  • 心脏不停跳手术中心肌NF-κB转录活性、ICAM-1的表达及其临床意义

    目的 探讨心脏不停跳与心脏停搏手术对心肌核转录因子κB(NF-κB)转录活性、细胞间黏附分子-1(ICAM-1)表达水平的影响及其临床意义。方法 将40例先天性心脏病患者随机分为两组,每组20例。组Ⅰ:行心脏不停跳心内直视手术;组Ⅱ:行常规体外循环手术(灌注冷晶体心脏停搏液)。两组患者均于心内操作前、后取右心房壁心肌组织检测NF-κB转录活性、ICAM-1表达水平,用透射电子显微镜观察心肌超微结构;分别于术前、主动脉开放或心内操作完成后1、24、48和72h测定两组心肌肌钙蛋白I(cTnI)、肌酸激酶同工酶MB(CK-MB),并对其结果进行比较。结果术后组Ⅰ NF-κB转录活性、ICAM-1表达水平较术前无显著变化,组Ⅱ NF-κB转录活性较术前升高(Plt;0.01);术后NF-κB转录活性组Ⅱ显著高于组Ⅰ(Plt;0.01)。术后两组血清cTnI、CK-MB水平较术前均有不同程度升高(Plt;0.01),主动脉开放后/心内操作完成后各时点,组Ⅱ均显著高于组Ⅰ(Plt;0.01)。透射电子显微镜观察,组Ⅰ术后心肌超微结构无明显变化,组Ⅱ心肌损伤变化显著。结论 心脏不停跳下心内直视手术术后短期内心肌NF-κB转录活性、ICAM-1表达水平无明显变化,减轻了心肌缺血-再灌注损伤及由NF-κB激活而引起的心肌炎性反应,有较好的心肌保护效果。

  • 心脏不停跳与冷晶体停搏在常见先心病矫治术中对肌钙蛋白I的影响

    目的 探讨常见先天性心脏病矫治术中心脏不停跳与心脏停搏两种术式对心肌肌钙蛋白 I(c Tn I)的影响及其心肌保护的效果。 方法  36例先天性心脏病矫治术患者按入院顺序以奇偶数随机分为心脏不停跳组 (组II)和冷晶体停搏组 (组I)两组 ,每组 18例。分别于术前、主动脉开放后 (组 为缝合右心房壁后 ) 1、2 4、4 8、72和 96 h取患者中心静脉血 ,测定 c Tn I、肌酸激酶 (CK)、肌酸激酶同工酶 (CK- MB)、乳酸脱氢酶 (L DH) ,并用透射电子显微镜观察两组各前 10例患者心内操作前、后心肌超微结构。 结果 两组术后血清 c Tn I、CK、CK- MB和 L DH水平均不同程度升高。两组 c Tn I均于术后 1h达峰值 ,主动脉开放后各时点组II显著高于组I(t=- 32 3.0 4 ,Plt;0 .0 1) ;组ICK-MB术后 2 4 h达峰值 ,组II术后 1h达峰值 ,主动脉开放后各时点组II显著高于组I(t=- 72 .5 3,Plt;0 .0 1) ;CK、L DH变化趋势与 CK- MB相似。组I术后心肌超微结构变化轻微 ,组II明显改变 ,...更多其受损程度分级较术前重。 结论 常见先天性心脏病矫治术中心脏不停跳可明显减轻心肌缺血、缺氧及再灌注损伤 ,减少 c Tn I的释放 ,较冷晶体停搏术有良好的心肌保护效果。

  • Application of rotationplasty in treatment of osteosarcoma of distal femur in children

    ObjectiveTo investigate the effectiveness of rotationplasty in treating osteosarcoma of distal femur in children.MethodsA clinical data of 10 children with osteosarcoma of distal femur treated with rotationplasty between March 2014 and June 2016 was retrospectively analyzed. There were 7 boys and 3 girls with an average age of 6.7 years (range, 4-10 years). There were 4 cases of osteoblastic osteosarcoma, 4 cases of mixed osteosarcoma, and 2 cases of chondroblastic osteosarcoma. All children were staged as Enneking stage ⅡB. The disease duration ranged from 3.5 to 6.0 months (mean, 4.6 months). The lower limb functional scoring system of 1993 Musculoskeletal Tumor Society (MSTS93), Toronto Extremity Salvage Score (TESS), and knee mobility were used to evaluate postoperative function. Tumor recurrence and metastases were monitored by radiograph.ResultsPoor superficial incision healing occurred in 1 patient, and healed after dressing change. The other incisions healed by first intention. All children were followed up 24-72 months (mean, 52.6 months). No local recurrence was observed during follow-up. Three of the ten patients suffered from metastases including 1 dying of multiple organ dysfunction syndrome, 1 alive with tumor, and 1 tumor free survival. Painful callosities and ulcers which related to prosthetic wear occurred in 2 patients and turned up after optimizing prosthetic fit and physiotherapy. The fracture healing time was 2.5-5.0 months (mean, 3.5 months). All children could walk independently at 4 months postoperatively. At last follow-up, the MSTS93 score was 19-25 (mean, 22) and the TESS score was 87-93 (mean, 90). The extension of knee joint mobility with artificial limbs was 0°-10° (mean, 5°), and the flexion of knee joint mobility with artificial limbs was 85°-95° (mean, 90.5°).ConclusionRotationplasty in treating osteosarcoma of distal femur in children with limb salvage difficulties can effectively preserve the limb function and improve the quality of life, and it can be used as an alternative to amputation.

  • Evaluation of total scapular arthroplasty after total scapulectomy for scapular tumors

    ObjectiveTo evaluate the effectiveness of total scapular arthroplasty after total scapulectomy for scapular tumors.MethodsA clinical data of 17 patients with scapular tumors treated with total scapulectomy and total scapular arthroplasty between January 2010 and December 2017 were retrospectively reviewed. There were 9 males and 8 females with an average age of 34.4 years (range, 13-64 years). Seven patients were diagnosed with chondrosarcoma, 3 with osteosarcoma, 2 with Ewing’s sarcoma, 1 with high-grade sarcoma, 1 with polymorphic dedifferentiated sarcoma, 1 with fibrosarcoma, 1 with plasmacytoma, and 1 with bone giant cell tumor. According to the surgical staging system described by Enneking et al, 1 patient was rated as stage 3, 8 as stageⅠB, 8 as stageⅡB. According to the classifications of shoulder girdle resections of Malawer et al, 11 patients were type ⅢB, 5 were type ⅣB, 1 was type ⅥB. The disease duration ranged from 0.5 to 8.0 months (mean, 3.2 months) and tumor size ranged from 11.0 cm×7.5 cm×6.0 cm to 18.5 cm×18.0 cm×12.5 cm. The 1993 Musculoskeletal Tumor Society (MSTS) upper limb function scoring system and shoulder mobility were used to evaluate postoperative shoulder joint function. Tumor recurrence and metastases were monitored by radiograph.ResultsPoor superficial incision healing occurred in 1 patient, the rest incisions achieved healing by first intention. All patients were followed up 20-72 months (mean, 45.4 months). Two of the 17 patients died of multiple organ dysfunction syndrome caused by tumor metastases; 3 patients suffered from pulmonary metastases and were alive with disease. No local recurrence occurred in all patients. The overall survival rate was 88.2% (15/17) and the disease-free survival rate was 70.6% (12/17). Rib fracture after trauma, aseptic loosening, and atrophy of the deltoid muscle occurred in 1, 1, and 1 case, respectively. The other related complication was not observed. At last follow-up, the MSTS score was 26.1±1.4, and the flexion, extension, and abduction range of motion of shoulder joint were (70.0±7.5), (31.2±11.3), and (54.4 ±12.5) °, respectively.ConclusionReconstruction with total scapular arthroplasty after total scapulectomy can obtain a satisfactory shoulder contour and an acceptable functional outcomes in patients with scapular tumors.

  • Investigation on preventive behaviors of occupational exposure in medical staff based on health belief model

    Objective To explore the influencing factors of medical staff’s preventive behaviors of occupational exposure using health belief model. Methods A questionnaire was used to investigate the health beliefs and prevention-related behaviors of medical staff in Shuguang Hospital Affilliated to Shanghai University of Traditional Chinese Medicine from July 2018 to June 2019. The preventive behaviors and health beliefs of health workers with different occupational categories and working years were analyzed descriptively, and the influence of health beliefs on preventive behaviors was analyzed with multiple linear regression. Results There were 400 copies of questionnaire dispatched and 384 recovered. Among the survey respondents, 258 (67.19%) were females, 169 (44.01%) were aged 20-30 years, and 203 (52.86%) had worked for 5 or less years; 158 (41.15%) had sharp injury experience, 171 (44.53%) had experience of exposure to blood or body fluids, and 290 (75.52%) were actively immunized by injecting vaccines. The total score of compliance with standard preventive behaviors of the 384 respondents was 6.94±3.12. The scores of perceived susceptibility, perceived severity, perceived barriers, perceived benefits, and perceived self-efficacy were 24.87±4.56, 31.27±5.92, 16.50±5.77, 36.06±5.60, and 15.27±2.98, respectively. The total score of health beliefs was 123.97±15.91. The compliance with standard preventive behaviors and total score of health beliefs of logistics staff were lower than those of other respondents (P<0.05). The scores of perceived barriers of medical technicians and clinicians were significantly higher than that of nurses (P<0.05). The scores of perceived benefits and perceived self-efficacy of clinicians were lower than those of nurses (P<0.05). Respondents who had worked for 6-10 years had better compliance with standard preventive behaviors than interns and the ones who had worked for 11-15 years (P<0.05), and had a lower total score of health beliefs than the ones who had worked for over 16 years (P<0.05). Respondents who had worked for over 16 years had better scores of perceived benefits and perceived self-efficacy (P<0.05), and had a lower score of perceived barriers (P<0.05). The results of multiple linear regression analysis showed that gender [nonstandardized partial regression coefficient (b)=–0.644, 95% confidence interval (CI) (–1.162, –0.125), P=0.015], educational background [b=1.367, 95%CI (1.245, 1.587), P<0.001], and score of perceived barriers [b=0.044, 95%CI (0.004, 0.084), P=0.031] were influencing factors of medical staff’s preventive behavior compliance. Conclusions The score of perceived barriers is a main factor that prevents medical staff from implementing standard preventive measures. In the occupational exposure training program, it may be effective to help medical staff establish good standard prevention habits by improving the awareness of obstacles.

  • Research and analysis on the reasonable selection of personal protective equipment for medical staff in the designated hospital for treatment of coronavirus disease 2019

    Objective To provide theoretical basis for the reasonable selection of personal protective equipment by analyzing the willingness of protection and the contamination of severe acute respiratory syndrome coronavirus 2 of medical staff in the designated hospital for treatment of coronavirus disease 2019. Methods The medical staff of Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine who entered the contaminated area from May 1 to 31, 2022 were collected as the study subjects. A simple random sampling method was adopted to investigate the willingness of protection of different medical staff leaving the cabin. Contamination of severe acute respiratory syndrome coronavirus2 was detected by fluorescence polymerase chain reaction. Results A total of 70 medical staff were included. There were 61 nurses and 38 in intensive care unit. The survey showed that 47 medical staff chose disposable isolation clothes, 44 medical staff chose protective face screen and 69 medical staff chose double-layer shoe cover/boot cover. A total of 640 specimens were collected. Six positive samples for severe acute respiratory syndrome coronavirus2 nucleic acid test were detected, with a positive rate of 0.94%. All the positive samples were sampled from the sole of a protective clothing. Six positive samples were willing to choose double-layer shoe cover/boot cover. Conclusions The medical staff in designated hospital for treatment of coronavirus disease 2019 tend to take high protection during daily medical activities. However, personal protective equipment is less likely to be contaminated by severe acute respiratory syndrome coronavirus2, and it should be selected rationally to avoid excessive protection.
