【摘要】　目的　讨论发生门静脉海绵样变的可能机制。　方法　对1例疑似门静脉海绵样变伴梗阻性黄疸患者，术前经MRI上腹部轴位增强扫描和腹部彩色多普勒超声检查后行“胆总管空肠Roux-en-Y吻合术”。　结果　术中证实病变系门静脉海绵样变，胆总管下端完全不通畅。　结论　门静脉海绵样变病因复杂，但极少引起梗阻性黄疸。患者黄疸的原因可能由海绵样变的门静脉分支压迫胆总管引起。
