  • Interleukin-1&beta|stimulates the secretion of vascular endothelial growth factor and basic fibroblast growth factor in human retinal pigment epithelial cells

    Objective To investigate the effect of interleukin (IL-1beta;) on vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF) secretion in cultured human retinal pigment epithelial (RPE) cells.Methods Enzyme linked immunosorbent assay (ELISA) was used to detect the production of VEGF and bFGF by RPECs. The cells were cultured in the presence of IL-1beta; at different concentrations,which were 0,1,10, and 20 ng/ml,respectively.VEGF and bFGF levels were measured at various time points. Semiquantitative reverse transcription polymerase chain reaction (RT-PCR ) was conducted to detect the expression of VEGF and bFGF mRNA in RPE cells with and without IL-1beta; stimulation. The proliferation of cells in all groups was measured by the MTT assay and expressed as light absorption values.Results IL-1beta; significantly stimulated the secretion of VEGF at the concentration of 1 ng/ml and 10 ng/ml (P<0.01),bFGF was significantly stimulated when the concentration of IL-1beta; reached 10 ng/ml (P<0.01);at the concentration of 10 ng/ml, IL-1beta; increased the production of VEGF and bFGF in a time dependent manner (P<0.01); VEGF and bFGF mRNA levels were significantly increased by stimulation of 10 ng/ml IL-1beta; for 48 hours.There was no significant difference between all groups in light absorption values (P>0.05).Conclusion Proinflammatory cytokine IL-1beta; increased the secretion of VEGF and bFGF by human RPE cells to some extent.

  • Safety analysis of thulium laser in thoracoscopic pulmonary wedge resection

    Objective To explore the feasibility and safety of 2 μm thulium laser in thoracoscopic wedge resection. Methods The clinical data of 137 patients who underwent thoracoscopic wedge resection with thulium laser (as a thulium laser group, 64 patients, including 22 males, 42 females, average age of 58.39±10.40 years) and staplers (as a stapler group, 73 patients, including 33 males, 40 females, average age of 60.79±10.96 years) in thoracic Department of Xuanwu Hospital between April 2016 and August 2018 were retrospectively analyzed. In the study, the intraoperative blood loss, the operative time, chest tube duration, daily amount of fluid leak, hospital stay and hospitalization costs were compared between two groups. Results The intraoperative blood loss of the thulium laser group (16.05±23.67 mL) was significantly shorter or lower than that of the stapler group (28.56±32.09 mL) (P=0.011). Besides, the post operation hospital stay and hospitalization costs of the thulium laser group (4.72±2.49 d, 37 127.33±9 302.14 yuan) were also significantly shorter or lower than those of the stapler group (5.67±2.02 d, 49 545.76±13 831.93 yuan) (P=0.015, P=0.000). Furthermore, no statistical difference was found between the thulium laser group and the stapler group in the operative time (116.38±41.91 min vs. 108.36±47.25 min), total hospital stay (10.13±2.98 d vs. 11.05±3.26 d), daily amount of fluid leak (138.38±72.23 mL vs. 152.7±77.54 mL), chest tube duration (2.89±2.34 d vs. 3.52±1.48 d) and the frequency of postoperative fever (0.89±1.55 times vs. 1.23±1.70 times). Conclusion Applying 2 μm thulium laser to thorascopic wedge resection is safe and feasible. Besides, 2 μm thulium laser can achieve a similar result to that of the standard technique by using staplers.

  • Reconstruction of elasticity modulus distribution base on semi-supervised neural network

    Accurate reconstruction of tissue elasticity modulus distribution has always been an important challenge in ultrasound elastography. Considering that existing deep learning-based supervised reconstruction methods only use simulated displacement data with random noise in training, which cannot fully provide the complexity and diversity brought by in-vivo ultrasound data, this study introduces the use of displacement data obtained by tracking in-vivo ultrasound radio frequency signals (i.e., real displacement data) during training, employing a semi-supervised approach to enhance the prediction accuracy of the model. Experimental results indicate that in phantom experiments, the semi-supervised model augmented with real displacement data provides more accurate predictions, with mean absolute errors and mean relative errors both around 3%, while the corresponding data for the fully supervised model are around 5%. When processing real displacement data, the area of prediction error of semi-supervised model was less than that of fully supervised model. The findings of this study confirm the effectiveness and practicality of the proposed approach, providing new insights for the application of deep learning methods in the reconstruction of elastic distribution from in-vivo ultrasound data.

  • Retinal manifestations of mitochondrial encephalomyopathy

      Objective To observe the retinal manifestations and classification of mitochondrial encephalomyopathy,and explore the relationship between retinopathy and systemic manifestations.Method The clinical data of 88 inpatients with mitochondrial encephalomyopathy were retrospectively analyzed,in whom 12 patients(24 eyes)with retinal manifestations who diagnosed by ophthalmology consultation and complete medical records were collected. There were nine males and three females aged from 14 to 33 years with the mean age of(20.1±7.0)years. The disease duration ranged from 2.5 to 20 years,with the mean of(9.5±6.8)years. All the patients had the eye symptoms of the different degree,such as limbs weakness,hearing decline and central nervous system symptoms. Ophthalmologic examination including best corrected visual acuity,slit lampa microscope,indirect ophthalmoscopy,noncontact Tonometer,ptosis,ocular movement,pupillary reflex and color fundus photography. Among the patients,three,one,two and five patients had undergone fundus fluorescein angiography(FFA),optical coherence tomography(OCT),lectroretinogram(ERG)and visual field examination respectively. Diabetic retinopathy were divided into “salt and pepper”, retinitis pigmentosa(RP),retinal pigment epithelium(RPE),choroidal capillary atrophy and simplex optic atrophy according to the inspection results.Results All the patients′ both eyes were involved,the disease degree of bilateral eyes was accordant. The ptosis and(or)eye movement limitation were found in nine patients(75.0%),and decreased visual acuity was in six patients(50.0%).“Salt and pepper” was found in six patients(12 eyes),presenting retinal granular pigmentation and depigmentation;the visual acuity was 0.4-1.2;no central nervous system symptoms were found in patients,such as hearing decline,twitch,ataxia and hypophrenia. RP was found in one patient(two eyes),presenting retinal cells sample pigmentation,retinal vessel shrink,optic atrophy;the vision were light perception in both eyes;hypophrenia,hearing decline,bilateral lower limbs pain and onset twitch were also found in them. RPE and choroidal capillary atrophy were found in three patients(six eyes),the choroidal great vessels and flake pigment accumulation surrounding the retina were observed;the visual acuity was hand movement0.7;limbs weakness was found in two patients;hearing decline was found in three patients;barylalia and hypophrenia were found in two patients;somnolence was found in one patient. Simplex optic atrophy was found in two patients(four eyes);the vision was 0.1-0.7;central nervous system symptoms were found in patients,such as limbs weakness,twitch,hypophrenia and headache.Conclusion Retinopathy types is concerned with visual prognosis and central nervous system symptoms.

  • Efficacy of intravitreal expansile gas alone for treatment of idiopathic full-thickness macular hole

    ObjectiveTo evaluate the efficacy of intravitreal injection (IVI) of expansile gas alone to treat idiopathic full-thickness macular hole (FTMH).MethodsThis is a prospective interventional case series. Twenty FTMH patients (26 eyes) who underwent IVI with expansile gas alone were enrolled in this study. There were 5 males (5 eyes) and 21 females (21 eyes), with the mean age of (59±12) years. All patients received the best corrected visual acuity (BCVA), slit lamp microscope, indirect ophthalmoscopy, fundus color photography and three-dimensional optical coherence tomography (OCT) examinations. The BCVA was measured using the international standard visual acuity chart, and the results were converted to the logarithm of the minimum angle of resolution visual acuity. The diameters of macular holes and the interface between vitreous and macular were observed by OCT (Topcon, OCT-2000). Based on the diameter, the holes were classified as small FTMH (equal or lesser than 250 μm), medium FTMH (more than 250 μm but equal or lesser than 400 μm) and large FTMH (more than 400 μm). The mean BCVA was 0.85±0.29. There were 7, 10 and 9 eyes with small, medium and large FTMH. There were 10 eyes with vitreous- macular traction (VMT). All the eyes received IVI of 0.2 ml C3F8 followed facedown positioning for 7-14 days. The follow-up ranged from 1 to 23 months. The BCVA, FTMH closure and complications were observed. If holes failed to close at 1 month after IVI, vitrectomy combined with internal limiting membrane (ILM) peeling and C3F8 tamponade would be performed for these eyes.ResultsFTMHs was able to close in 17/26 eyes (65.4%) had hole closure, failed to close in 9 /26 eyes (34.6%). All 10 eyes with VMT achieved vitreous-macula separation after IVI of gas. The eyes failed in the closure initially with IVI of gas alone, all succeed with hole closure after vitrectomy combined with ILM peeling and C3F8 tamponade. The closure rate of small (6 eyes), medium (8 eyes) and large FTMH (3 eyes) was 85.7%, 80.0% and 33.3% respectively. The diameter of FTMHs in holes-closure eyes and failed-closure eyes was (307.8±122.8), (431.6±128.4) μm respectively, the difference was significant (t=−2.407, P=0.024). VMT was found in 6 eyes and 4 eyes in holes-closure group and failed-closure group, respectively, the difference was significant (t=−2.196, P=0.038). The mean preoperative BCVA was 0.51±0.36. There was a significant difference between pre-and postoperative BCVA (t=4.758, P<0.05). Two eyes developed local retinal detachment, which achieved hole closure and retinal reattachment after vitrectomy.ConclusionIVI of expansile gas alone is an effective way in treating FTMH with a diameter smaller than 400 μm and with VMT before surgery.

  • Use of diagnostic vitrectomy in eyes with uveitis of unknown etiology

    Objective To analyze the results of diagnostic pars plana vitrectomy (PPV) in patients with uveitis of unknown cause. Methods This is a retrospective case series study. Sixty-five patients (67 eyes) with uveitis of unknown cause were enrolled in this study. There were 31 males (32 eyes) and 34 females (35 eyes). The ages were from 6 to 84 years, with the mean age of (55.00±18.56) years. All eyes were received PPV. Examination of vitreous samples consisted of microbial stains and culture, microbial DNA and antibody detection, cytokine measurement, cytology, flow cytometry and gene rearrangement detection. Results Vitreous analysis was positive in 40 of 67 eyes (59.7%). Positive results indicated bacterial endophthalmitis in 20 of 40 eyes (50.0%), lymphoma in 11 eyes (27.5%), viral IgM and IgG increased significantly in 3 eyes (7.5%), fungal endophthalmitis in 3 eyes (7.5%), IgG of toxocara increased significantly in 2 eyes (5.0%), IgG of toxoplasma Gondii increased significantly in 1 eye (2.5%). Conclusion The diagnostic yield of vitreous samples in uveitis eyes of unknown cause is 59.7%.
