  • SURGICAL TREATMENT OF TRAUMATIC SUBCLAVIAN ARTERY

    Objective To explore the treatment of traumaticsubclavian artery. Methods From July 1990 to January 2006, 12 cases of traumatic subclavian artery were treated byusing of combined incision of superior-inferior clavian. All patients were male,aging 18-36 years(mean 22.6 years). The locations were section 1 of subclavianartery in 1 case, section 2 in 4 cases and section 3 in 7 cases. All patients had incomplete rupture and defect. Time from injury to operation was 3 hours to 1.5 months. The methods of vascular repair included primary repair, end-to-end anastomosis and artificial vascular prosthesis grafts. Results There was no death. Extremities survived in all cases and got good function in 10cases.All patients were followed up 2 months to 12 years (mean 5 yeras and 2 months). The pulse of radial artery restored to normal in 10 cases and did not be felt in 2 cases. The function of extremities restored to normal in 2 cases withpartial injury of brachial plexus nerve and did not improve in 2 cases with complete injury ofbrachial plexus nerve. 〖WTHZ〗Conclusion The exposure of subclavian artery is difficult because of its particular anatomy region. The repair and reconstruction of subclavian artery should be selected according to the type of vascular injuries. Combined superiorinferior clavian approach can satisfy the exposure and repair for the subclavian artery.

  • RESEARCH PROGRESS IN LOWER LIMB LENGTHENING BY INTRAMEDULLARY NAIL

    ObjectiveTo summarize the research progress in the lower limb lengthening by intramedullary nail. MethodsThe domestic and foreign related literature about the lower limb lengthening by intramedullary nail was reviewed, summarized, and analyzed. ResultsThe intramedullary nail lengthening systems can be divided into 3 types:distraction by mechanical strength, by motorized electronic power, or by motorized magnetic force. The new technology has obvious advantages in complication incidence, limb function, bone healing rate, comfortable and cosmetic degree of the patients, and hospitalization days compared with the external fixation, but it also has the disadvantages including lengthening failure of system breakdown, intramedullary infection, injuries of vessel and nerve, limited distraction length, and expensive price of the apparatus. And the method also has several contraindications:narrow medulla, multiple curves of medulla, osteomyelitis, skin infection, and unclosed epiphyseal plate. ConclusionThe lower limb lengthening by intramedullary nail is a major improvement of Ilizarov technology. Although the method has been used in limited cases, the preliminary clinical results are excellent, and it can be regarded as a new trend of the limb lengthening, bone reconstruction, and deformity correction.

  • Research progress of in vivo bioreactor for bone tissue engineering

    ObjectiveTo review the research progress of in vivo bioreactor (IVB) for bone tissue engineering in order to provide reference for its future research direction.MethodsThe literature related to IVB used in bone tissue engineering in recent years was reviewed, and the principles of IVB construction, tissue types, sites, and methods of IVB construction, as well as the advantages of IVB used in bone tissue engineering were summarized.ResultsIVB takes advantage of the body’s ability to regenerate itself, using the body as a bioreactor to regenerate new tissues or organs at injured sites or at ectopic sites that can support the regeneration of new tissues. IVB can be constructed by tissue flap (subcutaneous pocket, muscle flap/pocket, fascia flap, periosteum flap, omentum flap/abdominal cavity) and axial vascular pedicle (axial vascular bundle, arteriovenous loop) alone or jointly. IVB is used to prefabricate vascularized tissue engineered bone that matched the shape and size of the defect. The prefabricated vascularized tissue engineered bone can be used as bone graft, pedicled bone flap, or free bone flap to repair bone defect. IVB solves the problem of insufficient vascularization in traditional bone tissue engineering to a certain extent.ConclusionIVB is a promising method for vascularized tissue engineered bone prefabrication and subsequent bone defect reconstruction, with unique advantages in the repair of large complex bone defects. However, the complexity of IVB construction and surgical complications hinder the clinical application of IVB. Researchers should aim to develop a simple, safe, and efficient IVB.

  • 穿支蒂腓肠神经营养血管螺旋桨皮瓣修复跟后软组织缺损

    目的总结采用穿支蒂腓肠神经营养血管螺旋桨皮瓣修复跟后软组织缺损的疗效。 方法2010年1 月-2012年6月,收治7例跟后软组织缺损患者。其中男5例,女2例;年龄14~52岁,平均31岁。致伤原因:碾压伤3例,撕脱伤2例,撞击伤2例。受伤至入院时间1 d~3周,平均6.8 d。软组织缺损范围4 cm × 3 cm~7 cm × 5 cm。采用大小为11 cm × 4 cm~15 cm × 7 cm的穿支蒂腓肠神经营养血管螺旋桨皮瓣修复。供区直接拉拢缝合。 结果术后1周内皮瓣肿胀程度根据顾玉东提出的标准进行评定,均为2级。皮瓣均顺利成活,创面及供区切口均Ⅰ期愈合。术后患者均获随访,随访时间6个月~2年,平均11.5个月。皮瓣外观无臃肿,蒂部平滑,质地良好。术后6个月皮瓣两点辨别觉为7~14 mm,平均12 mm;踝关节功能采用美国矫形足踝协会(AOFAS)评分系统,获优5 例,良2 例。 结论穿支蒂腓肠神经营养血管螺旋桨皮瓣术中移位简便,静脉回流并发症少,修复跟后软组织缺损后可获较好外形。

  • LUMBAR SPONDYLOLISTHESIS MANAGEMENT USING EXPANDABLE PEDICLE SCREW AND INTERBODY FUSION CAGE

    Objective To evaluate the therapeutic effects of expandable pedicle screw (EPS) combined with interbody fusion cage on lumbar spondylolisthesis. Methods From June 2004 to March 2008, 23 patients with lumbar spondylolisthesiswere treated, including 9 males and 14 females aged 24-72 years old (average 48.7 years old). The course of the disease varied from 6 months to 6 years (average 30.4 months). There were 18 cases of degenerative spondylol isthesis and 5 of isthmic spondylolisthesis, including 1 case at L3,4 level, 14 at L4,5 level, and 8 at L5 and S1 levels. There were 17 cases of grade I, 4 of grade II, and 2 of grade III (including 1 case of recurrent L5 spondylolysis) according to Meyerding classification system. Posterior lumbar interbody fusion was performed using 48 PLIVIOS interbody fusion cages, and spondylolisthesis reduction and internal fixation were conducted using 84 pieces of EPS. The indications for use of EPS were initial operation with bone mass reduction or osteoporosis, reoperation of previous pedicle instrumentation, intraoperative screw relocation, sacral anchoring, and construct reinforcement. Cl inical outcomes, radiographic reduction of spondylol isthesis and bone fusion of lumbar interbody were evaluated based on JOA score, Boxall index, and Cook criteria. Results The incision healed by first intension in all cases except for one revision case that suffered from postoperative cerebrospinal fluid leakage and obtained delayed-healing without infection 23 days after operation using conservative treatment. No operative compl ications such as nerve and organ injuries were found.All cases were followed up for 12-39 months (average 17.8 months). X-ray exams of spine AP, lateral, fully extended position and fully flexed position view showed all 84 EPS were fully expanded within vertebral body. The improvement rate of JOA at the final follow-up visit was markedly effective in 14 cases, and effective in 5 cases, and noneffective in 4 cases, with a total effective rate of 82.61%. Anatomic reduction was achieved in 14 cases, improvement was obtained in 6 cases, no improvement was observed in 3 cases, and the reduction rate was 86.69%. Lumber interbody fusion was achieved in 20 cases, fixation was achieved in 2 cases, failure was observed in 1 cases, and the fusion rate was 86.69%. Conclusion EPS in complex with interbody fusion cage provides effective reduction, internal fixation and interbody fusion for the reconstruction of lumbar spondylolisthesis.

  • Comparative study of different concentrations of methicillin-resistant Staphylococcus aureus in the preparation of chronic femoral osteomyelitis models

    ObjectiveTo investigate the appropriate concentration of methicillin-resistant Staphylococcus aureus (MRSA) in establishing chronic femoral osteomyelitis model in rabbits.MethodsForty-eight adult New Zealand white rabbits were randomly divided into 6 groups with 8 rabbits in each group. Animals in groups B, C, D, E, and F were injected 1×109, 1×108, 1×107, 1×106, 1×105 CFU/mL MRSA on the location of 2 cm of the femoral supracondyle, respectively, and group A was injected with aseptic saline as a control. The general observation were performed at 4 weeks after operation, and the wound secretions were taken for bacteriological examination. The serum C-reactive protein content was detected at preoperation and 2 weeks and 4 weeks after operation. The X-ray, CT scan, and Norden imaging scoring were performed at 4 weeks after operation. At 4 weeks after operation, the animals were sacrificed, and the specimens were observed and evaluated by general scores; and the HE staining and histological score were also performed.ResultsFive rabbits died of severe infection in group B, 2 died in group C, and no rabbit died in groups D, E, and F. General observation showed that the incision healed without soft tissue swelling in group A; most animals had visible incision swelling and sinus formation, femoral thickening, bone destruction, and damage decreased with the decreasing of the concentration of liquid bacterial in groups B-D; the infection signs were seen in groups E and F, and the degree of infection were less than that of group D. Bacteriological examination showed that fistula formation animal in groups B, C, D, and E were cultured with positive results, and with the decrease of concentration, the number of animal fistula formation decreased gradually; and bacteriological culture did not be performed in group F because of no sinus formation. There was no significant difference in the content of C-reactive protein between groups before operation (P>0.05). The contents of C-reactive protein in groups B-F were significantly higher than those in group A at 2 and 4 weeks after operation (P<0.05). At 4 weeks after operation, the content of C-reactive protein was in the order of groups B, C, D, E, F, and A in turn from high to low, showing significant differences between groups (P<0.05). Imaging examination showed that there was no soft tissue swelling and bone destruction in group A; bone destruction, massive sequestrum formation, and soft tissue swelling were found in groups B and C; bone destruction was observed in groups D and E, and the degree of sequestrum formation was not as good as that in group C; and there was a small amount of bone infection in group F. The Norden scores in groups B-F were significantly higher than that in group A, and in groups B and C than those in groups D, E, and F, and in groups D and E than that in group F (P<0.05); there was no significant difference between groups B and C, and between groups D and E (P>0.05). The specimens general observation scores in groups B-F were significantly higher than that in group A, while in groups B and C than those in groups D, E, and F (P<0.05); there was no significant difference between groups D, E, and F (P>0.05). HE staining showed that the structure of bone trabecula in group A was clear and the structure was arranged neatly; in groups B-F, trabecular bone destruction and inflammatory cell infiltration were seen and the degree gradually decreased. The histological scores in groups B-F were significantly higher than that in group A, and in group B than those in groups C-F, in groups C and D than that in group F (P<0.05); there was no significant difference between groups C, D, and E, and between groups E and F (P>0.05).ConclusionThe optimal MRSA concentration of rabbit model of chronic osteomyelitis of femur is between 1×106 and 1×107 CFU/mL.

  • Progress in the application of silk fibroin in tissue engineered drug delivery system

    ObjectiveThe properties and characteristics of different types of silk fibroin (SF) drug-loaded sustained-release carriers and their effects on the drug release behavior were reviewed, and the existing problems and development prospects of SF drug-loaded sustained-release carriers in tissue engineering drug delivery system were discussed.MethodsThe literatures about drug-loaded SF sustained-release carriers in recent years were extensively consulted, and the types of sustained-release carriers, characteristics of drug release, range of applications, advantages and disadvantages, and solutions were summarized and analyzed.ResultsAt present, the SF drug-loaded sustained-release carriers are mainly divided into SF microparticles, SF scaffolds, SF membranes, SF hydrogels, SF nanofibers, SF sponges, and so on. These types of SF drug-loaded sustained-release carriers have their own advantages and problems, of which the most prominent problem is the burst release of drugs at the initial stage. While, the initial burst release of drugs can be effectively solved by improving the preparation process and adjusting the material ratio. Different types of drug-loaded sustained-release carriers can be prepared by combining different materials to achieve different application scopes and drug release behaviors under different conditions.ConclusionSF is a good drug-loaded carrier for tissue engineering, the burst release of drugs at the initial stage can be solved by improving the preparation process and changing the material structure; through the combination of the advantages of various types of SF drug-loaded sustained-release carriers, it is expected to prepare SF drug-loaded sustained-release carriers that meet different clinical needs.

  • BIOMECHANICAL STUDY ON EFFECTS OF BONE MINERAL DENSITY ON FIXATION STRENGTH OF EXPANSIVE PEDICLE SCREW

    Objective To evaluate the fixation strength of expansive pedicle screw (EPS) at different bone mineral density (BMD) levels, further to provide theoretical evidence for the clinical application of the EPS in patients with osteoporosis. Methods Fresh human cadaver spines (T12-L5 spines) were divided into 4 levels: normal BMD, osteopenia, osteoporosis, and severe osteoporosis according to the value of BMD, 12 vertebra in each level. Conventional pedicle screw (CPS) or EPS was implanted into the bilateral vertebra in CPS group and EPS group, respectively, 12 screws in each group per BMD level. Screw pullout tests were conducted. The maximum pullout strength, stiffness, and energy absorption were determined by an AG-IS material testing machine with constant rate of loading in a speed of 5 mm/ min. Results With the decline of BMD from normal to severe osteoporosis level, the maximum pullout strength and the stiffness correspondingly declined (P lt; 0.05). In CPS group, the energy absorption gradually decreased (P lt; 0.05); in EPS group, significant difference was found between other different BMD levels (P lt; 0.05) except between normal BMD and osteopenia and between osteoporosis and severe osteoporosis (P gt; 0.05). At the same BMD level, the maximum pullout strength of EPS group was significantly larger than that of CPS group (P lt; 0.05); the stiffness of EPS group was significantly higher than that of CPS group (P lt; 0.05) except one at normal BMD level; and no significant difference was found in the energy absorption between 2 groups (P gt; 0.05) except one at osteopenia level. No significant difference was found in maximum pullout strength, stiffness, and energy absorption between EPS group at osteoporosis level and CPS group at osteopenia level (P gt; 0.05); however, the maximum pullout strength, stiffness, and energy absorption of EPS group at severe osteoporosis level were significantly lower than those of CPS group at osteopenia level (P lt; 0.05). Conclusion Compared with CPS, the EPS can significantly improve the fixation strength, especially in patients with osteopenia or osteoporosis.
