  • Outcome of Left Ventricular Outlet Tract and Aortic Valve Function after Arterial Switch Operation for Patients with Transposition of the Great Arteries and Left Ventricular Outlet Tract Obstruction

    ObjectiveTo assess the function of left ventricular outlet tract and aortic valve after arterial switch operation (ASO) for patients with transposition of the great arteries (TGA) and left ventricular outlet tract obstruction (LVOTO). MethodsFrom 2002 to 2013, 549 pediatric TGA patients received ASO in Fu Wai Hospital. Among them, 42 patients had LVOTO, including 31 males and 11 females with their median age of 12 months (range, 7 days to 96 months), median body weight of 6.5(3.5-26.0) kg and percutaneous oxygen saturation of 52%-85%. LVOTO anomalies included pulmonary valve stenosis, subaortic membrane, tunnel-like subaortic stenosis, muscular subaortic stenosis, subvalvular apparatus and combined anomalies. Different surgical procedures were performed according to respective anomalies. Echocardiographic characteristics, intraoperative findings, surgical methods, early and follow-up results were summarized. ResultsCardiopulmonary bypass time was 147-344 (193.5±73.1) minutes, mean aortic cross-clamping time was 139(109-305) minutes, mean mechanical ventilation time was 36(3-960) hours, and mean length of ICU stay was 5(1-48) days. Three patients received and later successfully weaned from extracorporeal membrane oxygenation. Two patients died postoperatively including 1 patient with multiple organ dysfunction syndrome and another patient with severe infection. One patient died during follow-up for unknown reason, and 3 patients were lost during followup. Thirty-six patients were followed up for 24 (3-116) months. During follow-up, there were 1 patient with LVOTO recurrence, 1 patient with new-onset mild aortic valve stenosis, 11 patients with new-onset mild aortic regurgitation (AR), and 2 patients with new-onset moderate AR. Median systolic left ventricular-aortic pressure gradient[4 (2-49) mm Hg] was significantly lower than preoperative value[37.2 (12.1-70.6) mm Hg] (Z=-5.153). Cardiac event-free rate was 91%±5% at 1 year and 78%±8% at 5 years after discharge. ConclusionFor TGA patients with LVOTO, ASO can produce satisfactory mid-and long-term results if proper surgical indications and strategies are chosen according to different severity of LVOTO which can be evaluated by anatomic features of TGA and systolic left ventricular-aortic pressure gradient.

  • Surgical Strategies for Hypertrophic Obstructive Cardiomyopathy (HOCM) with Severe Mitral Valve Regurgitation

    Objective To summarize the surgical treatment strategies and the clinical outcomes of hypertrophic obstructive cardiomyopathy (HOCM) with severe mitral regurgitation. Method We retrospectively analyzed the clinical data of 23 patients of HOCM with severe mitral regurgitation in our hospital from January 2004 through January 2014 year. There were 14 males and 9 females, aged from 15-71(50.2±15.4) years. The preoperative left ventricular outflow tract gradient (LVOTPG) of these patients was 75-161(98.1±19.3) mm Hg. And the septal thickness was 25.8±2.8 mm. All 23 patients had at least moderate mitral regurgitation and systolic anterior motion (SAM). All of them had extend septal myectomy (extend Marrow procedure) and mitral valve repair(MVP),while 4 patients with atrial fibrillation had left atrial ablation and left atrial appendage operation. Results All patients were successfully operated. The left ventricular outflow tract pressure gradient was 16-39(26.9±4.9) mm Hg when the cardiopulmonary bypass stopped and SAM phenomenon was completely eliminated. Except for 2 mitral valve patients with trace amounts of regurgitation, 1 patient with mild regurgitation, the other 20 patients of mitral regurgitation were completely corrected. All patients survived after operation and only 1 patient suffered from transient complete atrioventricular block and then back to normal sinus rhythm. A long-term follow-up from 6 months to 126 months with an average of 53.1±34.9 months showed no late postoperative death. No mitral regurgitation need reoperation. Two patients had mild reflux. Four patients were of trace reflux. The left ventricular outflow tract the maximum pressure gradient was less than 42 mm Hg. The thickness of interventricular septum dropped from preoperative 25.8±2.8 mm to postoperative 14.1±1.3 mm (P<0.001) . No recurrence was noted in the 3 patients with atrial fibrillation. And one patient still had paroxysmal atrial fibrillation. Long term follow-up of the patients' symptoms disappeared or with only mild symptoms. And quality of their life improved significantly. And there was no long-term complication, reoperation, or death. Conclusions The extensive septal myectomy can completely dredge left ventricular outflow tract stenosis and eliminate SAM phenomenon. The mitral valve repair can correct mitral regurgitation. The comprehensive surgical treatment strategy can achieve a good long-term therapeutic effect.

  • 先天性左心室流出道梗阻的手术治疗

    目的分析我们中心近年来先天性左心室流出道梗阻患儿的外科治疗经验,总结手术方式及治疗效果。方法回顾性分析 2011 年 11 月至 2017 年 12 月我院心血管外科共收治先天性左心室流出道梗阻患儿 58 例的临床资料。男 37 例、女 21 例,年龄 2~16 个月(8.49 ± 3.21)岁,体重 2.70~56.00(31.00±6.72)kg。所有患儿均在全身麻醉低温体外循环下行外科矫治术。结果所有患儿顺利出院,随访 6~72 个月。患儿术后心功能良好,行主动脉瓣置换的患儿人工瓣功能正常,无瓣周漏、传导障碍及明显的出凝血并发症。结论先天性左心室流出道梗阻的外科矫治效果良好。选择合适的手术方式、完全有效的解除流出道梗阻及坚持长期随访等,往往能够取得满意的外科治疗效果,促进患儿术后康复。

    Left ventricular outflow tract obstruction (LVOTO) in Ebstein's anomaly is a rare complication, and LVOTO related to surgery is rarer. We present a 46 years old female patient who was dignosed with Ebstein's anomaly, then suffered from cardiac arrest because of LVOTO secondary to cone reconstruction in ICU.

  • Mitral valve management in hypertrophic obstructive cardiomyopathy and its controversies

    Hypertrophic obstructive cardiomyopathy (HOCM) is a relatively common hereditary cardiomyopathy, which is featured by asymmetric myocardial hypertrophy and dynamic left ventricular outflow tract (LVOT) obstruction. Other than septal hypertrophy, mitral valve abnormalities are also quite common in HOCM patients, and they also contribute to systolic anterior motion of the mitral leaflets and LVOT obstruction. Septal myectomy is believed as the standard surgical treatment for HOCM, but whether to perform mitral valve procedures at the same time of myectomy is still debatable. In this article, we thoroughly explained the mitral valve abnormalities in HOCM patients and their surgical corrections. Besides, we also explained the controversies over mitral valve procedures based on the current clinical studies.
