    肿瘤干细胞学说是近年来快速发展和备受关注的一个理论,根据该学说,具有自我更新、多向分化、无限生长和很强成瘤能力的癌干细胞可能是肿瘤发生、发展、侵袭和复发转移的根源。当前,已有多种类型的癌干细胞被证实或探讨,然而胃癌干细胞的研究尚存在困难,相关报道很少。现就国内外近年来关于癌干细胞和胃癌干细胞的相关研究进展进行综述,探讨胃癌干细胞的培养、分离、确认鉴定及其复杂的生物学特征研究中尚面临的一些亟待解决的问题。

  • Clinical Effect of Preoperative Administration of Enteral Nutrition Enriched ω-3 Fatty Acids for Gastric Cancer

    ObjectiveTo evaluate the safety and efficacy of preoperation administration of enteral nutrition enriched ω-3 fatty acids for gastric cancer patients. MethodsA single center randomized controlled clinical trial was performed in 60 cases of gastric cancer in West China Hospital during January 2014 to June 2014, and cases were equally randomized divided into treatment group and control group. Cases of treatment group were given enteral nutrition enriched ω-3 fatty acids which was manufactured by Fresenius Kabi Deutschland GmbH for 5 consecutive days before operation, and cases of control group were given an isocaloric and isonitrogenous homogenized diet for 5 consecutive days before operation. The laboratory indexes of nutritional status and imflammatory factors were observed and compared between 2 groups on admission, preoperative day 1, postoperative day 3, and postoperative day 5. Liver and kidney function indexes which as the safety indexes were detected on admission and preoperative day 1. Vomiting, diarrhea, and infectious complications were recorded in addition. ResultsOn 3 days after operation, levels of interleukin-6 (IL-6) and α-acid glycoprotein (AAG) of treatment group were both lower than those of control group (P<0.05); on 5 days after operation, levels of C-reactive protein (CRP) of treatment group was lower than that of control group too (P<0.05); but at other time points, there were no significant differences in any index between the 2 groups (P>0.05). During the period of enteral nutrition, only 1 case suffered from bloating and 1 case suffered from diarrhea, both in treatment group, and the incidence of adverse reactions didn't differed between treatment group[6.7% (2/30)]and control group[0 (0/30)], P>0.05. Moreover, there were no significant differences between treatment group and control group in incidences of wound infection[3.3% (1/30) vs. 10.0% (3/30)], abdominal infection[0 (0/30) vs. 3.3% (1/30)], urinary infection[0 (0/30) vs. 3.3% (1/30)], and pulmonary infection[0 (0/30) vs. 6.7% (2/30)], but the total incidence of complication was lower in treatment group than that of control group[3.3% (1/30) vs. 23.3% (7/30)], P=0.026. ConclusionEnteral nutrition enriched ω-3 fatty acids can reduce the rate of infection-related complication for patients with gastric cancer, and has a sense of safety.

  • Expression of miR21 in Rectal Cancer of Different Tumor Stages

    ObjectiveTo quantitate expression of miR21 in rectal cancer of different tumor stages and discuss their significances. MethodThe expression of miR21 was detected and quantitated in the rectal cancer tissues and corresponding adjacent normal tissues of 40 patients with rectal cancer in this hospital from August 2012 to October 2012 by Taq Man microRNA assay. ResultsThe significant overexpression of miR21 was observed in the rectal cancer tissues (4.122±1.973 versus 1.825±0.661, P=0.000)as compared with the corresponding adjacent normal tissues. The expressions of miR21 in the rectal cancer tissues of N1-N2 and Dukes C-D stages were significantly higher than those in the rectal cancer tissues of N0(4.852±2.344 versus 3.391±1.171, P=0.019)and Dukes A-B stage(4.787±2.304 versus 3.386±1.203, P=0.021). From N0 to N2 stage, the expression of miR21 increased progressively in the rectal cancer, and the expression in the rectal cancer tissues of N2 stage was significantly higher than that in the N0 stage(5.556±1.500 versus 3.391±1.171, P=0.010). And receiver operating characteristics curve analysis showed that miR21 could discriminate N 0 stage from N1-N2 stage with a 0.698 area under curve(AUC), 50.0% sensitivity and 90.0% specificity, Dukes C-D stage from A-B stage with a 0.689 AUC, 42.9% sensitivity and 94.7% specificity. ConclusionmiR21 appears to have a potential correlation with N and Dukes stages of rectal cancer, which cautiously and optimistically suggests that it could be a potential novel biomarker for predicting tumor stage preoperatively.

  • Effects of early enteral nutrition containing ω-3 polyunsaturated fatty acids combined withintravenous infusion of alanyl-glutamine on inflammatory response and immune function of postoperative gastric cancer patients

    ObjectiveTo investigate the effects of early enteral nutrition containing ω-3 polyunsaturated fatty acids combined with intravenous infusion of alanyl-glutamine on inflammatory response and immune function of postoperative gastric cancer patients.MethodsA total of 110 patients, accepting radical operation for gastric cancer in West China Hospital of Sichuan University during October 2017 to December 2018, were prospectively incorporated in the study and were randomly divided into 2 groups equally. Patients in the control group were enterally fed with a formula containing ω-3 polyunsaturated fatty acids for 6 consecutive days after surgery. Patients in the experimental group accepted the same enteral feeding but combined with intravenous infusion of alanyl-glutamine (20 g/d). Both enteral feeding and intravenous infusion started within 24 hours after surgery. Peripheral venous blood was gathered within 3 days before surgery and on the morning of the first, third, and seventh postoperative days to detect inflammatory, immunological, and nutritional indexes. Complications, length of hospital stay, and hospital cost were also taken notes.ResultsFifty-two patients in the control group and fifty-two patients in the experimental group respectively finished the study. In both groups, 3 patients withdrew from the study for inadequacy of radical operation. Neutrophilic granulocyte percentage, interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α) on the third postoperative day, C-reactive protein (CRP), procalcitonin (PCT), IL-6, and TNF-α on the seventh postoperative day, were significantly lower in the experimental group (P<0.05). Immunological indexes including immunoglobulin G (IGG), immunoglobulin A (IGA), percentage of CD3+ T cells, and percentage of CD4+ T cells, nutritional markers including total protein (TP), plasma albumin (ALB), and prealbumin (PAB) were significantly higher in the experimental group on the seventh postoperative day (P<0.05). When the study ended, none significant differences of the rates of both infectious complications (wound infection, intra-abdominal infection, pulmonary infection, urinary system infection, blood system infection, and anastomotic fistula) and noninfectious complications (diarrhea, abdominal distension, and abdominal pain) were observed between the two groups (P>0.05). Time of the first anal discharge, length of hospital stay, and hospitalization cost between the two groups were not significantly different neither (P>0.05).ConclusionEarly enteral nutrition containing ω-3 polyunsaturated fatty acids combined with intravenous infusion of alanyl-glutamine contributes to reduce inflammatory response and improve immune function and nutrition status of patients with gastric cancer after surgery.
