  • Treatment Strategy for Patients with Subacute Myocardial Infarction and Severe Ischemic and Functional Mitral Regurgitation

    Objective To investigate appropriate treatment strategy and timing for patients with subacute myocardial infarction and severe ischemic mitral regurgitation (IMR). Methods A total of 89 patients with subacute myocardial infarction and severe IMR underwent surgical treatment from January 2005 to December 2011 in Beijing Anzhen Hospital. There were 66 male patients and 23 female patients with their mean age of 64 (55-73) years. All the patients received only coronary artery bypass grafting (CABG) after 3 months of medication treatment without specific management for their IMR. Echocardiography was examined before medication treatment,preoperatively and 6 months after CABG to analyze their IMR degree and measure left ventricular end-systolic dimension (LVESD),left ventricular end diastolic dimension (LVEDD) and left ventricular ejection fraction (LVEF). Results There was no surgery-related death,perioperative myocardial ischemia or other severe postoperative complication. Eighty-one patients (91.0%) were followed up for 6-60 months. At 6 months after CABG,mitral regurgitation area (3.1±1.3 cm2 vs. 5.6±2.3 cm2),LVEDD (51.3±4.2 mm vs. 54.3±5.5 mm) and LVESD (31.7±3.9 mm vs. 34.6±4.3 mm) were significantly decreased than preoperative values (P<0.05),but LVEF was not statistically different from preoperative value (59.1%±3.9% vs. 58.9%±5.6%,P>0.05). From the third year during follow-up,all the patients received annual CT examination of their coronary artery,and no significant graft stenosis (graft stenosis>50%) was found. Conclusion With appropriately delayed CABG and right medication treatment,patients with subacute myocardial infarction and severe IMR may no longer need concomitant surgical management for their IMR, which can decrease surgical risks and reduce treatment cost.

  • Off-pump Surgical Therapy for Mild-to Moderate-Ischemic Mitral Regurgitation Using a Self-designed Device

    Abstract: Objective To evaluate the effect of a surgical method for treating mild- to moderate-ischemic mitral regurgitation(IMR) using a self-designed device during off-pump coronary artery bypass grafting(OPCAB). Methods From September 2009 to August 2011, six patients(4 males, 2 females; age was 52-73 years) with mild- to moderate-IMR underwent OPCAB and concomitant mitral valvuloplasty using a self-designed device in Beijing An Zhen Hospital. Their degree of IMR, anteroposterior diameter of mitral annulus, left ventricular long-axis diameter, left ventricular short-axis diameter and left ventricular spherical index(left ventricular short-axis diameter/left ventricular long -axis diameter)were measured using transesophageal Doppler echocardiography before and after mitral valvuloplasty. Their mean aorta pressure, mean pulmonary artery pressure and central venous pressure were also measured via Swan-Ganz catheter before and after mitral valvuloplasty. Perioperative cardiac function indexes were compared. Results There was no in-hospital death. IMR of all patients disappeared postoperatively. After mitral valvuloplasty their anteroposterior diameter of mitral annulus(3.43±0.08 cm vs.3.68±0.08 cm;t=5.430, P=0.001), left ventricular short-axis diameter(4.80±0.21 cm vs.5.53±0.11 cm;t=7.530, P=0.001)and left ventricular spherical index(0.64±0.02 vs.0.74±0.01;t=11.110, P=0.002)significantly decreased than those before mitral valvuloplasty . But their left ventricular long-axis diameter and hemodynamic indexes did not change significantly after mitral valvuloplasty. All the six patients were followed up at the out-patient department 3 months postoperatively without autonomous symptoms. Their heart function improved to I class(New York Heart Association). Echocardiography showed 4 patients without IMR and 2 patients with trace of minimalIMR. Conclusion Off-pump surgical therapy for mild- to moderate- IMR during OPCAB can help the patients reverseremodeling of the left ventricle, avoid the risks of cardiopulmonary bypass and improve cardiac function with good short-term effects. This method may be a good choice for treating patients with IMR.

  • Clinical Outcomes of Preservation of Posterior Leaflet and Subvalvular Structures in Mitral Valve Replacement

    Abstract: Objective To summarize our experience and clinical outcomes of preservation of posterior leaflet and subvalvular structures in mitral valve replacement(MVR). Methods We retrospectively analyzed the clinical data of 1 035 patients who underwent MVR in Beijing An Zhen Hospital from January 2006 to March 2011. There were 562 male patients and 473 female patients with their age of 37-78(53.84±13.13)years old. There were 712 patients with rheumatic valvular heart disease and 323 patients with degenerative valve disease, 389 patients with mitral stenosis and 646 patients with mitral regurgitation. No patient had coronary artery disease in this group. For 457 patients in non-preservation group, bothleaflets and corresponding chordal excision was performed, while for 578 patients in preservation group, posterior leafletand subvalvular structures were preserved. There was no statistical difference in demographic and preoperative clinical characteristics between the two groups. Postoperative mortality and morbidity, and left ventricular size and function were compared between the two groups. Results There was no statistical difference in postoperative mortality(2.63% vs. 1.21%, P =0.091)and morbidity (8.53% vs. 7.44%, P=0.519)between the non-preservation group and preservation group, except that the rate of left ventricular rupture of non-preservation group was significantly higher than that of preservation group(1.09% vs. 0.00%, P=0.012). The average left ventricular end-diastolic dimension (LVEDD)measured by echocardiography 6 months after surgery decreased in both groups, but there was no statistical difference between the two groups. The average left ventricular ejection fraction (LVEF) 6 months after surgery was significantly improved compared with preoperative average LVEF in both groups. The average LVEF 6 months after surgery in patients with mitral regurgitation in the preservation group was significantly higher than that in non-preservation group (56.00%±3.47% vs. 53.00%±3.13%,P =0.000), and there was no statistical difference in the average LVEF 6 months after surgery in patients with mitral stenosis between the two groups(57.00%±5.58% vs. 56.00%±4.79%,P =0.066). Conclusion Preservation of posterior leaflet and subvalvular structures in MVR is a safe and effective surgical technique to reduce the risk of left ventricle rupture and improve postoperative left ventricular function.

  • Clinical Application of Homemade Flexible Annuloplasty Ring in Mitral Valve Repair

    Abstract: Objective To summarize the clinical results of homemade flexible annuloplasty ring in mitral valve repair, in order to discuss the appropriate ring size. Methods Sixtysix patients (55 males,and 11 females with a mean age of 44.62±15.94 years) with mitral insufficiency underwent mitral valve repair with homemade flexible annuloplasty ring from April 2002 to November 2009 in Fu Wai Hospital. In order to choose the ring with an appropriate size, we made and kept to the following principles: if the intercommissural distance was bigger than size 30, we chose a ring 2size smaller; if the measured distance was smaller than size 30, 1size smaller ring would be chosen. Patients were followed by echocardiography to observe the mitral valve function. Results All patients were cured and discharged from the hospital. The results of echocardiography showed mild to moderate regurgitation in 1 patient, mild regurgitation in 11 patients, and normal mitral function or trace regurgitation in the rest 54 patients. Mitral valve forward velocity was 1.40±0.30 m/s with no mitral stenosis or systolic anterior motion (SAM) of the anterior mitral leaflet. Fiftyone patients were followed up from 2 months to 7 years(24.60±25.90 months). The results of echocardiography on 38 patients showed that 1 patient had moderate regurgitation, 5 patients had mild to moderate regurgitation, 9 patients had mild regurgitation and others had normal mitral function or trace regurgitation. For these 38 patients included in the followup study, mitral valve forward velocity was 1.50±0.40 m/s with no mitral stenosis, SAM or left ventricular outlet tract obstruction. During the followup, the left atrium size (43.19±10.48 mm vs. 48.59±9.40 mm, t=4.524, P=0.000) and left ventricular end diastolic diameter (52.64±7.35 mm vs. 6269±8.77 mm, t=7.607, P=0.000) decreased significantly than the preoperative size and diameter respectively.  The application of restrictive homemade flexible annuloplasty ring in mitral valve annuloplasty had satisfactory, durable and stable clinical results.

  • Mild Functional Tricuspid Regurgitation Approach to Mitral Valve Repair

    Abstract: Objective To explore whether clinically mild functional tricuspid regurgitation should be addressed at the time of mitral valve repair (MVP) for moderate or severe mitral regurgitation due to myxomatous degeneration. Methods We retrospectively analyzed the outcomes of 135 patients with moderate or severemitral regurgitation due to myxomatous degeneration with mild functional tricuspid regurgitation. All patients were treated between January 1993 and March 2008 in the Department of Cardiothoracic Surgery of Changhai Hospital, the Second Military Medical University. We divided the patients into a MVP group (n=76) and a MVP+tricuspid valvuloplasty (TVP) group(n=59) according to whether they underwent combined TVP, and observed the perioperative mortality rate, degree of tricuspid regurgitation, and compared survival rate, and freedom from longterm moderate or severe tricuspid regurgitation after operation. Cox regression was used to analyzethe risk factors for longterm moderate or severe tricuspid regurgitation after operations. Results (1) There were no deaths during the perioperative period, and postoperative transthoracic echocardiography of all patients indicated that tricuspid regurgitation was mild or less. (2) Survival rate at 5 years, 10 years after operations in MVP group was 98.4%, 95.0%, respectively, and survival rate at 5 years, 10 years after operations in MVP+TVP group was 100.0%, 93.7%, respectively, and there was no significant difference in the survival rate after operations between the two groups(P=0.311), butthere was a significant difference in the freedom from longterm moderate or severe tricuspid regurgitation after operations between the two groups (P=0.040). Multivariate Cox regression showed that preoperative pulmonary artery pressure gt;30 mm Hg (95%CI 1.127 to 137.487, P=0.040 )and atrial fibrillation (95%CI 1.177 to 23.378, P=0.030) wereindependent risk factors for longterm moderate or severe tricuspid regurgitation afteroperations.Conclusion TVP is necessary for most patients undergoing MVP for moderate or severe mitral regurgitation due to myxomatous degeneration who have coexistent mild functional tricuspid regurgitation, especially those patients with preoperative pulmonary artery pressure gt;30 mm Hg or atrial fibrillation.

  • 非体外循环冠状动脉旁路移植术后缺血性二尖瓣关闭不全患者的心功能改变

    目的 探讨非体外循环冠状动脉旁路移植术(offpump CABG)对缺血性二尖瓣关闭不全患者心功能改变的影响,以提高治疗效果。 方法 53例缺血性二尖瓣关闭不全患者,男45例,女8例;年龄 51~73 岁,平均年龄63.3岁。53例患者均行offpump CABG,术前及术后12个月分别采用超声心动图检测心功能改变及二尖瓣反流情况。 结果 术后3 h死于突发心室颤动1例;术后发生大出血1例,于术后6 h行二次开胸止血手术。随访52例,随访时间12个月,其中行超声心动图检查48例。术后12个月左心室收缩期末内径(34.75±6.83 mm vs. 38.80±7.21 mm,t=-3.550, P=0.001),左心室舒张期末内径(50.50±10.31 mm vs. 53.85±11.20 mm, t=-6.364, P=0000)和二尖瓣反流面积(25.32%±9.80% vs. 29.45%±12.30%,t=2.129, P=0.001)均较术前减小, 左心室射血分数(62.10% ±24.20% vs. 57.75%±22.10%, t=2.132, P=0.038)较术前增加。 结论 冠心病合并中度二尖瓣关闭不全患者行offpump CABG术后12个月二尖瓣反流量减少,左心室功能改善。

  • 冠心病合并室壁瘤及二尖瓣关闭不全的外科治疗

    摘要: 目的 总结冠心病合并室壁瘤和二尖瓣关闭不全的外科治疗经验及效果。 方法 回顾性分析78例冠心病合并室壁瘤和二尖瓣关闭不全患者的临床资料,其中男45例,女33例;年龄42~70岁,平均年龄55岁。室壁瘤位于心尖部44例,外侧壁14例,下壁20例; 二尖瓣中度关闭不全48例,重度关闭不全30例。手术在低温体外循环下施行,室壁瘤线性缝合术24例,心内膜环缩、三明治缝合术36例,心内膜环缩、左心室补片成形术18例,同期血栓清除术13例;二尖瓣成形术42例,二尖瓣置换术36例;移植旁路血管1~6支(2.5±1.5支)。 结果 围术期死亡5例,死于败血症1例,多器官功能衰竭2例,心力衰竭2例。术中停机困难5例。术后主要并发症:低心排血量综合征、恶性心律失常和多器官功能衰竭。左心室舒张期末内径(55.6±1.2 mm vs. 68.2±4.0 mm),射血分数(45%±23% vs.34%±14%),心功能分级(NYHA,1.82±0.26级 vs. 3.36±0.56级)与术前比较差异均有统计学意义(Plt;0.05)。随访69例,随访率94.52%,随访时间25±8个月。随访期间无远期死亡,无症状复发。心功能Ⅰ~Ⅱ级58例,Ⅲ级9例,Ⅳ级2例, 较术前明显改善(Plt;0.05)。 结论 根据病理生理特点选择对心腔、血管和瓣膜全面处理的手术方式,加强围术期管理,手术治疗冠心病合并室壁瘤及二尖瓣关闭不全的近、远期疗效满意。

  • 二尖瓣成形术的临床应用及疗效分析

    摘要: 目的 总结二尖瓣成形术的临床经验,分析其疗效,探讨临床应用方法。 方法 自2000年6月至2009年6月上海交通大学附属胸科医院心血管外科对236例二尖瓣关闭不全患者施行二尖瓣成形术,其中男105例,女 131例;年龄8个月~76岁(53.27±21.60岁)。术前均经彩色超声心动图检查诊断:二尖瓣中度关闭不全79例,重度关闭不全157例。针对236例三种类型不同病变部位的二尖瓣关闭不全,采用人工瓣环植入22例,前叶三角形切除5例,人工腱索植入45例,腱索转移34例,缘对缘技术31例,后叶Sliding技术16例,矩形或楔形切除77例,裂缺修复6例。 结果 全组无死亡,术毕经食管超声心动图检查提示:无反流156例(66.10%),轻微轻度反流68例(28.81%),轻-中度反流12例(5.08%);出现血红蛋白尿3例;术后随访236例,随访时间6~108个月(62.00±38.20个月),超声心动图检查提示:无反流138例,轻微-轻度反流78例,轻中度反流17例,中度反流2例,中重度反流1例; 平均左心室射血分数 58.34%。再手术率为0.84%(2/236)。 结论 灵活应用多种二尖瓣成形技术纠治不同类型的二尖瓣关闭不全,中长期效果良好。

  • Surgical Treatment of Partial Atrioventricular Canal Defect in 66 Cases

    Objective To summarize the experiences of surgical treatment for partial atrioventricular canal defect. Methods The data of 66 patients of surgical treatment for partial atrioventricular canal defect from January 1984 to December 2007 were analyzed retrospectively. The cleft of mitral valve presented in all of those patients. There were 52 cases with direct suture on cleft, 8 cases with direct suture with commissurroplasty, 1 case with posterior leaflet plasty, 3 cases with direct suture St.Jude ring and 2 cases mitral valve replacement. The ostium primum atrial septal defects were repaired with patches of Dacron in 12 cases and autologous pericardium in 54 cases. Coronary sinus was situated on the left atrium in 5 and ostium primum atrial septal defects were repaired in Kirklin’s way; the others in MeGoon’way. Meanwhile other heart abnormalities were done. Results There were two early deaths (3.03%), one patient died of heart arrhythmia and one patient died of respiratory failure. Complications of total A-V block was in 2 cases. Both of them were replanted with pace makers.52 cases were followed up, followup time was 5 months to 22 years(mean follow-up 15 years). All patients had better life. Four patients have been re -operated for different reasons post primary operation. One had good result after re-mitral valve replacement. One case died of acute renal failure and the other two died of low cardiac output syndrome. Conclusions Early operation is definitely recommended when the diagnosis is confirmed. Because the structure or function of mitral valve is saved, pulmonary hypertension is avoided and the mortality is lower in the future. The key points of operation are to rectify the mitral insufficiency, repair ostium primum atrial septal defects and avoid atrioventricular block. The patients of mild regurgitation of mitral valve have good results. Provided in those have more than middle regurgitation of mitral valve then their longterm results are poor.

  • The Effect of Edgetoedge Mitral Valve Plasty on Left Ventricular Diastolic Function

    Objective To investigate the effect of edgetoedge mitral valve plasty on left ventricular diastolic function and in order to find the validity and safety of this procedure. Methods From Feb. 2006 to Dec. 2007, thirty cases with mitral regurgitation were divided into two groups. Quadrangular resection was performed on fifteen cases with posterior proplapse in control group, and edgetoedge mitral valve plasty was performed on fifteen cases with anterior or bileaflet proplapse in experimental group, and ring annuloplasty(Medtronic ring) was used in both groups. The hemodynamics were monitored and recorded with SwanGanz catheter at the time of postoperation,2 h, 4 h, 6 h and 12 h after operation. Left ventricular diastolic function was also evaluated with echocardiography using color Doppler and tissue Doppler imaging in the patients with sinus rhythm. The ratio of the peak E velocity and A velocity(E/A), the ratio of the early diastolic peak flow velocity to the early diastolic mitral valve annular movement velocity(E/Em), and the ratio of early diastolic mitral valve annular movement velocity to late diastolic mitral valve annular movement velocity(Em/Am)were measured before operation and 1 week after operation respectively. Results Mitralvalve area were significantly reduced at 1 week after operation compared with that before operation in both groups (control group 3.63±1.06 cm2 vs. 7.18±2.41 cm2, experimental group 3.44±1.02 cm2 vs. 6.51±3.06 cm2, Plt;0.05); and mitral regurgitant grade were significantly reduced at 1 week after operation in both groups as well(control group 0.53±0.64 cm2 vs.3.60±0.51 cm2, experimental group 0.67±0.82 cm2 vs.3.40±0.63 cm2, Plt;0.05). However, there was no significant difference for mitral valve area and mitral regurgitant grade between two groups before and after operation(Pgt;0.05). In experimental group, there were no significant change of evaluations of E/A,E/Em and Em/Am before and after operation(E/A 1.28±0.36 vs. 1.95±1.06,E/Em 8.79±2.16 vs. 8.13±3.02, Em/Am 1.39±0.38 vs. 1.31±041,Pgt;0.05). There was no significant change of pulmonary artery wedge pressure (PAWP) before and after operation between two groups(13.60±4.37 mm Hg vs.12.20±3.53 mm Hg, Pgt;0.05). Conclusion Edgetoedge mitral valve plasty technique is available and has no significant influence on left ventricular diastolic function, and a doubleorifice mitral valve has similar hemodynamic change compared with a physiological mitral valve.
