  • Causal Analysis of HIV Risk Behavior among Male Who Have Sex with Male in a Community

    Objective To explore the key influencing factors of HIV risk behavior among male who have sex with male (MSM). Methods 36 MSM subjects in a community were recruited for HIV risk behavior characteristics, social environment and the attitude of exposure of high risk sexual intercourse, using behavior scales and qualitative research methods. The collected data were orderly input and analyzed using Nvivo 8.0 software. Then, after three-level transcription, the data were further summarized and extracted based on the method of the grouding theory. Results The HIV Risk Assessment Questionnaire score of 36 subjects was 8.08±2.46, of whom, 72% scored at a medium level (5 to 10 scores) and 19% scored at a high level (more than 10 scores). The social support rating scale (SSRS) score was 32.38±5.99 in MSM population, lower than in undergraduates and floating population. The results of qualitative analysis showed that, after open coding, 11 key message and 4 categories contributed to HIV risk in MSM populaiton, including: a) low levels of fear for AIDS; b) male role and uncertain sexual orientation; c) low degree social support; and d) poor availability of condom in the setting of sexual intercourse. Conclusion The interventions against AIDS/HIV for MSM need to be further studied. Besides, we should strengthen the community intervention mode based on fear for AIDS, social support, and condom distribution methods

  • Factors Influencing Patients’ Compliance with Hypertension Management in the Community: Qualitative Research Synthesis

    Objective To collect and analyze the qualitative studies on patients’ compliance with hypertension management in the community, and to summarize the factors influencing the compliance from the view of patients. Methods Eleven electronic databases and search strategies and words were determined through discussion by experts and the review group. After the selection and critical appraisal of the retrieved studies were performed by two reviewers independently, meta-summary was employed for the results of qualitative studies. Results Five studies were included. The results showed some factors had great impact on the compliance of patients, including patients’ knowledge of hypertension and its treatment, doctor-patient relationship, patients’ financial status, social support, designing of treatment plans, and patients’ individual conditions. Conclusion To improve doctors’ technical competence, building good doctor-patient relationship, strengthening patients’ individualized health education, formulating the proper treatment plan, and making good use of the social support system would promote the compliance with hypertension management in the community.

  • Expansion of PICO Model in Evidence-based Medicine and Its Application in Qualitative Research

    The PICO model is a general model in building problems of evidence-based medicine (EBM). However, with the deepening and refinement of the medical research and the rising of qualitative research, the PICO model could not satisfy all problems. This article introduces the expansions of the PICO model and puts forward the SPIDER model according to the qualitative problem and its relationship with the PICO model, which can build the best search strategy of clinical problems in a short time.

  • How to Write the Qualitative Systematic Review: An Introduction

    Evidence-based medicine (EBM) emphasizes the combination of the physician's experience, the best research evidence and patient's values to ensure the best prevention and treatment effect. Evidence is the core of the EBM, and quantitative systematic review can synthesize interventional studies; however, it can't provide synthetic evidence for patients' demands, opinions and attitudes and so on. Qualitative systematic review can offset these limitations. With the development of methodology of qualitative research and systematic review of qualitative studies, the number of qualitative studies increased year by year and they provided more and more evidence for decision making in public health, social work, management and education. The international research institutions including the Cochrane Collaboration, the Campbell Collaboration and the 3ie have mature methods for qualitative systematic review; however, few studies introduced how to write it in China. Therefore, this paper briefly introduces how to write the qualitative systematic review.

  • 医疗人员对心因性非痫性发作的观点研究:基于定性与定量研究的系统回顾

    近期一篇关于心因性非痫性发作(Psychogenic nonepileptic seizures,PNES)的系统分析表明,该类患者总是会有不愉快的就医经历,因此详细了解其中相互关系至关重要。文章旨在探究医疗人员对 PNES 的态度与看法。系统性检索截至到 2017 年 11 月前发表在 3 个主要数据库的文章(Web of Science、PubMed、CINAHL),并回顾 1997 年以后发表的定性或定量的方法学研究。基于扎根理论来解释数据,使用 Mixed Methods Appraisal Tool 进行质量评价。总共 30 篇独立文章,其中包含了至少 3 900 名专业人员的观点。在分析中出现了 5 个解释性概念:① 不确定性,医疗人员表示他们对 PNES 的许多方面都感到不确定,包括诊断和治疗;② 理解,医疗人员对 PNES 的理解是基于二元论,他们认为 PNES 与心理精神因素的关系极大;③ 挫折感与挑战性,对 PNES 患者的诊断治疗被认为是有挑战性的,并令人感到沮丧的;④ 责任,医疗人员对于“谁应该为 PNES 患者的治疗负责”是有争论的;⑤ 预后与残疾,与癫痫相比,医疗人员普遍认为 PNES 症状相对较轻,致残率也没有癫痫高,并且与自身的自制力有关。尽管有一些医师对 PNES 有较深刻的理解,但是仍有一部分医师的观点需要更新和提高。有一部分研究在调查中是直接询问医师“对 PNES 疾病的观点”,这样的研究存在局限性。此外,有一些学科专业的医师并没有被研究调查(如精神健康专业医师)。研究还表明,对医师们进行 PNES 额外教学训练是有必要的,但同时也需要改变医师们对 PNES 的态度。

  • Technical essentials for collecting patient experience data in patient-focused drug development

    As subjects in drug clinical trials and participants in medical practice, patients can best understand their own conditions and needs. With this in mind, the FDA proposed "patient-centered drug discovery" and issued a set of guidelines to incorporate patient experiences, perspectives, needs, and preferences into the drug development and evaluation process. Guideline (2), methods for identifying important patient information, mainly describes methods and precautions for collecting and extracting patient experience data. This paper will focus on the characteristics, common methods and precautions of qualitative, quantitative and mixed research methods in the collection of patient experience data, in order to provide help for the comprehensive collection of patient experience data.
