    2005年的癫痫概念性定义:一种具有持久性产生癫痫样发作倾向的大脑疾患。实际上,这个定义通常用于间隔24 h以上的两次非诱发性痫性发作。国际抗癫痫联盟(ILAE)接受了一个特别工作组的建议,在没有满足两次非诱发性发作标准的特殊情况下,改变了实用性定义。特别工作组建议,在符合下列任何一项条件时,癫痫可以考虑为一种脑部疾病:①至少两次间隔>24 h的非诱发性(或反射性)痫性发作;②一次非诱发性(或反射性)痫性发作,在未来10年内,再次发作的可能性与两次非诱发性发作后的再发可能性相当(至少60%);③癫痫综合征的诊断,具有年龄相关的癫痫综合征的个人,目前已经超过该患病年龄,或保持无发作至少10年,并停用抗癫痫药物至少5年,这类癫痫被认为是可以解除的。"解除"与传统上"缓解"或"治愈"的观点不同。不同的实用性定义的形成服务于各种不同的目的。这次修订癫痫定义所使用的术语与常用的术语一致。

  • Design of Electrocardiogram Signal Generator Based on Typical Electrocardiogram Database

    Using LabVIEW programming and highspeed multifunction data acquisition card PCI6251, we designed an electrocardiogram (ECG) signal generator based on Chinese typical ECG database. When the ECG signals are given off by the generator, the generator can also display the ECG information annotations at the same time, including waveform data and diagnostic results. It could be a useful assisting tool of ECG automatic diagnose instruments.

  • Near-infrared Spectra Noninvasive Measurement Method of Blood Oxygen Saturation Based on the "M+N" Theory

    M+N theory can be used as a method to improve the prediction accuracy in spectral analysis. The measured component, M kinds of non-measurement component, and N kinds of outside interference are induced into the entire measuring system, with the impact of "M" factors and "N" factors on the measurement accuracy considered systematically and comprehensively. Our human experiment system testing blood oxygen saturation based on "M+N" theory has been established. Dynamic spectrum method was used to eliminate the effects of different persons and different measuring parts which belonged to the system error of "N" factors. And then the D-value estimation was used to eliminate the effects of motion pseudo signal which belonged to the random error of "M" factors. Sixty two groups of valid data were obtained. The prediction model of blood oxygen saturation was built based on partial least squares regression method. The correlation coefficient and relative error were 0.796 8 and ±0.026 6, while the result of oximeter was 0.595 7 and relative error was ±0.076 0, respectively. The results show that the prediction accuracy of the measurement method based on the "M+N" theory is much higher than that of the oximeter.

  • Testing Method of Human Body's Current Threshold for Perception Based on EEG Analysis

    Electric and electronic products are required to pass through the certification on electrical safety performance before entering into the market in order to reduce electrical shock and electrical fire so as to protect the safety of people and property. The leakage current is the most important factor in testing the electrical safety performance and the test theory is based on the perception current effect and threshold. The traditional method testing the current threshold for perception only depends on the sensing of the human body and is affected by psychological factors. Some authors filter the effect of subjective sensation by using physiological and psychological statistical algorithm in recent years and the reliability and consistency of the experiment data are improved. We established an experiment system of testing the human body's current threshold for perception based on EEG feature analysis, and obtained 967 groups of data. We used wavelet packet analysis to detect α wave from EEG, and used FFT to do spectral analysis on α wave before and after the current flew through the human body. The study has shown that about 97.72% α wave energy changes significantly when electrical stimulation occurs. It is well proved that when the EEG feature identification is applied to test the human body current threshold for perception, and meanwhile α wave energy change and human body sensing are used together to confirm if the current flowing through the human body reaches the perception threshold, the measurement of the human body current threshold for perception could be carried out objectively and accurately.

  • Two-incision Approach for Video-assisted Thoracoscopic Sleeve Lobectomy for Lung Cancer

    ObjectiveTo discuss the experiences of video-assisted thoracoscopic surgery (VATS) sleeve lobectomy with bronchoplasty for lung cancer, using only two incisions. MethodWe retrospectively analyzed the clinical data of 16 patients of VATS sleeve lobectomy with bronchoplasty in our hospital between January 2013 and January 2014. There were 14 males and 2 females at age of 55 (39-70) years. The clinical effect was analyzed. ResultsThe patients underwent sleeve lobectomy with bronchoplasty were no severe postoperative complications. Median operative time was 183 min. Median bronchial anastomosis time was 39 min. Median blood loss was 170 ml. The result of pathological examination showed 11 patients with squamous cell carcinomas and 5 patients with adenocarcinoma. Median postoperative chest tube drainage duration was 4.5 days. Median hospital stay was 6.9 days. ConclusionVATS sleeve lobectomy with bronchoplasty is a feasible and safe surgical approach, using only two incisions.

  • Clinical characteristics and influencing factors of lower respiratory tract infection of Acinetobacter baumannii in respiratory intensive care unit

    ObjectiveTo investigate clinical characteristics and influencing factors of lower respiratory tract infection of Acinetobacter baumannii (AB-LRTI) in respiratory intensive care unit (RICU).MethodsClinical data were collected from 204 RICU patients who were isolated Acinetobacter baumannii (AB). The bacteriological specimens were derived from sputum, bronchoscopic endotracheal aspiration, bronchoalveolar lavage fluid, pleural effusion and blood. The definition of bacterial colonization was based on the responsible criteria from Centers for Disease Control and Prevention/National Medical Safety Network (CDC/NHSN). The patients were divided into three groups as follows, AB colonization group (only AB was isolated, n=40); simple AB-LRTI group (only AB was isolated and defined as infection, n=63), AB with another bacteria LRTI group (AB and another pathogen were isolated simultaneously, n=101). The epidemiology, clinical characteristics and influencing factors of each group were analyzed and compared. ResultsCompared with the AB colonization group, the AB with another bacteria LRTI group had higher proportion of patients with immunosuppression, specimens from sputum and bronchoalveolar lavage fluid, more than 4 invasive procedures, 90-day mortality, white blood cell count >10×109/L (or <4×109/L), neutrophil percent >75% (or <40%), lymphocyte count <1.1×109/L, platelet count <100×109/L, albumin <30 g/L, high sensitivity C-reactive protein >10 mg/L, and neutrophil-to-lymphocyte ratio (NLR). The frequency of bronchoscopy and days of infusing carbapenem within 90 days before isolating AB, the Acute Physiology and Chronic Health Evaluation Ⅱ score, the proportion of patients with invasive mechanical ventilation and the duration of invasive mechanical ventilation in the AB with another pathogen LRTI group were higher than those in the AB colonization group (all P<0.05). Days of infusing carbapenem and β-lactams/β-lactamase inhibitors within 90 days before isolating AB, proportion of septic shock, NLR and 90-day mortality of the patients from the AB with another pathogen LRTI group were more than those in the simple AB-LRTI group (all P<0.05). After regression analysis, more than 4 invasive procedures, or immunosuppression, or with more days of infusing carbapenem within 90 days before isolating AB were all the independent risk factors for AB-LRTI.ConclusionsThere are significant differences in epidemiology, clinical symptoms and laboratory indicators between simple AB-LRTI, AB with another pathogen LRTI and AB colonization in RICU patients. For RICU patients, who suffered more than 4 invasive procedures, immunosuppression, or with more days of infusing carbapenem within 90 days before isolating AB, are more susceptible to AB-LRTI.
