ObjectiveTo evaluate the efficacy of XiaochengqiMixture (XM) on promoting healing of colonic stoma. MethodsForty Wistar rats were divided into two groups randomly after colonectomy: experimental group (n=20) and control group (n=20). In early postoperatively stage rats were given gastric administration of XM in the experimental group and pure water in the control group. On day 3, 7, and 14 after establishment of animal models, laparotomy was performed in two groups of rats, respectively. Anastomotic stoma and surrounding tissues were harvested to detect the context of hydroxyproline and collagen fiber proportion by Masson dying. ResultsOn day 3 after establishment of animal models, hyperplastic collagen with small fiber was observed while no fasciculus was found. Hydroxyproline context and collagen fiber proportion of rats were higher in experimental group than those in control group (Plt;0.05). On day 7 after operation, many fasciculuses were found in two groups of rats, hydroxyproline context and collagen fiber proportion of rats were higher in experimental group than those in control group (Plt;0.01). On day 14 after operation, fasciculuses became bigger and more regular in arrangement, but there was no significant difference between the two groups (Pgt;0.05). ConclusionXM is capable of promoting healing of colonic stoma and might prevent the occurrence of anastomotic fistula.
目的 探讨应用双吻合器低位直肠癌前切除术后吻合口漏的早期诊断和防治措施。方法 回顾性分析 2005~2011年期间笔者所在医院收治的160例应用双吻合器行低位直肠癌前切除患者的临床资料。结果 本组患者术后发生吻合口漏13例(8.1%),发生吻合口漏的时间为术后 3~12d,平均7d;8例经保守治疗后治愈,4 例经二次手术行结肠或回盲部造瘘后好转,1 例于术后12d死亡。结论 直肠癌前切除术后吻合口漏的早期正确诊断和合理治疗是降低患者死亡率的关键;早期的造瘘手术和通畅引流是治愈吻合口漏的必要措施。
目的 探讨低位直肠癌双吻合器保肛手术后吻合口漏的预防。 方法 回顾性分析2000年5月至2005年5月我院肛肠外科行双吻合器保肛手术的78例低位直肠癌患者的资料。结果 所有病例应用双吻合器进行直肠闭合并吻合成功,术后切缘病理检查均未见癌细胞浸润,无吻合口漏及手术死亡。随访73例(93.6%),随访时间9~65个月,平均35个月。盆腔复发2例(2.6%),腹腔广泛转移1例(1.3%),肝脏转移7例(9.0%),吻合口局部复发1例(1.3%,术后11个月再次行Miles术)。结论 双吻合技术为低位直肠癌患者提供更多的保肛机会,使用得当可有效预防吻合口漏的发生。
【Abstract】ObjectiveTo study the positive effect of recombinant human epidermal growth factor (rhEGF) on rabbit intestinal anastomotic wound healing after bowel resection. MethodsFortyeight white rabbits were randomly divided into study group in which rhEGF was injected and spinged in the submucosa and mucosa respectively during intestinal anastomosis after bowel resection, and control group in which only intestinal resection and anastomosis was performed. The leukocyte was counted. The incidence of anastomotic leakage and the synthesis of collagen fibrils and hydroxyproline were observed. ResultsThe leukocyte numbers in the anastomotic tissue in two groups rabbits increased slightly 3 d, 5 d and 7d after intestinal anastomosis, but the difference between study group and control group was insignificant (Pgt;0.05). The incidence of anastomotic leakage in the control group (16.7%) was higher than that of the study group (4.3%). The area of collagen fibrils 3 d, 5 d and 7d after intestinal anastomosis in the study group were significantly more than that in the control group (P<0.05). Number of fibroblast was higher in the study group and the cells appeared bigger nucleus and dense colouration as well as enriched plasm. Angiogenesis in anastomosis tissue in the study group was significant and normal structure was present. Cell structure of anastomosis mucosa was damaged in the control group. Synthesis of hydroxyproline in anastomotic tissue 5 d and 7 d after anastomosis in the study group was more than that in the control group (P<0.05).ConclusionInflammation was present in the whole process of wound healing, and local using of EGF had insignificant effect on system inflammation. EGF functions as chemoattractant and increases the recruitment of leukocytes, monocytes and fibroblasts into the wound area. EGF increases the production of collagen, angiogenesis and the synthesis of hydroxyproline. So EGF could promote wound healing and protect from anastomosis leakage in this study.
ObjectiveTo investigate the cause of stoma recurrence after anterior resection of rectal cancer and discover the methods of prevention and treatment.MethodsA total 91 patients with stoma recurrence after anterior resection of rectal cancer (or Dixon) were analysed retrospectively between 1985 and 1996. Fourtyseven patients experienced reradical resection (Miles), 27 cases palliative resection, and 11 cases only exploration. Thirtytwo cases had been followed up for 5 years and obtained 1,3,5year survival rate for reradical radical resection (Miles). Diagnosis and treatment of stomal recurrence after Dixon were evaluated. ResultsOne, three and fiveyear survival rate of reradical resection (Miles) was 93%,77%,45% respectively.ConclusionTo amplify blindly the adaptation of Dixon is to raise the rate of stoma recurrence. Digital rectal examination and fiberopic colonoscopy (and biopsy) are very essential methods for the diagnosis of stoma recurrence, and we strive to do reradical resection (Miles) for the patients with stoma recurrence after Dixon’s operation.
目的对单层吻合在消化道手术中临床应用的安全性和实用性进行评价。方法对该院1 600例消化道单层吻合术的临床资料进行回顾性分析,并结合国内外文献进行讨论。结果全组1 600例消化道单层吻合手术均顺利完成,发生吻合口漏25例(1.56%),吻合口狭窄3例(0.19%),无一例发生吻合口大出血。结论消化道单层吻合不会增加吻合口漏的发生率,并能减少吻合口狭窄、梗阻和出血,是安全、实用且有效的吻合方法。
In this study, the effect of neostigmine on the healing of colonic anastomoses has been investigated following onestage resection and anastomosis for complete leftsided colomic obstruction.It was found that neostingmine promoted colonic anastomotic healing either experimentally or clinically. Further, the authors discuss the pathogenesis of anastomotic leakage but suggest that neostigmine should be used in the first 5 hours after operation.
In a series of experiments on rats,we have observed that the tensile strength and hydroxyproline content of left colonic anastomosis presented the lowest level on the 3rd and 4th postoperative days. Aprotinin,dimethyl sulfoxide,superoxide dismutase and vitamin A could improve the early coures of healing of colonic anastomosis;hydroxycortisone could retard the early course of healing of anastomosis and vitamin A antaonize the deleterious effect as mentioned above. Perioperative chemotherapy did not impair the early coures of healing of colonic anastomosis. The primary rasection and anastomosis of left colon for complete obstruction could be safety done if colonic content was decompressed before performing anastomosis and abdominal cavity was irrigated with antibiotic solutions afterwards. The mechanism of some factors influencing the healing of colonic anastomosis is discussed.
Objective To investigate the use of intraoperative transit time flow measurement (TTFM) to accuratelyevaluate graft patency during sequential coronary artery bypass grafting (CABG). Methods Clinical data of 131 patientsundergoing sequential off-pump coronary artery bypass grafting (OPCAB) with the great saphenous vein (with or without internal mammary artery) as graft vessels in Beijing Anzhen Hospital from April 2012 to January 2013 were retrospectivelyanalyzed. There were 92 male and 39 female patients with their age of 61.35±8.24 years. During the operation,2 methods were used to measure mean blood flow volume,pulsatility index (PI) and diastolic filling (DF) of the graft vessels. For thenon-blocking method,blood flow in graft vessels was maintained,and TTFM was applied 2 cm proximal to the anastomoticsite in graft vessels to record above parameters. For the blocking method,blood flow in graft vessels was temporally blockedby clipping distal graft vessels with an atraumatic bulldog clamp,and TTFM was applied 2 cm proximal to the anastomotic site in graft vessels to record above parameters. Results Blood flow volumes of the diagonal branch (27.43±15.22 ml/minvs. 59.28±30.13 ml/min),obtuse marginal branch (26.14±19.74 ml/min vs. 47.19±24.27 ml/min) and posterior left ventr-icular branch (19.16±8.92 ml/min vs. 38.83±20.11 ml/min) measured by the blocking method were significantly smallerthan those measured by the non-blocking method (P<0.05) . PI values of the diagonal branch (2.93±1.30 vs. 2.31±0.91),obtuse marginal branch (2.62±1.17 vs. 2.01±0.87) and posterior left ventricular branch (2.33±0.92 vs. 1.80±0.73) meas-ured by the blocking method were significantly higher than those measured by the non-blocking method (P<0.05). There was no statistical difference in DF values measured by 2 methods at respective anastomotic sites,and all DF values were higher than 50%. Non-blocking method identified 1 anastomotic site and blocking method identified 3 anastomotic sites that were not patent,and these anastomotic sites became patent after graft reconstruction. Conclusion Blood flow velocity of sequential grafts is higher than that of single grafts,indicating that sequential grafts have the advantages of high blood flow volume and low risk of thrombosis. Blocking method can more accurately evaluate graft patency at the anastomotic sites and sensitively identify graft vessels that are not patent,which is helpful for anastomotic site reconstruction in time and enhancement of successful rate of CABG.
Objective To evaluate preventive effectiveness of pocket esophagogastric anastomosis for postoperativeanastomotic leak,stricture and gastroesophageal reflux disease (GERD),and investigate clinical significance of Montreal definition and classification of GERD after esophageal reconstruction. Methods Clinical data of 1 078 patients whoreceived 2 different surgical procedures for resection of esophageal or cardiac carcinoma from June 2007 to June 2011 in our hospital were retrospectively analyzed. In the experimental group,there were 582 patients who received pocketesophagogastric anastomosis,including 403 male and 179 female patients with their age of 60.4±12.6 years. There were 399 patients with esophageal carcinoma and 183 patients with cardiac carcinoma,392 patients receiving esophagogastrostomyabove the aortic arch and 190 patients receiving esophagogastrostomy below the aortic arch respectively. In the control group,there were 496 patients who received conventional end-to-side esophagogastric anastomosis,including 343 male and 153 female patients with their age of 59.2±12.8 years. There were 322 patients with esophageal carcinoma and 174 patients with cardiac carcinoma,317 patients receiving esophagogastrostomy above the aortic arch and 179 patients receivingesophagogastrostomy below the aortic arch respectively. A survey questionnaire was made on the basis of relevant diagnosticstandards to investigate the incidence of postoperative anastomotic stricture and GERD of the 2 groups during follow-up.Results The incidence of postoperative anastomotic leak of the experimental group was significantly lower than that of the control group [0% (0/582)versus 1.0% (5/496),χ2=5.835,P=0.016]. Patients in the experimental group had less severeGERD symptoms,and the percentage of patients who needed antacid therapy for extraesophageal symptoms of GERD ofthe experimental group was significantly lower than that of the control group [1.6% (33/541) versus 12.6% (57/453),χ2=23.564,P=0.000]. The incidence of anastomotic stricture of the experimental group was significantly lower than that of thecontrol group [0.9% (5/539) versus 7.3% (34/465),χ2=25.124,P=0.000],and especially,the incidence of severe anastomoticstricture of the experimental group was significantly lower than that of the control group [0% (0/539) versus 4.7% (22/465),χ2=24.883,P=0.000]. There was no statistical difference in five-year survival rate. Conclusion Pocket esophagogastric anastomosis is better than conventional end-to-side esophagogastric anastomosis for the prevention of postoperative anastomoticleak,stricture and GERD. Montreal definition and classification of GERD is suitable for the diagnosis of postoperativeGERD after esophageal reconstruction.
