  • Application of iris fluorescein angiography in the diagnosis of intraocular diseases

      Objective To evaluate the application value of iris fluorescein angiography (IFA) in the diagnosis of ischemic center retinal vein occlusion (CRVO). Methods Fifty-one patients (51 eyes) with CRVO which had been diagnosed by fundus fluorescein angiography (FFA)were studied. All patients underwent the examination of visual acuity, slit lamp biomicroscope,anterior segment color photography, intraocular pressure, FFA and IFA. The patients were classified as non-ischemic CRVO (20 eyes)and ischemic CRVO (31 eyes). The 20 non-ischemic CRVO patients included 11 males and nine females, aged from 41 to 59 years. The 31 ischemic CRVO patients included 21 males and 10 females, aged from 28 to 62 years.FFA and IFA were performed for all the patients using Heidelberg retina angiograph, and the classic pictures were analyzed by the computer image processing system. The detection rate of iris neovascularization (NVI) by slit lamp biomicroscope and IFA was analyzed. All ischemic CRVO eyes underwent panretinal photocoagulation (PRP), and PRP was completed in 27 eyes and not completed in four eyes. Six months after PRP the regression of iris NVI was followed up. Results All non-ischemic CRVO eyes (100.0%) had no neovascularization on papillary margin and iris by slit lamp biomicroscopy, and had no fluorescence (pigment blocked fluorescence) on IFA. Thirteen eyes (41.9%) and 23 eyes (74.2%) of the 31 ischemic eyes had NVI by slit lamp biomicroscope and IFA, respectively. The NVI detection rate of those two methods was statistically different (Z=-3.425,P=0.001). NVI showed b fluorescence and leakage with variable patterns (small blocks, thin lines and irregular cross-links) by IFA. There was no fluorescence staining and leakage on papillary margin and iris in 27 eyes who completed the PRP, but the neovascular glaucoma (NVG) occurred in one eyes who discontinued the PRP treatment after one to two months. Conclusions IFA has a high specificity in CRVO which hints the ischemic state of anterior segment. It is helpful to the early diagnosis of ischemic CRVO and the turnover of NVG.

  • Research on extubation time and arterial blood gas analysis of ex vivo liver resection followed by autotransplantation in patients with advanced hepatic alveolar echinococcosis

    Objective To explore the correlations between the time of tracheal extubation and the intraoperative basic factors of ex vivo liver resection followed by autotransplantation in patients with advanced hepatic alveolar echinococcosis (HAE), and analyze the change trend of blood gas analysis during operation. Methods The data of 24 patients with advanced HAE who underwent ex vivo liver resection followed by autotransplantation in West China Hospital of Sichuan University between February 2014 and August 2017 were retrospectively analyzed. Results There were significant correlations between the extubation time and the duration of anesthesia (r=0.472, P=0.031), the amount of bleeding (r=0.524, P=0.015), the amount of erythrocyte suspensions infusion (r=0.627, P=0.002), and the amount of plasma infusion (r=0.617, P=0.003). There was no statistical difference in extubation time between patients with and without pulmonary complications in 3 months postoperatively [(23.74±15.84), (15.52±19.40) h, P=0.327]. Compared with those arterial blood gas results before the interruption, the pH value, blood glucose, lactic acid and base excess were statistically significantly different (P<0.05) at each time point after the interruption. Blood potassium increased at the end of operation compared with that before interruption (P<0.05); and the free calcium after blocking and opening increased with a temporary decrease (P<0.05); the hemoglobin decreased significantly after interruption and clamping (P<0.05). Conclusions Anesthesia length and bleeding should be reduced in ex vivo liver resection followed by autotransplantation, thus the extubation time would be shortened and the prognosis of the patients might be improved. Because of the longer anhepatic phase, the blood gas analysis varies largely. During operation, blood gas analysis and monitoring should be strengthened, and the acid-base balance and electrolytes should be maintained in time.

  • Status and progress of peripheral interventions via trans-radial access

    Trans-radial access (TRA) has been a common approach to percutaneous coronary intervention (PCI). Comparing with trans-femoral access (TFA), TRA is used as an alternative approach for PCI with less local complications, higher comfort level, and better outcome. In recent years, TRA has been paid more and more attention in peripheral vascular interventions. We reviewed recent developments in peripheral vascular intervention using TRA, with detail summary of the effectiveness, safety, limits, and future developments of it, aiming to improve the understanding and performance of TRA in interventionalists to benefit patients.

  • Intravitreal injection with bevacizumab combined with extra panretinal photocoagulation for highrisk proliferative diabetic retinopathy

    Objective To evaluate the therapeutic effect of intravitreal injection with bevacizumab (Avastin) (IVB)combined with extra panretinal photocoagulation (E-PRP) for highrisk proliferative diabetic retinopathy (PDR).Methods A total of 57 eyes of 53 patients with highrisk PDR underwent intravitreal injection combined with E-PRP. The examinations of vision acuity, intraocular pressure, iris fluorescein angiography (IFA),fundus photos and fundus fluorescein angiography (FFA) were performed on all of the patients before and 1,2,3,and 6 months after the treatment; the results of the examinations before and after the treatment were compared and analyzed.The average follow up was 6 months.Results The mean visual acuity was (0.143plusmn;0.072) before the treatment and (0.218plusmn;0.128) 7 days after the tretment; the difference was significant (t=-7.940, Plt;0.05). The mean visual acuity 1,3,and 6 months after E-PRP (0.228plusmn;0.138, 0.223plusmn;0.125,0.220plusmn;0.134, respectively) differed much from that before IVB (Plt;0.05), but not so much from that after IVB (Pgt;0.05).The mean intraocular pressure of 21 eyes which had the neovascularization of pupil margin and iris surface before and 7 days after IVB was (26.632plusmn;2.629) and (19.316plusmn;3.092) mm Hg(1 mm Hg=0.133 kPa), respectively; the difference was significant (t=12.838, Plt;0.05) . The mean intraocular pressure 1,3,and 6 months after E-PRP was (16.947plusmn;2.345),(16.474plusmn;1.611), and (16.421plusmn;4.702) mm Hg, respectively, which differed much from that before and after IVB (Plt;0.05). Neovascularization on the disc and the retinae of 57 eyes were subsided partly, and a significant reduction or disappeared of the area of retinal neovascularization and the blood vessel leakage were observed 7 days after IVB. The neovascularization of pupil margin and iris surface of 21 eyes disappeared, and the IFA leakage decreased. The results of FFA 2 months after E-PRP showed that the one-off efficiency of E-PRP was 68.4%;12 eyes (21.1%) needed an additional laser, in which 6 eyes (10.5%) underwent vitreous surgery. Conclusion IVB combined with E-PRP as a treatment for highrisk PDR may improve the regression of retinal neovascularization and the reduction of vascular permeability,and prevent or reduce the complications and improve the therapeutic effect.

  • 汶川地震灾民心理健康调查

    摘要:目的: 了解汶川地震后灾民的心理健康状况,为对灾民进行心理干预提供科学依据。 方法 :采用创伤后应激障碍症状清单平民版(PCLC)问卷对四川省绵阳市安县京安小区安置点的1 100名灾民进行了随机调查。 结果 :安县灾民PCLC均分者占调查人数的8.8%。女性PCLC 均分高于男性,不同年龄段PCLC均分差异无统计学意义。文化程度方面文盲受损程度较高,其他不同学历组间PCLC均分差异无统计学意义。地震前存在躯体疾病者PCLC均分较高,地震中有掩埋经历者PTSD症状较严重。 结论 :地震灾害造成灾民的心理健康问题较为突出,加强心理卫生教育及对灾民的心理干预是非常必要的,尤以女性、震前存在躯体疾病以及有掩埋经历者应给予重点关注。

  • Efficacy of fibrin glue after thyroidectomy: a systematic review

    ObjectiveTo systematically review efficacy application of fibrin glue (FG) after thyroidectomy.MethodsPubMed, EMbase, The Cochrane Library, ClinicalTrials.gov, CBM, CNKI, WanFang Data and VIP databases were searched to collect randomized controlled trials (RCTs) regarding the use of FG after thyroidectomy from inception to October 29th, 2019. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of included studies. Then, meta-analysis was performed by using RevMan 5.3 software.ResultsA total of 15 RCTs involving 2 406 patients were included. The results of meta-analysis showed that compared with non-FG group, the use of FG could reduce postoperative drainage amount at the initial 24 hours (MD=−17.98, 95%CI −28.35 to −7.60, P=0.000 7), total amount of wound drainage (MD=−40.92, 95%CI −46.25 to −35.59, P<0.000 01), and postoperative discomfort (RR=0.48, 95%CI 0.35 to 0.66, P<0.000 01), as well as shorten drainage time (MD=−9.99, 95%CI −15.74 to −4.23, P=0.000 7) and stitches removal time (MD=−1.49, 95%CI −2.1 to −0.87, P<0.000 01). However, there was no statistically significant difference concerning postoperative short-term complications such as swelling (RR=0.78, 95%CI 0.48 to 1.28, P=0.32), recurrent laryngeal nerve injury (RR=0.83, 95%CI 0.21 to 3.29, P=0.79) and wound infection (RR=0.28, 95%CI 0.07 to 1.21, P=0.09) between two groups.ConclusionsThe current evidence shows that FG can reduce postoperative drainage amount and shorten postoperative recovery time in thyroidectomy. Due to the limited quality and quantity of included studies, more high quality studies are required to verify above conclusions.

  • Allogeneic islets combined with autologous peripheral blood stem cell transplantation in treatment of type 1 diabetes mellitus: a 3-year follow-up clinical study

    ObjectiveTo evaluate the 3-year follow-up results of allogeneic islets combined with autologous peripheral blood stem cells (PBSC) transplantation in treatment of type 1 diabetes mellitus (T1MD). MethodsThe clinicopathologic data of recipients underwent allogeneic islet cells combined with (or without) PBSC transplantation in the Sichuan Provincial People’s Hospital from January 2017 to December 2019 were retrospectively analyzed. The allogeneic islet cells combined with (or without) PBSC were infused into the liver by percutaneous hepatic portal vein puncture. Then the modified Edmonton immunosuppressive regimen was used after the operation. The recipients were followed-up for 3 years, and the insulin usage, C-peptide, and glycosylated hemoglobin A1c (HbA1c) were analyzed. ResultsThere were 5 recipients of allogeneic islet cell transplantation alone (named as alone group) and 5 recipients of allogeneic islet cell combined with PBSC transplantation (named as combined group). The gender, age, body mass index, preoperative insulin dosage, and HbA1c concentration of the recipients in the two groups were similar. ① The numbers of islets transplanted in the alone group and combined group were (50.60±2.51)×104 islet equivalent quantity (IEQ) and (48.70±4.76)×104 IEQ, respectively, and the number of PBSC transplanted in the combined group was (9.2±2.6)×107. ② In the alone group and combined group, there were 3 cases and 3 cases of insulin withdrawal at the 12th month, as well as 2 cases and 3 cases of insulin withdrawal at the 36th month after operation, respectively. The postoperative insulin dosages of the two groups were decreased as compared with that before operation, and which of the combined group showed a lower trend as compared with the alone group within 12 months after operation. ③ The C-peptide level at the fasting in the alone group peaked at 24th month after operation and began to decline, and which at the fasting and postprandial 2 h peaked at 12th month after operation and began to decline, but that at postprandial 2 h began to increase slightly at 24th month in the combined group, and which showed a higher trend at 12th and 36th month after operation in the combined group as compared with the alone group. ④ Within 36 months after operation, the HbA1c concentrations of the two groups decreased at different times after operation as compared with those before operation, and basically maintained a normal and stable level at 6 months after operation, and the change trend of the two groups was basically similar. ⑤ There were no obvious immune rejection and adverse reactions in the two groups. ConclusionFrom the preliminary results of this study, PBSC has a certain protective effect on islet transplantation, which improves the clinical efficacy of allograft islet transplantation for 3 years, but the long-term efficacy still needs to be observed, and the mechanism still needs to be further explored.
