  • Correlation between Hematocrit and Slow Coronary Flow in Male Patients

    Objective To explore the correlation between hematocrit and slow coronary flow (SCF) in male patients. Methods We studied 205 patients with angiographically no more than one stenosis lt;40% in each major coronary artery who had admitted to the department of cardiology of the Beijing Anzhen Hospital Affiliated to Capital Medical University from August 2011 to August 2012. According to the level of hematocrit, 101 patients were classified into the trial group whose hematocrit was more 42.9%, while 104 patients into the control group whose hematocrit was no more than 42.9%. Clinical variables were analyzed and compared between the two groups. Results The age was younger in the trial group than the control group, whereas the levels of white blood cell count, mean platelet volume, triglyceride, low-density lipoprotein cholesterol, left anterior descending artery (LAD) and right coronary artery (RCA) TIMI frame count and the proportion of SCF in the LAD were higher in the trial group than in the control group (Plt;0.05). The results of correlation analysis showed that, LAD TIMI frame count (r=0.238, P=0.001), proportion of SCF in the LAD (r=0.206, P=0.003) and RCA TIMI frame count (r=0.209, P=0.003) were positively correlated with hematocrit. The results of multivariate analysis (using logistic regression with adjusted confounding factors such as age) showed that, LAD TIMI frame count (OR=1.031, 95%CI 1.006 to 1.056, P=0.014), proportion of SCF in the LAD (OR=1.919, 95%CI 1.038 to 3.547, P=0.038) were independently correlated with hematocrit. Conclusion The proportion of SCF in the LAD is independently correlated to hematocrit, which suggested that increased hematocrit may contribute to the pathophysiological change of SCF in male patients.

  • Causal Analysis of HIV Risk Behavior among Male Who Have Sex with Male in a Community

    Objective To explore the key influencing factors of HIV risk behavior among male who have sex with male (MSM). Methods 36 MSM subjects in a community were recruited for HIV risk behavior characteristics, social environment and the attitude of exposure of high risk sexual intercourse, using behavior scales and qualitative research methods. The collected data were orderly input and analyzed using Nvivo 8.0 software. Then, after three-level transcription, the data were further summarized and extracted based on the method of the grouding theory. Results The HIV Risk Assessment Questionnaire score of 36 subjects was 8.08±2.46, of whom, 72% scored at a medium level (5 to 10 scores) and 19% scored at a high level (more than 10 scores). The social support rating scale (SSRS) score was 32.38±5.99 in MSM population, lower than in undergraduates and floating population. The results of qualitative analysis showed that, after open coding, 11 key message and 4 categories contributed to HIV risk in MSM populaiton, including: a) low levels of fear for AIDS; b) male role and uncertain sexual orientation; c) low degree social support; and d) poor availability of condom in the setting of sexual intercourse. Conclusion The interventions against AIDS/HIV for MSM need to be further studied. Besides, we should strengthen the community intervention mode based on fear for AIDS, social support, and condom distribution methods

  • Correlation between Glycosylated Hemoglobin A1c and Severity of Coronary Artery Lesions in Young Men with Acute Myocardial Infarction

    Objective To investigate the correlation between glycosylated hemoglobin A1c (HbA1c) and severity of coronary artery lesions in young men with acute myocardial infarction (AMI). Methods Total 278 young men with AMI less than 45 years old were retrospectively studied, and all of them were admitted to hospital from January 2009 to December 2011, and had undergone coronary angiography. According to the results of coronary angiography, the patients were divided into three groups based on the number of artery lesions: the single group (156 cases), the double group (64 cases) and the triple group (58 cases). The relationship between the severity of coronary artery lesions and the following factors were observed: HbA1c, systolic blood pressure (SBP), diastolic blood pressure (DBP), hemoglobin (Hb), serum uric acid (UA), total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), body mass index (BMI), smoking history, drinking history and family history of early coronary artery disease. Results a) HbA1c levels were gradually raised in all the three groups, but the single group (6.39±1.67%) was significantly lower than the double group (6.91±1.63%) and the triple group (7.41±2.12%), with significant differences (Plt;0.05); the HbA1c level of the single group was significantly lower than the triple group in both the ST-segment elevation AMI (6.42±1.68% vs. 7.17±1.86%, Plt;0.05) and the non-ST-segment AMI (5.57±0.37% vs. 8.56±2.83%, Plt;0.05); the HbA1c level of the single group was significantly lower than the triple group in patients with diabetes millitus (8.31±1.83% vs. 8.59±2.02%, Plt;0.05) and in patients without diabetes millitus (5.56±0.33% vs. 5.74±0.37%, Plt;0.05); b) There were significant differences in SBP, TC, HDL-C, LDL-C and drinking history between the single group and the other two groups (all Plt;0.05), and there were significant differences in DBP and TG between the single group and the double group (all Plt;0.05); and c) The results of logistic regression analysis showed that, LDL-C (OR=1.790), HbA1c (OR=1.287) and SBP (OR=1.042) were the independent risk factors (all Plt;0.05) for multiple lesions in coronary arteries of young men with AMI. Conclusion Glycosylated hemoglobin A1c is an independent risk factor for multiple lesions in coronary arteries of young men with AMI.

  • Clinicopathological Features, Postoperative Survival and Prognostic Influencing Factors of Male Patients with Hepatocellular Carcinoma

    Objective To investigate the clinicopathological features, postoperative survival and prognostic influencing factors of male patients with hepatocellular carcinoma (HCC). Methods The clinicopathological features and the follow-up data of 155 male HCC patients who received hepatectomy from Jan. 1995 to Dec. 2002 were retrospectively analyzed and the prognostic influencing factors were defined by uni- and multi-variate analysis. Results Compared with 24 female patients at the same period, males were about six-year older and both of their hepatitis B surface antigen (HBsAg) and liver cirrhosis positive rates were higher (P<0.05), but there were no significant differences of the other clinicopathological parameters between the male group and the female group. Multivariate analysis showed that Edmondson-Steiner grade and portal vein tumor thrombosis (PVTT) were two independent prognostic influencing factors of both the overall survival and the tumor-free postoperative survival of male patients with HCC, while satellite nodule and tumor size only influenced the overall survival. Conclusion The main clinicopathological features and the postoperative survival of male HCC patients were similar than those of female’s. Tumor differentiation and biological behaviors were major factors affecting postoperative survival of male patients with HCC.

  • TREATMENT OF GYNECOMASTIA BY A COMBINED METHOD OF LIPOSUCTION AND SEMICIRCULAR PERIAREOLAR INCISION GLANDULAR ORGAN PARTIAL RESECTION

    Objective To evaluate the effect of the combined method of l iposuction and semicircular periareolar incision glandular organ partial resection in the treatment of gynecomastia. Methods From June 2004 to June 2006, 40 patients, aged 11-41 years old, were treated, with no-nodule (n=10), nodule (n=22) and female-breast-l ike with nodules (n=8). Three patients were unilateral and 37 ones were bilateral. The levels of serum prolactin, luteinizing hormone, foll icle stimulating hormone, estradiol, testosterone and cortisol were normal in 38 patients, while in the other 2 patients, the levels ofserum prolactin, luteinizing hormone, foll icle stimulating hormone and estradiol were higher than normal, and the testosterone level was lower. Li posuction alone was performed in 10 no-nodule patients (lei po-type), and combined l i posuction and semicircular periareolar incision glandular organ partial resection were conducted in the other 30 patients (lei po-glandular type). Results Except for 2 cases in which hematoma and a small amount of effusion were found on the first and second day postoperatively and then obtained heal ing by first intention right after hematoma removal in time, all the other patients’ incisions obtained heal ing by first intention. Ni pple numbness occurred in 3 cases on the first day postoperatively and no special treatment was conducted. There was still nipple hypesthesia in these 3 cases after 6-month follow-up. There were no compl ications such as hematoma, effusion, nipple and mammary areola necrosis, and nipple hypesthesia in other patients. All the 40 patients were followed up for 6-24 months (13 months on average). They were satisfied with their chest figures and no recurrence was observed. Conclusion The combined method of l iposuction and semicircular periareolar incision glandular organ partial resection in the treatment of gynecomastia has many advantages, such as safe, micro-scars, natural and beautiful male breast figures as well as high patients’ satisfaction.

  • Relationship between Different Diagnostic Criteria for Metabolic Syndrome and Non-alcoholic Fatty Liver Disease in the Elderly Male

    Objective To explore the relationship between different diagnostic criteria (ATPIII2002, IDF2005 and CDS2007 criteria) for metabolic syndrome (MS) and non-alcoholic fatty liver disease (NAFLD). Methods A total of 666 elderly males admitted to West China Hospital for routine physical examination were involved in this study in May, 2010. The diagnostic agreement rates of different criteria were compared, along with the relationship between different diagnostic criteria for MS and NALFD. Results The diagnostic agreement of CDS2007 criteria with either IDF2005 or ATPIII2002 criteria was good. However, the agreement of ATPIII2002 with IDF2005 was compromised. The prevalence of NAFLD in MS group was significantly higher than that of non-MS group (Plt;0.01). On the basis of CDS2007 criteria, there was significant correlation between NAFLD and MS (Plt;0.000). Conclusion There is a close relation between NAFLD and all three diagnostic criteria of MS. NAFLD is one of the most important risk factors of MS. The diagnostic agreement of CDS2007 criteria with the other two is good, and there is significant correlation between NAFLD and criteria CDS2007 of MS. CDS2007 is found to be of high accuracy and applicability in the diagnosis of MS in Chinese population including the elderly.

  • Alendronate for the Prevention and Treatment of Men Osteoporosis: A Systematic Review

    Objective To systematically review the efficacy and adverse events of alendronate on bone mineral density and fractures in men with osteoporosis. Methods We electronically searched MEDLINE (1990 to 2005), EMBASE (1990 to 2005), The Cochrane Library (Issue 3, 2005), Controlled Trials Register and The National Research Register, CBM disc, VIP, and CNKI. We also handsearched some related journals. The search was conducted in Nov., 2005. The quality of included randomized controlled trials (RCTs) was evaluated and meta-analysis was conducted by RevMan 4.2.8. Results We identified 7 studies including 817 patients. Sufficient evidence showed that alendronate plus calcium was superior as preventive treatment to calcium in increasing the bone mineral density (SMD 0.59, 95% CI 0.15 to 1.03, P=0.009) of the lumbar spine. The incidence of withdrawal and lost to follow-up due to adverse events of the alendronate plus calcium was lower than that of calcium (RR 0.32, 95% CI 0.11 to 0.87). Two studies showed that alendronate was superior to placebo in increasing the bone mineral density in men with osteoporosis but with no significantly statistical difference in reducing fractures. Two studies showed alendronate was superior to alfacalcidol in increasing the bone mineral density and reducing the vertebral fractures in men with osteoporosis. One study showed alendronate was not superior to calcitonin or alfacalcidol in increasing the bone mineral density in men at high risk of osteoporosis. One study comparing anledronate or parathyroid hormone with combination of these drugs in men with osteoporosis suggested that anledronate wasn’t superior to parathyroid hormone in increasing the bone mineral density, and the combination did not show any difference compared to parathyroid hormone alone. Conclusions Alendronate is more effective in prevention and treatment of men with osteoporosis compared to placebo. Alendronate is more effective than alfacalcidol in increasing bone mineral density and reducingvertebral fractures in men with osteoporosis. Alendronate is not superior to alfacalcidol or calcitonin in increasing the bone mineral density in preventing men osteoporosis. Alendronate compared to combination of parathyroid hormone does not show more effectiveness in increasing the bone mineral density in men with osteoporosis. More RCTs of high quality, especially multiple center trials are needed to generate ber evidence.

  • A Randomized Controlled Trial of Testosterone Undecanoate for Improving Quality of Life of the Elderly Male Patients with Chronic Disease in Advanced Stage

    Objective To study the effectiveness, safety and tolerance of testosterone undecanoate (TU) in improving exhaustion and quality of life (QOL) of the elderly male patients with chronic disease in advanced stage. Methods Using a simple randomized controlled trial design, eighty patients were randomized into two groups. The treatment group took routine therapy and TU and the control group with routine therapy alone. Results 1. Baseline characteristics in both groups were similar. 2. The PADAM grade decreased, and the QOL scale increased in the treatment group (P〈0.05 ). 3. Serum total testosterone in the treatment group rose, upper arm circumference and right hand grip improved (P〈0.05 ). 4. There were no significant differences in adverse effects between both groups. Conclusions TU can improve the clinical health status in the elderly male patients with chronic disease in advanced stage and increase serum testosterone level, there is a satisfactory tolerances and few adverse reactions.
