  • 尺神经手背支损伤的一期修复

    目的 总结尺神经手背支急性锐器伤的治疗方法及疗效。 方法 2007 年10 月- 2009 年3 月,对36 例尺神经手背支急性损伤采用显微外科技术一期修复。男29 例,女7 例;年龄20 ~ 59 岁,平均28 岁。玻璃切伤6 例,刀伤11 例,电锯伤19 例。损伤部位:尺神经手背支起始处至发出横支处13 例,尺神经手背支中间支与尺侧支联合损伤18 例,尺侧支损伤5 例。单纯尺神经手背支损伤22 例,合并尺骨茎突骨折1 例,第4 掌骨骨折2 例,第5 掌骨骨折3 例,环指伸肌腱断裂1 例,小指伸肌腱断裂7 例。 结果 术后伤口均Ⅰ期愈合,无感染等并发症发生。36 例均获随访,随访时间6 ~ 24 个月,平均16 个月。根据中华医学会手外科分会上肢部分功能评定试用标准中尺神经功能评定试用标准:获优33 例,良2 例,中1 例,优良率97.2%。 结论 尺神经手背支急性锐器伤采用显微外科技术一期修复,腕背及手背尺侧重要感觉功能恢复良好,效果满意。

  • CLINICAL OBSERVATION OF ONE STAGE CRANIOPLASTY FOR SKULL DEFECT WITH SELF-CRANIAL BONE POWDER

    Objective To investigate the clinical application of self-cranial bone powder in one stage cranioplasty.Methods From October 1999 to December 2002,self-cranial bone powder and medical adhesive were used to repair the skull defect, for one stage cranioplasty, caused by operations on cranium in 128 casesof severe dangerous craniocerebral injury, acute intracranial hematome, sick skull and intracranial tumor.The bone growth was observed by CT or X-ray examination 3-24 months after replantation of cranioplasty.Results The decompression and cranioplasty were performed simultaneously, the time prolonged 5-10 minutes than that of routine, the appearance of repaired cranial bone was normal, without concavity and convexity. After 12 months of operation, the replanted bone merged with the normal bone completely, with normal appearance. The operation successful rate was 96.1%(123/125) without any complication. Only fivecases were not better in growing because of less bone powder, but withoutcerebral pulse and defective syndrome. All the cases did not need secondary cranioplasty.Conclusion The effect of cranioplasty with self-cranial bone powder effect is good in taking shape. This new method can avoid the traditional secondary cranioplasty for skull defect and complications. 

  • PRIMARY MICROSURGICAL REPAIR OF MULTI-STRUCTURAL DEFECTS OF HAND

    Forty-eight cases of multi-structural defects of hands were primarily repaired or reconstructed from July 1989 to 1997. The structural defects included: the defects of radial or ulnar aspect of hands involving fingers and skin, multiple fingers defects and the fingers and skin defects of whole hand. In this series, there were 32 males and 16 females with age ranged from 17 to 46 years old. The composite tissue grafts were obtained from wrap-around flap or 2nd toe skin flap of the foot. The result showed that composite 108 tissues transplantations, or 48 cases, were all survived. After a follow-up of 38.5 months (ranged from 5 months to 6 years), the grasp, pinch and opposition function of the reconstructed finger were restored, the two-point discrimination sensation was 4 mm-12 mm. Most of the patients had resumed their original works. So that the primary repair of multi-structural defects of hands by composite tissues transplantation was feasible and valuable, but thorough debridement and skilled microsurgecal technique were required.
