  • Changes and Significancy of Adrenomedullin, Atrial Natriuretic Polypeptide and Heart Rate Variability in Patients with Obstructive Sleep Apnea-Hypopnea Syndrome

    Objective To investigate the changes and clinical relationship of plasma adrenomedullin( ADM) , atrial natriuretic polypeptide( ANP) , and heart rate variability( HRV) in patients with obstructive sleep apnea-hypopnea syndrome ( OSAHS) . Methods Seventy-five inpatients with OSAHS were enrolled in this study. According to the apnea hypopnea index ( AHI) by polysomnography, the subjects were divided into a mild group, a moderate group, and a severe group. Meanwhile, HRV was screened bydynamic electrocardiogram in sleep laboratory. HRV parameters were obtained including LF ( low frequency power) , HF( high frequency power) , pNN50( percentage of NN50 in the total number of N-N intervals) ,SDNN( standard deviation of the N-N intervals) , rMSSD( square root of the mean squared differences of successive N-N intervals ) . Plasma levels of ADM/ANP were measured by radioimmunoassay. Results The levels of SDNN ( P lt;0. 05) , rMSSD, pNN50, LF ( P lt; 0. 05) and HF were gradually reduced, and the levels of ADM ( P lt;0. 05) and ANP ( P lt; 0. 05) were increased with increasing severity of OSAHS. Linear correlation analysis demonstrated that SDNN was negatively correlated with ADM( r = - 0. 423, P lt;0. 05)and ANP( r = - 0. 452, P lt; 0. 05) , and LF was also negatively correlated with ADM( r = - 0. 348, P lt;0. 05) . Conclusion Lower HRV is associated with more sever OSAHS, and it may be modulated neurohumorally by ADM and ANP.

  • 风湿性心脏病患者心瓣膜置换术前后血浆肾上腺髓质素及肺血流动力学变化

    目的 观察风湿性心脏病患者心瓣膜置换术前、后血浆肾上腺髓质素(AM)及肺血流动力学的变化,探讨AM在风湿性心脏病肺动脉高压中的作用.方法 采用放射免疫法测定37例风湿性心脏病患者瓣膜置换术前、后血浆AM的浓度,Swan-Ganz漂浮导管测定肺血流动力学指标.结果 风湿性心脏病患者术前血浆AM水平明显高于对照组(P<0.05),AM水平与平均肺动脉压、肺动脉楔压、肺血管阻力呈正相关(r=0.736,0.593和0.655,P<0.01);术后1周血浆AM水平都有不同程度的下降,并与肺动脉高压的改善有一定的关系.结论 AM在风湿性心脏病肺动脉高压的病理机制中具有一定的作用.

  • Expression and Role of Adrenomedullin in Patients with Liver Cancer

    ObjectiveTo investigate the expression of adrenomedullin (ADM) in human liver cancer and to evaluate its clinical signigicance. MethodsA total of 54 patients with liver cancer who underwent surgery between January 2012 and November 2013 and another 18 benigh tissue samples were selected. Immuohistochemical staining (SP staining) was performed to detect the expression of ADM in liver cancer, and enzyme-linked immunosorbent assay was applied to determine the concentration of ADM in the patients and controls, and the relationship was retrospectively analyzed between the expression of ADM in liver cancer and the clinicobiological features. Besides, 60 inpatients with liver cancer and 40 health people under went physical examination between January and June 2013 were selected, whose expression of ADM in plasma was deteced. ResultsThe expression of ADM in liver cancer was significantly higher than that in the normal tissues (P<0.01). There was a significant relationship between the expression of ADM and the differentiation grade in liver cancer (P<0.05), and there was no relationship between the expression of ADM and age, histological type and American Joint Committen on Cancer (AJCC) stage (P>0.05). The expression of ADM in the plasma in liver cancer group was significantly higher than that in the control group (P<0.05). There was a significant relationship between the expression of ADM in the plasma and AJCC stage (P<0.05). ConclusionThe expression of ADM significantly increases in patients with liver cancer, which is associated with the differentiation grades of the liver cancer. ADM may play an important role in the occurrence and development of liver cancer.

  • The Diagnostic Value of Serum Proadrenomedullin in Ventilator-Associated Pneumonia

    ObjectiveTo investigate the clinical value of serum proadrenomedullin (pro-ADM) for diagnosis of ventilator-associated pneumonia(VAP). MethodsA prospective study was carried out in eighty-nine patients with clinically suspected diagnosis of VAP who underwent invasive mechanical ventilation between June 2014 and July 2015.The patients were divided into a VAP group (n=52) and a non-VAP group (n=37) according to clinical and microbiological culture results.The levels of serum pro-ADM were measured by sandwich ELISA on 1st, 3rd and 5th day of VAP suspicion.The diagnostic value of pro-ADM for VAP was assessed by receiver operating characteristic (ROC) curve analysis. ResultsOn 1st day, 3rd day and 5th day, the pro-ADM levels [3.10(2.21, 4.61) nmol/L, 3.01(2.04, 4.75)nmol/L and 1.85(1.12, 3.54)nmol/L, respectively] in the VAP group were significantly higher than those in the non-VAP group [1.53(1.07, 2.24)nmol/L, 1.52(1.05, 2.17) nmol/L and 1.26(1.02, 2.17) nmol/L, respectively] (all P < 0.05).For diagnosis of VAP, the area under the ROC curve (AUC) for pro-ADM on 1st, 3rd and 5th were 0.896 (95%CI 0.799-0.940), 0.863(95%CI 0.791-0.935) and 0.651 (95%CI 0.538-0.765), respectively.When using 2.53 nmol/L as the best cutoff on 1st day, pro-ADM had 84.6% sensitivity and 86.5% specificity.When using 2.40 nmol/L as the best cutoff on 3rd day, pro-ADM had 82.7% sensitivity and 83.8% specificity. ConclusionSerum level of pro-ADM in the diagnosis of VAP has good sensitivity and specificity, which may be used as a marker to diagnose VAP early.

  • 肾上腺髓质素与血小板相关参数对社区获得性肺炎的诊断价值

    目的探讨血清肾上腺髓质素(ADM)、血小板(PLT)、平均血小板容积(MPV)、血小板分布宽度(PDW)等指标水平变化对社区获得性肺炎(CAP)患者的诊断价值。方法收集我院 140 例 CAP 患者,采用酶联免疫吸附法测定血清中 ADM 的浓度,同时收集 PLT 相关参数、PLT 计数水平以及相关炎症指标数据。根据肺炎严重指数(PSI)评分将 CAP 患者分为低危组、中危组、高危组,采用 SPSS 17.0 统计软件分析数据,并与正常对照组的 140 例健康成人进行比较和分析。结果CAP 组血清 ADM 浓度、PLT、MPV、PDW 及白细胞(WBC)计数均高于正常对照组,差异有统计学意义(P<0.05)。血清 ADM 浓度及 WBC 计数在高危组、中危组、低危组之间两两比较差异均有统计学意义(P<0.05),而中危组与低危组间的 PLT、MPV 及 PDW 水平差异无统计学意义(P>0.05)。血清 ADM 浓度变化水平与 PSI 评分系统呈强相关(0.8>r>0.6,P<0.05);PLT 及 WBC 水平变化与 PSI 评分系统呈中度相关(0.6>r>0.4,P<0.05);MPV、PDW 水平变化与 PSI 评分系统相关性相对较弱(r<0.4)。对于细菌性肺炎与其他病原体感染性肺炎的鉴别,联合检测 ADM 和 PLT 的受试者工作特征曲线下面积(AUC)为 0.950,明显高于单独检测 PLT(AUC=0.772)、降钙素原(AUC=0.802)和 C 反应蛋白(AUC=0.913),但与单独检测 ADM 的 AUC 一致。结论血清 ADM 浓度及血小板参数测定对 CAP 的诊断具有一定的价值。血清 ADM 浓度及血小板参数随着 CAP 加重而明显增加,其水平越高提示 CAP 病情越重。血清 ADM 浓度及血小板参数与 PSI 评分呈正相关。联合检测血清 ADM 浓度和 PLT 水平对于鉴别细菌性肺炎有一定临床价值。
