  • THE APPLICATION OF W SHAPE WIRE TO THE EARLY TREATMENT FOR THE METACARPAL BONY LOSS FROM WAR INJURY

    From 1989 to 1991, eighteen cases of the metacarpal bony loss from war injury were treated in Africa. Their ages ranged from 17~56 years old. Fourteen patients were injuried from gunshot; another four were injuried from blast. All of the eighteen were performed with W shape wire, and these can be done as early as 4~16 hours after the initial injuries. The author introduced the way of making W wire and the important points of the surgical technics. We suggest that the key to the successful operation is the good debridement,covertage of wound with healthy soft tissue and the good drainage.

  • BASIC RESEARCH AND CLINICAL APPLICATION OF CARTILAGE-DERIVED RETINOIC ACID-SENSITIVE PROTEIN

    Objective To know the basic research and the clinical application of cartilage-derived retinoic acid-sensitive protein (CD-RAP) in orthopedic and in other clinical fields. Methods The literature related to CD-RAP in basic research and clinical application were extensively reviewed. Results CD-RAP, as a protein, which is cartilage-specific,could be a marker of the joint diseases. It also can be used to monitor metastsais of melanoma. Conclusion CD-RAP test provides a new way to study repair of cartilage and metastsais of melanoma.

  • EFFECT OF TRANSPLANTING MARROW MESENCHYMAL STEM CELLS VIA SUBARACHNOID SPACE ON SPINAL CORD INJURY AND T CELL SUBPOPULATION IN RATS

    Objective To investigate the therapeutic effects of transplanting allogeneic marrow mesenchymal stem cells (MSCs) via subarachnoid space on spinal cord injury(SCI) and the T cell subpopulation. Methods Density gradient centrifugation was used to isolate and expand MSCs from bone marrow of 10 six-week-old SD rats. The SCI model was produced by weightbeating from 60 eight-week-old female SD rats. Forty survival SCI rats,which BBB scores were zero, were divided randomly into 2 groups:experimental group(group A) and control group(group B). In addition, 20 normal eightweekold SD ratswere used as blank group (group C). In group A, 1 ml cells suspention containing MSCs(the 6th generation, 2×106/ml) was injected via subarachnoid space. Ingroup B, equal volume of L-DMEM was injected in the same way. The BBB score was obtained after 1st,2nd and 3rd weeks of injection. At the same time,T cell subpopulation was detected by flow cytometry. Results The BBB score in group A was better than that in group B, but fewer than that in group C in the 3rd week. CD4+T cells in group A were less than those in groups B and C in the 1st, 2nd, and 3rd weeks. CD8+T cells in group A were less than those in groups B and C in the 2nd and 3rd weeks. The ratio of CD4+/CD8+T cells in group A was less than those in groups B and C in the 1st week. Above differences showed statistically significant difference(P<0.05). However, there were no statistically significant differences in the ratio of CD4+/CD8+T cells between group A and groups B, C in the 2nd and 3rd weeks (P>0.05). Conclusion The above results suggest that allogeneic MSCs transplantation via subarachnoid space is beneficial to SCI to some extend, do not result in rejection in vivo. Furthermore, it can lead to immunosuppression in short time. So, it provides clues to apply MSCs to treat SCI and other diseases.

  • EXPLORATION OF SURGICAL APPROACHES OF COMPLEX TIBIAL PLATEAU FRACTURES

    Objective To evaluate the surgical treatment and its results of complex tibial plateau fractures.Methods From May 2003 to November 2004, 24 cases of complex tibial plateau fractures were treated by two-incision approaches. There were 16 males(including 1 case of bilateral), and8 females, with mean age of 40.6 years(21-65 years), eleven fractures occurred in left extremity and 14 in the right. No patients suffered open fractures. Fracture was caused by fall from height in 10 cases, by traffic accident in 12 cases and by another causes in 2 cases. According to Schatzkerclassification, type Ⅴ fracture was found in 14 extremities,type Ⅵ in 11 extremities. All cases were operated with twoincision approaches and double-plates fixation. The operation was performed 6-15 days after injury with mean 10.4 days. Results All cases were followed up from 12 to 23 months (mean 15.6 months). Time of the bone healing was 69 months, with mean 7.4 months. No reduction of anatomy was achieved because of comminuted fractures of articular surfaces in 2 limbs. The line of the lower extremity did not recover because of metaphysis comminution in 1 limb, which led to knee varus. Epidermal necrolysis was observed in 3 limbs, and the wound healedafter change dressing. The function of the knee wasevaluated according to Rasmussen’s criteria, the results were excellent in 10limbs, good in 8 limbs, fair in 3 limbs, and poor in 4 limbs. Conclusion The twoincision approaches reduced the complication of softtissueinjury, which is conductive to reduction of articular surface. Double-plates provide a sufficient internal fixation. It can permit early motion of knee joint. Therefore, two-incision approach are an effective surgical approach of treatment of complex tibial plateau fractures.

  • THE EFFECT OF PHENYTOIN IN HEALING OF FRACTURE OF RABBITS

    It was reported that the systemic use of phenytoin could promote healing of fracture. In order to observe the effect of local application of phenytoin in the healing of fracture, the experiment was performed. Seventy-two rabbits were divided into three groups. Fractures were created on both radius of all rabbits. Group 1, intraperitoneal injection of phenytoin with a dosage of 50 mg/kg per day; Group 2, local use of phenytoin with a dosage of 40 mg/kg was injected in the fracture site every seventy-two hours, and Group 3, injection mormal saline of in the control group. Eight rabbits in each group were sacrificed in the 9th, 16th and 30th days after operation respectively. By X-ray excuiualtion, the healing of fracture was observed. Dry and wet weights of the callus were determined. After HE and Mallory’s stain, the samples were examined under microscope. Results showed that both local and systemic use of phenytoin promoted healing of fracture. The effects of phenytoin in the two groups were the same and had no significant difference.

  • PROTEOMICS STUDY ON EFFECT OF BASIC FIBROBLAST GROWTH FACTOR LONG CIRCULATION LIPOSOME ON SPINAL CORD TRACTION INJURY IN RATS

    ObjectiveTo explore the possible active mechanism of the basic fibroblast growth factor (bFGF) long circulation l iposome (LCL) (bFGF+LCL) on spinal cord traction injury in rats at the level of proteomics. MethodsTwenty Sprague Dawly rats were randomly divided into groups A and B, 10 rats in each group. The models of spinal cord traction injury was established at T12-L3 spines. The rats were not treated in group A, and the rats were treated with bFGF+LCL (20μg/ kg) in group B. At 3 weeks after operation, the rats were sacrificed for harvesting T13-L2 spinal tissue specimens. The protein was extracted and quantified in the spinal tissue firstly. The proteins from spinal tissue were separated by two-dimensional gel electrophoresis and identified by mass spectrometry. The different expression profiling was established in each group, and the differentially expressed protein was determined by comparing the level of each spot with gel imaging software and manually. The proteins were identified by nano ultra-high performance liquid chromatography-electrospray tandem mass spectrometry (NanoUPLC-ESI-MS/MS), and the proteins were classified. ResultsThe differentially expressed protein spots were found in 2 groups. Compared with group A, 4 spots were up-regulated and 6 were down-regulated in group B. NanoUPLC-ESI-MS/MS results showed that 18 significant proteins were identified in 26 differentially expressed proteins, including 4 apoptosis-related proteins, 3 nerve signal transduction related proteins, 7 proteins involved in metabolism, 1 unknown function protein, and 3 unnamed proteins. ConclusionThe differentially expressed proteins are found in spinal cord traction injury of rats treated with bFGF+LCL. bFGF+LCL can affect the proteins expression in rats with spinal cord traction injury. The possible active mechanism is that it has protective and repair effects on injured spinal cord by nerve signal transduction, and regulation of nerve cells apoptosis and metabolism.

  • CORRELATION ANALYSIS BETWEEN RESIDUAL DISPLACEMENT AND HIP FUNCTION AFTER RECONSTRUCTION OF ACETABULAR FRACTURES

    【Abstract】 Objective To investigate the relationships between residual displacement of weight-bearing and non weight-bearing zones (gap displacement and step displacement) and hip function by analyzing the CT images after reconstruction of acetabular fractures. Methods The CT measures and clinical outcome were retrospectively analyzed from 48 patients with displaced acetabular fracture between June 2004 and June 2009. All patients were treated by open reduction and internal fixation, and were followed up 24 to 72 months (mean, 36 months); all fractures healed after operation. The residual displacement involved the weight-bearing zone in 30 cases (weight-bearing group), and involved the non weight-bearing zone in 18 cases (non weight-bearing group). The clinical outcomes were evaluated by Merle d’Aubigné-Postel criteria, and the reduction of articular surface by CT images, including the maximums of two indexes (gap displacement and step displacement). All the data were analyzed in accordance with the Spearman rank correlation coefficient analysis. Results There was b negative correlation between the hip function and the residual displacement values in weight-bearing group (rs= — 0.722, P=0.001). But there was no correlation between the hip function and the residual displacement values in non weight-bearing group (rs=0.481, P=0.059). The results of clinical follow-up were similar to the correlation analysis results. In weight-bearing group, the hip function had b negative correlation with step displacement (rs= — 0.825, P=0.002), but it had no correlation with gap displacement (rs=0.577, P=0.134). Conclusion In patients with acetabular fracture, the hip function has correlation not only with the extent of the residual displacement but also with the location of the residual displacement, so the residual displacement of weight-bearing zone is a key factor to affect the hip function. In patients with residual displacement in weight-bearing zone, the bigger the step displacement is, the worse the hip function is.

  • ESTABLISHMENT OF TRACTIVE SPINAL CORD INJURY MODEL IN RATS WITH A NOVEL SPINAL DISTRACTOR

    Objective To develop a tractive spinal cord injury model in rats with a novel spinal distractor so as to supply the rel iable animal model for researching the pathological mechanism and rehabil itation treatment of tractive spinal cordinjury. Methods A novel spinal distractor was prepared based on previous study. Sixty adult Sprague Dawley rats (weighing 250-300 g) were randomly divided into 5 groups, 12 rats in each group. T12-L3 spinal structures in the rear area were exposed and then T13-L2 spinal cords were revealed via dual laminectomy and kept integrity. In group A, a novel spinal distractor was placed without distraction; in groups B, C, D, and E, the T12-L3 spines were tracted with a novel spinal distractor which put on transverses process of T12-L3 vertebrae. During the tractive period, the somatosensory evoked potential (SEP) was used to monitor spinal cord function. The SEP ampl itudes descended 50% and kept distracting for 5 minutes in group B and for 10 minutes in group C, and descended 70% and kept distracting for 5 minutes in group D and for 10 minutes in group E, respectively to establ ish the tractive spinal cord injury model of T11-L2. The improved combine behavioral score (ICBS) was recorded at 1 and 7 days after injury in 6 rats of each group. The T13-L2 spinal tissue specimens were harvested for the morphological observation by HE and Nissl’s staining and for neurons counting. Results In group A, the ICBS score was 0 at 1 and 7 days after operation, showing significant difference when compared with the scores of the other groups (P lt; 0.05). The ICBS scores of groups D and E were significantly higher than those of groups B and C (P lt; 0.05). Edema and hemorrhage were observed in spinal cord surface and normal morphological structures were destroyed at different extent in groups B, C, D, and E at 1 day. There were adherence and congestion between spinal cord surface and peripheral issue without luster at 7 days, and dura depression was observed at the injury section, especially in group E. Necrosis and dissolution occurred in some neurons, and Nissl body structure dissolved or disappeared in groups B, C, D, and E. The neuron counting gradually decreased in accordance with the aggravation of injury in groups B, C, D, and E, showing significant difference when compared with group A (P lt; 0.05). Significant differences in neuron counting were found among groups B, C, D, and E (P lt; 0.05). Conclusion The tractive spinal cord injury model in rats can be successfully establ ished with novel spinal distractor, and the model establ ished by SEP ampl itude descending 70% and keeping distracting for 10 minutes is more suitable for study in tractive spinal cord injury.

  • 踝关节骨折的治疗现状及进展

    踝关节骨折的发生率正随着人口老龄化的增加而逐步增长,其中,跌倒伤是踝关节骨折的主要受伤机制。关于不稳定性踝关节骨折,目前的治疗金标准仍是切开复位内固定术。关于三踝骨折的处理,应先行内外踝解剖复位坚强内固定后再行后踝处理,而后踝骨折的固定与否,由后踝骨折块的大小或胫距关节的稳定性决定。若存在踝关节上方5 cm以外的腓骨骨折或踝关节上方3.0~4.5 cm以外的腓骨骨折合并不能修复的内踝损伤应行下胫腓联合螺钉固定。若踝关节骨折合并严重的软组织损伤且明确的固定方式未定之前,临时性跨踝外支架是一种明智的选择。踝关节术后因创伤性关节炎引起的持续疼痛,去神经支配可望成为未来一种尝试性的治疗方式。踝关节镜为新型微创手术的代表,简单骨折类型如Cedell骨折、撕脱性骨折、创伤后的异化骨,都能通过该方式治疗。关于小于关节面25%的后踝骨折块是否应该固定以及下胫腓联合螺钉使用的适应证的判断有待于未来进一步的生物力学及临床研究的探索。
