ObjectiveTo discuss nursing measures and experiences for children with extensive skin avulsion injury of lower limbs treated by vacuum sealing drainage (VSD). MethodsWe reviewed the perioperative nursing data of 48 child patients with extensive skin avulsion injury in lower limbs who underwent VSD treatment between December 2010 to October 2012. And the data were compared with those for 56 children with the same disease and treatment between January 2007 and November 2010. ResultsIn these 48 children, 19 received once, 23 received twice, and 6 received 3 times of vacuum sealing drainage. During the treatment, 5 drainage tubes were blocked and were solved with normal saline, and 3 drainage tubes had leakage and two of them were reconnected and the other one became normal after changing the dressing. For VSD, the time between being wounded and skin-grafting was 13.6 days, and the average hospitalization time was 24.3 days. Compared with the traditional method group, children in the VSD group had fewer drug changes during hospitalization, lower infection rate, and shorter hospitalization stay, and the differences were significant (P<0.05). ConclusionKeeping drainage tube airtight, normal and effective is as important as nutrition and mental nursing in the treatment of children's extensive skin avulsion injury in lower limbs, which can decrease infection rate, reduce average hospitalization time, and increase operative efficacy.
ObjectiveTo explore the impact on the retention and effective rate of the mode of continuous guidance with wechat group in children with infantile spasms (IS) treated by ketogenic diet (KD).MethodsThe clinical data of 40 children who received KD treatment in Jiangxi Children’s Hospital from January 2017 to June 2019 were retrospectively analyzed, including 23 males and 17 females, the average age was (19.90±13.10) months and the average course was (9.95±7.61) months. They were randomly divided into control group (20 cases) and observation group (wechat group continuity Guidance Group, 20 cases), and followed up one year to compare the retention rate and efficacy.ResultsThere was no statistical difference between the general data of the observation group and the control group (P>0.05). After 9 and 12 months of KD treatment, the retention and effective rate of the observation group was significantly higher than the control group (P<0.05). And the seizure-free rate was higher than the control group (35% vs. 10%, 35% vs. 15%).ConclusionThis model of continuous guidance with wechat group can increase the KD retention rate of IS children, And increase the effective rate.
ObjectiveTo discuss the efficacy of type Ⅱ hybrid aortic arch repair for type A aortic dissection in patients of different age groups.MethodsWe retrospectively analyzed the clinical data of 126 patients with type A aortic dissection admitted to the Fuwai Hospital between January 2016 and December 2018, including 78 (61.9%) males and 48 (38.1%) females, with an average age of 61.8±6.9 years. The patients were divided into an elderly group (≥60 years, n=82) and a non-elderly group (<60 years, n=44). The preoperative, intraoperative and postoperative data of patients in the two groups were compared.ResultsThe age between the elderly and non-elderly group was significantly different (65.9±4.1 years vs. 54.3±4.1 years, P<0.010), and no significant difference was found between the two groups in other preoperative baseline data. There were 6 (4.8%) patients of early death, 3 (2.4%) patients of stroke and 2 (1.6%) patients of paralysis. A total of 194 stents were implanted, and the average dimeter of the stents was 33.6±1.8 mm and the average length was 199.0±6.7 mm. The non-elderly group had shorter mechanical ventilation time (31.9±41.7 h vs. 61.0±89.2 h, P=0.043) and ICU stay time (77.8±51.4 h vs. 143.1±114.4 h, P<0.001) than the elderly group. There was no significant difference in in-hospital mortality rate, reoperation rate or survival rate between the two groups (P>0.05). Follow-up time was 1-43 (22.6±10.8) months, and 3 patients were lost. There were 104 (82.5%) patients of complete thrombus formation of false lumen in stent and endoleak was reported in 11 (9.2%) patients.ConclusionType Ⅱ hybrid aortic arch repair offers an alternative approach to acute type A aortic dissection with acceptable early and mid-term clinical effects. The non-elderly patients have a similar early treatment effect to the elderly patients, but have a better mid-term outcome.
