  • Progress in hydrogel implantation in treatment of heart failure

    Heart failure affects quality of life and life expectancy of tens of millions of individuals. There are no available economic and effective treatments for end-stage heart failure. Hydrogels are novel tissue engineering materials, which have the potential to ameliorate myocardium remodeling, increase cardiac output, improve quality of life and prolong life span by implantation into myocardium. The preclinical experiments and clinical trials have greatly explored the function of hydrogels in heart failure. In this review, we summarized the approaches of implantation, mechanism and clinical outcomes of the hydrogels.

  • Application of artificial intelligence in prevention and treatment of cardiovascular diseases

    With the development of science and technology, artificial intelligence is gradually integrated into every aspect of daily life and the medical field is no exception. Cardiovascular diseases, as the first killer to global health, is the focus of new technologies and methods. In this study, the application of computer vision, natural language processing, robotics and machine learning in cardiovascular disease studies were reviewed and prospected, in order to promote the development for new technologies and applications in the future.

  • Clinical efficacy of trans-jugular transcatheter closure of atrial septal defect solely under echocardiography guidance in infants

    ObjectiveTo explore the key points, indications and safety of trans-jugular transcatheter closure of atrial septal defect (ASD) in infants.MethodsThe clinical data of 53 infants with ASD from January 2017 to May 2019 in our hospital were retrospectively reviewed. There were 20 males and 33 females with the age of 1.2 (0.5-2.9) years, and body weight of 9.0 (6.8-10.6) kg. The ASD diameter was 9.8 (8.0-14.0) mm. Thirty-one patients were treated under the guidance of transesophageal ultrasound (TEE), and the other 22 patients under the guidance of transthoracic echocardiography (TTE). We used the steerable curved sheath through the internal jugular vein under the guidance of echocardiography, and the average occluder size was 13.5±4.5 mm.ResultsAll of the 53 patients were successfully occluded, and none of them changed to radiation-guided or transthoracic surgery. Postoperative hospital stay was 3.35±0.70 d. There was no complication such as peripheral vascular injury, occluder malposition or displacement, serious arrhythmia or pericardial effusion. The patients were followed up for 14.3±5.1 months without arrhythmia, residual shunt, occluder malposition or displacement or thrombus.ConclusionEchocardiography-guided trans-jugular closure of ASD for infants with low weight and large ASD shunt or patients with inferior vena cava abnormalities not suitable for femoral vein treatment, not only overcomes the radiation risk of radiation guidance, but also maintains the advantages of minimal invasiveness and safety, providing a new treatment option for such patients.

  • Interpretation of the section of congenital heart diseases in Annual Report on Cardiovascular Health and Diseases in China (2019)

    Congenital heart disease (CHD) is a birth defect with the highest incidence in China. In September 2020, Annual Report on Cardiovascular Health and Diseases in China (2019) was published by National Center for Cardiovascular Diseases. The present situation of prevention and treatment of CHD was briefly summarized in this report. In this paper, we presented the main opinions of Annual Report on Cardiovascular Health and Diseases in China (2019), supplied with relevant data, such as epidemiology, surgical treatment and interventional treatment of CHD, to further expand and explain this report.

  • Artificial intelligence application in the perioperative period of cardiovascular surgery

    With the advancement and development of computer technology, AI-based medical assistance decision-making systems are widely used in clinical settings. In the perioperative period of cardiovascular surgery, artificial intelligence can be applied to preoperative diagnosis, intraoperative, and postoperative risk management. This article introduces the application and development of artificial intelligence during the perioperative period of cardiovascular surgery, including preoperative auxiliary diagnosis, intraoperative risk management, postoperative management, and full process auxiliary decision-making management. At the same time, it explores the challenges and limitations of the application of artificial intelligence and looks forward to the future development direction.

  • Application of novel wire system for echo-guiding percutaneous atrial septal defect closure

    ObjectiveTo evaluate the efficacy and safety of a novel wire for echo-guiding percutaneous atrial septal defect (ASD) closure.MethodsTwenty sheep were randomly divided into two groups, an experimental group and a control group (n=10 in each group). In both groups, an animal model of ASD was established by transthoracic balloon dilatation, and percutaneous closure of atrial septal defect was finished only by echography guiding. The total procedure time, passing time (time needed for the guide wire to enter the left atrium), frequency of delivery sheath dropping into right atrium, frequency of arrhythmias and rate of complications were compared between the two groups.ResultsASD model was uneventfully established in all animals. ASD devices were successfully implanted in all sheep. Compared with the control group, the experimental group had significantly less mean procedure time and mean passing time, lower frequency of guide wire dropping into right atrium and frequency of atrial and ventricular premature contractions, respectively (15.36±4.86 min vs. 25.82±7.85 min, 2.69±0.82 min vs. 5.58±3.34 min, 0% vs. 40.0%, 4.41±2.61 vs. 9.60±3.68, 0.75±0.36 vs. 1.34±0.68; all P<0.05) . Two groups did not have serious complications within postoperative one week.ConclusionThe novel wire system is not only safe and effective in the treatment of ASD occlusion, but also easy to operate.

  • Recent clinical development of minimally invasive closure for atrial septal defect

    Atrial septal defect (ASD) is a congenital heart disease that causes blood communication between the left and right ventricles due to partial atrial septal tissue defects, accounting for about 13% of all heart malformations. Secondary ASD is the most common type of ASD and can generally be treated with minimally invasive closure. At present, the commonly used minimally invasive methods in clinical practice mainly include X-ray-guided percutaneous occlusion, transesophageal ultrasound-guided transthoracic occlusion and ultrasound-guided percutaneous occlusion. This review focuses on the basic research process of occluder materials, and advantages and disadvantages of three different surgical methods.

  • Current situation and prospect of interventional therapy for congenital heart disease in China

    Transcatheter intervention for congenital heart disease has been developed for 40 years in China, it has experienced the transition of learning to self-dependent innovation, and witnessed the intervention therapy system starting from scratch and gradually reaching the top level and gaining high achievements in the world scale. With the continuous development of interventional technology and devices, cutting-edge ideas and viewpoints are constantly discussed. This review summarized the development of intervention techniques and devices, hoping to provide some experience for the further development of transcatheter interventions for congenial heart disease.
