  • Reasons of Missed Diagnosis and Misdiagnosis of Glandularis Cystitis by Ultrasonography

    【摘要】 目的 分析超声对腺性膀胱炎的误、漏诊原因,探讨减少其误、漏诊的方法。 方法 回顾性分析2006年1月-2010年2月经病理证实的135例腺性膀胱炎的声像图表现。 结果 135例腺性膀胱炎患者中,超声误诊26例,误诊率19.3%,漏诊11例,漏诊率8.2%。误诊的主要原因:乳头结节型和团块型与膀胱肿瘤声像图极为相似、容易混淆,超声医师对膀胱壁各层次的观察不仔细,对病史重视不够;漏诊的主要原因:膀胱充盈不佳或不充盈,病变体积太小、位于前壁或顶部,或病变位于膀胱后壁及颈部被明显增生的前列腺、膀胱内血凝块及膀胱结石等掩盖。 结论 超声是诊断腺性膀胱炎常用方法,但存在一定的误、漏诊,改进检查方法,可减少其误、漏诊发生。【Abstract】 Objective To analyze the reasons of missed diagnosis and misdiagnosis of glandularis cystitis by ultrasonography. Methods The sonographic outcomes of 135 patients with glandular cystitis diagnosed by pathological examination from January 2006 to February 2010 were retrospectively analyzed. Results In 135 patients, misdiagnosis was in 26 with a rate of 19.3%, missed diagnosis was in 11 with a rate of 8.2%. The reasons of misdiagnosis included: the sonographic outcomes of mastoid and tuberculous conglomeration were similar to those of bladder tumour; the ultrasonographic professionals didn’t clearly observe each layer of the bladder wall, and didn’t pay enough attention to the disease history. The reasons of missed diagnosis included: the bladder was under-filled or unfilled, the size of the lesions were too small and were located at the anterior wall or the top, and the lesions were located at the posterior wall and neck of the bladder which were covered up by obvious prostate hyperplasia,and gores or stones of bladder so that the lesions could not be observed. Conclusion Ultrasonography is a usual method for diagnosing glandular cystitis,and we should ameliorate the examination to decrease the misdiagnosis and missed diagnosis.

  • Chemotherapeutics Bladder Irrigation after Transurethral Resection for Cystitis Glandularis: A Meta-Analysis

    Objective To evaluate the effectiveness and safety of chemotherapeutics bladder irrigation (CBI) after transurethral resection (TR) in the treatment of cystitis glandularis (CG). Methods Databases including MEDLINE, The Cochrane Library, EMbase, VIP, CNKI and CBM were searched from January 2001 to November 2011 to collect randomized controlled trials (RCTs) and case-control studies (CCSs) on pirarubicin or mitomycin bladder irrigation after TR in the treatment of CG. Two reviewers independently screened articles according to the inclusion and exclusion criteria, extracted data and evaluated the quality of the included studies. Then meta-analysis was performed using RevMan 5.0. Results A total of 11 articles involving 5 RCTs and 6 CCSs were included. Among the total 1032 patients involved, 497 patients were in the control group treated by TR alone, while the other 535 patients were in the treatment group given CBI after RT. There were two subgroups, one involving 347 patients irrigated by pirarubicin in 7 studies, and the other involving 188 patients irrigated by mitomycin in 4 studies. The results of meta-analysis showed: (a) pirarubicin bladder irrigation after TR could increase both short-term and long-term cure rates and decrease both short-term and long-term relapse rates, but no significant differences were found in both short-term and long-term improvement rates, compared with the control group. As for the safety, pirarubicin was similar to the control group in the incidence of urinary irritation, but it was superior in the incidence of bloody urine; and (b) mitomycin bladder irrigation after TR could increase long-term cure rate and decrease long-term relapse rate, but no significant differences were found in short-term cure rate and short-term improvement rate, compared with the control group. Mitomycin was similar to the control group in incidence of urinary irritation and bloody urine. Sensitivity analyses indicated the outcomes regarding to some indexes in different studies were inconsistent. Conclusion Based on the current evidence, pirarubicin or mitomycin bladder irrigation after TR can increase long-term cure rate and decrease long-term relapse rate in treating CG, but pirarubicin tends to easily cause bloody urine. For the inconsistent outcomes of different studies, the results of this meta-analysis are instable and highly possible to be inconsistent to the future outcomes, hereby it is uncertain of the better effectiveness of CBI after, TR compared with TR alone, and more high-quality and large-scale RCTs are needed to be performed.

  • Comparison between the Clinical Outcome of Early and Deferred Intravesical Instillation Therapy in Treating Cystitis Glandularis after Transurethral Resection

    ObjectiveTo evaluate the clinical effect of early and deferred intravesical instillation in the treatment of cystitis glandularis after transurethral resection (TUR). MethodsWe retrospectively analyzed the clinical data of 95 patients with cystitis glandularis during February 2007 to April 2012. Among them, 37 patients underwent the first intravesical instillation within 24 hours after transurethral resection (group A), while the others underwent the same treatment within a week (group B). Then, intravesical instillation in all patients were carried out once every week for 8 weeks, and after that, it was carried out once every month for 5 to 10 months. All the patients were followed up for 12 to 16 months. The cure rate, improvement rate, total effective rate, recurrence rate and incidence of adverse events associated with therapy were observed. ResultsRecovery rate, improvement rate, side effects were observed in group A and B respectively, and there was no significant difference between the two groups (P>0.05). But there was significant difference in the total effective rate and recurrence rate (P<0.05). ConclusionThe first intravesical instillation within 24 hours after transurethral resection in the treatment of cystitis glandularis can improve curative effect and lower recurrence rate, without the increase of side effects.
