  • Disease Burden and Quality of Life of Rheumatoid Arthritis in China: A Systematic Review

    Objective To get known about the disease burden and quality of life (QOL) of rheumatoid arthritis (RA) in China by conducting a systematic review. Methods The observational studies about the disease burden and QOL of RA in China were systematically searched in the following databases: CNKI, CBM, VIP, WanFang Data, MEDLINE/Pub Med, EMbase, and Science Citation Index. The retrieval time was from January 1st, 1990 to July 31st, 2010. According to the inclusion and exclusion criteria, the literature was screened, the data were extracted, and the methodological quality of the included studies was assessed. The morbidity of RA was pooled by adopting generic inverse variance model, the meta-analyses on 8 dimensions of SF-36 life quality score (LQS) was conducted by suing RevMan5.0 software, the weighted mean difference (WMD) was regarded as the indicator of intervention effect, and the impact of studies’ quality on the results was assessed by sensitivity analysis. Results A total of 20 studies with medium quality in general were included. The morbidity of RA was 14.7/100 000. The disability adjusted of life years (DALYs) per capita were 4.92. If excluding the cost resulted from DALY, the overall economic cost was RMB 1 250.45 yuan per capita per year; While considering DALY, the cost was RMB 15 717.91 yuan per capita per year. The average cost of outpatient medications was RMB 8 018±17 238 yuan per capita per year. The weighted morbidity was 0.42% (95%CI 0.39% to 0.45%), and it was higher in female than male (Plt;0.05). There was no secular trend and difference between southern and northern (Pgt;0.05), but there were statistical significances in the WMD of 8 dimensions of SF-36 LQS. Sensitivity analyses revealed that the weighted pooled results were stable. Conclusion The epidemiological and economic burden of rheumatoid arthritis are heavier in China, which needs to be concerned by both government and society.

  • Influence of Hospital-Community-Family Mental Rehabilitation Mode on Quality of Life of Schizophrenia Patients

    Objective To explor the influence of the hospital-community-family mental rehabilitation mode on the quality of life of patients with schizophrenia. Methods We selected 101 patients diagnosed as schizophrenia according to ICD-10, who were hospitalized in mental health center of the West China Hospital and took part in rehabilitation voluntarily after discharge. Those patients were randomly assigned to two groups. Hospital-community-family mental rehabilitation mode intervention was applied to the patients based on inpatient rehabilitation in the trial group (n=52), while inpatient rehabilitation alone was applied in the control group (n=49). The total score of quality of life, psycho-social dimension, motivation and energy dimension, score of mental disability and social function, and family social care index were recorded. Then, statistical analysis was performed using SPSS 17.0 software. Results After 3 months and 6 months, the trial group had lower scores of the total score of quality of life, psycho-social domain, and motive and energy dimension than those of the control group (Plt;0.05). After 6 months, the trial group had lower scores of mental disability and social function (Plt;0.05) but a higher score of family social care index (Plt;0.05). The scores of WHO-DSA II and SQLS were positively correlated, while the scores of APGAR and SQLS were negatively correlated. Conclusion The integral mode of hospital-community-family mental rehabilitation effectively improves the quality of life of patients with schizophrenia, which also positively improves patients’ rehabilitation.

  • Effects of Pulmonary Rehabilitation on Patients with Stable COPD of Different Severity

    ObjectiveTo investigate the effect of pulmonary rehabilitation on pulmonary function,perception of dyspnea and quality of life in stable COPD patients of different severity. Methods300 patients with COPD in stable stage were divided into a moderate COPD group (n=120),a severe COPD group (n=100) and a very severe COPD group (n=80). Each group was randomly subdivided into a control group and a treatment group. The treatment groups received pulmonary rehabilitation for 6 months in addition to usual care,and the control groups received usual care without pulmonary rehabilitation. Pulmonary function(FEV1),6 minute walking distance (6MWD),modified medical research council (mMRC) scale,and acute exacerbation frequency of COPD were compared before and after intervention and among groups. ResultsAfter pulmonary rehabilitation for 6 months,the quality of life score and 6MWD were significantly improved in the treatment groups with moderate,severe,very severe COPD,and the increscent of 6MWD was greatest in the severe COPD patients. The mMRC of the patients with very severe COPD improved significantly after pulmonary rehabilitation(P<0.05). Lung function before and after the intervention in three groups all showed no significant difference (P>0.05). The acute exacerbation frequency of the severe COPD patients was significantly reduced by pulmonary rehabilitation (P<0.05), while there was no significant change in the moderate and very severe groups (P>0.05). ConclusionPulmonary rehabilitation can improve exercise tolerance and quality of life of COPD patients with different severity,reduce acute exacerbation frequency in severe COPD,reduce the dyspnea degree in very severe COPD. Pulmonary rehabilitation is a cost-effective treatment for stable COPD.

  • Reliability and Validity of MOS-36-item Short Form of Health Survey Measuring the Quality of Life among Disabled People

    【摘要】 目的 评价SF-36量表在测量地震伤残人员生存质量中的信度和效度。 方法 利用自填法及访谈相结合的方式调查201例绵竹市某镇地震伤残人员,用重测信度和Cronbach’s α系数分析SF-36信度;因子分析方法分析效度。 结果 SF-36各领域的重测信度分别为:生理功能(PF)0.78、生理问题对功能的限制(RP)0.85、躯体疼痛(BP)0.92、健康总体评价(GH)0.82、活力(VT)0.77、社会功能(SF)0.71、心理问题对功能的限制(RE)0.79、精神健康(MH)0.66;各领域的Cronbach’s α系数分别为:PF 0.89、RP 0.75、BP 0.84、GH 0.86、VT 0.78、SF 0.72、RE0.86、MH 0.50。因子分析共提取了6个主成分,基本反映了量表的8个维度,与量表的结构构思基本相符。 结论 自填法及面对面访谈相结合的方式,将SF-36量表用于该地地震伤残人员生存质量测定具有较好的信度和效度。【Abstract】 Objective To evaluate the validity and reliability of the MOS-36-item Short Form of Health Survey (SF-36) measuring the quality of life (QOL) of disabled people injured in the earthquake. Methods A total of 201 disabled people injured in the earthquake in a town of Mianzhu city were investigated via questionnaire combined with a face-to-face interview. The reliability of the SF-36 was assessed by test-retest reliability and Cronbach’s α coefficient. The validity was assessed through factor analysis. Results The test-retest reliability of the SF-36 included: physical functioning (PF) 0.78, role limitation due to physical problems (RP) 0.85, body pain (BP) 0.92, general health (GH) 0.82, vitality (VT) 0.77, social functioning (SF) 0.71, role limitation due to emotional problems (RE) 0.79, and mental health (MH) 0.66. The Cronbach’s α coefficients were as the follows: PF 0.89, RP 0.75, BP 0.84, GH 0.86, VT 0.78, SF 0.72, RE 0.86, and MH 0.50. Six principal components were extracted by factor analysis and the constructs of the obtained instrument were consistent with the conceived concept in essence. Conclusion The SF-36 survey could measure the QOL in disabled people injured in the earthquake with better reliability and validity by questionnaire combined with a face-to-face interview.

  • The relationship of depression status with asthma control and quality of life in asthma patients before and after guided treatment including health care education

    Objective To investigate the depression status,score of asthma control test (ACT) and quality of life in asthma patients before and after health care education according to Global Initiative for Asthma guidelines.Methods 59 enrolled outpatients with asthma were asked to self-administer the CES-D (center for epidemiologic studies-depression) scale,ACT scale and the quality of life (QOL) questionnaire respectively.All the patients were educated and treated by health care professionals under the guidance of GINA 2004.After average of 2.5 months ± 15 days,they were asked to self-administer all the scales and questionnaire mentioned above once again on return visit.The data was collected and analyzed statistically based on whether or not the patient had depression according to the CES-D score and the data before and after the education was compared statistically.Results (A)40.7% (24/59) of the patients had depression emotion before guided treatment,and after that the percentage significantly decreased to 13.6% (8/59) (Plt;0.05).(B)Comparing the depressive and non-depressive groups,there was significant difference in 3 of 5 domains in QOL excluding limitation of activity and self concern about health (Plt;0.05) before guided treatment.While on return visit 3 domains in QOL excluding limitation of activity and psychologic status had shown a significant difference (Plt;0.05).There was no significant difference in ACT score before guided treatment,while wise versa after that (Plt;0.05).(C)Before guided treatment the depression status was inversely correlated with 3 in 5 domains of QOL (symptoms of asthma,psychologic status and response to irritant),as well as QOL as a whole (Plt;0.05),but not with the other two domains of QOL scale and ACT score.After guided treatment,the depression status had inverse correlation with QOL and 3 in 5 domains of QOL scale (excluding limitation of activity and psychologic status) (Plt;0.05),as well as ACT score (Plt;0.05).Conclusions The symptoms of asthma and response to irritants are common factors that influence the depressive emotion in asthma population.While psychologic status and self concern about health are both important factors that can not be overlooked.Health care education is important for asthma patients in view of appropriate treatment,symptom control and relief of depression emotion.

  • Relationships Between Health-Related Quality of Life and Social Support in Patients with Obstructive Sleep Apnea-Hypopnea Syndrome

    Objective To study the relationships among health-related quality of life( HRQL) ,social support, excessive daytime sleepiness ( EDS) and PSG parameters in patients with obstructive sleep apnea-hypopnea syndrome ( OSAHS) . Methods Eighty-five patients were recruited who were diagnosed as OSAHS by overnight polysomnography from August 2007 through November 2007 in West China Hospital.The Calgary sleep apnea quality of life index ( SAQLI) was used for HRQL, social support rating scale ( SSRS) was used for social support, and Epworth sleepiness scale( ESS) was used for EDS. The Pearson linear correlation and stepwise multiple regression analysis were used to analyze the correlation among SAQLI, SSRS, ESS, and PSG. Results The SAQLI was correlated with SSRS score ( r =0. 402, P lt;0. 01) ;ESS score ( r = - 0. 505, P lt;0. 01) ; apnea-hypopnea index ( AHI) ( r = - 0. 269, P lt; 0. 05) and lowest artery oxygen saturation ( LSaO2) ( r = 0. 226, P lt; 0. 05) . Stepwise multiple regression analysis determined two variables, the SSRS and ESS score, as independent factors for predicting the total score of SAQLI which accounted for 37. 3% of the total variance in the total score on SAQLI ( R2 = 0. 373, P lt; 0.001) .Conclusions The HRQL of patients with OSAHS was correlated with the SSRS score, ESS score and PSG parameters. The former two were the more important factors to affect the HRQL of patients with OSAHS.

  • The major factors which affect quality of life of patients with diabetic retinopathy

    Objective To study the major factors which affect quality of life of Chinese patients with diabetic retinopathy,and provide basis raising the patients condition of health and quality of life. Methods Quality of life of 179 patients with different stages of diabetic retinopathy and 30 healthy persons was measuerd by the medical outcomes study 36-item short form health survey(SF-36)and the brief scale of quality of life of WHO(WHOQOL-BREF),then the affectting factors were analysed by step regression. Results The major factors which affect quality of life of patients with diabetic retinopathy included:bilateral visual acuity(BVA),different category of retinopathy,course of disease,economic condition,personality,medical care and age. Conclusion The study suggests that bringing about the transformation from the biologic medical model to bio-psycho-social medical model is important to improve quality of life of the patients with diabetic retionpathy.   • A Survey on Beichuan Cadres’ Quality of Life One Year after Wenchuan Earthquake

    Objective To investigate the quality of life (QOL) of Beichuan cadres 1 year after Wenchuan earthquake, so as to provide references for promoting psychological intervention and improving QOL. Methods With the adoption of WHOQOL-BREF, questionnaire survey on basic conditions of cadre victims, and PCL-C scale, cross-section survey was conducted for 2 055 cadres of Beichuan public institutions in aspects of their QOL, social demographic characteristics and psychological trauma. Results a) Compared with domestic norm, although the Beichuan cadres had higher score in social relation domain (14.31 ± 2.59, Plt;0.01), their QOL scores in each dimensions (physical domain score: 14.16 ± 2.43; psychological domain score: 13.79 ± 2.59, environmental domain score: 11.20 ± 2.74) were lower with significant difference (Plt;0.01); b) The group with population under the age of 30 scored higher than other age groups in the psychological and environmental domains; men scored higher than women in the psychological domain; the widowed group scored lower than other groups in all domains; the Han nationality group scored higher than the Qiang nationality group in all domains; the primary education group scored lower than other groups in the social relation domain; the group with relatives, houses and physical loss or damage scored lower than the non-loss group in all domains; and the higher the score in PCL-C, the lower the scores in all domains (all Plt;0.05). Conclusion Beichuan cadres are severely affected in their physical, psychological and environmental domains even one year after Wenchuan earthquake, especially the female, the Qiang nationality, the old, the lost, and the psychological injured cadres are more badly affected with worse QOL, so those population should be concerned in particular with sustainable psychological service and other measures. The finding of social relation domain score being higher than general population suggests that it is important to promote the satisfaction with social relation through the timely effective support from both government and civil society, and the resettlement of sample housing area as well.

  • A Randomized Controlled Trial of Testosterone Undecanoate for Improving Quality of Life of the Elderly Male Patients with Chronic Disease in Advanced Stage

    Objective To study the effectiveness, safety and tolerance of testosterone undecanoate (TU) in improving exhaustion and quality of life (QOL) of the elderly male patients with chronic disease in advanced stage. Methods Using a simple randomized controlled trial design, eighty patients were randomized into two groups. The treatment group took routine therapy and TU and the control group with routine therapy alone. Results 1. Baseline characteristics in both groups were similar. 2. The PADAM grade decreased, and the QOL scale increased in the treatment group (P〈0.05 ). 3. Serum total testosterone in the treatment group rose, upper arm circumference and right hand grip improved (P〈0.05 ). 4. There were no significant differences in adverse effects between both groups. Conclusions TU can improve the clinical health status in the elderly male patients with chronic disease in advanced stage and increase serum testosterone level, there is a satisfactory tolerances and few adverse reactions.

    Objective We searched the Cochrane Library(Issue 3, 2005 )to identity evidence related to palliative treatment. We found that Opioids are effective for the palliation of breathlessness in terminal illness. Oral Morphine and hydromorphone is effective for cancer pain. Radiotherapy and Bisphosphonates can relief pain secondary to bone metastases. Corticosteroids can resolve malignant bowel obstruction in advanced gynaecological and gastrointestinal cancer.
