  • Clinical Analysis and Recognition on Surgical Treatment of Remnant Gastric Cancer

    Objective To analyze surgical treatment and clinicopathologic features of remnant gastric cancer,and to recognize the strategies of treatment. Methods The clinical data of 26 cases patients with remnant gastric cancer diagnosed by endoscopy and pathological examination and underwent surgical treatment were retrospectively analyzed in our hospital between January 2004 and March 2011.In this study,14 cases of remnant gastric cancer from benign disease (RGCB) and 12 cases of remnant gastric cancer after stomach cancer operation (RGCC) were included.The clinical findings,Helicobacter pylori (HP) infection,surgical methods, histopathological features,and prognosis were analyzed.Results The patients developed a carcinoma in the gastric remnant about 15-44 years after operation for benign disease (median 26.3 years) and about 1-10.5 years after gastric cancer operation (median 4 years),and there was significant differences of the two groups(P<0.05).HP infection of the gastric remnant was found in 73.1%(19/26) patients,and infection rate of patients was 71.4%(10/14) in RGCB and 75.0%(9/12) in RGCC, but there was no statistical different in two groups (P>0.05).All 26 patients underwent surgical procedure,and the rate of radical resection was 46.2% (12/26), which was 57.1% (8/14) in RGCB and 33.3% (4/12) in RGCC respectively,there was no statistical different (P>0.05).Among the 12 cases underwent radical resection, the highest lymphatic metastasis rate was in No.3 group (83.3%, 10/12),which came in second in lymph node of mesojejunum or splenic hilum (33.3%, 4/12).Postoperative pathological staging was as follows:stageⅠin 3 cases, stageⅡ in 2 cases, stage Ⅲ in 14 cases and stage Ⅳ in 7 cases. In 18 patients underwent tumor excision,the metastasis of lymph node occurred in 13 cases (72.2%),and the pancreas,transverse colon,or spleen were invaded in 13 cases according to histopathological results. Meanwhile,peritoneal metastasis were founded in 8 cases patients,and the peritoneal metastasis rate of patients with RGCB (14.3%;2/14) was significantly lower than that with RGCC (50.0%, 6/12), P<0.05.The overall one-year survival rate and three-year survival rate was 54.5% and 38.5%,respectively, and the survival time was 2-61 months (median 12 months).Survival analysis indicated that pathological stage and radical resection were significant prognostic factors for patients with remnant gastric cancer(P<0.01),and radical resection was an independent prognostic factor (P<0.05),while age,gender,disease of first operation, degree of differentiation and HP infection were not (P>0.05). Conclusions Early detection and standard radical resection are the key factor to improve the prognosis of patients with remnant gastric cancer and laparoscopic exploration may minimize unnecessary injures of surgery.Because of the different clinical characteristics,strategy of treatment for RGCC and RGCB shall be discriminatory.

  • A Clinicopathological Analysis of Minimal Deviation Adenocarcinoma of the Uterine Cervix:Report of 8 Cases

    目的:探讨宫颈微小偏离性腺癌的诊断与治疗。方法:按照WHO(2003)宫颈腺癌分类标准,回顾性分析我院收治的8例宫颈微小偏离性腺癌的临床病理资料。结果:宫颈微小偏离性腺癌占同期收治宫颈浸润腺癌的4.73%,临床表现为水样白带和/或生殖道出血、宫颈肥大变硬和赘生物,阴道B超检查示宫腔积液,宫颈细胞学诊断较困难,常需深部活检组织诊断。结论:诊断应结合临床表现,影像学检查,宫颈细胞学。当高度怀疑时应取深部组织或宫颈锥切组织诊断,早期诊治预后较好。

  • Analyses of Clinicopathologic Characteristics for Remnant Gastric Cancer

    Objective To analyze the clinicopathologic characteristics of remnant gastric cancer (RGC). Methods The clinical data of 114 patients with RGC treated in The Second Affiliated Hospital of Northern Sichuan MedicalCollege and The General Hospital of Chinese People’s Liberation Army from March 2000 to May 2008 were reviewed and analyzed retrospectively. The clinicopathologic characteristics between the patients with primary benign diseases and those with malignant diseases were evaluated. Results A total of 114 cases,the age was (62.6±11.3) years,and the males versus females was 4.7∶1.0. Most patients (76.2%,64/84) were diagnosed at advanced stages (consistent with pT),and the proportion of pT1 stage cases was only 23.8% (20/84),tumor invasion pT4 was 60.7% (51/84). It was more common that tumor directly invaded adjacent organs or structures (27.4%,23/84),lymph nodes positive (42.9%,36/84),and distant metastasis (27.2%,31/114). The location of distant metastasis was usually confined in the abdominal cavity (93.5%,29/31),and the peritoneum disseminated was the most commonly structures (67.7%,21/31). Histologically,the incidence of poorly differentiated adenocarcinoma (76.7%,79/103) was the mostly histologic grade as well as the diffuse type (78.6%,81/103) was the mostly Laurén classification. Between the patients with primary benign diseases and those with initial malignant disease,the initial gastrectomy or the methods of reconstruction had significantly differences (both P=0.000). The median time from initial resection to development of RGC was 30.0 years in the patients with original benign disease,contrary to 3.3 years in those with previous malignant disease (P=0.000). Both primary diseases (benign or malignant) and the age at initial gastrectomy were the major influencing factors for the time of RGC developed (P<0.05). For pathohistology characters,except signet-ring cell carcinoma (P=0.045), pT4b (P=0.049),pN stage (P=0.025),and Borrmann classification (P=0.005),there were no significant differences between the patients with previous benign diseases and those with original malignant disease,as well as the resectability rate,curative resection (R0) rate,and overall survival rate (P>0.05). Conclusions It is almost unaffected by originalbenign diseases or malignant diseases for clinicopathologic characteristics including the treatment option and prognostic factors.It is necessary and feasibility to form a pattern of endoscopic follow-up for RGC.

  • Expression of CD133 Protein in Primary Lesions of Gastric Cancer and Its Clinical Significance

    ObjectiveTo investigate the expression of CD133 protein in primary lesions of gastric cancer and its clinical significance. MethodsThe expressions of CD133 protein in the primary lesion of tumor and normal gastric mucosa tissues confirmed by using histopathologic examination of 99 patients were detected by immunohistochemical staining. The correlation of CD133 protein expression with the clinicopathologic parameters and features after operation were analyzed. ResultsPositive cells of CD133 protein were localized in the gland parietal and cell membrane surface. The expression of CD133 protein in the cancer and normal gastric mucosa tissues were 29.29% (29/99) and zero, respectively (P=0.000). Expression of CD133 protein in tumor with diameter gt;5 cm was significantly higher than that in the tumor with diameter ≤5 cm (P=0.041). The expression of CD133 protein was correlated with TNM stage (P=0.044), lymph node metastasis (P=0.017), lymphatic vessel invasion (P=0.000), and vascular invasion (P=0.000). Logistic regression analysis revealed that invasion depth of tumor (P=0.011), lymph node metastasis (P=0.043), and TNM stage (P=0.049) were independent risk factors for CD133 protein expression. Survival time of patients with positive expression of CD133 protein was significantly shorter than that negative expression of CD133 protein (P=0.046). Cox proportial hazard regression model analysis demonstrated that lymph node metastasis (P=0.042), TNM stage (P=0.046), and positive expression of CD133 protein (P=0.046) were independent risk factors for patients survival. ConclusionThe CD133 protein expression in primary lesions is closely related with development, metastasis, and prognosis of gastric cancer.

  • Prognosis and Clinical Characteristics of Inflammatory Breast Cancer

    ObjectiveTo investigate the prognostic factors for inflammatory breast cancer based on the data from West China Hospital with a relatively large sample. MethodsClinical data of 41 patients with histopathologically confirmed inflammatory breast cancer (IBC) who received treatment at West China Hospital Oncology Center of Sichuan University between January 2009 and December 2014 were collected and analyzed. Log-rank test and Cox regression model were used for statistical analysis. ResultsIn the study, negative estrogen receptor, negative progestrone receptor and positive human epidermal growth factor receptor-2 were identified in 58.5%, 61.0% and 34.2% of the inflammatory breast cancer tissues, respectively. Progress free survival (PFS) were between 2 and 60 months, with a median of 35 months. Univariate analysis showed that Tumor Node Metastasis (TNM) stage (P=0.016) and therapeutic effect (P=0.002) influenced the survival. Multivariate analysis showed that TNM stage (P=0.006), therapeutic effect (P=0.002), and anthracycline-taxane based chemotherapy (P=0.041) were the significant prognostic factors. ConclusionTNM stage is the major prognostic factor for IBC. Preoperative chemotherapy with paclitaxel-epirubicin combination can improve the PFS of IBC. Comprehensive treatment mode with operation is recommended for the treatment of IBC.

  • Expressions of CD133 and CD44 Protein in Primary Lesion of Gastric Cancer and Its Clinical Significance

    ObjectiveTo investigate the expressions of CD133 and CD44 protein in primary lesions of gastric cancer and its clinical significance. MethodsThe expressions of CD44 and CD133 protein in gastric cancer tissues of 100 patients with gastric cancer were detected by immunohistocheimcal stainings.The relation between the expressions of CD44 and CD133 protein and the clinicopathologic characters were analyzed. ResultsBoth CD44 and CD133 protein were expressed on the cell membranes.No correlation were found between CD44/CD133 and the clinicopathologic parameters include gender and age (P > 0.05), but the positive expression rate of CD44/CD133 with diameter>5 cm was significantly higher than that tumor with diameter≤5 cm (CD44 P=0.150;CD133 P=0.056), and correlated with the tissue differentiation (CD44 P=0.008;CD133 P=0.007), vascular invasion (CD44 P=0.043;CD133 P=0.023), lymphatic vessel invasion (CD44 P=0.020;CD133 P=0.044), lymph nodes metastasis (CD44 P=0.002;CD133 P=0.004), inva-sion depth of tumor (CD44 P=0.006;CD133 P=0.021), and pTNM stage (CD44 P=0.034;CD133 P=0.001).No correlation were found between the co-expression of CD44 and CD133 protein and the clinicopathologic parameters include gender, age, tissue differentiation, and vascular invasion (P > 0.05), but the positive co-expression rate of CD44 and CD133 with diameter>5 cm was significantly higher than that tumor with diameter≤5 cm (P=0.010), and correlated with lymphatic vessel invasion (P=0.003), lymph nodes metastasis (P=0.045), invasion depth of tumor (P=0.041), and pTNM stage (P=0.049).The Spearman rank correlation analysis showed that there was positive correlation between the expressions of CD44 and CD133 protein (r=0.207, P=0.039).Univariate analysis showed that lymph nodes metastasis (P < 0.001), pTNM stages (P=0.013), CD44 protein expression (P=0.005), CD133 protein expression (P=0.002), and co-expression of CD44 and CD133 protein (P < 0.001) were significantly correlated with 3-year survival rate of pati-ents with gastric cancer respectively.Logistic regression analysis revealed that lymph node metastasis (P=0.038) was independent risk factor for co-expression of CD44 and CD133 protein.Multivariate analysis with the Cox regression models showed that co-expression of CD44 and CD133 protein (P=0.003) and lymph node metastasis (P=0.006) were significantly associated with poor prognosis. ConclusionsCD44 and CD133 protein may be considered as robust cancer stem cell markers in gastric cancer.The co-expression of CD44 and CD133 protein is the independent prognosis factor for gastric cancer and strongly associated with poor prognosis when they are expressed more high.

  • 肝癌患者血清中肿瘤特异性生长因子水平与临床病理特征及预后的关系

    目的探讨肝细胞癌(简称“肝癌”)患者血清中肿瘤特异性生长因子(TSGF)水平与患者临床病理特征及预后的关系。 方法选取2010年1月至2013年12月期间我院收治的76例行手术治疗的肝癌患者(肝癌组)和45例正常健康体检者(对照组),比较2组间血清TSGF水平,并观察血清TSGF水平与肝癌患者的临床病理特征和预后之间的关系。 结果肝癌组血清TSGF水平为(86.25±10.52)U/mL,表达阳性率为76.32%(58/76),均明显高于对照组的(25.32±6.54)U/mL和6.67%(3/45),差异有统计学意义(t=35.019,P<0.001;χ2=52.097,P<0.001)。血清TSGF水平与肝癌患者有无癌栓、TNM分期、组织分化程度、有无肝硬变有关(均P<0.05)。血清TSGF水平阳性表达者3年总生存率为55.0%,明显低于阴性表达者的3年总生存率(85.7%),两者之间比较差异有统计学意义(χ2=4.502,P=0.034)。 结论TSGF在肝癌患者血清中高表达,与有无癌栓、TNM分期、组织分化程度、有无肝硬变有关,且其表达高者预后差,有助于肝癌的诊断和预后评价。

  • Correlation Between The Clinicopathological Features of Patients with Colorectal Liver Metastases and Their Postoperative Survival

    ObjectiveBy analyzing the correlation between the clinicopathological features of patients with colorectal liver metastases (CRLM) and their postoperative survival, this study is aimed to identify new and accurate prognostic indicators on the prognoses to provide a reference of the treatment strategy selection for patients with CRLM. MethodsThe clinical data of 233 patients with CRLM who received operation treatments in the Eastern Hepatobiliary Hospital of the Second Military Medical University from January 2006 to December 2009 were retrospectively investigated, and their clinicopathological features, as well as their prognosis were analyzed. The survival curve was drawn by Kaplan-Meier method, and the survival rates were analyzed by log-rank test. Parametric survival analysis was used to identify predictors of cancer-specific survival. ResultsThe median survival time after cancer resection was 37.0 months, with cumulative 1-year, 3-year, and 5-year survival rates of 93.0%, 61.0%, and 17.0%, respectively. The median survival time, with cumulative 3-year, and 5-year survival rates of patients who had received radical operations was better than the others who received palliative operations:40.53 months vs 27.20 months, 59.0% vs 29.0%, and 20.0% vs 0(P < 0.05), respectively. In overall surviva, the results of univariate analysis showed that 13 factors, including surgical method, the first relapse after liver metastasis resection, the number of liver metastases, surgical margin, other unresectable extrahepatic metastases or resectable invasion in blood vessels or the surrounding tissue, whether any chronic liver disease was associ-ated, preoperative serum CEA level, preoperative serum CA19-9 leve, the position of the liver metastases, whether the liver metastasis capsule was complete, TNM stagethe of primary cancer, whether the liver metastasis was simultaneous liver metastases, and the maximum diameter of the liver metastases, were closely related to the clinicopathological features associated with prognosis and the differences were statistically significant (P < 0.05). The results of multivariate survival analysis demonstrated that received palliative operations, simultaneous liver metastases, there were other unresectable extrahepatic metastases or resectable invasion in blood vessels or the surrounding tissue, liver metastases without a complete capsule, the number of liver metastases appeared as multiple and widedistribution, unassociated chronic liver disease of the patients, the maximum diameter of the liver metastases>3 cm, were the independent risk factors affecting the postoperative survival of the patients with CRLM (P < 0.05). ConclusionsIt is important for long-term survival of patients with CRLM who were received operations. Received palliative operations, simultaneous liver metastases, there were other unresectable extrahepatic metastases or resectable invasion in blood vessels or the surrounding tissue, liver metastases without a complete capsule, the number of liver metastases appeared as multiple and widedistribution, unassociated chronic liver disease of the patients, the maximum diameter of the liver metastases>3 cm, were the independent risk factors affecting the postoperative survival of the patients with CRLM.

  • Expression and Clinical Significance of miR-155 in Human Esophageal Squamous Cell Carcinoma

    ObjectiveTo investigate the expression of MicroRNA 155 (miR-155) in esophageal squamous cell carcinoma (ESCC) and analyze its correlation with clinicopathological features of ESCC. MethodsThis study included 54 patients with primary ESCC who underwent radical esophagectomy in Department of Thoracic Surgery, Henan Cancer Hospital of Zhengzhou University between January 2010 and November 2012. There were 47 males and 7 females with median age of 61 years (range, 45 to 82 years). Forty patients were in stage Ⅰ or Ⅱ and 14 patients in stage Ⅲ a+b. Expression of miR-155 was determined by SYBR Green qRT-PCR in ESCC tissue and corresponding adjacent normal mucosa in surgical samples from the 54 patients, and its correlation with clinicopathological features was analyzed. ResultsExpression of miR-155 was significantly lower in ESCC tissue than that in adjacent normal mucosa (Z=-4.258, P=0.000).Expression level of miR-155 was significantly correlated with lymph node metastasis (P=0.040), but not significantly correlated with smoking (P=0.430), drinking (P=0.429), age (P=0.769), gender (P=0.671), depth of invasion (P=0.230), differentiation degree (P=0.896) or pTNM (P=0.407) of ESCC. ConclusionUnder-regulation of miR- 155 expression in ESCC tissue may lead to disorders of inflammation response, immune response and relevant tumor suppressor, and may play a significant role in carcinogenesis and progression of ESCC.

  • Clinical and Pathological Characteristics of Lung Carcinoma in Young Adults

    ObjectiveTo explore the clinical and pathological characteristics of lung carcinoma in young adults, and improve the early diagnosis and treatment. MethodsWe retrospectively analyzed 129 patients with lung carcinoma aged from 18 to 35 years old who came to West China Hospital of Siduan University from January 1st 2008 to December 31st 2014.Also we collected 32 young adults with lung benign mass to make the comparison of the clinical and pathological characteristics with the patients. ResultsIn the 129 patients with lung carcinoma, there were 72 females and 57 males.Adenocarcinoma (88/129) was the most common type of pathology, while others included small cell carcinoma (18/129), squamous cell carcinoma (13/129), and other types of carcinoma (10/129).Among patients with lung carcinoma, 41(31.8%) complained of refractory dry cough and another 40(31.0%) presented coughing and expectoration.Twenty-nine patients (22.5%) were misdiagnosed in their first visits.In young adults with lung carcinoma, 55.8% were at the Ⅳ stage with metastasis at the first-time diagnosis.Comparing with patients with benign lung mass, patients with lung carcinoma presented significantly more dry cough (P=0.011), chest distress (P=0.007) and weight loss (P=0.007);while fever was found more patients with lung benign mass (P=0.040).The differences in cough, expectoration, and emptysis between the two groups were not significant (P > 0.05). ConclusionsLung carcinoma in young adults often has atypical clinical presentations.Therefore, it is more commonly misdiagnosed.
