  • Investigation of Awareness and Requirements of Care in the Elderly with Chronic Diseases in Community

    目的 了解成都市社区老年慢性病患者对关爱的感知和需求,为更好地关爱老年慢性病患者提供依据。 方法 于2011年8月-10月采用随机抽样和问卷调查的方法,对成都市玉林社区、二仙桥社区、草堂街社区和驷马桥社区的180名老年慢性病患者的关爱感知和需求进行调查,并根据调查结果提出相应对策。 结果 180例老年慢性病患者中有98.89%能感受到关爱,1.11%自觉缺乏关爱;感知到的关爱主要来源于家庭成员,占91.01%,其次来源于亲戚朋友和邻居,占7.87%,最少来源于单位同事,占1.12%。关爱需求主要为家人团聚、关心体贴、尊重理解、日常照顾和心理情感支持、帮助解决困难、给予经济资助、提供情感支持等;护理关爱需求以尊重理解排在首位,其次是慢性病日常护理、慢性病的防治、老年保健和慢性病基本知识等。 结论 加强对社区卫生服务人员的能力培训,强化尊老爱老家庭氛围和社会风气,提高老年慢性病患者的关爱感知,有效地为老年慢性病患者提供关爱,更好地促进他们的健康。

  • Enhancing Nurse′s Satisfaction by Improving the Quality of Nursing Services

    【摘要】 目的 总结开展“优质护理服务示范工程活动”以来,护士满意度提高的原因与经验。 方法 分别于2010年1月和11月采用一般情况调查表及明尼苏达工作满意度问卷短式量表调查干部/老年科的护士在开展“优质护理服务示范工程活动”前后的工作满意度。 结果 开展“优质护理服务示范工程活动”1年以来,护士的内在满意度上升了35.27%,外在满意度上升了29.25%,一般满意度上升了27%。 结论 干部/老年科通过提高护士对各岗位的价值与责任的认可,科学规划护士的职业生涯,完善科室文化建设、薪酬与激励机制使护士的职业成就感、自身价值满意度均有不同程度提高。【Abstract】 Objective To summarize the reasons and experience of enhancing nurse′s satisfaction after improving the “high-quality nursing services”. Methods A general questionnaire and Minnesota Satisfaction Questionnaire (MSQ) were used to investigate satisfactions of nurses working at the senior leader/ person′s wards before and after improving the activity on “high-quality nursing service”. Results One year later, the inner satisfactions of participates increased 35.27%, the outer satisfactions increased 29.25%, and the general satisfactions increased 27%. Conclusion Nurse′s professional achievability and the satisfaction on self-value increase after enhancing professional value and responsibility of nurse, planning reasonably professional career of nurse, and perfecting culture construction, and the salary and encourage mechanism.

  • Depression in Geratic Malignant Tumor Patient: the State and Nursing Management

    【摘要】 目的 了解老年恶性肿瘤患者的抑郁状况及引起抑郁的因素,并针对抑郁的主要因素制定多层次、全方位、科学的护理干预措施,改善老年恶性肿瘤患者的抑郁心理,提高其生活质量。 方法 对2009年12月-2010年4月183例老年恶性肿瘤患者分别应用一般资料调查表和 Brink的老年抑郁量表(GDS)进行调查。从文化程度,疾病认识程度,对社会、家庭支持的满意度,付费方式等方面进行了比较和分析。 结果 老年恶性肿瘤患者的抑郁患病率为80.87%。明显高于一般老年人及老年慢性病患者;而不同文化程度、对疾病认知程度、患者对社会、家庭支持的满意度与抑郁情绪的发生有一定的关系(Plt;0.05)。 结论 护理人员需加强对患者的疾病知识的健康教育,努力提高患者的社会支持满意度,以减轻患者的抑郁情绪。【Abstract】 Objective To explore the state and the etiology of depression in patients with geratic malignant tumor, and to develop the global and scientific nursing management for patient with geratic malignant tumor, and improve the patients′ depression and life quality. Methods A total of 183 patients with geratic malignant tumor from December 2009 to April 2010 were investigated by questionnaire survey with Brink′s geratic-depression-scale (GDS). The education level, disease′s awareness level, satisfaction level for family and scocial supports, and the payment mode were analyzed. Results Depressive prevalence in malignant tumor patients (80.87%) was much higher than that in the normal elder people (10%-15%) and in the patients with chronic disease (31.0%). Different education level, disease′s awareness level, satisfaction level for family and social supports were related to the depressive prevalence (Plt;0.05).  Conclusion Nursing faculty should enhance the health education to the patients with geratic malignant tumor, increase the satisfaction for social support and decrease their depression.

  • Research on the Nursing of Pressure Ulcer in Patients with Diabetes in Sichuan Area

    ObjectiveTo explore the best nursing interventions for advanced-age diabetes with pressure ulcer in Sichuan area. MethodsWe retrospectively analyzed the reasons for pressure ulcer and the nursing process for 37 patients with diabetes treated between March 2012 and March 2013 in our hospital. Then, we analyzed and summarized the susceptibility factors, wound nursing, nutritional support and blood sugar monitoring in all the patients. ResultsThrough intensive nursing, pressure ulcer in 36 patients were cured, and one patient died due to lung infection. ConclusionThe risk of pressure ulcer is high in advanced-age diabetes patients, and they need promising nursing. Individualized plan is necessary for them to improve their life-quality.

  • The application of WeChat software in the follow-up of patients with primary hypertension

    Objective To explore the application of WeChat software in the follow-up of patients with primary hypertension. Methods Seventy-two patients with primary hypertension treated between June and December 2014 were randomly divided into two groups, WeChat group and phone group, with 36 in each. The patients in both the two groups were followed up once a week for three months. We recorded patients’ blood pressure, heart rate, and body mass index (BMI), assessed their life style, and compared follow-up time, rate of readmission, satisfaction degree, medication compliance and follow-up loss rate between the two groups. Results There were no significant differences in blood pressure, heart rate and BMI between the two groups (P>0.05). At the end of the follow-up, there were no significant differences in blood pressure, heart rate, BMI, salt-intake compliance, medicine-intake compliance, patient satisfaction, follow-up loss rate, and readmission rate between the two groups (P>0.05), but patients in the WeChat group were less than those in the Phone group in terms of average weekly smoking, alcohol consumption, and follow-up time (P<0.05), and were better in terms of average weekly sport-time (P<0.05). Conclusion The application of WeChat software for follow-up in patients with primary hypertension can effectively improve the patients’ life style and follow-up efficiency.

  • Survey on Financial Burden of In-patients with Thyroid Diseases in Department of Endocrinology and Metabolism of West China Hospital in 2011

    Objective To investigate the financial burden of in-patients with thyroid diseases in the West China Hospital in Chengdu, Sichuan province, from January 2011 to December 2012, so as to provide baseline data for further research. Methods The data of in-patients (who had been discharged from the department of endocrinology and metabolism or discharged after being transferred to other departments for diagnosis and treatment in the West China Hospital in 2011) were collected from the Hospital Information System (HIS) of the West China Hospital, including basic information, initial diagnosis when the patients were discharged, hospital costs, the information about whether the patients had been registered the insurance in hospital, etc. We classified diseases according to ICD-10 based on the initial diagnosis when the patients were discharged on the first page of case reports. The data were input using Excel 2010 software, and statistical analysis was performed using SPSS 13.0 software. Results The results showed that: a) in 2011, 205 person-times were hospitalized in the department of endocrinology and metabolism, of which, 84 were male and 121 were female, with mean age of 45.3±15.7 years; b) for patients with thyroid diseases, median hospital stay was 10 days, the average cost of hospital stay for each patient was RMB 2 881.43 yuan, most of which was for lab tests and examination; c) the person-times of patients with hyperthyroidism was 162, accounting for 79.5% of the total of thyroid diseases, median hospital stay was 10 days, and the average cost of hospital stay was RMB 2 958.36 yuan; and d) there was no association between the number of hyperthyroidism complications and hospital stay and costs. Conclusion Thyroid diseases are a commonly-seen disease in the department of endocrinology and metabolism, of which, hyperthyroidism accounts for the most. There is no association between the number of hyperthyroidism complications and hospital stay/costs.

Results The results showed that: in 2011, 352 person-times of in-patients with hypothalamus-pituitary-adrenal gland/gonad disease as first diagnosis were hospitalized in the department of endocrinology and metabolism, of which, 139 were male and 213 were female, with mean age of 42.9±15.0 years; and b) median hospital stay was 11 days, the average cost of hospital stay for each patient was RMB 4 361.09 yuan, most of which was for lab tests, examination, and biomedicine cost. Conclusion Hypothalamus-pituitary-adrenal gland/gonad diseases are an important health problem in the department of endocrinology and metabolism in a Triple-A Hospital. Most of hospitalization costs are for lab tests, examination, and biomedicine cost.

  • Survey on Constitution and Financial Burden of Disease in In-patient Department of Endocrinology and Metabolism of West China Hospital during 2011-2012

The data were input using Excel 2010 software, and statistical analysis was performed using SPSS 13.0 software. Resultsa) During 2011-2012, in the department of endocrinology and metabolism, the top five commonly-seen diseases of in-patients were:diabetes, followed by hypothalamic-pituitary-adrenal/gonadal disease, diseases other than endocrine disorders, thyroid disease, and metabolic bone disease. b) The diseases of the top five average hospital stay included:diabetes, followed by malnutrition, metabolic bone disease, diseases other than endocrine disorders, and hypothalamic-pituitary-adrenal/gonadal disease. c) The diseases of the top five total costs of included:diabetes, followed by metabolic bone disease, diseases other than endocrine disorders, malnutrition, and other nutrients deficiency. ConclusionDiabetes and its complications are the major disease as well as the major cause of the burden of disease in the in-patient department of endocrinology and metabolism of the West China Hospital of Sichuan University.
