  • An Analysis of the Relationship between Resting Heart Rate Controlling and Pulse Wave Velocity in Patients with Long-standing Persistent Atrial Fibrillation

    目的 研究长期持续性心房颤动患者静息心率控制与脉搏波传导速度(PWV)的关系。 方法 序贯收集于2011年12月-2012年3月在四川大学华西医院心脏内科门诊就诊的长期持续性心房颤动患者84例,将人群按静息心率是否低于80次/min,分为标准组(心率<80次/min)和对照组(心率≥80次/min),采用Pearson相关及多元线性回归分析方法,研究静息心率控制与PWV的关系。 结果 ①标准组人群的PWV显著低于对照组,而饮酒率显著高于对照组,差异皆有统计学意义(P<0.05)。②Pearson相关分析显示静息心率与PWV存在线性相关关系(r=0.355,P=0.001);多元线性回归分析显示,在调整了年龄、性别、BMI、收缩压、舒张压、吸烟、饮酒、空腹血糖、总胆固醇、甘油三酯、高密度脂蛋白、低密度脂蛋白等混杂因素影响后,心率与PWV仍独立相关。 结论 长期持续性心房颤动患者的静息心率控制不良与PWV升高关系密切。

  • Application of Arterial Stiffness and Ankle Brachial Index in Evaluating Peripheral Arteriosclerosis in Elderly Patients with Type 2 Diabetes

    目的 了解老年2型糖尿病动脉僵硬度与踝臂指数(ABI)对评价外周动脉硬化的价值。 方法 2010年1月-3月,应用超声、回声跟踪技术和动脉硬化检测仪检测80例老年2型糖尿病患者(观察组)和40例正常老年人(对照组)的颈总动脉内中膜厚度(IMT)、僵硬系数(β)、脉搏波传导速度(PWVβ)、ABI及动脉斑块情况。 结果 与对照组比较,观察组患者的IMT增厚、β增高、PWVβ增快、ABI降低及动脉斑块发生率增多,差异均具有统计学意义(P<0.001)。 结论 老年2型糖尿病可导致IMT增厚、动脉僵硬度增高、PWVβ增快、ABI降低及更易发生动脉粥样硬化斑块,这些参数可以预测外周动脉粥样硬化的程度,利用彩色多普勒超声诊断仪和动脉硬化检测仪可以较容易得到这些参数,是评价外周动脉粥样硬化病变一种简便、易行,有临床实际意义的方法和手段。

  • Changes of Plasma Homocysteine and Peripheric Arterial Stiffness in Patients with Subclinical Hypothyroidism

    ObjectiveTo observe the changes of plasma homocysteine (Hcy) and brachial ankle pulse wave velocity (baPWV) in patients with subclinical hypothyroidism, and discuss the relationship between subclinical hypothyroidism and arterial stiffness. MethodSeventy-three patients with subclinical hypothyroidism who were not treated before were divided into two groups according to thyroid stimulating hormone (TSH) level between January 2013 and June 2014. There were 35 patients in group A (4 mU/L < TSH < 10 mU/L) and 38 in group B (TSH ≥ 10 mU/L). Another 30 healthy individuals were selected as controls. Hcy and baPWV were determined in all subjects. ResultsCompared with the controls, patients had significantly higher level of TSH, Hcy and baPWV in group A, and had significantly higher TSH, triacylglycerol (TG), low density lipoprotein cholesterol (LDL)-C, Hcy, and baPWV in group B (P<0.05). Compared with group A, TSH, TG, LDL-C, Hcy, and baPWV in group B patiens were significantly higher (P<0.05). Pearson correlation analysis showed that Hcy was positively correlated with TSH (r=0.353, P<0.01) and baPWV was positively correlated with TSH (r=0.416, P<0.01). ConclusionsHcy level and peripheric arterial stiffness increase in patients with subclinical hypothyroidism. Both of them are correlated positively with TSH.

  • The role of increased preoperative ventricular-arterial stiffening in the hypertension of infants with coarctation of the aorta

    ObjectiveTo explore the role of increased preoperative ventricular-arterial stiffening in hypertensive infants with coarctation of the aorta (CoA).MethodsA retrospective study was conducted in 314 infants with CoA (CoA group, 193 males and 121 females, aged 5.4±2.2 months), and 314 infants receiving tumor chemotherapy with normal cardiovascular function and without pneumonia (control group, 189 males and 125 females, aged 4.4±3.8 months), who were admitted to our hospital from 2015 to 2017. The clinical data of the two groups were compared.ResultsThere were statistical differences in effective aortic elastance index (Eai), effective left ventricular end-systolic elasticity index (Eesi), ventricle-artery coupling index (VACi), N-terminal pro-brain natriuretic peptide (NT-proBNP) levels, and ratios of left ventricular hypertrophy, dilation and systolic dysfunction between the two groups (P<0.05). Compared with non-hypertensive CoA infants, CoA infants with hypertension had higher Eai, Eesi, NT-ProBNP level and proportion of left ventricular hypertrophy (P<0.05). Compared with concomitant ventricular septal defect, infants with isolated CoA had higher Eai, Eesi, NT-proBNP level, incidence of hypertension and higher proportion of left ventricular hypertrophy, dilation and systolic dysfunction (P<0.05). Eai and Eesi were positively correlated with NT-proBNP level, left ventricular hypertrophy and fractional shortening of left ventricle (P<0.05), while Eai and Eesi were negatively correlated with left ventricular end diastolic volume index, left ventricular end systolic volume index and concomitant ventricular septal defect (P<0.05). Hypertension was related to preoperative left ventricular hypertrophy, Eai, Eesi and NT-ProBNP. Eai was an independent risk factor for hypertension.ConclusionPreoperative ventricular-arterial stiffening is increased in infants with aortic coarctation, which is related to the occurrence of hypertension. Isolated CoA shows more significant increase in ventricular-arterial stiffening, higher ventricular overload and incidence of hypertension.

  • Application of biomechanical modeling and simulation in the development of non-invasive technologies and devices for cardiovascular testing

    The prevalence of cardiovascular disease in our country is increasing, and it has been a big problem affecting the social and economic development. It has been demonstrated that early intervention of cardiovascular risk factors can effectively reduce cardiovascular disease-caused mortality. Therefore, extensive implementation of cardiovascular testing and risk factor screening in the general population is the key to the prevention and treatment of cardiovascular disease. However, the categories of devices available for quick cardiovascular testing are limited, and in particular, many existing devices suffer from various technical problems, such as complex operation, unclear working principle, or large inter-individual variability in measurement accuracy, which lead to an overall low popularity and reliability of cardiovascular testing. In this study, we introduce the non-invasive measurement mechanisms and relevant technical progresses for several typical cardiovascular indices (e.g., peripheral/central arterial blood pressure, and arterial stiffness), with emphasis on describing the applications of biomechanical modeling and simulation in mechanism verification, analysis of influential factors, and technical improvement/innovation.
