  • The Parameter Estimation Method of Gaussian Point Spread Function in Microscopic Images

    As the most popular simplified model of the optical imaging system, the acquisition of the Gaussian point spread function (PSF) parameter is one of the hotspots and key points on which people do research in the field of image restoration. Based on the idea by which there exists deterministic mathematical relationship between Gaussian OTF feature points as well as its parameter and the frequency representation of the image in an existed literature, we proposed an automatic, accurate, stable, and improved approach. This method is able to give prominence to the related calculation feature by a Gaussian convolution and degeneration operation and finally realize the automatic estimation of PSF parameter of a microscopic image. Experiments have proved that a good restoration result can be achieved utilizing the estimated PSF by the present method, which is of considerable application and reference value in restoration of other sorts with Gaussian approximate PSF model or 3D microscopic image restoration .

  • Clinical Efficacy of Immediate Breast Reconstruction with Extended Latissimus Dorsi Musculocutaneous Flap after nipple and areola Sparing modified radical mastectomy for patients with Early-Middle Breast Cancer

    ObjectiveTo explore the clinical efficacy of immediate breast reconstruction with extended latissimus dorsi musculocutaneous flap after nipple and areola sparing modified radical mastectomy for patients with early-middle breast cancer. MethodsSixty patients diagnosed with early-middle breast cancer by needle biopsy from November 2009 to October 2012 in this hospital were divided into two groups according to the surgical method.The breast reconstruction group (30 cases) were performed immediate breast reconstruction with extended latissimus dorsi musculocutaneous flap after nipple and areola sparing modified radical mastectomy, the control group (30 cases) were performed traditional modified radical mastectomy.The postoperative complications, clinical efficacy, quality of life, and recurrence and metastasis were compared in two groups. Results①The ipsilateral breast was completely missing in the control group.The aesthetic effect in the breast reconstruction group was satisfactory including excellent in 22 patients, good in 7 patients, general in 1 patient.②The rates of flaps effusion, flap necrosis, and affectied limb activity limitation had no significant differences in two groups (P > 0.05).The wound healing time, operation area drainage time, hospital stay, and post-operative chemotherapy start time all had no significant differences in two groups (P > 0.05).③The frequency of psycho-logical pressure (inferiority/anxiety/fear) and rate of un-satisfaction of secondary sexual characteristics (body/curve) in the breast reconstruction group were lower than those in the control group (P < 0.001).The rate of social interaction fitness in the breast reconstruction group was higher than that in the control group (P < 0.001).④The following-up time was 12 to 38 months with an average 25 months, one case died because of brain metastasis in the control group, the rest 59 patients had no local recurrence and distant metastasis. ConclusionsThe immediate breast reconstruction with exte-nded latissimus dorsi musculocutaneous flap after nipple and areola sparing modified radical mastectomy can cure cancer on the basis of satisfactory cosmetic results of breast, excellent quality of life of patients, and fewer surgical complica-tions.It does not affect postoperative adjuvant radiochemotherapy and short-and long-term efficacy for breast cancer.

  • Interpretation of the Consolidated Standards of Reporting Trials (CONSORT) guideline for within person trials

    Within person trial is an efficient study design for randomized controlled trials which has been widely used in fields of dentistry, dermatology and ophthalmology. However, due to a series of inherent methodological difficulties, the design, conduct and reporting of within person trials usually requires additional methodological knowledge and considerations from researchers. To standardize and improve the reporting quality of these trials, the BMJ recently published the CONSORT statement extension for within person trials. The present article aimed to provide interpretation of this reporting guideline, and thereby promote its understanding and use among Chinese researchers.

  • Reporting guideline for health care simulation research: extensions to the CONSORT and STROBE statements

    背景 基于仿真研究(simulation-based research,SBR)的数量迅速增加,但是这类研究的报告质量却需亟待提高。为使读者能够批判性地评估研究,研究报告的要素需要在文章中清晰地报告出来。我们旨在通过扩展试验报告的统一标准(Consolidated Standards of Reporting Trial,CONSORT)和加强流行病学观察性研究报告(Strengthening the Reporting of Observational Studies in Epidemiology,STROBE)声明来制定卫生保健仿真研究的报告规范。 方法 在制定报告规范的建议步骤基础上,使用迭代的多步共识法建立流程。多步共识法包括以下内容:① 建立指导委员会;② 定义报告规范的范围;③ 确定共识小组参与者;④ 通过在线会前调查生成拟讨论项目清单;⑤ 召开共识会议;⑥ 起草报告规范及解释与说明文件。 结果 对 CONSORT 的 11 个条目进行了扩展,包括条目 1(文题和摘要),条目 2(背景),条目 5(干预措施),条目 6(结局指标),条目 11(盲法),条目 12(统计方法),条目 15(基线数据),条目 17(结果和估计值),条目 20(局限性),条目 21(可推广性)和条目 25(资金来源)。对 STROBE 的 10 个条目进行了扩展:条目 1(文题和摘要),条目 2(背景/原理),条目 7(变量),条目 8(数据来源/数据测量),条目 12(统计方法),条目 14(描述性数据),条目 16(主要结果),条目 19(局限性),条目 21(可推广性)和条目 22(资金来源)。工作组已创建详细说明文档,提供每个扩展条目的示例和说明。 结论 制订基于 CONSORT 和 STROBE 声明的仿真研究扩展版,可帮助提高仿真研究的报告质量。

  • Current status and surgical advances in adult heart transplantation in the United States

    Heart transplantation remains the most effective treatment for patients with end-stage heart failure. Over the past decade, significant advancements have been made in the field of heart transplant surgery. However, the enormous demand from heart failure patients and the severe shortage of available donor hearts continue to be major obstacles to the widespread application of heart transplantation. With the development of donor heart recovery, preservation, and evaluation techniques, the use of extended criteria donors and donation after circulatory death has increased. These technological advancements have expanded the safe ischemic time and geographic range for donor heart procurement, significantly enlarging the donor pool and driving a rapid increase in heart transplant cases. Concurrently, many new techniques have emerged in heart transplant surgery and perioperative management, particularly the rapid advancements in mechanical circulatory support and artificial intelligence, which hold the potential to revolutionize the field. This article reviews and discusses the current status and major surgical advancements in adult heart transplantation in the United States, aiming to provide insights and stimulate ongoing exploration and innovation in this field.
