  • REPAIR OF FINGER SOFT TISSUE DEFECT WITH ISLAND FLAP BASED ON VASCULAR CHAIN OF CUTANEOUS BRANCH OF DORSAL METACARPAL ARTERY

    Objective To investigate the operative procedure and the clinical results of the island flap based on the vascular chain of the cutaneous branch of dorsal metacarpal artery for repairing finger soft tissue defect. Methods Between January 2008 and March 2012, 28 cases of tissue defect of fingers (32 fingers) were repaired with the island flaps based on the vascular chain of the cutaneous branch of dorsal metacarpal artery. There were 20 males (23 fingers) and 8 females (9 fingers), with an average age of 29.5 years (range, 14-67 years). The injury causes included 14 cases of crush injury, 6 cases of pressing injury, 5 cases of cutting injury, and 3 cases of avulsion injury. The locations included 10 index fingers, 13 long fingers, 6 ring fingers, and 3 little fingers. There were 9 defects of proximal segment, 12 defects of middle segment, and 11 defects of distal segment. The area of defect ranged from 1.0 cm × 0.8 cm to 5.2 cm × 3.5 cm. The disease duration was 1 hour to 15 days. The area of flaps ranged from 1.2 cm × 1.0 cm to 5.5 cm × 3.8 cm. The donors were closed by suture or were repaired with skin graft. Results Tense blister occurred in 3 cases, which was cured after dressing change; the other flaps survived. Wound obtained primary healing. Twenty-five patients (27 fingers) were followed up 6-25 months (mean, 16.8 months). The flaps had soft texture and satisfactory appearance. Two point discrimination was 6-9 mm (mean, 7.7 mm) at 6 months after operation. The total active movement of fingers was 105-230° (mean, 204.6°). The results were excellent in 17 fingers, good in 8 fingers, and fair in 2 fingers with an excellent and good rate of 92.6%. Conclusion The island flap based on the vascular chain of the cutaneous branch of dorsal metacarpal artery has the advantages of the deverting point from the dorsal point to the palm, the extended vessel pedicle, and expanded operation indications, so it is not necessary to cut the dorsal metacarpal artery. It can be used to repair finger tissue defect.

  • 尺动脉腕上皮支游离皮瓣修复手指掌侧软组织缺损

    目的 总结尺动脉腕上皮支游离皮瓣修复第2~5指掌侧皮肤软组织缺损的临床效果。 方 法 2003 年5 月- 2009 年10 月,收治第2 ~ 5 指掌侧皮肤软组织缺损17 例19 指。男12 例,女5 例;年龄21 ~ 53 岁,平均38.6岁。机器伤12 例,交通事故伤3 例,切割伤2 例。损伤指别:示指5 例,中指6 例,环指4 例,小指4 例;其中2 例为相邻两指。14 例为急性损伤,伤后至入院时间为10 min ~ 7.5 h;3 例为伤后彻底清创后感染创面。创面范围5.0 cm × 1.5 cm ~ 7.5 cm × 4.5 cm。应用大小为6.5 cm × 2.5 cm ~ 9.0 cm × 6.0 cm 的尺动脉腕上皮支游离皮瓣修复缺损,供区直接缝合或游离植皮修复。 结果 术后1 ~ 6 d 4 例出现表皮张力性水疱,3 例皮瓣远端部分坏死,经对症处理后皮瓣成活;其余皮瓣均顺利成活,创面Ⅰ期愈合。供区切口均Ⅰ期愈合,植皮均成活。术后14 例获随访,随访时间6 个月~ 2 年6 月,平均1.3 年。皮瓣外形、质地、色泽均满意,两点辨别觉8 ~ 20 mm。手指功能参照关节主动活动度法(TAM)评定,获优8 例,良5 例,可1 例。 结论 尺动脉腕上皮支游离皮瓣是修复第2 ~ 5 指掌侧皮肤软组织缺损的有效方法之一。

  • REPAIR THE PALMAR SOFT TISSUE DEFECT OF THE FINGER WITH CROSS-FINGER FLAP WITH CUTANEOUS BRANCH OF THE ULNAR DIGITAL FINGER

    Objective To discuss the clinical effect of cross-finger flap with cutaneous branch of the ulnar digital finger on repairing the palmar soft tissue defect of the finger. Methods From October 1996 to June 2004, crossfinger flaps were used to repair the palmar soft tissue defect of the finger in 25 cases( 32 fingers ) with tendon or bone exposed. There were 18 males and 7 females, and theirages ranged from 13 to 45 years. Among them, 6 cases were incised injury, 8 cases were impact and press injury, 11 cases were crush injury; and 2 cases were thumb, 8 cases were index, 5 cases were middle finger, 3 cases were ring finger, 2 cases were little finger, 2 cases were index and middle finger, 2 cases were middle and ring finger, and 1 cases were index, middle, ring and little finger. Thetime from injury to diagnosis was 30 min to 48 h, and the size of the tissue defect was 1.5 cm×1.0 cm to 4.1 cm×2.0 cm. All cases were treated with emergent operation, and the sense of the flap was recovered by anastomosing the cutaneous branch of the ulnar digital finger and the distal digital nerve of injured finger. The flap pedicle was dissected 3 weeks later. Results Followup was conducted for 6 to 26 months and it showed that the cross-finger flaps all survived with full digital fingertip, satisfactory appearance, good function, and normal sense. The discrimination of two points was 5-8 mm. Conclusion As it is easy to operate and with satisfactory appearance and good function restoration, cross-finger flap with cutaneous branch of the ulnar digital finger is effective in repairing the palmar soft tissue defect of the finger.
