Objective To determine the clinical effect of sequence and cross bypass or combined with endovascular procedure for mutilevelarterial occlusive disease of lower extremity so as to investigate the credibletreatment for mutilevel arterial occlusive disease of lower extremity. Methods Between April 2004 and July 2005, 11 patients (14 limbs)underwent sequence and cross bypass, thromboendarterectomy or combined with endovascular procedure. Among of them, 10 cases were male and 1 case was female, aging 62-79 years(mean 70.5 years), and including 8 cases of intermittent claudications (Fontaine stage Ⅱ), 3 cases of rest pain (Fontaine stage Ⅲ) and 1 case of toe ulcerwith gangrene (Fontaine stage Ⅳ). Colour Doppler ultrasonography showed that 14 lower limbswere diagnosed as having multilevel arterial occlusive disease and the preoperation and ankle brachial index(ABI) was 0.36±0.11 . Lower limb digital subtraction angiography(DSA) showed 3 cases of bilateral iliac artery stenosis,extrailiac artery occlusion and bilateral superficial femoral artery occlusions,1 case of right common iliac artery stenosis, extrailiac artery occlusion and bilateral superficial femoral artery occlusions and 8 cases of unilateral extrailiac artery stenosis and superficial femoral artery occlusions. Postoperation tests of DSA,colour Doppler ultrasonography and ABI were done to observe cumulative patency rate after operation. Results The follow-up period was from 3 to 26 months(mean 14.5 months ). All patients survived. The symptoms of intermittent claudication and rest pain disappeard in all patients. ABI was improved by0.89±0.13 after procedure(Plt;0.01). The overall salvage rate of limb was 100%. DSA was performed from 3 to 280 days after operation, the overall primary graft patency rate was 92.86%(13/14).Conclusion Sequence and cross bypass, thromboendarterectomy or combined with endovascular procedure in treatment of mutilevel occlusive disease of lower extremity was effective, less invasive and safe.
目的探讨减少胰十二指肠手术切除的死亡率和并发症的方法。方法总结分析了我院20年来因胰十二指肠切除手术死亡的10例患者,分析其死亡原因。结果发现应激性溃疡出血和胰瘘是手术死亡的主要因素。结论术后如何防止应激性溃疡出血,如何进行良好的胰肠吻合,是减少手术死亡的关键。
ObjectiveTo explore the role of arteriovenous axillary loop graft (AVALG) on chest for establishing hemodialysis access in patients with chronic renal failure. MethodsA retrospective analysis was made on the clinical data of 12 patients with chronic renal failure who underwent an AVALG on chest for hemodialysis access between December 2010 and May 2014. There were 2 males and 10 females with an average age of 65.25 years (range, 46-75 years). The main causes were chronic glomerulonephritis in 6 cases, diabetic nephropathy in 4 cases, and both kidney resection because of urinary tract tumors in 2 cases. The disease duration was 2-12 years (mean, 6 years). The 12 patients all underwent 5-14 times (mean, 7 times) failed prior vascular accesses [arteriovenous fistula (AVF)and arteriovenous graft (AVG)] leading to exhaustion of venous access sites on the upper extremities. ResultsThe AVALG on chest were functionally useful for hemodialysis access, 2-3 times per week, and the blood flow was 250-350 mL/minute; the average time for the first dialysis was 48 days (range, 42-93 days). All patients were followed up 12-54 months (mean, 20.92 months). There was no death during perioperative period. The primary patency rates at 6 and 12 months were 91.7% and 83.3% respectively, and the secondary patency rates at 6 and 12 months were both 100%. After operation, infection (1 case), thrombosis (2 cases), bleeding (2 cases), and swollen (1 case) occurred, which were all cured after corresponding treatment. ConclusionAVALG on chest is a supplementary option for chronic renal failure patients with inadequate upper extremity venous access sites after repeat occlusion.
ObjectiveTo explore the role of chest wall arteriovenous graft (CWAVG) for establishing hemodialysis access in patients with end-stage renal disease.MethodsA retrospective analysis was made on the clinical data of 12 patients with end-stage renal disease who underwent CWAVG for establishing hemodialysis access between January 2014 and June 2015. There were 3 males and 9 females with an average age of 63.6 years (range, 54-82 years). The renal disease causes were chronic glomerulonephritis in 2 cases, hypertensive renal damage in 4 cases, diabetic nephropathy in 1 case, both kidney resection because of urinary tract tumors in 3 cases, and causes unknown in 2 cases. Hemodialysis time ranged from 1 to 144 months, with an average of 38.4 months. The 12 patients all underwent 1-14 times (mean, 4.2 times) anterior pathway failure in CWAVG, all of which were caused by repeated occlusion of dialysis pathway or poor vascular condition of upper extremity, resulting in the exhaustion of vascular pathway in upper extremity.ResultsAll patients were followed up 30-48 months (mean, 35.4 months). Two patients died, including 1 case of digestive tract hemorrhage, 1 case of heart failure. The other 10 CWAVGs were functionally useful for hemodialysis access about 6 weeks after operations. The primary patency rates at 6, 12, 18, 24, and 30 months were 83.3%, 75.0%, 33.3%, 33.3%, and 16.7%, respectively, and the cumulative patency rates at 6, 12, 18, 24, and 30 months were 83.3%, 75.0%, 50.0%, 33.3%, and 16.7%, respectively. Among 8 cases of CWAVG dysfunction, 6 cases had thrombosis, 1 case had seroma, and 1 case had vertebral artery stealing. Among them, 4 patients underwent hemodialysis using tunneled-cuffed catheter, 3 patients using fistula or graft on other limbs, and 1 patient was not treated with hemodialysis.ConclusionAlthough the long-term patency rate of CWAVG is yet to be further increased by improvement of treatment strategies, but it is still a supplementary option for end-stage renal disease patients with inadequate upper extremity venous access sites.
ObjectiveTo put forward the concept of acute peroneal artery ischemia syndrome, and to study its typical clinical manifestations and imaging features so as to provide the basis of the evidence-based medicine for the diagnosis and treatment of acute peroneal artery ischemia. MethodsBetween October 2009 and December 2012, 3 cases (2 males and 1 female, aged 57, 68, and 71 years) of acute peroneal artery ischemia syndrome were treated. All the patients displayed typical three symptoms of "peroneal artery blood supply zone pale/red + severe pain of the gastrocnemius muscle + acute drop foot". Medical examination revealed that they all had localized tenderness in the lateral and inferior part of the gastrocnemius muscle, with decreasing of skin temperature. The pulse of dorsalis pedis artery and the posterior tibial artery on affected limbs could be felt. Blood coagulation function and biochemical assay showed that D-dimer, fibrinogen degradation products, creatine kinase, or myoglobin markedly elevated. Magnetic resonance angiography revealed proximal peroneal artery stenosis. All patients were treated with intravenous thrombolysis, anticoagulation, vasodilation, and improving circulation therapy. ResultsThe symptoms of lower limb swelling, pain, and fatigue were improved obviously in all patients after treatment. Blood coagulation function and biochemical assay showed D-dimer, fibrinogen degradation products, creatine kinase, or myoglobin were gradually reduced. The patients were discharged at 9-13 days after treatments, without recurrence during the follow-up of 1year. ConclusionAcute peroneal artery ischemia syndrome is a special type of acute lower limb ischemia, with the three symptoms of "peroneal artery blood supply zone pale/red+ severe pain of the gastrocnemius muscle + acute drop foot" as the main characteristics and should be treated by early active anticoagulant and recanalization therapy.
