  • EFFECTS OF LEUCOCYTE- AND PLATELET-RICH PLASMA ON OSTEOGENIC DIFFERENTIATION OF BONE MARROW MESENCHYMAL STEM CELLS IN TREATING AVASCULAR NECROSIS OF FEMORAL HEAD IN RABBITS

    ObjectiveTo evaluate the effect of leucocyte- and platelet-rich plasma (L-PRP) on the osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs) in treating avascular necrosis of the femoral head (ANFH) in rabbits. MethodsTwenty-four New Zealand white rabbits (4-6 months old, both genders, weighing 2.0-3.0 kg) were used for the establishment of bilateral ANFH models and divided into 4 groups (n=6). BMSCs were isolated from the bone marrow of iliac crest, cultured and identified. L-PRP was prepared by Landesberg method. Core decompression only (group A), core decompression and L-PRP implantation (group B), core decompression and BMSCs implantation (group C), and core decompression and implantation of BMSCs and L-PRP were performed in 4 groups. To evaluate bone formation and remodeling of the defects, X-ray photography was taken at 2, 4, and 8 weeks postoperatively. The modified Lane-Sandhu scoring system was used to evaluate the bone formation. Two rabbits were sacrificed at 2, 4, 8 weeks after operation to harvest the specimens for histological observation, new blood vessel count and new bone area ratio. ResultsThe observations of radiology and histology displayed different degrees of bone regeneration at bone defect sites in each group. At 2, 4, and 8 weeks postoperatively, the results of Lane-Sandhu X-ray photography scoring, new blood vessel count, and new bone area ratio showed that groups C and D were significantly better than groups A and B, group D was significantly better than group C. and group B was significantly better than group A (P<0.05). ConclusionThese findings demonstrate that L-PRP can promote osteogenic differentiation of BMSCs in treating ANFH in rabbits, and core decompression associated with BMSCs and L-PRP is an effective and feasible method to treat ANFH.

  • Platelet-Rich Plasma for the Treatment of Periodontal Intrabony Defects: A Meta-Analysis

    Objective To systematically evaluate the clinical effectiveness of platelet-rich plasma (PRP) combined with grafting material for the treatment of periodontal intrabony defects. Methods The following databases such as PubMed, The Cochrane Library, EMbase, CNKI, CBM and WanFang Data were searched on computer from inception to August, 2012 to collect the relevant randomized controlled trials (RCTs) on PRP combined with grafting material versus grafting material alone for periodontal intrabony defects. Two reviewers independently screened the literature according to the inclusion and exclusion criteria, extracted the data, and assessed the methodological quality of the included studies. RevMan 5.2 software was applied for meta-analysis. Results A total of 11 RCTs involving 342 patients were included. The pooled analysis on 7 RCTs showed that there was a significant difference in lower increase of clinical attachment loss (WMD=0.70, 95%CI 0.51 to 0.90, Plt;0.000 01) between the PRP combined with grafting material group and the grafting material alone group. But there was no significant difference in the gingival recession (WMD= −0.01, 95%CI −0.15 to 0.13, P=0.86). The pooled analysis on 9 RCTs showed that there was no significant difference in the reduction of plaque index (WMD= −0.04, 95%CI −0.09 to 0.02, P=0.20) between the two groups. Conclusion PRP combined with grafting material is superior to grafting material alone in the clinical attachment loss. But, there are no significant differences in gingival recession and plaque index. However, given the limited sample size and incomplete measure indexes of included studies, this conclusion still needs to be further proved by conducting more high-quality and large-scale RCTs.

  • Effectiveness of Platelet Increasing Capsule Combined with Hormone in Treating Idiopathic Thrombocytopenic Purpura: A Systematic Review

    Objective To systematically evaluate the effectiveness of platelet increasing capsule combined with hormone in treating idiopathic thrombocytopenic purpura (ITP). Methods Randomized controlled trials (RCTs) of ITP treated by platelet increasing capsule combined with hormone were electronically searched from PubMed (1966 to 2012), The Cochrane Library (CENTRAL, Issue 3, 2012), CBM (1978 to 2012), CNKI (1979 to 2012), WanFang Data (1998 to 2012), and VIP (1991 to 2012). References of included studies were also retrieved. The literature was independently screened according to exclusion and inclusion criteria by two researchers independently and meta-analysis was conducted using RevMan 5.1 software after data extraction and quality assessment. Results 10 RCTs were included involving 588 patients. The results meta-analysis showed that, the group which was treated by platelet increasing capsule combined with hormone was superior to the hormone alone group in the aspects of overall effectiveness rate (RR=1.18, 95%CI 1.06 to 1.32, P=0.003), the significant effectiveness rate (RR=1.57, 95%CI 1.29 to 1.91, Plt;0.000 01), blood platelet count (MD=21.54, 95%CI 13.85 to 29.23, Plt;0.000 01), and the recurrence rate (RR=0.49, 95%CI 0.34 to 0.69, Plt;0.000 01) which was lower. Conclusion Current evidence has showed that platelet increasing capsule combined with hormone in treating ITP is better than hormone alone. However, more high quality RCTs are needed to verify the above conclusion in future.

  • Intracoronary Glycoprotein IIb/IIIa Inhibitor for Patients with Acute Coronary Syndrome Undergoing Percutaneous Coronary Intervention: A Meta-Analysis

    Objective To systematically review the effectiveness and safety of intracoronary glycoprotein IIb/IIIa inhibitors (GPIs) undergoing percutaneous coronary intervention (PCI) for acute coronary syndrome (ACS) compared with intravenous administration. Methods Databases including PubMed, EMbase, The Cochrane Library (Issue 9, 2012), Ovid, CBM, CNKI and VIP were electronically searched for randomized controlled trials (RCTs) about intracoronary GPIs administration versus intravenous administration undergoing PCI for ACS from inception to September 30th, 2012. Meanwhile, domestic relevant papers published in recent 1 year were also retrieved manually. References of the included studies were retrieved, too. According to the inclusion and exclusion criteria, two reviewers independently screened literature, extracted data, and assessed the methodologically quality of the included studies. Then, meta-analysis was performed using RevMan 5.1 software. Results 10 RCTs involving 3 553 ACS patients were finally included. The results of meta-analysis showed that: compared with intravenous administration, intracoronary GPIs administration decreased the major adverse cardiovascular event (MACE) (OR=0.54, 95%CI 0.34 to 0.85, P=0.008). The incidences of re-infarction (MI), revascularization (TVR) and heart failure were (OR=0.62, 95%CI 0.39 to 0.97, P=0.04), (OR=0.59, 95%CI 0.36 to 0.97, P=0.04), (OR=0.52, 95%CI 0.32 to 0.84, P=0.008), respectively. But for the mortality, there were no significant differences between the two groups (OR=0.81, 95%CI 0.58 to 1.14, P=0.23). Intravenous administration and intracoronary administration were alike in the incidences of mild/serious bleeding (mild: OR=0.94, 95%CI 0.75 to 1.19, P=0.63; serious: OR=1.18, 95%CI 0.76, 1.84, P=0.47). Conclusion Compared with routine GPIs regimen of intravenous bolus, intracoronary administration with initial dosage showed significant benefits in clinical outcomes in ACS patients undergoing PCI, which could not increase the incidence of bleeding.

  • Relationship between Platelet Parameters and Traumatic Cerebral Infarction in Acute Phase of Craniocerebral Injury

    目的 探讨颅脑损伤急性期血小板(PLT)参数与外伤性脑梗死(TCI)的关系。 方法 选取2010年9月-2012年1月符合纳入除标准的颅脑损伤患者191例。将伤后14 d内发生脑梗死的患者划入观察组,余为对照组。分别于伤后第24、48小时,第3、7、14天,采集肘静脉血对PLT计数、PLT平均体积(MPV)、PLT体积分布宽度(PDW);伤后24 h行格拉斯哥昏迷评分(GCS),伤后6个月随访行格拉斯哥结果评分(GOS)。分别分析脑梗死与上述PLT参数的关系、PLT参数与颅脑损伤病情轻重的关系以及与预后的关系。 结果 观察组PLT计数降低、MPV升高、PDW升高;观察组患者GCS/GOS与PLT计数呈正相关,与MPV、PDW呈负相关。 结论 PLT计数、MPV、PDW均与TCI有关,三者中PLT最具临床意义。PLT计数越低、MPV和PDW越高,脑梗死可能性越大,颅脑损伤可能越严重,预后可能越差。

  • Clinical Observation of Thrombocytopenia Induced by Heparin for Sealing Venous Remaining Needle

    目的 探讨静脉留置针封管用肝素诱导的血小板减少症(HIT)的临床特点。 方法 分析2010年1月-2011年12月,在1 215例静脉留置针肝素封管患者中发生的14例HIT患者的临床表现,血小板(PLT)、HIT抗体的变化,氯吡格雷、阿加曲班、地塞米松等治疗的结果。 结果 1 215例患者中:发生HIT 14例,发生率1.15%;HIT并血栓形成综合征4例,发生率0.33%。14例HIT患者中:经典型12例,占85.7%。出血7例,发生率50.0%,其中1级、2级出血发生率分别为42.9%(6/14)、7.1%(1/14),分别占出血的85.7%(6/7)、14.3%(1/7)。血栓形成4例,发生率28.6%,其中静脉血栓3例,占血栓形成的75.0%。14例均发生于肝素封管后第1~14天,其中第6~7天8例,占57.1%。PLT降低到最低值的时间为肝素封管后2~10 d内,其中2~5 d内13例,占92.9%。PLT降低的最低值为(1.81~101)×109/L,其中PLT(20~70)×109/L12例,占85.7%;PLT降至最低时下降的比值为51.1%~90.1%,其中50%~80%10例、占71.4%。1例至28 d死亡时血小板没有恢复至肝素封管前水平,其余13例患者PLT减少持续时间5~13 d,其中5~10 d 10例,占76.9%。14例(100%)HIT患者HIT抗体阳性,其中13例(92.9%)在PLT开始减少时即阳性,1例在PLT减少2周后呈阳性。14例HIT患者中12例(85.8%)痊愈,1例(7.1%)脑血栓后遗症,1例(7.1%)死于急性肺栓塞。 结论 静脉留置针封管用肝素可导致HIT,PLT减少及血栓、HIT抗体是诊断的可靠依据。及时停用肝素,必要时抗凝、抗PLT、类固醇激素治疗效果好,部分患者进展快,死亡率高。

  • Significance of Expression of Platelet Derived Growth Factor and Survivin of Hepatocellular Carcinoma with Portal Vein Tumor Thrombosis

    Objective To investigate the expressions of platelet derived growth factor (PDGF) and survivin in hepatocellular carcinoma (HCC) with portal vein tumor thrombosis (PVTT) and to evaluate the relationships among the expressions of PDGF, survivin and the proliferation of cancer cells. Methods The expression of PDGF mRNA in 16 cases in HCC with PVTT group was observed by in situ hybridization and the results were compared with that in HCC tissue group. The expressions of PDGF and survivin protein in 36 cases in HCC with PVTT group were detected with immunohistochemistry and it was found that there was a correlation between them. Flow cytometry (FCM) was used to measure the proliferation of cancer cells and it was also used to analyze the relations among PDGF, survivin and the proliferation of cancer cells. Results The expression level of PDGF mRNA in HCC with PVTT group was significantly higher than that in HCC tissue group (P<0.01). There was a positive correlation between the expressions of PDGF and survivin protein in HCC with PVTT (P<0.01). The degree of proliferation of cancer cells in PDGF and survivin protein positive group was significantly higher than that in negative group (P<0.01). Conclusion PDGF and survivin gene over-expressed in HCC with PVTT group and there is a positive correlation between the expressions of PDGF and survivin protein. The proliferation of cancer cells increases as the expressions of PDGF and survivin increase.

  • Clinical Observation of Splenectomy on Chronic Idiopathic Thrombocytopenic Purpura

    ObjectiveTo study the results of splenectomy in patients with idiopathic thrombocytopenic purpura. MethodsSeven patients who failed to respond to conservative management were treated with splenectomy and followed up for 6 months to 8 years (1990~1999).ResultsThe presplenectomy patients had symptoms of bleeding and their platelet count on average was 32×109/L. The 3th,7th day and 1th,2th, 6th month after splenectomy, the average platelet count was 191×109/L,354×109/L,317×109/L,200×109/L and 151×109/L respectively. Their platelet recovered to normal during a week in 7 cases (≥100×109/L); In 6 patients the platelet count was normal in the 6th month after splenectomy, the success rate was 6/7, the rate of remission was 1/6. The platelet count after splenectomy was significantly higher than that before splenectomy.ConclusionThere are no correlation between the course of disease before splenectomy and the results of splenectomy. Splenectomy is safe and effective in the treatment of idiopathic thrombocytopenic purpura.

  • THE CHANGE AND SIGNIFICANCE OF PLASMA LEVELS OF ENDOTHELIN IN ACUTE NECROTIZING PANCREATITIS IN RATS

    This prospective animal study was designed to investigate the changes of plasma endothelin (ET) levels in acute necrotizing pancreatitis (ANP). Sprague-Dawley rats were randomly devided into 3 groups: acute necrotizing pancreatitis (ANP) group in which ANP was induced by infusion of 5% sodium taurocholate (STC) into biliopancreatic duct, sham operation (SO) group and platelet activating factor antagonist BN50739 (BN) group. Blood levels of ET and platelet activating factor (PAF) were detected. Pancreatic microcirculatory blood flow was measured and pancreatic histological scores were evaluated. Results showed that the pancreatic microcirculatory blood flow in ANP group was decreased to a great extent immediatly after induction of ANP and soon began to rise slowly for 3 hours and again decreased steadily after that. The blood levels of ET, PAF and histological scores in ANP group were significantly higher than those in SO group. In BN group, the blood flow was significantly improved and the levels of blood ET, PAF and histological scores were all significantly lower as compared to those in ANP group. It is concluded that ischemia/ reperfusion is present in the initiation of acute necrotizing pancreatitis induced by STC in the rat. This leads to injuries of endothelial cells and increase in the production of ET and PAF. I/R lesions,and interaction of ET and PAF lead to a vicious circle, thus augmenting the pathological changes in the pancreas.

  • THE EFFECTS OF KADSURENONE IN ISCHEMIA-REPERFUSION-INDUCED LIVER INJURY IN RATS

    To investigate the role of platelet-activating factor (PAF), neutrophils in ischemia-reperfusion-induced liver injury and their possible mechanism, PAF and the degree of neutrophil infiltration in liver tissue and the preventive effects of PAF antagonist kadsurenone were evaluated in this study by means of a partial liver ischemia model, in which it was induced by clamping only left and median lobes of the liver without causing intestinal congestion. The present study was undertaken to find out the mechanism of liver ischemia-reperfusion injury and preventive effect of kadsurenone. The results indicate that in early stage of reperfusion liver injury possibly caused by the generation of free radicals, declined of autioxidant defence and increased Ca2+ influx, and in the later stage of reperfusion injury was mainly mediated by accumulation of PAF in the liver, which elicits the release of polymorphonuclear leukocytes induced toxical free radical, endothelial damage, microcirculatory collapse. The authors conclude that the effectiveness of antagonist kadsurenone in protecting against ischemiareperfusioninduced liver injury is due not only to their action in preventing the direct effects of PAF, but also to their ability to inhibit both PAF priming and PAF dependent feedback processes, thus preventing escalation of auto generated inflammatory damage.
