  • CT of Inverted Papilloma in the Nasal Cavity and Nasal Sinuses

    目的:着重探讨鼻腔及鼻窦内翻性乳突状瘤CT表现特征及诊断价值。材料和方法:收集经手术和病理确诊的鼻腔及鼻窦内翻性乳突状瘤65例CT资料,进行回顾性研究。结果:65例IP的CT表现特征,以鼻中道为中心的一侧鼻窦和/或鼻腔内不规则软组织肿块。其中,仅鼻腔受累者18例,一侧鼻腔及鼻窦均受累者47例。鼻腔及鼻窦有膨大并骨质破坏者43例。软组织肿块强化者23例。术后IP复发者13例(20%);恶变者9例(14.3%)。结论:CT检查对IP的病变部位、范围、程度等能准确确定,可为手术治疗提供重要依据。

  • Association of Myocardial Enzymes’ Changes with Critical Illness Score in Neonatal Infection

    Objective To evaluate the relationship between the severity of neonatal infection and the activity change of cardiac enzyme. Methods A total of 102 infectious neonates in NICU were evaluated with neonatal serious illness scoring system and then divided into non-critical, critical, and extremely-critical groups. The activity of such serum myocardial enzymes as AST, ALT, LDH, CK, and CKMB was tested in the first 24 hours on admission. Comparison between groups and analyses were conducted. Resultes Myocardial enzyme of the critical and extremely-critical groups was obviously higher than that of the non-critical group (Plt;0.01). Conclusion  The serious illness score of infectious neonates is closely associated with the activity change of myocardial enzyme and the level of serum myocardial enzymes has certain advantages in judging the myocardial injury and the disease severity of neonatal infection.

  • Integrated analysis of a gene correlation network identifies critical regulation of fibrosis by lncRNAs and TFs in idiopathic pulmonary fibrosis

    ObjectiveTo investigate the key long non-coding RNAs (lncRNAs) and transcription factors (TFs) in idiopathic pulmonary fibrosis (IPF) by Bioinformatics analysis.MethodsBioinformatics analysis of three gene expression profiles from the Gene Expression Omnibus dataset (GSE2052, GSE44723, and GSE24206), including 42 IPF and 21 normal lung tissues, was performed in this study. Subsequently, differentially expressed genes (DEGs) were filtered, and key genes involved in signaling pathways and the DEG-associated protein-protein interaction network (PPI) were further analyzed. The filtered genes expression was determined by real-time quantitative polymerase chain reaction analysis.ResultsA total of 8483 aberrantly expressed genes were screened, and 29 overlapping genes were identified among these three datasets. A significant enrichment analysis of DEG-associated functions and pathways was further performed. A total of 18 modules were obtained from the DEG PPI network, and most of the modules were involved in polyubiquitination, Golgi vesicle transport, endocytosis and so on. The key genes were obtained through hypergeometric testing, and most of the corresponding genes were closely associated with ubiquitin-mediated proteolysis, the spliceosome, and the cell cycle. These differential expressed genes, such as lncMALAT1, E2F1 and YBX1, were detected in the peripheral blood of IPF patients when compared with those normal control subjects.ConclusionlncMALAT1, E2F1 and YBX1 might be possible regulators for the pathogenesis of idiopathic pulmonary fibrosis.

  • The amount of endothelial progenitor cells at different stages of diabetic retinopathy in patients with type 2 diabetes mellitus

      Objective To observe the amount of endothelial progenitor cells (EPCs) at different stages of diabetic retinopathy (DR) in patients with type 2 diabetes mellitus (DM). Methods Sixty patients with type 2 DM were divided into no DR (NDR) group, non-proliferative DR (NPDR)group and proliferative DR (PDR)group according to the examination of fundus and fundus fluorescein angiography, 20 patients in each group. Twenty healthy people were collected as the control group. 6 ml blood samples were taken from all the subjects, and then the EPCs contents in peripheral blood were detected by flow cytometry. Results  The EPCs contents in peripheral blood of the control, NDR, NPDR and PDR group were (0.0179plusmn;0.0047)%, (0.0151plusmn;0.0086)%, (0.0123plusmn;0.1137)%, (0.0316plusmn;0.0 294)%. The EPCs contents in peripheral blood of the PDR group was significantly higher than those in others (chi;2=43.780, P<0.05); the EPCs contents in peripheral blood of the NDR and NPDR group were slightly lower than that in the control group (chi;2=5.244, P=0.73); the EPCs contents in peripheral blood of the NPDR group was lower than that in the NDR group (chi;2=6.016, P=0.12). Conclusion The EPCs contents in peripheral blood decreases in NDR, NPDR patients, while significantly increases in PDR patients.

  • Research progress of tibial-graft fixation methods on anterior cruciate ligament reconstruction

    Objective To review the studies about the tibial-graft fixation methods on anterior cruciate ligament (ACL) reconstruction, in order to provide clinical reference. MethodsThe literature about the tibial-graft fixation methods on ACL reconstruction at home and abroad was extensively reviewed, and the factors that affect the selection of fixation methods were summarized. Results The knee flexion angle, graft tension, and graft fixation device are mainly considered when the tibial-graft was fixed on ACL reconstruction. At present, the graft is mainly fixed at 0°/30° of knee flexion. The study shows that the knee joint is more stable after fixed at 30°, while the incidence of knee extension limitation decrease after fixed at 0°. In terms of graft tension, a good effectiveness can be obtained when the tension level is close to 90 N or the knee flexion is 30° to recover the affected knee over-restrained 2 mm relative to the healthy knee. In terms of the graft device, the interference screw is still the most commonly used method of tibial-graft fixation, with the development of all-inside ACL reconstruction in recent years, the cortical button fixation may become the mainstream. Conclusion Arthroscopic reconstruction is the main treatment of ACL rupture at present. However, there is no optimal fixation method for the tibial-graft, the advantages and disadvantages of each fixation methods need to be further studied.

  • Comparative study of I.D.E.A.L. technique and transtibial technique in anterior cruciate ligament reconstruction

    Objective To compare the effectiveness of I.D.E.A.L technique and transtibial (TT) technique in anterior cruciate ligament (ACL) reconstruction. Methods A clinical data of 60 patients with ACL injury, who were admitted and met the selection criteria between January 2020 and September 2022, was retrospectively analyzed. All patients underwent arthroscopic ACL reconstruction with autologous tendon. During operation, the femoral tunnel was prepared by using I.D.E.A.L technique in 30 cases (I.D.E.A.L group) and using TT technique in 30 cases (TT group). There was no significant difference in baseline data such as age, gender, body mass index, cause of injury, injured side, interval from injury to operation, constituent ratio of combined cartilage and meniscus injury, and preoperative Lysholm score, International Knee Documentation Committee (IKDC) score, visual analogue scale (VAS) score, anterior tibial translation difference, and Blumensaat angle between the two groups (P>0.05). The length of hospital stay and the occurrence of early and late complications were recorded. During follow-up, the Lysholm score, IKDC score, and VAS score were used to evaluate knee joint function and pain degree, and the anterior tibial translation difference was measured. MRI reexamination was performed to observe the healing of the graft, and the signal to noise quotient (SNQ) values of the femoral end, middle section, and tibial end of the graft, as well as the Blumensaat angle of the knee joint were measured. The differences in tibial anterior translation difference and Blumensaat angle before and after operation (change values) were calculated and compared between the two groups. Results The incisions in both groups healed by first intention after operation, and there was no significant difference in the length of hospital stay between the two groups (P>0.05). All patients were followed up 12-18 months, with an average of 14.9 months. The Lysholm score and IKDC score of the knee joint in both groups after operation increased when compared with those before operation, and the VAS score decreased. Compared to preoperative scores, except for the VAS score of the TT group at 1 week after operation (P>0.05), there were significant differences in all scores at different time points postoperatively in the two groups (P<0.05). The above scores in both groups showed a further improvement trend with the prolongation of time after operation. There were significant differences in Lysholm score and VAS score among 1 week, 1 month, 3 months, 6 months, and 12 months after operation in the two groups (P<0.05). The IKDC score of both groups at 1 month after operation was significantly different from that at 1 week after operation (P<0.05). At 1 week after operation, the Lysholm score and IKDC score in the I.D.E.A.L group were significantly higher than those in the TT group (P<0.05), and the VAS score was significantly lower (P<0.05); there was no significant difference between the two groups at 1, 3, 6, and 12 months after operation (P>0.05). At 12 months after operation, the anterior tibial translation differences in both groups were significantly lower than those before operation (P<0.05); and the change value in the I.D.E.A.L group was significantly higher than that in the TT group (P<0.05). The incidences of early and late complications in the I.D.E.A.L group were significantly lower than those in the TT group (P<0.05). At 12 months after operation, MRI examination showed that the grafts of the knee joint in both groups survived well, and the Blumensaat angles of both groups were significantly smaller than those before operation (P<0.05). The change value of the Blumensaat angle in the I.D.E.A.L group was significantly higher than that in the TT group (P<0.05). The SNQ values of the femoral end, middle section, and tibial end of the graft in the I.D.E.A.L group were significantly higher than those in the TT group (P<0.05). Conclusion The early effectiveness of ACL reconstruction by using the I.D.E.A.L technique is better, the knee joint is more stable, and the incidence of postoperative complication is lower. However, the maturity of the graft after reconstruction using the TT technique is higher.
