  • Healthcare Alliance through Vertically Integrating Resources: An Endeavor in Shanghai, China

    It is one of the priorities of the new round of healthcare reform to develop regional healthcare alliances through vertically integrating resources. This paper reviews the framework and characteristics of Shanghai’s healthcare system, and then analyzes its strengths and weaknesses, underscoring Shanghai’s exploration in the vertical integration of medical resources as a response to the pressing issues in healthcare. The paper outlines the main tasks and groundwork of the Shanghai healthcare alliance initiative and gives an outlook on the expected outcomes. It ends with some thoughts on the problems and challenges confronting regional healthcare alliances in Shanghai.

The practice of lead-type close medical consortium in West China Hospital of Sichuan University is an innovative exploration in the context of the new national medical reform. Combining the social responsibilities, location characteristics, functional orientation of West China Hospital of Sichuan University, and the remarkable results of the lead-type close medical consortium, this article elaborats three distinctive features of the lead-type close medical consortium, namely the cooperation between the government and the hospitals as the cornerstone, the hierarchical collaboration as the core, and the medical care and great health as the guiding principle, to provide a reference for the construction of a high-quality and efficient medical and health service system in China.

  • Analysis of the economic benefit and business levels of community hospitals in the medical consortium in a tertiary grade A hospital

    ObjectivesTo explore the impact of the medical consortium of a tertiary grade A hospital on the economic benefits and business levels of the community hospitals, to summarize the problems of the current medical joint mode, and to provide suggestions for promoting the medical association model.MethodsWe collected the data of economic benefit and business level of a tertiary grade A hospital and its affiliated community hospitals before and after the implementation of the medical consortium model [2016 (pre-implementation) and 2017 (post-implementation)], and analyzed the differences between the two groups. Patients were selected by stratified sampling method before and after the implementation, investigations and follow-up were conducted. The satisfaction and compliance were analyzed.ResultsCompared with the data in 2016, in 2017, the total income of community hospitals increased by 25.13% (2.671 2 million yuan vs. 2.134 7 million yuan), the number of outpatients increased by 15.55% (7 496 vs. 6 487 cases), the number of hospitalizations increased by 59.49% (1 673 vs. 1 049 cases), and the number of students increased the most, reaching 366.67% (14 vs. 3). Finally, a total of 472 patients were included, including 213 in pre-implementation group and 259 in post-implementation group. There was no significant difference in the general data between the two groups (P>0.05). The patients’ satisfaction and compliance in post-implementation group were better than those in pre-implementation group (P<0.05). In June and December 2017, the hospital’s medical staff business test level increased significantly compared with those in the same period in 2016 (t=−7.467、−9.998, P<0.001), and the results in December 2017 were significantly higher than those in June 2017 (t=−4.719, P<0.001). But there was no significant difference in the performance of business level assessment between the results in June 2016 and in December 2016 (t=−1.158, P=0.250).ConclusionAn effective “Three Construction and One Management” model can promote the advancement of medical standards in community hospitals while promoting the effective improvement of economic benefits and improving the patients’ satisfaction.

  • Innovative application and effects of lead-type close medical consortium under the epidemic situation of coronavirus disease 2019

    ObjectiveTo explore the innovative application and effect of lead-type close medical consortium under the epidemic situation of coronavirus disease 2019 (COVID-19).MethodsDuring the epidemic of COVID-19, Jintang First People’s Hospital implemented a series of innovative countermeasures under the guidance of West China Hospital of Sichuan University, the leading hospital of a lead-type close medical consortium. To verify the implementation effect of the countermeasures, the patient satisfaction questionnaire and medical personnel satisfaction questionnaire were administered in confirmed or suspected COVID-19 patients and anti-epidemic healthcare workers. The relevant health economic indicators were extracted through the hospital information system for descriptive analysis.ResultsA total of 16 patients were included, including 10 confirmed patients and 6 suspected patients. The median score of patient satisfaction was 66. All patients were cured and discharged. A total of 56 healthcare workers were included, including 18 doctors and 38 nurses, with a median satisfaction score of 81. The average length of hospital stay of patients was 11.00 d, the average hospitalization cost was 5 117.35 yuan; the average drug cost was 1 099.95 yuan, accounted for 21.49%; the average material cost was 38.63 yuan, accounted for 0.75%.ConclusionsThe innovative application of the lead-type close medical consortium in the treatment of patients with COVID-19 plays an important role in the prevention and control of epidemic, and has achieved remarkable results in patients treatment, patients satisfaction, and hospital management. It is worthy of being widely popularized.
