  • Feasibility of sleeve lobectomy after neoadjuvant chemoimmunotherapy in non-small cell lung cancer

    Objective To evaluate the surgical outcomes of neoadjuvant chemoimmunotherapy for sleeve lobectomy. Methods The clinical data of patients diagnosed as non-small-cell-lung-cancer (NSCLC) and evaluated to undergo sleeve lobectomy surgery at TianJin Chest Hospital were retrospectively analyzed. Patients were divided into two groups: a neoadjuvant treatment group and a surgery alone group. The clinical characteristics, perioperative parameters, and postoperative outcomes of two groups were compared. Results Finally 22 patients were collected, including 19 males and 3 females with a median age of 63 years. There were 7 patients in the neoadjuvant treatment group, and 15 patients in the surgery alone group. There was no statistical difference in surgical time, intraoperative bleeding, lymph node dissection, postoperative catheterization time, and postoperative drainage volume between the two groups (P>0.05). In the neoadjuvant treatment group, 1 patient had a second thoracotomy exploration for hemostasis due to bronchial artery bleeding, 2 patients had wound infection, 1 patient had immune-associated pneumonia before surgery, and 1 patient had immune-associated pneumonia before postoperative adjuvant therapy. In the the neoadjuvant treatment group, 1/7 (14.3%) patients had a pathological complete response, and 3/7 (42.9%) patients achieved a major pathological response. Conclusion The surgical difficulty and post-surgical complication rate of sleeve lobectomy with neoadjuvant chemoimmunotherapy are higher than those of surgery alone but also offer appropriate oncological outcomes. Overall, neoadjuvant chemo-immunotherapy sleeve lobectomy can be considered as a treatment option for patients with locally advanced lung cancer.

  • Exploration of porcine thyroidectomy via submaxillary approach using MP1000 robotic surgical system in porcine animal model

    ObjectiveTo investigate the safety and feasibility of domestic MP1000 robotic surgical system assisted thyroidectomy via submaxillary approach in porcine animal model. MethodThe thyroidectomy process assisted by the MP1000 robotic surgical system via submaxillary approach for a Bama pig in the 960th Hospital of the Joint Logistics Support Force was retrospectively analyzed. ResultsThe operation was performed as planned programme using the MP1000 robotic surgical system without opening, adding or lengthening the surgical incision. There was no mechanical problems during the MP1000 robotic surgical operation. The operative time was 53 min and the estimated intraoperative blood loss was 10 mL. There was no shaking of instruments and robotic arm during the operation, and the 3 surgical instruments cooperated skillfully, the establishment of surgical operation space successfully was completed, the thyroid blood vessels accurately and finely was dissected, and the separation, coagulation and cutting of blood vessels were smoothly completed. The recurrent laryngeal nerve and parathyroid gland were delicately dissected and protected. The carotid sheath, trachea, esophagus, and other important organs around the thyroid did not be damaged. The master-slave mapping frequency was high, and there was no delay sense during the operation. The lens resolution of MP1000 was 1 920×1 080, the surgical field of vision was clear, no visual field was defected and the visual field was stable and not shaking, light source front and intelligent adaptive temperature control system reduced the fogging of the lens, and the lens was scoured for 4 times during the operation. ConclusionAccording to the preliminary results of the experimental animal in this study, MP1000 robotic surgical system can successfully complete thyroidectomy via submaxillary approach in porcine animal model.

  • 甲状腺癌颈淋巴结清扫术后难治性乳糜漏临床分析

    目的 分析甲状腺癌手术后难治性乳糜漏的临床特点,探讨难治性乳糜漏的治疗策略。方法 回顾性分析解放军第九六〇医院(原济南军区总医院)2009年 1 月至 2021 年1 月期间收治的 5 021例行甲状腺癌颈淋巴结清扫术患者的临床资料,术后发生乳糜漏122例,其中治疗时间超过2周的难治性乳糜漏34例,对难治性乳糜漏患者的一般情况、治疗方式、峰值引流量(日最高引流量)、带管时间等进行分析总结。结果 5 021例患者中发生乳糜漏122例,占所有患者的2.4%。经非手术治疗2周内治愈者88例,占乳糜漏总数的72.1%;治疗时间超过2周的难治性乳糜漏34例,占乳糜漏总数的27.9%。难治性乳糜漏经非手术方法治愈者18例,经再次手术治愈者16例,2组患者的乳糜漏位置左侧多于右侧;34例难治性乳糜漏峰值引流量(日最高引流量)手术组(16例)高于非手术组(P<0.05),带管时间手术组短于非手术组(P<0.05)。结论 低峰值引流量乳糜漏经非手术治疗多可治愈;难治性乳糜漏左侧多见,高流量乳糜漏(峰值引流量≥1 000 mL)宜早期实施手术治疗。

  • Application of new surgical clinical classification and staging of myasthenia gravis in the perioperative period

    Objective To exploring the effectiveness of perioperative application of new surgical clinical classification and staging for myasthenia gravis (MG) in reducing the incidence of postoperative myasthenic crisis (MC). Methods The clinical data of patients with generalized MG admitted to the Comprehensive Treatment Center for Myasthenia Gravis of Henan Provincial People’s Hospital from January 2018 to June 2022 were retrospectively analyzed, who were scored with myasthenia gravis-activities of daily living (MG-ADL) score and quantification of the myasthenia gravis (QMG) score at the first visit, 1 day before surgery, and 3 days after surgery. The patients were divided into a group A (typeⅡ) and a group B (typeⅢ+Ⅳ+Ⅴ) by the new surgical clinical classification and staging of MG according to the disease progression process, and all patients underwent expanded thoracoscopic thymus (tumor) resection after medication and other interventions to control symptoms in remission or stability. The incidence of MC and the efficiency rate after surgery were analyzed. The normal distribution method and percentile method were used to calculate the unilateral 95% reference range of the QMG score and MG-ADL score. Results Finally 126 patients were enrolled, including 62 males and 64 females, aged 13-71 years, with an average age of 46.00±13.00 years. There were 95 patients in the group A and 31 patients in the group B, and the differences of the preoperative baseline data between the two groups were not statistically significant (P>0.05). The incidence of postoperative MC was 1.05% (1/95) in the group A and 3.23% (1/31) in the group B (P>0.05). The effective one-sided 95% reference range of the QMG score and MG-ADL score 1 day before surgery was 0-7.75 and 0-5.00, and there was no postoperative death in both groups. Conclusion The new surgical clinical classification and staging of MG can guide the timing of surgery, which can benefit patients undergoing surgery for MG and greatly reduce the incidence of postoperative MC.

  • Expression and function of α7 nicotinic acetylcholine receptor in thymocytes of myasthenia gravis patients

    ObjectiveTo investigate the expression of α7 nicotinic acetylcholine receptor (α7 nAChR) in thymocytes of patients with myasthenia gravis (MG) and its effect on cytokine secretion and T cell proliferation. MethodsPatients with MG who underwent expanded thoracoscopic thymectomy in the Comprehensive Diagnosis and Treatment Center of the Henan Provincial People’s Hospital from June 2021 to June 2022 were selected and allocated to a MG group. Patients who underwent partial thymectomy to expose the surgical field during the cardiac disease surgery from June 2021 to September 2022 in the Department of Adult Cardiac Surgery of Fuwai Huazhong Cardiovascular Hospital were selected as the control group. Thymic single cell suspensions were prepared from MG and control groups, and the expression of α7 nAChR in thymocytes of the two groups was detected by real-time polymerase chain reaction and Western blotting. Then CD3/CD28 monoclonal antibody coupled with magnetic beads was used to induce T cell activation, and the levels of cytokines interferon-gamma (IFN-γ), tumor necrosis factor-α (TNF-α), interleukin-4 (IL-4), IL-6, IL-10, IL-17, and IL-21 in thymocytes of the two groups were detected by enzyme-linked immunosorbent assay (ELISA). The activated T cells of the MG group were divided into a blank control group, an α7 nAChR antagonist group, and an α7 nAChR agonist group according to different treatment methods. After 72 hours of culture, IFN-γ, TNF-α, IL-4, IL-6, IL-10, IL-17, and IL-21 expression levels in the culture supernatant were measured by ELISA. Afterwards, CD4-PE and CD8-APC antibodies were added, and the proliferation of T cell subsets was detected by flow cytometry. ResultsA total of 10 MG patients were collected, including 3 males and 7 females with an average age of 19.25±6.28 years; and 15 control patients were collected, including 6 males and 9 females with an average age of 26.18±6.77 years. Compared with the control group, the mRNA and protein levels of α7 nAChR in the thymocytes of MG group were decreased, and the expression levels of IFN-γ, TNF-α, IL-4, IL-6 and IL-21 in the supernatant were increased (P<0.05), but there was no statistical difference in the expression of IL-10 and IL-17 (P>0.05). The cell-culture experiment showed that compared with the blank control group, the levels of IFN-γ, TNF-α, IL-6 and IL-21 secreted by T cells in the α7 nAChR antagonist group were increased (P<0.05), while they were decreased in the α7 nAChR agonist group (P<0.05). There was no statistical difference in the secretion levels of IL-4, IL-10 or IL-17 among the three groups (P>0.05). CD4+ T and CD8+ T cells in the α7 nAChR agonist group were significantly less than those in the blank control group and α7 nAChR antagonist group (P<0.001), while they were significantly more in the α7 nAChR antagonist group than those in the blank control group (P<0.001).ConclusionThe expression of α7 nAChR in thymocytes of MG patients is decreased, and α7 nAChR may be involved in the inflammatory response in thymocytes and thus in thymic function.

  • Expression and significance of nicotinoid acetylcholine receptor α7 in T helper cells of thymic follicles of myasthenia gravis

    Objective To investigate the role of T follicular helper cells (TFH) and their expression of α7 nicotinic acetylcholine receptor (α7nAChR) in patients with Myasthenia gravis (MG). Methods Fifteen MG patients who underwent surgical treatment in the Myasthenia Gravis Comprehensive Diagnosis and Treatment Center of Henan Provincial People's Hospital from June 2022 to June 2023 were selected as the MG group, including 7 males and 8 females, aged 12-30 years. Twelve healthy individuals who underwent partial thymectomy without obstruction during cardiac surgery in Fuwai Central China Cardiovascular Hospital from June 2022 to June 2023 were selected as the healthy control group, including 5 males and 7 females aged 20-35 years. Thymus single cell suspension was obtained by grinding the thymus tissue, and flow cytometry was used to detect the expression of α7nAChR in TFH cells. The thymus cell suspension was purified using density gradient centrifugation, followed by immunomagnetic bead separation to obtain CD4+T cells. CXCR5 antibody and coupled magnetic beads were added to isolate TFH cells. Real-time fluorescent polymerase chain reaction (RT-PCR) and Western blotting were performed to further investigate the expression of α7nAChR in TFH cells. Results Compared with the healthy control group, there was a significant decrease in α7nAChR levels within thymic TFH cells in the MG group (P<0.001), along with significantly decreased mRNA and protein expression levels of α7nAChR within these cells (P<0.01). Conclusion The findings suggest that there is a reduced expression of α7nAChR within thymic TFH cells in MG patients, leading to weakened immunosuppressive function which may indirectly contribute to disease onset and progression.
