  • Characteristic and Surgical Treatment for Gastric Cancer in Elderly

    Objective To discuss the clinical characteristics and surgical treatment for gastric cancer in the elderly. MethodsRelated literatures were reviewed to summarize the characteristics of elderly patients with gastric cancer, thus to evaluate the feasibility and strategy of surgical treatment for elderly patients with gastric cancer. Results Elderly gastric cancer patients often had one or more comorbidities, but most patients could accept gastrectomy and had a good prognosis. Conclusions Age is not the contraindication for surgery. Dealing with comorbidities in elderly patients, preventing the complications and performing the appropriate surgical method can result in a good therapeutic effect.

  • Effects of p38 Mitogen-Activated Protein Kinase on Apoptosis of Small Intestinal Epithelial Cells after Transplantation in Rats

    【Abstract】ObjectiveTo explore the effects of p38 mitogenactivated protein kinase (MAPK) on apoptosis of small intestinal epithelial cells after transplantation in rats. MethodsSmall intestinal transplantation was performed in SD and Wistar rats. The recipients were divided into three groups: isograft group (Wistar→Wistar group), allograft group (SD→Wistar group) and allograft+cyclosporine A group (SD→Wistar+CsA group). The grafts were harvested on day 1, 3, 5 and 7 after operation. All graft samples were subjected to histological examination. The apoptosis of graft epithelial cells was detected by TUNEL method. p38 MAPK was measured by Westernblotting method and serum TNFα was determined by ELISA. ResultsMild, moderate and severe rejection reaction occurred in the SD→Wistar group, it was showed that the number of apoptotic cells increased with the severity of the rejection reaction by TUNEL. In SD→Wistar group, the numbers of apoptotic cells were significantly higher than those of the other two groups (P<0.01). The severity of rejection reaction in SD→Wistar+CsA group was less than that of SD→Wistar group and the number of apoptotic cells increased with the severity of the rejection reaction (P<0.01). The level of serum TNFα varied with the apoptotic degree of small intestinal epithelial cells in SD→Wistar group and SD→Wistar+CsA group (P<0.01). The expression of p38 MAPK increased with the number of the apoptotic cells in SD→Wistar group and SD→Wistar+CsA group (P<0.01), but there was no evident change in Wistar→Wistar group (Pgt;0.05). The expression of p38 MAPK and the level of serum TNFα were positively correlated with apoptosis in small intestinal rejection after transplantation (r=0.875, P<0.01; r=0.837, P<0.01). p38 MAPK and TNFα were also positively correlated (r=0.826,P<0.01). ConclusionApoptosis plays an important role in small intestinal rejection. p38 MAPK is involved in apoptosis and is an important regulator in signal pathway of cell apoptosis.

  • Management and Prophylaxis of Complications in Laparoscopic Gastrectomy

    ObjectiveTo summarize the current status and the management and prophylaxis of complications in laparoscopic gastrectomy. MethodsThe literatures on laparoscopic gastrectomy in domestic and abroad were reviewed and analyzed combined with our experiences. ResultsThe complications of laparoscopic gastrectomy primary attributed to surgery itself and pneumoperitoneum. Limited field under laparoscopy, loss of threedimensional space and finger touch, new instrument and technology and working conditions increased the difficulty of operation and the possibility of surgical complications. Clear anatomical layer under laparoscopy, accurate dissection of lymph nodes, and digestive tract reconstruction were the basis to reduce the complications in laparoscopic gastrectomy. ConclusionCorrect surgical procedures may reduce the occurrence of complications in laparoscopic gastrectomy.

  • Clinical Outcomes of Combined Complete Thoracoscopic and Laparoscopic Esophagectomy for Esophageal Carcinoma

    Objective To explore the feasibility,safety and clinical applicability of combined complete thoracoscopicand laparoscopic esophagectomy for the treatment of esophageal carcinoma. Methods Clinical data of 34 patients with esophageal carcinoma who underwent combined complete thoracoscopic and laparoscopic esophagectomy in the Departmentof Thoracic Surgery of our hospital from January 2012 to June 2013 were analyzed retrospectively. There were 33 males and1 female with their age of 63 (41-76) years. Results One patient received conversion to laparotomy and 1 patient diedpostoperatively. Mean operation time was 362.1 (300-560) minutes,including 90.6 (60-220) minutes for thoracoscopicprocedure in 34 patients and 61.1 (45-85) minutes for laparoscopic procedure in 33 patients. Mean intraoperative blood loss was 206.5 (100-500) ml. Average number of dissected lymph nodes was 18.0 (13-31) for each patient with positivemetastatic rate of 44.1% (15/34). Postoperative pathological diagnosis was squamous cell carcinoma in 33 patients and smallcell carcinoma in 1 patient. Postoperative pathological staging was stageⅠB in 1 patient,stageⅡA in 1 patient,stageⅡB in 15 patients,stageⅢA in 11 patients,stageⅢB in 3 patients and stageⅢC in 3 patients. Postoperative hospital stay was 15.2 (6-35) days. Two patients received bedside bronchoscopic sputum suction. Postoperative complications occurred in32.4% (11/34) of all patients,including pulmonary infection in 4 patients (11.8%),respiratory failure in 2 patients (5.9%),chylothorax in 1 patient (2.9%),cervical anastomotic leak in 4 patients (11.8%) and hoarseness in 2 patients (5.9%). We followed up 33 patients for 1-16 months. Two died,Two were lost. The other 29 patients survived. Conclusion Combined complete thoracoscopic and laparoscopic esophagectomy is a minimally invasive,feasible and safe surgical procedure for the treatment of esophageal carcinoma with quick postoperative recovery,and is worthy of furtherclinical application.

  • Comparison of long-term prognosis in elderly stageⅠnon-small cell lung cancer patients undergoing lobectomy or segmentectomy: A propensity score matching study

    ObjectiveTo compare the long-term prognosis of elderly patients with stageⅠnon-small cell lung cancer (NSCLC) after lobectomy or segmentectomy. MethodsData of elderly patients with stageⅠNSCLC were collected from the SEER database. According to the resection method, patients were divided into a lobectomy group and a segmentectomy group. The overall survival (OS) and lung cancer-specific survival (LCSS) of the two groups were compared by propensity score matching (lobectomy : segmentectomy=2:1). ResultsA total of 9990 patients were included, including 5840 (58.46%) females and 4150 (41.54%) males, with an average age of 70.48±6.47 years. Among them, 9029 patients were in the lobectomy group and 961 patients were in the segmentectomy group. After propensity score matching, a total of 2883 patients were matched, including 1922 patients in the lobectomy group and 961 patients in the segmentectomy group. There was no statistical difference in baseline data between the two groups (P>0.05). The 10-year OS rate and LCSS rate of the lobectomy group were higher than those of the segmentectomy group (OS: 51.15% vs. 38.35%, P<0.01; LCSS: 79.68% vs. 71.52%, P<0.01). Subgroup analysis found that the survival advantage of lobectomy was only limited to patients aged 60-70 years; for patients>70 years, there was no statistical difference in the 5-year and 10-year OS rates between the two surgical methods (P>0.05). In addition, for patients with tumor diameter ≤2 cm (stages ⅠA1-ⅠA2), lymph node dissection number≥10, and receiving adjuvant radiotherapy/chemotherapy, segmentectomy could also achieve a similar prognosis as lobectomy. ConclusionOverall, for elderly patients with stage ⅠNSCLC, lobectomy can achieve better OS and LCSS. However, individual differences, tumor characteristics, and perioperative treatment plans should be considered comprehensively to determine whether elderly patients with stageⅠNSCLC should undergo segmentectomy.

  • Diagnosis and Surgical Treatment of Adult Hirschsprung’s Disease (Report of 8 Cases)

    目的 探讨成人先天性巨结肠的诊断和外科治疗。 方法 回顾性分析1990年1月至2004年12月我院收治的8例成人先天性巨结肠患者的临床资料。结果 男6例,女2例,年龄19~49岁,平均31岁,均行手术治疗,其中2例因急性肠梗阻行结肠造瘘术,另6例患者行Duhamel手术一期切除,其中2例行Kocker钳夹法,4例采用吻合器吻合,保留端肠管术中冰冻病理检查均可见神经节细胞。8例患者随访至今无复发,生活质量佳,排便功能良好,男性患者无性功能障碍。结论 Duhamel手术是治疗成人先天性巨结肠的有效方法,吻合器吻合可明显减轻患者的痛苦,术后无复发,排便功能及性功能良好,值得临床推广。

  • Application of composite tissue flaps pedicled with distal perforating branch of posterior tibial artery for repairing distal leg defects

    Objective To investigate the clinical application and effectiveness of the composite tissue flaps pedicled with perforating branch of posterior tibial artery for repairing distal leg defects. Methods Between September 2014 and August 2017, 12 patients with skin and bone defects of distal leg were repaired with the composite tissue flaps pedicled with perforating branch of posterior tibial artery. There were 8 males and 4 females with an average age of 41.3 years (range, 25-66 years). The causes of injury included traffic accident injury in 7 cases, heavy crushing injury in 2 cases, tibial osteomyelitis with soft tissue ulcer and necrosis in 2 cases, and bone and soft tissue defect after resection of bone tumor in 1 case. Eight patients underwent primary repair, and 4 patients underwent second-stage repair. The size ranged from 6 cm×4 cm to 10 cm×7 cm in skin flap, from 4.0 cm×2.5 cm to 8.0 cm×6.0 cm in muscle flap, and from 4 cm×2 cm×2 cm to 5 cm×4 cm×4 cm in tibial bone flap. Tibial defects of the donor region were repaired by autologous iliac bone grafting, and the wounds were sutured directly in 7 cases and repaired by autologous skin grafting in 5 cases. Results All composite tissue flaps survived and both the recipient and the donor wounds healed primarily. All patients were followed up 6-12 months, with an average of 10.8 months. The appearance, color, texture of the composite tissue flaps and ankle function were satisfactory. X-ray films showed that the bone flap at the tibia defect and the ilium graft at the donor site both healed well at 6 months after operation. Conclusion The composite tissue flaps pedicled with perforating branch of posterior tibial artery has abundant blood, and it is a good donor region for repairing the distal leg defects combined with circumscribed bone defect.
