  • The valuation of quantitative lung data of vibration response imaging in diagnosis of obstructive lung diseases

    Objective Vibration response imaging(VRI) is a new lung sound imaging technology.It provides quantitative lung data(QLD) of vibration in respiratory system.The study is to explore the value of QLD in diagnosis of obstructive lung diseases.Methods The QLD of 61 chronic obstructive pulmonary disease(COPD) patients,58 asthma patients and 64 healthy volunteers were reviewed.The QLD were transferred to abnormity and variation by a formulation and were analyzed.Results The mean QLD of healthy volunteers were 8.4,14.5,22.0,11.1,18.5,25.5 with mean abnormity as 10.0 and mean variation as 2.0.The mean QLD of the COPD patients were 11.6,16.7,21.9,12.6,17.2,20.1 with mean abnormity as 47.1 and mean variation as 10.9.The mean QLD of the asthma patients were 12.8,17.2,19.9,13.3,17.5,19.3 with mean abnormity as 58.1 and mean variation as 12.2.The abnormity and variation of the patients were different from those of volunteers(Plt;0.05).When abnormity≥20.0 or variation≥5.0 was define as threshold value,the specificity was 87.5%.The diagnosis sensitivity for COPD is 82.0% and sensitivity for asthma is 82.8%.Conclusion COPD and asthma patients can be detected by quantitative lung data from vibration response imaging.

  • The Difference of Clinical Manifestation in Patients of Churg Strauss Syndrome Between the Patients with Antineutrophil Cytoplasmic Antibody Positive and Negative

    Objective To clarify the differences of clinical manifestations in patients of Churg Strauss syndrome (CSS) with or without antineutrophil cytoplasmic antibody (ANCA) positive,and primarily explore the roles of ANCA and eosinophilia in CSS according to current literatures. Methods The clinical data of 25 patients with CSS,admitted to Peking Union Medical College Hospital from January 2001 to November 2010,were retrospectively investigated. Results The positive rate of ANCA in Churg Strauss was 32.0%,with P-ANCA predominantly (in 6 patients). The lung involvement in the patients between ANCA positive and ANCA negative patients was 16.7% and 82.4% respectively with significant difference. The heart involvement in the patients between ANCA positive and ANCA negative patients were 60.0% and 64.2%,respectively.The peripheral neuropathy between ANCA positive and ANCA negative patients were 85.7% and 66.7%,respectively.The renal manifestations between ANCA positive and ANCA negative patients were 75.0% and 35.3%,respectively. But the differences in heart,peripheral neuropathy,orrenal manifestations were not significant. Conclusion The status of ANCA may represent different clinical phenotype of CSS.

  • AppHcation of bronchial provocation test on etiological diagno~s of chronic cough

    Objective To investigate the changes of small airway function,airway resistance and responsiveness of extrathoracic airway in chronic cough patients before and after bronchial provocation test (BPT).Methods 68 chronic cough patients were requested to conduct lung function test and BPT.The airw ay resistance were measured via forced oscillationary technology before and after BPT.Results BPT revealed airway hyperresponsiveness in 52%subjects.MEF50 and R0 before BPT were significantly different between the patients with or without airway hyperresponsiveness.Post BPT changes in MEF50(MEF50%) were correlated positively to the changes in FEV1(FEVl%),and negatively to the changes in R0[Ro-d]. Extrathoracic airway hyperresponsivenes(EAHR)was f0und in13 patients,in which 6 patients were not revealed by routine BPT.Conclusion There is small airway function abnormalities in chronic cough patients.Extrathoracic airway responsiveness test is a valuable supplementary index to routine BPT.

  • Asynchrony Between Left and Right Lungs in Evaluation of Early Smoking-related Lung Abnormalities

    ObjectiveTo observe the asynchrony patterns between left and right lungs in smokers and non-smokers,to assess the role of vibration response imaging(VRI) in the early detection and evaluation of smoking-related lung abnormalities. MethodsData were collected as follows:(1)past history and smoking history were collected;(2)exhaled CO test to confirm smoking status was performed;(3)VRI test was performed and the curve of Breath Energy Unit(BEU)was drawn,which is an energy versus time graph of the breath energy.The asynchrony between left and right lungs was derived from this graph;(4)pulmonary function test was performed.In the end,26 villagers with normal spirometry findings were included in the study.The subjects were divided into an ever-smoking group and a never-smoking group. ResultsThe BEU lung asynchrony was 2.0(3.0) frame in the never-smoking group,and 2.0(3.0) frame too in the ever-smoking group.Rank sum test showed that there was no significant difference(Z=-0.29,P=0.77) between the never-smokers and the ever-smokers in the lung asynchrony.Rank correlation analysis suggested that in the ever-smoking group,smoking index and BEU asynchrony had significant correlation(r=0.61,P=0.03).In the never-smoking group,the coefficient of passive smoking index and lung asynchrony was 0.52(P=0.07).The P value of the coefficient between passive smoking index and lung asynchrony was nearly 0.05,scatter between them could be seen a presence of a certain trend. ConclusionThe lung asynchrony in VRI has dose-effect relationship with ever-smokers' smoking level(smoking index).Thus,the lung abnormalities in VRI caused by the exposure to passive smoking is maybe the same as the abnormalities caused by direct smoking.

  • The Relationship Between Tumor Biomarkers and Idiopathic Pulmonary Fibrosis

    【Abstract】 Objective To evaluate the relationship between multiple tumor biomarkers and idiopathic pulmonary fibrosis ( IPF) , and analyze the prognostic value of these biomarkers in IPF. Methods Clinical data of 43 confirmed IPF patients with no evidence of malignant disaeses, admitted in Peking Union Medical College Hospital between January 2000 and June 2010, were retrospectively analyzed. All IPF patients had detected serum alpha fetoprotein ( AFP) , cancer antigen 50 ( CA50) , cancer antigen 24-2( CA24-2) , carcinoembryonic antigen ( CEA) , carbohydrate antigen 19-9 ( CA19-9) , cancer antigen 125( CA125) , cancer antigen 15-3 ( CA15-3) , tissue polypeptide antigen ( TPA) , neuron specific enolase( NSE) , and cytokeratin-19-fragment ( Cyfra211) . Results The serum levels of CEA, CA19-9, CA125,CA15-3, and TPA were obviously higher than normal range, while the serum levels of AFP, CA50, CA24-2,NSE, and Cyfra211 were within normal range. Neither tumor biomarkers had correlation with 6-minute walk distance, FVC% pred, TLC% pred, DLCO/VA, PaO2 , PaO2 /FiO2 , P( A-a) O2 , BALF cell differentiation counting,or CD4 /CD8. The patients with increased CA19-9 level had shorter survival time than those with normal CA19-9 level ( P lt; 0. 05) . There was no significant difference in survival time between the patients with increased CEA/TPA levels and those with normal CEA/TPA levels( P gt;0. 05) , neither between the patients with glucocorticoid treatment and those with non-glucocorticoid treatment ( P gt; 0. 05) . Conclusions Multiple tumor biomarkers, especially CA19-9, increase in IPF patients. The degrees of those increases arenot associated with the severity of disease, but closely relate to prognosis, and may also indicate the progression. The increases of multiple tumor biomarkers may be a sign of poor prognosis of IPF with no evidence of malignant disaeses.

  • Clinical analysis of obliterative bronchiolitis

    Objective To investigate the etiology and the clinical features of obliterative bronchiolitis (OB). Methods Sixteen admitted patients were diagnosed as OB in Peking Union Medical College Hospital from Jan 1990 to May 2018. The case records were reviewed retrospectively. Results Twelve patients were female and 4 patients were male. The median age was 33.5 years (ranged from 17 to 55). The common symptoms included: exertional dyspnea in 15 patients (94%), cough in 10 patients (63%), phlegm production in 8 patients (50%). All of the 16 patients had associated conditions: paraneoplastic pemphigus in 6 patients (Castleman’s disease, lymphoproliferative disorder, inflammatory myofibroblastic tumor and follicular dendritic cell tumor respectively), chronic graft-versus-host disease after hematopoietic stem-cell transplantation in 3 patients, thymoma in 2 patients, Behcet’s disease, Sjögren syndrome, multiple myeloma, myasthenia gravis, and oral lichen planus in 1 patient respectively. The pulmonary function tests revealed severe or very severe obstructive ventilatory defect in all 16 patients. The CT scans showed slight ground-glass opacities in 10 patients, decreased attenuation of lung in 9 patients, mosaic attenuation in 8 patients, bronchiectasis in 7 patients, and air trapping in 3 patients. Conclusions OB is a clinical syndrome characterized by progressive airway obstruction, resulting from a variety of exposures or diseases. In most cases, diagnosis can be obtained on the basis of typical clinical, physiological, and radiological features, combined with OB-related diseases or exposures. Surgical lung biopsy can help diagnosis but risk-benefit should be taken into consideration seriously.

  • Pulmonary Involvement in Patients with Systemic Sclerosis

    Objective To investigate the lung involvement in Chinese patients with systemic sclerosis ( SSc) and its functional impact. Methods 68 patients with SSc were enrolled in Scleroderma study of PUMCH ( Peking Union Medical College Hospital) . All the patients underwent high resolution computed tomography ( HRCT) , pulmonary function testing, 6-minute walk testing, and echocardiography. And 36 patients filled in the Saint George’s Respiratory Questionnaire ( SGRQ) for assessment of healthrelated quality of life. Results HRCT revealed interstitial lung disease ( ILD) in 52 ( 76. 5% ) patients, 20 out of them without respiratory symptoms. Reticular opacification, ground-glass opacity ( GGO) , traction bronchiectasis, and honeycomb were presented respectively in 80.8% , 73.1% , 59.6% , and 30.8% of patients with SSc-ILD. Fibrosis was the predominant HRCT finding, and pure GGO ( in the absence of reticulation or architectural distortion) was only present in 8 ( 15. 7% ) patients. 57 (83.8% ) patients presented with diffusion defect, with most of them having moderate to severe impairent. Reduced FVC or TLC presented in 20 ( 29.4% ) and 28 ( 41.2% ) of patients respectively. The significant correlation was observed between the DLCO and the extent of ILD on HRCT ( rs = -0.476, P =0.000) . DLCO showed significant correlations with all the four components of the SGRQ ( Plt;0.05) . Significant correlations were also observed among the SGRQ scores and SpO2 ( maximum desaturation) or Borg index. Stepwise multiple regression analysis confirmed that the DLCO, SpO2 , and Borg index contributed to the SGRQ. Conclusions Lung involvement in Chinese SSc patients is common and insidious. The HRCT features of SSc-ILD are predominant fibrosis plus GGO, indicating little reversibility. Thus HRCT should be performed routinely in newly diagnosed SSc patients for early screening of lung involvement. The lung function defect of SSc is characterized by reduced diffusing capacity, and DLCO show better correlations with HRCT abnormities or SGRQ than FVC or TLC. Thus DLCO is of great value for early detection or severity assessment of SSc-ILD. SGRQ can be used to examine the health related quality of life of SSc patients and may reflect severity of lung involvement.

  • Prevalence of anxiety and depression in chronic obstructive pulmonary disease

    ObjectiveTo investigate prevalence of anxiety and depression in chronic obstructive pulmonary disease (COPD) in order to increase the awareness of physicians on this comorbidity.MethodsPatients were from a multicenter prospective cohort study and 13 institutions or hospitals participated in the study. Four hundred and ten patients with a prior diagnosis of COPD were prospectively recruited from January 2017 to January 2019, and baseline date were analyzed. Hospital Anxiety and Depression Scale was used to identify anxiety and depression.ResultsThe cohort had a mean age of (60.7±10.0) years with 76.8% males and 74.3% smokers. Seventy-nine patients (19.3%) had anxiety or depression. Among them, 36 subjects (8.8%) had anxiety, and 64 patients (15.6%) were with depression, and 21 patients (5.1%) with anxiety and depression. The prevalence of severe anxiety (2.7%) and severe depression (2.4%) was low. Compared to patients without anxiety or depression, patients with anxiety or depression had significantly higher CAT scores (17.8±9.3 with anxiety, 17.4±8.4 with depression, 12.5±7.3 without anxiety or depression, P 0.002 and 0.000 respectively). The risk of acute exacerbations was higher in patients with depression (37.5% vs. 22.7%, P=0.016). Proportion of patients categorized into GOLD group D was higher in patients with depression (P=0.001).ConclusionsAbout 20% COPD patients has anxiety or depression. Prevalence of depression is two times that of anxiety. The prevalence of severe anxiety or severe depression is low. Patients with depression have lower quality of life, higher risk of acute exacerbations, and higher proportion of patients categorized into GOLD group D.
