  • The expression of thymidine kinase 1 in patients with lung cancer and patients with acute exacerbation of chronic obstructive pulmonary disease

    ObjectiveTo preliminarily investigate the expression of thymidine kinase 1 (TK1) in serum of patients with lung cancer and patients with acute exacerbation of chronic obstructive pulmonary disease (COPD), and analyze the relationship of TK1 level with gender, age and smoking.MethodsFifty-two patients with lung cancer and 64 patients with acute exacerbation of COPD, who admitted in this hospital from January 2017 to April 2018 and underwent serum TK1 measurement, were retrospectively collected and analyzed. The serum level of TK1 in the patients was detected with immunoblot-enhanced chemiluminescence method.ResultsThe TK1 expression level had no relationship with gender, age or smoking (P>0.05). A total of 14 patients were tested positive for TK1 (TK1 level>2 pmol/L) in the lung cancer patients and the positive expression rate was 26.9%; a total of 23 patients were tested positive for TK1 in the COPD patients and the positive expression rate was 35.9%. The TK1 expression level had a more obvious increase in the patients with acute exacerbation of COPD than that in the patients with lung cancer, but there was no statistical difference between the two groups (U=1 482, P=0.314).ConclusionsThe TK1 expression level is not affected by patient’s gender, age or smoking. The TK1 expression level in patients with acute exacerbation of COPD is higher than that in patients with lung cancer. Abnormal cell proliferation in patients with acute exacerbation of COPD may be stronger than that in patients with lung cancer, but further studies are needed.

  • A Comparative Study on Different Methods of Sedation and Analgesia in Emergency Tracheal Intubations in RICU

    ObjectiveTo explore the efficacy and safety of different sedative and analgesic methods in emergent endotracheal intubations in RICU. Methods110 cases of tracheal intubation in critically ill patients were divided into 5 groups randomly: ① control group(given no sedative or analgesic drug before intubation); ② fentanyl group(given intravenous fentanyl 2 μg/kg before intubation,followed by fentanyl 2 μg·kg-1·h-1 maintenance); ③ dexmedetomidine hydrochloride+fentanyl group(given dexmedetomidine hydrochloride 1 μg/kg+fentanyl 2 μg/kg before intubation,followed by dexmedetomidine hydrochloride 0.5 μg·kg-1·h-1+fentanyl 2 μg·kg-1·h-1 maintenance); ④ midazolam+fentanyl group(given midazolam 0.05 mg/kg+fentanyl 2 μg/kg before intubation,followed by midazolam 0.05 mg·kg-1·h-1+fentanyl 2 μg·kg-1·h-1 maintenance); ⑤ Propofol+fentanyl group(given propofol 1 mg/kg+fentanyl 2 μg/kg before intubation,followed by propofol 0.4 mg·kg-1·h-1+fentanyl 2 μg·kg-1·h-1 maintenance).The mean arterial pressure(MAP),heart rate(HR),respiratory frequency(RR),PaO2/FiO2,Riker sedation score and agitation were monitored before,during and after intubations.The one-time success rate of intubation and severe arrhythmia (sinus bradycardia,frequent ventricular premature,ventricular fibrillation,and cardiac arrest) incidence rate were recorded. ResultsThe one-time success rates of intubations of the propofol+fentanyl group (95.4%) and the midazolam+fentanyl group (90.9%) were higher than that in the dexmedetomidine hydrochloride+fentanyl group (86.4%,P<0.05),while one-time intubation success rate of three groups were higher than that of the fentanyl group (45.4%) and the control group (31.8%,P<0.05).5 minutes after intubation,the PaO2/FiO2 index of 5 groups of patients were higher than those before intubation,but the PaO2/FiO2 index of the control group and the fentanyl group were lower than those in the other three groups(P<0.05).The occurrence of serious arrhythmia rate in the dexmedetomidine hydrochloride+fentanyl group (0%),the midazolam+fentanyl group (9%) and the propofol+fentanyl group (9%) were lower than that in the control group (13.6%) and the fentanyl group (18.2%).The MAP during intubation and 2 minutes after intubation of the propofol+fentanyl group and the midazolam+fentanyl group were lower than that in the other three groups(P<0.05).The proportion of patients with Riker sedation and agitation score≤4 at intubation in the dexmedetomidine hydrochloride+fentanyl group (68.2%) was lower than that in the propofol+fentanyl group(90.9%) and the midazolam+fentanyl group (86.4%,P<0.05),but higher than those in the fentanyl group(22.7%)and the control group(18.2%,P<0.05). ConclusionPropofol,midazolam or dexmedetomidine hydrochloride with fentanyl are all effective and safe methods of sedation and analgesia in emergent endotracheal intubation in RICU.Dexmedetomidine hydrochloride with fentanyl is an ideal sedative relatively with less influence on cardiovascular system and less myocardial oxygen consumption.

  • Effect of hypoxic preconditioning on rat lungs exposed to simulated high altitude hypoxia

    Objective To investigate the influence of hypoxic preconditioning on pulmonary structure of rats exposed to simulated high altitude hypoxia and to explore the role of hypoxia inducible factor-1α(HIF-1α).Methods Fifty-six Wistar rats were randomly divided into 7 groups(n=8 in each group),ie,a normal control group(N group),an acute hypoxic control group(H0 group),an acute hypoxic group(H1 group),a 3 000 m hypoxic preconditioning group(C3.0 group),a 3 000 m hypoxic preconditioning + acute hypoxic group (C3.1 group),a 5 000 m hypoxic preconditioning group(C5.0 group),and a 5 000 m hypoxic preconditioning + acute hypoxic group(C5.1 group).After treated with hypoxic preconditioning,the animals were exposed to simulated altitude of 6 000 m for 24 hours.Then the protein and mRNA expression of HIF-1α in lung of N,H0,C3.0 and C5.0 groups were assessed by Western blot and RT-PCR,respectively.The lung structure in N,H1,C3.1 and C5.1 groups was observed by light microscope and electron microscope.Results Pulmonary interstitial edema was apparently observed in H1 group,while significantly relieved in two hypoxic preconditioning groups.HIF-1α protein was not detected in rat lungs by Western blot analysis.Compared to N group,the levels of HIF-1α mRNA significantly increased in C3.0 group and C5.0 group(both Plt;0.01).Conclusions Hypoxic preconditioning can relieve hypoxic pulmonary interstitial edema and increase HIF-1α mRNA expression in rat lungs.HIF-1 may be involved in the process of hypoxic preconditioning in rat lungs.
