  • Early and Midterm Follow-Up Results of Endovascular Repair of Stanford Type B Aortic Dissection:Report of 85 Cases

    Objective To introduce the experience of treating 85 cases of stanford type B aortic dissection by endovascular implantment of stent-grafts,to provide more clinical evidence for endovascular repairment for stanford B type aortic dissections.Methods Imaging examination by computed tomography angiography (CTA) were done to obtain anatomical detail of dissection. Stent-grafts were implanted under the guide of DSA in all cases. The patients were followed-up for 3 months,6 months,12 months,and then annually by CTA examination to observe the complications and morphological changes of the aneurysm.Results The technical success rate was 95.3%(81/85),and the clinical success rate was 91.8%(78/85). Eight patients died after operation in hospital,who were all in acute phase,and no patients of chronic phase died,there had statistic differences (P<0.05). The perioperative complications’ incidence of endovascular repair which happened in patients of acute phase was higher than that patients of chronic phase (38.2% vs.13.3%,P<0.05). Left subclavian artery were covered completely for 13 cases,and partially covered for 22 cases,there was no obvious cerebralvascular insufficiency. There were 41 patients of whom other tears which were not covered during operation exsited at abdominal aortic and one year following-up showed that at those section thromblization of false cavity was not clear. The rest of patients whose abdominal aortic didn’t exist tears hadn’t new tears appeared.Conclusions Regardless in the acute or chronic stage,endovascular therapy is an effective mathod of treating stanford type B dissection. However,implanting stent-grafts have a higher security in chronic period than doing that in acute period. If the right carotid artery,vertebra and Willis ring are smoothly,there is little need to set up a by-pass to left subclavian artery before endovascular repairment. For those patients that other uncovered tears exited,following-up should be a must,and further strategy should be studied and prepared in advance.

  • Clinical Analysis on Perioperative Death of Endovascular Repair for Acute Aortic Dissection

    Objective To analyze the reasons for the perioperative death of endovascular repair of acute aortic dissection (AD). Methods The clinical data of 176 patients with acute AD and received endovascular repair from July 2001 to October 2012 were analyzed retrospectively. Results Among 176 patients with acute AD, 8 patients died during perioperatively, received endovascular repair in 1-5 days after admission (mean 2.4 d), and all of them admitted before 2008. Two cases were type A and 6 cases were type B. All cases with hypertension and 3 cases with pleural effusion. Three cases died on the day of operation, among them 2 cases occurred in 1 h after operation, the other 1 case occurred in 2 h after operation. Four cases died in 2 days after operation and 1 case died in 4 days after operation. Four cases died of rupture of the aortic dissection, 2 cases died of cerebral infarction, 1 case died of multiple organ failure, and 1 case died of gastrointestinal bleeding. Conclusion To avoid performing endovascular repair during the acute phase and improving operation skills may help to avoid the occurrence of perioperative death.

  • Experience of Arteriosclerosis Obliterans of Lower Extremities by Endovascular Treatment

    Objective To explore the clinical experience of endovascular treatment for arteriosclerosis obliterans (ASO) of lower extremities. Methods Endovascular treatment were performed on 22 patients (26 limbs) suffering from ASO which were diagnosed by magnetic resonance angiography (MRA). The clinical efficacy after operation was analyzed. Results Twentytwo lower extremities of 18 patients successfully accepted endovascular treatment with 12 stents planted without major complications. Four cases failed to endovascular treatment and 2 of them converted to bypass surgery. The clinical symptoms of limb ischemia vanished or significantly improved after treatment. The ankle brachial index (ABI) of affected extremities increased from 0.35±0.13 (before operation) to 0.70±0.15 (after operation), Plt;0.01. During the follow-up of 2-18 months, 3 cases suffered re-occlusion of artery of lower extremity, in them one case received drug treatment and 2 cases resolved by percutaneous transluminal angioplasty (PTA) and stenting again. Conclusion Endovascular treatment for ASO of lower extremities has such advantages as minimal invasiveness, reduced complications and repeatability. It may serve as a more promising choice of method to clinical treatment of ASO.

  • Clinical Analysis of Improving Thyroidectomy Via Isthmus

    目的探讨经峡部径路行甲状腺手术的优、缺点。方法回顾性总结分析近8年来我院收治的1 699例患者经峡部径路行甲状腺手术的临床资料。结果该术式术野暴露充分,手术时间平均65 min,术中出血量平均50 ml。术后喉返神经损伤8例(均为单侧),68例术后出现短暂性低钙血症,3例术后出血,其中2例发生于术后2 h,出血量200 ml, 行手术止血; 1例发生于术后1 h,出血量100 ml,给予压迫止血和药物止血后,出血停止,余恢复均好。结论经峡部径路行甲状腺手术,能开阔术野和拓宽手术空间,能立即解除患者颈部紧缩感,保持术野清晰,减少术中出血和喉返神经损伤,减少低钙血症及继发性甲状腺功能低下,避免术后呼吸困难、窒息等并发症的发生。

  • Application of The Femoral Vesseles Catheterization in Extracorporeal Membrane Oxygenation for Salvage Treatment (Report of 47 Cases)

    目的 分析股动静脉插管在体外膜肺氧合(extracorporeal membrane oxygenation,ECMO)抢救治疗中的临床应用。方法 回顾性分析笔者所在医院2002 年6 月至2010年7 月期间因重症心肺功能衰竭而施行股动静脉插管并进行静脉-动脉转流体外膜肺氧合(VA-ECMO)抢救的47例患者的临床资料。结果 本组47例患者均顺利施行ECMO支持,37例患者经过治疗后治愈出院,10例死亡。插管并发症有出血、肢体缺血等,均经相应处理后治愈。结论 ECMO对急性心肺衰竭是理想的支持方法,股动静脉为急救插管的首选通道。

  • Transplantation of Autologous Peripheral Blood Stem Cells for The Treatment of Lower Limb Arterial Ischemic Disease

    Objective  To evaluate the clinical efficacy of t ransplantation of autologous peripheral blood stemcells ( PBSC) for the t reatment of lower limb arterial ischemic disease. Methods  From March 2004 to February2007 , 16 patient s with severe lower limb arterial ischemic disease were t reated with autologous PBSC t ransplantation. Recombinated granulocytecolony stimulating factor ( G2CSF) was used to mobilize the proliferation of bonemarrow stem cells and then the stem cells were released into peripheral blood. Af ter 5 - 6 days , PBSC were collected by CS23000 PLUS blood2cells separator. Such concent rated stem cells fluid was int ramuscularly injected into theischemic areas of the lower limbs. Results  The result s of 3 to 242month following2up after the t ransplantation ofstem cells showed that the resting pains of the affected limb were greatly relieved , and ulcers were healed. The distance and duration of intermittent limping became farther and longer. Conclusion  Transplantation of autologousPBSC would be a novel and effective method for the t reatment of arterial ischemic disease. However , this method isstill at the stage of initial clinical application , so it still need to be further studied.
