  • 同期双侧单孔胸腔镜切除肺多发磨玻璃影的单中心经验

    目的总结双肺多发磨玻璃影(ground-glass opacity,GGO)患者同期行双侧单孔胸腔镜手术切除的经验。方法回顾性分析 2015 年 5 月至 2019 年 10 月同期行双侧单孔胸腔镜肺 GGO 切除 34 例患者的临床资料,其中男 6 例、女 28 例,平均年龄 41~69(57.9±6.7)岁。结果术中平均出血量(120.9±67.7)mL,平均手术时间(140.0±74.8)min,术后平均胸腔引流时间(4.8±3.1)d,术后平均住院时间(7.2±4.3)d。术后并发症包括肺部感染 2 例,心房颤动 3 例,肺持续漏气>3 d 5 例,经治疗后均好转,无围手术期严重并发症及死亡病例。共切除 GGO 病灶 76 个,总恶性率为 81.6%,其中纯 GGO 40 个,恶性 28 个(70.0%),平均直径(9.6±3.8)mm;混合 GGO 36 个,恶性 34 个(94.4%),平均直径(15.6±6.6)mm。平均随访时间 38.4 个月,未发现术后转移及复发。结论双肺多发 GGO 患者的病灶为恶性可能性大,在肺功能允许时可考虑同期双侧单孔胸腔镜多病灶切除,根据病灶位置、大小及术中快速病理结果可灵活采取亚肺叶或肺叶切除方法。双侧同期手术安全可行,不会增加术后并发症风险,短期预后良好。

  • Short-term efficacy of surgical treatment for multiple pulmonary nodules: A retrospective study in a single center

    ObjectiveTo summarize and analyze the clinical diagnosis, surgical treatment and prognosis of multiple pulmonary nodules (MPNs). Methods The clinical data of lung cancer patients who received surgical treatment in our hospital from 2018 to 2020 were collected. The short-term efficacy of surgical treatment for MPNs was analyzed. Results A total of 97 patients were enrolled, including 30 males and 67 females with an average age of 56.1±10.0 years at onset ill. There were 62 patients with double lesions, 22 patients with three lesions, 4 patients with four lesions, and 9 patients with more than four lesions. A total of 213 lesions were surgically treated, including 88 pure ground-glass nodules, 81 partially solid nodules, and 7 solid nodules. There were 87 simultaneous surgeries and 10 staged surgeries, with an average operation interval of 5.2 months. The pathological combination type included adenocarcinoma-adenocarcinoma in 96 (99.0%) patients, squamous cell carcinoma-squamous cell carcinoma in 1 (1.0%) patient, and no lymph node metastasis was found. The 2-year disease-free survival (DFS) rate was 92.1%, and the overall survival (OS) rate was 100.0%. Univariate analysis showed that high-risk lesion size>2 cm (P=0.316), residual lesions (P=0.782) and pathological combination type (P=0.913) had statistical effect on the 2-year DFS rate. Conclusion MPNs are mainly diagnosed with multiple primary lung cancers, and the pathological combination is mostly adenocarcinoma-adenocarcinoma combination. Imaging examination is of great help to the surgical approach selection, diagnosis and differential diagnosis of MPNs. During the operation, maximal preservation of lung function and complete resection of high-risk nodules should be taken as the principle, and the prognosis is satisfactory.

  • Clinical application of basic anesthesia combined with local anesthesia in preoperative localization of multiple pulmonary nodules: A retrospective cohort study

    Objective To evaluate the safety and efficacy of basic anesthesia combined with local anesthesia in the preoperative localization of multiple pulmonary nodules. Methods The clinical data of 200 patients who underwent preoperative localization for multiple pulmonary nodules resection under single-port thoracoscopy in Nanjing Brain Hospital from July 2023 to September 2023 were extracted. They were divided into a group A and a group B, according to the localization method. The group A consisted of 100 patients who were localized under local anesthesia, and the group B consisted of 100 patients who were localized with basic anesthesia combined with local anesthesia. The basic clinical characteristics, localization success rate, incidence of localization complications, localization time, and pain score of the two groups were compared and analyzed. Results The incidence of localization complications (4% vs. 13%), localization time [(19.9±8.66) min vs. (15.23± 5.98) min], and pain score (2.01±2.09 vs. 3.29± 2.54, P≤0.05) in the group B were significantly lower than those in the group A, and the differences were statistically significant. The localization success rate of the group B was significantly higher than that of the group A (98% vs. 92%, P≤0.05), and the difference was statistically significant. Conclusion Basic anesthesia combined with local anesthesia for preoperative localization of multiple pulmonary nodules under mobile CT has the advantages of short localization time, fewer localization complications, high safety, high success rate, and high patient comfort, and has certain clinical promotion value.
