  • 急性间质性肺炎的治疗进展

    急性间质性肺炎( acute interstitial pneumonia, AIP) 是一种突发起病、快速进展为呼吸衰竭并需机械通气的间质性肺疾病, 其病理特点为弥漫性肺泡损伤。最早在1944 年Hamman和Rich[ 1] 报告了一组以暴发起病、进展迅速并短时间内死亡为特点的病例, 在病理上主要表现为肺泡间隔增厚水肿、通明膜形成及肺间质广泛纤维增生, 此后有人称此为Hamman-Rich 综合症。1986 年Kuzenstein 等[ 2 ] 报道了8 例与Hamman-Rich 综合症相类似的病例, 并正式更名为急性间质性肺炎。2002 年ATS/ERS 将AIP列为特发性间质性肺炎的一个亚型。AIP 确切的病因及发病机制尚不清楚, 因其临床表现及病理特点类似于ARDS, 有人认为AIP是特发性ARDS[ 3] 。目前AIP 尚无有效规范的治疗方法, 临床上主要以大剂量糖皮质激素的应用及机械通气支持治疗为主。现结合有限的文献资料对AIP 的治疗作进一步探讨。

  • PROGRESS IN TREATMENT OF TERRIBLE TRIAD OF ELBOW

    Objective To review the injury mechanism and the treatment progress of terrible triad of the elbow, and to analyze the direction of further research. Methods Related literature concerning terrible triad of the elbow was extensively reviewed and comprehensively analyzed. Results The main treatment of terrible triad of the elbow is operation. The ultimate goal of treatment is to reconstruct sufficient stability of the elbow. The treatment includes fixation of the coronoid by suture, screw or plate; fixation of radial head by screw and plate, partial or complete replacement of the radial head; fixation of lateral collateral ligament and the medial collateral ligament by bone suture or anchors and the application of the external fixator. These surgical treatments have their own indications and advantages, most get satisfactory results. Conclusion Generally, surgery is needed to maintain the stability of the elbow for patients of terrible triad elbow. However, medial ligament repair or not, the choice of approach, and mechanism of injury still need further study.

  • TREATMENT PROGRESS OF EXTREMITY SKIN AVULSION

    Objective To review the treatment methods of the extremity skin avulsion, and to point out the problems at present as well as to predict the trend of development in this field. Methods Domestic and abroad l iterature concerning the treatment methods of the extremity skin avulsion in recent years was extensively and thoroughly analyzed. Results The timely and accurate evaluation of viabil ity and blood circulation of avulsed skin was the key to treat extremity skin avulsion. At present, there were many methods to treat extremity skin avulsion and the vacuum seal ing drainage (VSD) technology played an important role in the treatment of skin avulsion. Artificial skin had been used to treat skin avulsion. Conclusion The conventional method to treat skin avulsion is rigorous debridement, defatting of the avulsed flap, planting back avulsed skin flap. The combined use of the VSD technology helps skin avulsion treatment. It may be a trend that artificial skin will be used in treating extremity avulsion.

  • 外周T细胞淋巴瘤的治疗进展

    T细胞淋巴瘤(TCL)是来源于T淋巴细胞的一类恶性淋巴肿瘤,目前尚无统一的最佳治疗方法。虽近年有新的治疗药物,但都只基于少量的病例或小规模的Ⅱ期临床试验。美国国立卫生研究院对于非皮肤型外周TCL的原则为一线治疗方案首选临床试验。且一线巩固方案除低危患者外,应考虑行大剂量化学治疗(化疗)联合造血干细胞移植。另外由于间变性淋巴瘤激酶基因阳性的间变大细胞淋巴瘤预后较好,对处于完全缓解期者无需行干细胞移植。二线治疗方案中,对于耐受大剂量化疗的患者首选临床试验。对于不能耐受大剂量化疗的患者首先考虑参加临床试验,其次使用新药(如阿仑单抗、硼替佐米、地尼白介索、吉西他滨等)及放射治疗,或根据原癌基因或抑癌基因进行治疗。现就其治疗进展作一综述。

  • Progress of treatments for old calcaneal fractures

    Objective To review the progress of treatments for old calcaneal fractures. Methods The related literature of treatments for old calcaneal fractures were reviewed and analyzed from the aspects such as the pathoanatomy, classifications, and surgical treatments. Results Old calcaneal fractures are common in clinical, the anatomical changes are very complicated. In addition to classical open reduction and internal fixation, arthrodesis, and osteotomy, techniques of minimally invasive operation, external fixator, and three-dimensional printing are more and more widely applied, treatments for old calcaneal fractures nonunion have also received increasing attention. Conclusion Although the perfect strategy for treating old calcaneal fractures has not yet been developed, great progress has been achieved recently, the personalized therapy need to be further studied, and therapies for the early stage old calcaneal fractures and old calcaneal fractures nonunion need to be further explored.

  • Advances in cervical artificial disc replacement for cervical spondylosis

    Anterior cervical decompression and fusion (ACDF) treatment for cervical spondylosis has been more than half a century, and achieved good clinical results. However, with the continuous extension of follow-up time, the fusion segment-associated postoperative complications emerged gradually. Reserved cervical stability and activity, the concept of non-fusion was born. As a non-fusion technique, cervical artificial disc replacement (CADR) developed rapidly. With the continuous development of artificial prosthesis materials and design concepts, and specification and proficiency of surgical procedures, CADR has achieved better short- and mid-term clinical efficacy than ACDF. Compared with ACDF, the main advantages of CADR are that the postoperative recovery is quick, the activity and stability of cervical vertebra are maintained, the height of cervical intervertebral space is restored, and the stress of adjacent segments and the rate of surgical renovation are reduced. In clinical work, as an emerging technology, CADR requires spine surgeons to control the surgical indications, contraindications, and patients’ conditions strictly. This article reviews the research progress of CADR in order to provide new ideas for clinical treatment of cervical spondylosis.

  • Progress in surgical treatment of aortic regurgitation caused by Takayasu arteritis

    Takayasu arteritis (TA) is a primary, chronic, non-specific, inflammatory disease of the aorta and its larger branches. The pulmonary artery trunk and its branches could be impacted by TA, which could cause stenosis or occlusion of lesion vessels. TA also affects the normal function of the aortic valve and other heart valves, mainly due to valvular insufficiency. Aortic regurgitation caused by TA is mainly treated by surgical operation. In this review, the examination technique, operation timing, operation method and prognosis of aortic valve involved in TA are discussed systematically.
