  • Appropriateness Assessment and Correlation Analysis of Barthel Index and Modified Rankin Scales in a Stroke Data Register

    Objective To assess the appropriateness of Barthel Index (BI) and Modified Rankin Scales (MRS) used as long-term outcome measures in a stroke data register and to investigate the correlation between cutoff points of the two scales in different stroke patients with and without disability. Methods Nine hundred and twelve patients were registered prospectively. BI and MRS were evaluated at the end of 1, 3, 6 and 12 months after stroke onset. The distribution, ceiling effects and floor effects of the two scales were evaluated. A logistic regression model was established to investigate correlation of cutoff points of BI and MRS. Results There were a total of 2 829 evaluation points of BI and MRS. The percentages of patients reaching the maximum scores of BI at the end of 3, 6 and 12 months (54.8%, 62.2% and 68.3%, respectively) were higher than those of MRS. There was significant correlation between the two scales (Spearman’s correlation coefficient 0.887, P<0.05), when MRS scores of ≤1 and ≤2 were taken as cutoff points, the corresponding cutoff points of BI score were ≥90 and ≥85, respectively. Conclusions BI has significant ceiling effects when used as long-term outcome measurement in a stroke data register. There was significant correlation between BI and MRS scores. In future clinical studies, an MRS score ≤2 or BI score ≥85 could be used as cutoff points in predicting stroke patients with and without disability.

    目的 总结阻塞性睡眠呼吸暂停低通气综合征患者手术配合经验。 方法 对2006年1月-2011年11月259例阻塞性睡眠呼吸暂停低通气综合征患者实施改良腭咽成形术的临床资料及护理措施进行回顾性分析。 结果 259例患者顺利完成手术,未发生呼吸道梗阻及大出血。随访4~9年,治愈158例( 61.0%) ,显效73 例(28.2%),有效28例( 10.8%)。 结论 针对性的围手术期护理有助于保留悬雍垂改良腭咽成形术的顺利进行,减少手术后并发症,提高治疗效果。

  • 应用改良皮瓣修复颌面缺损(附12例报告)

    目的:为了进一步探讨颌面部缺损的理想修复方法。方法:应用改良后邻近滑行皮瓣和带蒂皮瓣对12例颌面部缺损患者进行I期修复。结果:12例中除1例皮瓣部分坏死导致失败外,其余均收到良好效果。结论:改良后皮瓣修复颌面缺损,方法简单皮瓣成活率高,供受区皮瓣厚度一致,临床效果满意。

  • The Efficacy of Modified De Vega Technique Compares with the Traditional De Vega Technique for the Correction of Severe Tricuspid Regurgitation

    摘要:目的:探讨改良De Vega环缩术与经典De Vega环缩术相比对于治疗重度三尖瓣返流是否具有更好的成形效果。方法: 2007年12月至2009年3月对29例重度三尖瓣返流的患者行De Vega环缩术。其中16例行改良De Vega环缩术,13例行经典De Vega环缩术,随访比较两组患者三尖瓣返流程度,右心室舒张期末内径,EF值及心功能分级。以秩和检验分析研究两组患者三尖瓣返流程度和心功能分级的差异,以t检验研究两组患者右心室舒张末期内径及EF值变化。结果:术前两组患者一般指标无显著差异。两组患者随访时间无显著差异。随访经典De Vega组重度返流1例,中度返流5例,轻度返流5例,微量及无返流2例;改良De Vega组无中、重度返流,轻度返流8例,微量及无返流8例。经分析显示两组患者三尖瓣返流程度结果差异有统计学意义(Plt;0.05)。经典De Vega组心功能分级I级5例,II级7例,III级1例;改良De Vega组I级7例,II级8例,III级1例,两组患者心功能差异无统计学意义(Pgt;0.05)。两组患者右室舒张期末内径及EF值组内比较随访与术前差异均有统计学意义(Plt;0.05),随访时组间比较差异有统计学意义(Plt;0.05), 改良De Vega环缩术随访时右室舒张期末内径缩小更显著,射血分数改善更明显。结论:改良De Vega环缩术治疗重度三尖瓣返流效果优于经典De Vega环缩术。Abstract: Objective: To compare the efficacy of one kind of modified De Vega technique and traditional De Vega technique for the correction of severe tricuspid regurgitation. Methods: From December 2007 to March 2009, 29 patients were treated with tricuspid valve annuloplasty. These were 16 patients in modified De Vega annuloplasty group and the others (13 patients) in traditional De Vega annuloplasty group. The grade of tricuspid regurgitation、New York Heart Association (NYHA) functional class、ejection fraction (EF) and the right ventricular enddiastolic dimension of two groups were followed and reviewed. Results: There was no statistically difference between two groups about preoperative characteristics and followup time. There was 1 patient with severe TR, 5 patients with moderate TR, 5 patients with mild TR and 2 patients without TR in traditional De Vega annuloplasty group after the operations. In modified De Vega annuloplasty group, no patient was observed with severe or moderate TR, 8 patients with mild TR, and 8 patients without TR. At interval time, there was significant difference in the grade of tricuspid regurgitation between two groups (Plt;0. 05). Both tricuspid valve plasty techniques could reduce the right ventricular enddiastolic dimension and improve ejection fraction significantly (Plt; 0. 05), and there was significant difference in the right ventricular enddiastolic dimension and ejection fraction at interval time between two groups (Plt;0.05). Conclusions: The outcome of modified tricuspid De Vega technique is superior to that of traditional De Vega technique in correcting severe tricuspid regurgitation.

  • New Double Tract Digestive Reconstruction of Total Gastrectomy : The Modif ied Functional Jejunal Interposition

    Objective  To describe a new technique for digestive tract reconst ruction of total gast rectomy.Methods  The modified functional jejunal interposition ( FJ I) was performed in 38 patient s who underwent total gastrectomy between June 2004 and March 2006. At digestive tract reconst ruction, the jejunum with suitable suture ligated at 2 cm distal to side-to-end jejunoduodenostomy was changed to sew up 2-3 needles and to narrow it . End-to-side esophagojejunostomy to Treitz ligament was shortened to 20-25 cm befittingly. Side2to2side jejunojejunostomy to Treitz ligament was 10 cm. Both esophagojejunostomy and jejunojejunostomy must not be tensioned. Results No patients died or had anastomotic leakage in perioperative period. Roux-en-Y stasis syndrome (RSS) was in 2 patients. The Visick grade: 35 patient s were grade Ⅰ, 3 patient s were grade Ⅱ. Serum nut ritional parameters in 2 patients hemoglobin was only lower than normal. At 6 months after operation , food intake per meal and body weight were recovered to the preoperative level in 36 patients, and only 2 patients appeared weight worse. One patient had reflux esophagitis and no dumping syndrome occurred. Through the upper gast rointestinal radiograph , the bariums entered into duodenal channels mostly , and a little into the narrow channels. Conclusion  The modified FJ I not only reserved all advantages of the primary procedure , but also could further lower the complications and improve of the quantity life of the patients who were underwent total gast rectomy. It would be necessary for further prospective randomized controlled trial in tlhe largescale cases.

  • MODIFIED RADICAL MASTECTOMY (AUCHINCLOSS OPERATION) WITH PRESERVATION OF PECTORAL NERVES FOR BREAST CANCER PATIENT

    Eight patients treated with modified radical mastectomy and fenestration of pectoralis muscle to preserve pectoral, nerves are reported and the practical procedure is introduced. The results indicate that this method can overcome the disadvantage of mastectomy (Auchincloss) in that only dissection of fatty tissue and lymph nodes in the lateral part of axilla is carried out. With fenestration of pectoralis major muscle, not only the pectoral nerves can be perserved but also the fatty tissue and lymph nodes, including of those medial to the pectoralis minor, subclavicular and interpectoral nodes can be dissected. This method almost reached Halsted’s demand and it can be used for stage Ⅰ-Ⅱ, and even stage Ⅲ breast cancer if no infiltration to pectoralis major muscle is found.

  • Application of Improved Artificial Airway in Interventional Therapy with Bronchoscope

    Objective To investigate the safety and effectiveness of a self-made bronchoscopic catheter ( an improved artificial airway) in bronchoscopic interventional therapy.Methods 126 patients planning to receive bronchoscope between October 2012 and February 2013 were divided into A, B and C groups. Three groups received inhalation of 2% lidocaine 5mL for surface anesthesia, and the venous channel was build up. Then the patients in group A ( n = 45) were treated with conventional bronchoscope. The patients in group B ( n =40) were treated with painless bronchoscope ( received intravenous injection with midazolam0. 06 mg/kg and fentanyl 1μg/kg before operation) . The patients in group C ( n = 41) were treated with painless bronchoscope through improved artificial airway ( after anesthesia similar to group B, the improved artificial airway was implanted through the mouth guided by bronchoscope, then the bronchoscopy was performed through artificial airway) . Blood pressure, respiration rate, heart rate and the pulse oxygen saturation were measured by multi-parameter ECG monitor before and during the operation, and the differences were compared among three groups. Body movement, transient respiratory depression during the operation, and postoperative feelings and reactions after operation were also observed. Meanwhile, the convenience of operation by physicians was evaluated. Results The blood pressure fluctuations in group C and group A had no significant difference ( P gt;0. 05) . Heart rate of three groups was somewhat increased,but there was no significant difference between group C and group A ( P gt; 0. 05) . Body movement and postoperative pain memory in group B and group C were better than those in group A ( P lt; 0. 05) .Respiratory depression of three groups had no significant difference ( P gt; 0. 05) . The operative convenience and the comfort of physicians in group C were better than those in group A and group B ( P lt; 0. 05) .Conclusions Operation safety of bronchoscopic interventional treatment with improved artificial airway is similar to the conventional procedure, but the reaction of the intra-operation and postoperative painful memories are significantly superior to conventional bronchoscopy. The convenience of operation and comfort of physicians are much better than the conventional bronchoscopy. It can be concluded that the improved artificial airways is worthy of clinical application.

  • Clinical Analysis of Concomitant Mitral Valve Replacement and Bipolar Radiofrequency Ablation for Chronic AtrialFibrillation

    Objective To evaluate clinical results of concomitant mitral valve replacement (MVR) and modified maze procedure with Atricure bipolar radiofrequency for chronic atrial fibrillation (AF). Methods Clinical data of 59 patients with mitral valve diseases and chronic AF who underwent concomitant MVR and bipolar radiofrequency ablation in Subei People’s Hospital from June 2010 to September 2012 were retrospectively analyzed. There were 22 male and 37 female patients with their age of 29-71 (48±11) years. The AF duration was 1.2-26.0 (7.2±3.4) years. Preoperatively,there were 20 patients with New York Heart Association (NYHA) class Ⅱ,31 patients with NYHA class Ⅲ and 8 patients with NYHA class Ⅳ. There were 32 patients with moderate to severe mitral stenosis,9 patients with moderate to severe mitral regurgitation and 18 patients with combined mitral stenosis and regurgitation. There were 42 patients with tricuspid regurgitation. The left artial dimension was 39-98 (55.2±8.9) mm. Left atrial thrombus was found in 9 patients. Atricure bipolar radiofrequency system was used for right atrial ablation under normothermic cardiopulmonary bypass (CPB) with beating heart first,then for ablations of the left and right pulmonary vein orifices and left atrium under moderate hypothermia with heart arrest. MVR was performed after ablation procedures were completed. Amiodarone was routinely used postoperatively and patients were periodically followed up after discharge. Results There was no in-hospital death. CPB time was 65-180 (99±28)minutes,aortic cross-clamping time was 46-123 (69±17)minutes,and ablation time was 15-28 (21±4)minutes. Postoperatively,heart rhythm immediately changed to sinus rhythm (SR) in 44 patients,remained AF in 10 patients and atrial flutter in 1 patient. Temporary pacemaker was used for 4 patients with bradycardia (3 patients recovered SR and 1 patient remained AF later). Fifty-eight patients were followed up after discharge for 6-33 months,and 1 patient was lost during follow-up. Patients’ SR rate was 86.2 % (50/58),91.4% (53/58),89.7 % (52/58),84.6 % (33/39)and 71.4 % (5/7)at discharge,3 months,6 months,1 year and 2 years after discharge respectively. There was no thrombotic event during follow-up. Conclusion Concomitant MVR and modified maze procedure with Atricure bipolar radiofrequency is a safe procedure for chronic AF with good short-term results.

  • Treatment of Stanford Type A Aortic Dissection with “Modified” Ascending Aorta and Hemiarch Replacement Combined with Stent-graft Elephant Trunk Technique

    Abstract: Objective To summarize and evaluate the clinical effect of “modified” ascending aorta and hemiarch replacement combined with stent-graft elephant trunk technique, a new surgical approach for patients with Stanford type A aortic dissection. Methods Between December 2009 and January 2011, the “modified” ascending aorta and hemiarch replacement combined with stent-graft elephant trunk technique was performed to a total of 47 patients suffering from Stanford type A aortic dissection in the First Affiliated Hospital of China Medical University. There were 35 male patients and 12 female patients. Their mean age was(57.9±16.0)years (ranging from 29 to 86 years). Preoperative computedtomography angiography(CTA) imaging was analyzed using three-dimensional (3D) reconstruction to clarify their diagnosis.All these patients underwent their procedures under cardiopulmonary bypass(CPB), hypothermic circulatory arrestand right axilary artery cannulation for selected cerebral perfusion. The treatment of proximal end to heart included: ascending aorta replacement in 29 patients, Bentall procedure in 11 patients, Wheat procedure in 4 patients, and David procedure in 3 patients. Five patients underwent concomitant coronary artery bypass grafting. Results Their average CPB time was (136±32) min, average aortic cross-clamp time was (97±28) min, and average selected cerebral perfusion andlower body arrest time was (27±11) min. The in-hospital mortality was (4.25% , 2/47). Postoperatively, two patients had transient neurological disorder, 1 patient had irreversible paraplegia, and 4 patients underwent reoperations for bleeding. All the 45 surviving patients underwent 3D CTA before discharge and 6 months after operation. The stented elephanttrunk-elastic metal stent were all well exhibited in the true lumen of the descending aorta, and the true lumens distal to the stent graft were also significantly enlarged compared with their preoperative diameters(P < 0.05). All the patients were followed up from 1 to 13 months. There was no aneurysm rupture and no reoperation related to residual dissected aorta wascarried out during follow-up. Conclusions “Modified” ascending aorta and hemiarch replacement combined with stentgraftelephant trunk technique is a safe and effective approach to treat patients with Stanford type A aortic dissection without involvement of 3 vessels of the arch. The main advantage of this approach is to simplify the surgical procedure, shorten the procedure time and CPB time, and reduce morbidity with a satisfying short-term result.
