  • SURGICAL TREATMENT OF TRAUMATIC SUBCLAVIAN ARTERY

    Objective To explore the treatment of traumaticsubclavian artery. Methods From July 1990 to January 2006, 12 cases of traumatic subclavian artery were treated byusing of combined incision of superior-inferior clavian. All patients were male,aging 18-36 years(mean 22.6 years). The locations were section 1 of subclavianartery in 1 case, section 2 in 4 cases and section 3 in 7 cases. All patients had incomplete rupture and defect. Time from injury to operation was 3 hours to 1.5 months. The methods of vascular repair included primary repair, end-to-end anastomosis and artificial vascular prosthesis grafts. Results There was no death. Extremities survived in all cases and got good function in 10cases.All patients were followed up 2 months to 12 years (mean 5 yeras and 2 months). The pulse of radial artery restored to normal in 10 cases and did not be felt in 2 cases. The function of extremities restored to normal in 2 cases withpartial injury of brachial plexus nerve and did not improve in 2 cases with complete injury ofbrachial plexus nerve. 〖WTHZ〗Conclusion The exposure of subclavian artery is difficult because of its particular anatomy region. The repair and reconstruction of subclavian artery should be selected according to the type of vascular injuries. Combined superiorinferior clavian approach can satisfy the exposure and repair for the subclavian artery.

  • Short-term Outcome of Olfactory Ensheathing Cells Transplantation for Treatment of Amyotrophic Lateral Sclerosis

    Objective To determine whether transplanting olfactory ensheathing cells (OECs) is effective in controlling or reversing the deterioration caused by amyotrophic lateral sclerosis (ALS). Methods Between February 2003 and April 2006, 327 patients (241 males and 86 females) with probable or definite ALS (diagnosed according to the El Escorial criteria) received the OECs transplantation. Their ages ranged from 20 to 84 years (51.6±11.1 years). The duration of symptoms before surgical treatment was 4-8 months to 13 years (2.9±2.0 years). OECs were cultured and injected into pathological regions of the spinal cord and/or bilateral corona radiata of the brain; the patients were divided into three groups, group A (cord only,n=29), group B (cord and brain,n=6), and group C (brain only,n=292) based on the transplant sites. Results The patient’s neurological function was assessed both before and at 4 weeks after transplantation by using the Amyotrophic Lateral Sclerosis Functional Rating Scale (ALSFRS) of the ALS CNTF Treatment Study (ACTS). The scores were increased from 17.2±8.6 preoperation to 20.1±9.7 postoperation in group A (P<0.05),from 24.2±6.8to 25.7±6.6 (P>0.05) in group B, and from 20.3±8.6 to 22.0±9.4 (P<0.001) in group C.There were no significant difference in increased ALSFRS scores among the threegroups (P>0.05). The total improvement rate of neurological function was 77.1% (252/327). The result of electromyographic examination showed that spontaneouspotential diminished and/or disappeared, the amplitude of the motor unit actionpotential decreased remarkably and the numbers of motor unit action potential greatly increased in 261 cases (79.8%). Sixteen patients (4.9%) experienced thevarious complications including headache, shortterm fever, seizure attack, central nerve system infection, pneumonia, respiratory failure, urinary tract infection, heart failure, and possible pulmonary embolism; of them, there were 4 deaths(1.2%). Conclusion These preliminary results suggest that the OECs transplantation is effective in controlling or reversing the physiological deterioration caused by ALS.

  • EVALUATION OF RIGID INTERNAL FIXATION IN MANDIBULAR RECONSTRUCTION WITH AUTOGENOUS BONE

    Objective To investigate the application of rigid internal fixation in mandibular reconstruction with autogenous bone and to evaluate its efficacy. Methods From January 1994 to May 2004, 98 patients with mandibular defect received mandibular reconstruction with autogenous bone by using rigid internal fixation. Seventy-two cases of benign tumor and 26 cases of malignant tumor were included. Four hundred and ono rigid fixation plates were inserted.The clinical results and X-ray films were analyzed and the healing processes were evaluated. The functional and aesthetic results of the mandibular reconstruction were also evaluated according to Lopez assessment system. Results After a follow-up of 1 to 3 years, 95 patients(96.9%)achieved successful effect. The forms and function of the mandibleswere resumed. Eightyone (82.7%)patients were satisfied with the results ofoperations. Thirteen patients(13.3%) achieved acceptable results. Four patients (4.1%) were dissatisfied with the results of operations. Conclusion The rigid internal fixation is conductive to healing and remodeling ofthe transplant bone in mandibular reconstruction.

  • EXPERIMENTAL STUDY ON CRYOPRESERVATION OF ARTERIAL ALLOGRAFTS BY VITRIFICATION

    Objective To evaluate the feasibility of preservation of arteriesby vitrification and the effectiveness of vitrified arteries as allografts. Methods Sixty rabbits were used in the research. Forty-eight femoral arteries wereharvested from 24 rabbits as the transplanted materials,and 24 femoral arteries were preserved by vitrification, 24 by freezing for 14 days,respectively. Theother 36 rabbits were used as the transplanted subjects,and were divided into three groups, 12 rabbits including 24 femoral arteries per group: Group A(fresharterial autografts), Group B(vitrified arterial allografts) and Group C(frozen arterial allografts). The morphologic changes of arterial grafts were observed macroscopically and histologically. The patent rate of arterial grafts were measured by angiography, and the rabbits were sacrificed on the 14th day, the 30th day, the 60th day and the 120th day after transplantation respectively. Arterial grafts were harvested to observe the morphological changes,and the immunological rejection was evaluated by measuring the ratio of tunica intima and tunica media. The results were compared between these groups. Results Before transplantation,theintegrated rate of Group B was 91.67%,which was significantly better than that of Group C(54.17%, Plt;0.01). After transplantation, the accumulative patent rate of Group B was 87.50%,which was significantly better than that of Group C(66.67%, Plt;0.05). There was statistically significant difference in the ratioof tunica intima and tunica media between Group B and Groups A, C(Plt;0.05).Conclusion The above results show that vitrification does less damage to cells and tissues because of ice-free in the process of cryopreservation. So vitrification can be used to preserve arteries, and the arterial allografts preserved by vitrification are better than those preserved by freezing.

  • EFFECT OF VASCULAR ENDOTHELIAL CELL GROWTH FACTOR ON REPAIR OF BONE DEFECT WITH CORTICAL BONE ALLOGRAFT

    Objective To study the effect of vascular endothelial cell growth factor (VEGF) on repair of bone defect with cortical bone allograft. Methods Forty five New Zealand white rabbits, weighted 2.5-3.0 kg, were made bone defect model of 1.5 cm in length in the bilateral radii and then were randomly divided into 3groups. The defect was repaired with only cortical bone allograft in the control group, with the cortical bone allograft and local injection of human recombinantVEGF in the experimental group, and with the cortical bone allograft and abdominal injection of VEGF PAb3 in the antagonist group. Roentgenography, immunohistochemical staining and tetracycline labelling were carried out to evaluate the reparative results 1, 3, 5, 8 and 16 weeks after operation. Results Immunohistochemical staining results showed that a great deal of blood vessels formed in the experimental group, and the number of blood vessels increased gradually with the time and reached the highest value at the 8th week. Tetracyclinelabelling showed the same result.The best results in callus formation, ossification rate and count of microvascular density were shown in the experimental group, while those in the control group were significantly better than those in the antagonist group (Plt;0.05),but there was no significant difference between the experimental group and the control group at the 8th week and the 16th week (Pgt;0.05). Conclusion VEGF can accelerates the bone formation and angiogenesis in the bone allografts, thus it can promote the repair of bone defects.

  • SURGICAL TREATMENT OF NECROSIS OF THE FEMORAL HEAD IN EARLY STAGES WITH CORE DEPRESSION AND ALLO-FIBULAR GRAFTING

    Objective To study the effect of surgical treatment with core depression and allo-fibular grafting on osteonecrosis of the femoral head in early stages. Methods From June 1998 to August 2004, 22patients with necrosis of the femoral head (39 hips) were treated with core depression and allo-fibular grafting, indluding 17 males and 5 females. Necrosis was classified as Stages Ⅰ-Ⅲ according to ARCO classification system. Their ages ranged from 22 to 60 years and pain duration was 2 to 12 months preoperatively (6.5 months on average). The regular examinations of joint function, X-ray, ECT,CT and MRI were conducted before surgerg and after 15 days, 3 months, and 6 months of surgery. Results All patients were followed up for 3 to 74 months (31.4 months on average). Symptoms 17 of the patients were relievedevidently. Harris hip score from 78 points preoperatively to 91.6 points.The X-ray films 18 of the patients showed that the geography of the hip joint kept intact with no deterioration. Osteogenesis was observed by regular examinations. Two cases (4 hips, 10.25%) were revised by total hip replacement, and 3 cases (4hips, 10.25%) without THR worsened postoperatively. Conclusion The method is less traumatic than common procedures. The joint function can berecovered within 2.4 weeks. Clinical symptoms are greatly improved. This method is effective at least in short term and the long-term effect needs further study.

  • OPERATIVE TREATMENT OF COMPLICATED DISTAL FEMORAL FRACTURES

    Objective To explore an effective way fortreating severe complicated distal femoral fractures. Methods Twenty-six patients with complicated distal femoral fracture who all belonged to 33C3.3type according to AO/ASIF lassification, were treated with a lateral condylar buttress plate or self-desinged aliform anatomical plate, and operated on with allogeneic bone grafting. Results All cases were followed up for an average of 14 months (ranging 5-25 months). Twenty-four wounds were primary healing postoperatively, 2 wounds were infected and healed after dressing change. Twenty-four had bone healing after 411 months, 2 needed to operate again because of earlier weight-bearing resulting in fixation failure. According to shelbourne and Brueckmann score, the excellent and good rate was 88.46%. Conclusion The internal fixation forcomplicated distal femoral fracture by self-designed aliform anatomical plate and lateral condylar buttress plate with a great deal of allograft bone is an effective surgical method. As it has long oval holes and the holes are consecutive ,the aliform anatomical plate is more suitable for severe complicated fractures. At the same time, autogenous-ilium transplantation can be substituted by the allograft bone.
