  • 小流量负压引流在腹壁排粪造口旁感染中的应用价值研究(附 18 例病例分析)

    目的 研究小流量负压引流装置在严重造口旁感染处理中的可行性与应用效果。 方法 回顾性分析川北医学院附属医院 2013 年 5 月至 2016 年 5 月期间收治的 18 例严重肠造口旁感染患者的临床资料,根据造口旁感染发生后处理方法的不同分为一般换药处理组(简称“换药组”,n=8)和小流量负压引流组(简称“引流组”,n=10),对 2 组的临床治疗效果进行比较。 结果 2 组患者的一般资料如年龄、性别、体质量指数、原发病、造口部位、合并症、感染发生的时间等比较差异无统计学意义(P>0.05)。引流组 10 例患者感染灶内脓液、积液引流效果佳,创面愈合良好,所有感染创口均愈合后患者顺利出院;随访期为 1~36 个月,其中 1 例发生造口旁疝。换药组 8 例患者中 7 例患者感染创面顺利愈合,另有 1 例因感染面积扩大伴局部肠管部分缺血坏死,自发现感染后第 5 天另行肠切除肠造口术,原切口术后经换药处理后于第 10 天愈合出院;随访期为 2~36 个月,无其他并发症发生。引流组患者的造口旁感染病灶愈合时间和总住院时间均明显短于换药组(P<0.05),2 组的并发症发生率比较差异无统计学意义(P>0.05)。 结论 从本研究有限的数据初步研究结果来看,小流量负压引流装置在造口旁感染中应用安全、有效,且易于掌握、取材便利。

  • Establishment and Characterization of Human Gastric Cancer Cell Line SGC-7901/HCPT Xenograft in Nude Mice

    Objective To establish a xenograft model of hydroxycamptothecine (HCPT)-resistant human gastric cancer cell line (SGC-7901/HCPT) in nude mice and study its biological characteristics. Methods The SGC-7901 and SGC-7901/ HCPT cells were cultured in vitro. The cell suspension was injected subcutaneously into the nude mice. When the subcutaneous carcinoma was 1.0 cm in diameter, it was cut off and divided into pieces of 0.1-0.2 cm in diameter. Then the small pieces of tumor were re-transplanted subcutaneously into the second generation nude mice until the fourth generation. The morphological feature, ultramicro-structure, and growth characteristics of the fourth generation transplanted tumor were examined. The drug resistance was measured by methyl thiazolyl tetrazolium (MTT) assay. Results The transplanted tumor in nude mice was round or oval, and many blood vessels were on its surface. Under the light microscope, the sizes of SGC-7901 transplanted tumor cells were similar, and sizes of cell nuclei were also similar; Meanwhile, the morphous of SGC-7901/HCPT transplanted tumor cells were irregular and in disorder, and the size of the cell nuclei was different from each other. Under the electron microscope, the mitochondria and endoplasmic reticulum of SGC-7901 transplanted tumor cells were nearly normal and no swelling in cell nuclei; Meanwhile the cell nuclei of SGC-7901/HCPT transplanted tumor cells were lightly swelled, a the mitochondria and endoplasmic reticulum were obviously swelled. By MTT assay, compared with SGC-7901 transplanted tumor cells, the resistance index of SGC-7901/HCPT transplanted tumor cells was 9.02±0.78 in HCPT, and resistance index to Adriamycin, Mitomycin C, 5-fluorouracil, and Etoposide was 1.24±0.09, 1.31±0.17, 0.96±0.12, and 1.07±0.16, respectively. Conclusions A transplanted tumor model of SGC-7901/HCPT in nude mice is established successfully, and showing stable drug resistance to HCPT and no cross-resistance to other chemotherapeutics, which can be used for further experiments.

  • Detection of 5-FU Concentration and Pathological Effects in Intraoperative Regional Chemotherapy for Colon Cancer

    ObjectiveTo detect 5-FU concentration and investigate the changes of pathology, and Ki-67 protein expression after intraoperative regional chemotherapy (RC) for colon cancer. MethodsAll the patients were randomized into two groups: RC group (n=20), received intraoperational RC with 100 ml physiological saline contained 5-FU (15 mg/kg) and camptothecine (0.06 mg/kg); control group (n=20), saline alone. The samples from portal vein blood, peripheral blood, peritoneal fluid, and peri-cancerous tissues in RC group were taken to detect the 5-FU concentration by high performance liquid chromatography (HPLC), respectively at 2, 5, 10, 20, 30, and 60 minutes after treatment. The pathological changes were observed and Ki-67 protein expressions were examined by immunohistochemical staining for all the cancer tissues postoperatively in two groups. ResultsPeak concentration of 5-FU appeared at 2 min after treatment, and decreased gradually. 5-FU concentration in peritoneal fluid was the highest, and the lowest in the peripheral blood (Plt;0.01). In RC group, light karyopyknosis, nuclear swelling, and coagulative necrosis of cancer cells, and light intercellular substance hydropsia, inflammatory cells invasion were observed under light microscopic examination; light vasculitis presented also in five cases. Nuclear swelling, heterochromatin agglutination, perinuclear gap expansion, mitochondrial swelling, endoplasmic reticulum expansion, and Golgi complex expansion were observed with transmission electron microscope. Ki-67 protein expression of colon cance tissues in RC group was lower than that in control group (Plt;0.05). Conclusions Intraoperative RC for colon cancer may sustain a high concentration of chemotherapy drugs in peritoneal fluid and portal vein blood, and alter histopathological morphology of cancer cells, and suppress Ki-67 protein expression. So, intraoperative RC may play an important role in preventing intraoperative spreading and postoperative recurrence of colon cancer.

  • Analysis of postoperative complications and their related factors after laparoscopic radical surgery in rectal cancer

    ObjectiveTo investigate factors associated with postoperative complications after laparoscopic radical surgery in rectal cancer.MethodsThe clinical data of patients with rectal cancer performed by the laparoscopic radical resection from February 2013 to December 2016 were analyzed retrospectively. All the data were analyzed by the t test, chi-square test or logistic regression analysis.ResultsThere were 343 patients with rectal cancer performed by the laparoscopic radical resection. The postoperative complications occurred in the 97 (28.3%) patients. The result of univariate analysis showed that the postoperative complications rate was associated with the gender, age, body mass index, preoperative anemia, preoperative comorbidity, location and diameter of tumor, operative time, and surgeon experience (all P<0.050). The results of logistic regression analysis revealed that the gender, age, body mass index, preoperative anemia, preoperative comorbidity, location of tumor, operative time, and surgeon experience were the independent risk factors for the postoperative complications (all P<0.050).ConclusionGender, age, body mass index, preoperative anemia, preoperative comorbidity, location of tumor, operative time, and surgeon experience are independent risk factors for postoperative complications in laparoscopic radical rectal surgery for rectal cancer.
