Objective To evaluate the role of preoperative 64 multi-detector spiral CT (MDCT) in predicting the extent of radical resection for advanced gastric cancer (AGC). MethodsThe imaging data of 70 patients with AGC were collected and analyzed. The N2 lymph node metastasis was predicted by the MDCT indications, and compared with that postoperative pathological results. Results Sixty-two patients were treated with surgical intervention. The sensitivity, specificity, and accuracy of N2 positive prediction by MDCT was 92.0% (46/50), 75.0% (9/12) and 88.7% (55/62), respectively. Extended resection was performed in 81.6% (40/49) patients who were predicted as N2 positive, and D2 resection was performed in 92.3% (12/13) patients who were predicted as N2 negative.Conclusion The MDCT is a valuable technique to predict N2 lymph node metastasis, and to determine the extent of resection for AGC.

【Abstract】ObjectiveBy using multidetector row spiral CT (MDCT) to investigate the CT imaging findings of gallbladder abnormalities caused by hepatic parenchymal diseases and those of inflammatory cholecystitis. MethodsCT and clinical data of 80 patients with gallbladder abnormalities were retrospectively reviewed. Fifty patients were in hepatic disease group, including 20 chronic hepatitis, 25 liver cirrhosis, and 5 cirrhosis with hepatocellular carcinoma. Thirty patients were in inflammatory group, including 19 chronic cholecystitis, 6 acute cholecystitis, 3 cholecystitis with acute pancreatitis, 1 gangrenous cholecystitis, and 1 xanthogranulomatous cholecystitis. All patients underwent MDCT plain scan and contrastenhanced dualphase scanning of upper abdomen. ResultsIn hepatic disease group, 48 cases had evenly thickened gallbladder wall (96%) with mean thickness of (3.67±0.49) mm; 38 cases had clear gallbladder outlines (76%); 38 cases had gallbladder wall enhancement of various degree (76%); 14 cases had gallbladder bed edema and localized nondependant pericholecystic fluid collection (28%). In inflammatory cholecystitis group, 28 cases had obscuring gallbladder outlines (93%) ; 26 cases had gallbladder wall evenly thickened (87%), 4 cases showed unevenly thicked wall (13%), the mean thickness being (4.54±1.14) mm; 30 cases had inhomogenous enhancement of the gallbladder wall (100%); 9 cases had highattenuation bile (30%); 4 cases had dependant pericholecystic fluid collection (13%); 5 cases had transient enhancement of adjacent hepatic bed in arterial phase (17%); microabscess and gas in the gallbladder wall was observed in 1 case respectively. ConclusionMDCT can offer imaging findings useful for differentiating abnormal gallbladder changes caused by hepatic parenchymal diseases from those due to inflammatory cholecystitis.

ObjectiveTo evaluate the value of spiral CT in diagnosing ischemic bowel changes in systemic lupus erythematosus (SLE) patients presenting with acute abdominal pain. MethodsThe clinical data and spiral CT imaging files of 23 SLE patients presenting with acute abdominal pain were retrospectively reviewed. Sixteen had contrastenhanced spiral CT scanning of the abdomen, the rest had plain CT study. Observation emphasis was placed on the changes of bowel wall (wall thickness, enhancement pattern, lumen size) and mesentery (mesenteric edema, engorgement of mesenteric vessels and their abnormal arrangement pattern). Other abnormal findings (e.g. fluid accumulation, changes of abdominal solid organs, lymphadenopathy) were also observed. ResultsNineteen patients had intestinal wall thickening (19/23, 82.6%), with the “target sign” in 12 patients (12/16, 75.0%); Bowel lumen dilatation was present in 16 patients (16/23, 69.6%). Mesenteric swelling with increased density of adipose tissue was noticed in 21 patients (21/23, 91.3%); 18 patients had engorgement of mesenteric vessels (18/23, 78.3%), with comb like arrangement in 4 patients (4/16, 25.0%). Other abnormal findings included ascites, hydrothorax, hydropericardium, hepatosplenomegaly and so on. ConclusionThe most common CT findings in SLE patients presenting with acute abdominal pain are the signs associated with ischemic bowel disease. Contrastenhanced spiral CT is a preferable imaging method for both the diagnosis and differential diagnosis of ischemic bowel disease associated with SLE.
ObjectiveTo investigate the CT presenting rate and features of gastric bare area (GBA, including the area posterior to GBA and the adipose tissue in the gastrophrenic ligament) without pathologic changes.MethodsThirty cases with superior peritoneal ascites, but without pathological involvement of GBA were included into the study to show the normal condition of GBA, including the presenting rate and CT features. We selected some cases with GBA invasion by inflammation or neoplasm to observe their CT features. ResultsAll cases with superior peritoneal ascites showed the GBA against the contrast of ascites with the presenting rate of 100%. The GBA appeared at the level of gastricesophageal conjunction and completely disappeared at the level of hepatoduodenal ligament and Winslow’s foramen. The maximum scope of GBA presented at the level of the sagital part of the left portal vein with mean right to left distance of (4.39±0.08)cm (3.8~5.7 cm) (distance between the left and right layer of the gastrophrenic ligament). In acute pancreatitis, the width of GBA increased, in which local hypodensity area could be seen. In gastric leiomyosarcoma invading GBA, the mass could not separate from the crus of the diaphragm. In lymphoma and metastasis invading GBA, the thickness of GBA increased and the density was heterogeneous, in which lymph nodes presenting as small nodes or fused mass. ConclusionThe results of this study show that it is helpful to use contrast enhanced spiral CT scanning to observe the change of GBA and to diagnose retroperitoneal abnormalities that involving GBA comprehensively and accurately.
肺栓塞( PE) 的确诊依赖于肺动脉的影像学检查。电子计算机断层扫描肺动脉造影( CTPA) 诊断PE 的敏感性和特异性高[ 1] , 而且该项检查是无创技术, 患者痛苦小, 并发症少, 已成为诊断PE 的一线技术[ 2,3] 。随着CT 仪器的不断升级和改进以及检查技术的不断研究, CT 在PE 中的应用不再仅限于PE 的定性诊断, 还用于肺动脉栓塞程度的量化、右心室改变的诊断、患者预后判断以及下肢深静脉血栓形成( DVT) 的诊断等。
Objective To evaluate the complication rate of CT-guided percutaneous lung biopsy and determine the risk factors. Methods A retrospective investigation of patients with CT-guided percutaneous lung biopsy in Ruijin Hospital, Shanghai Jiao Tong University School of Medicine between2002 and 2009 was performed. The risk factors for complications were determined by multivariate analysis of variables related to patients’demographics, lung lesions, biopsy procedures, and individual radiological features. Results 281 biopsy procedures were enrolled. The total complication rate was 55. 9% with pneumothorax 32. 4% ( 91/281) , hemoptysis 34. 5% ( 97 /281) , and cutaneous emphysema2. 1% ( 6 /281) , and with no mortality.The pneumothorax rate was correlated with lesion location, lesion depth, and number of pleural passes. The bleeding risk was correlated with lesion size, lesion depth, and age. Prediction models for pneumothorax and bleeding were deduced by logistic regression. The pneumothorax model had a sensitivity of 80. 0% and a specificity of 62. 4% . And the bleeding model had a sensitivity of 67. 4% and a specificity of 88. 8% .Conclusions Lesion location, lesion depth, and number of pleural passes were independent risk factors for pneumothorax. Lesion size, lesion depth, and age were independent risk factors for bleeding. The prediction models for pneumothorax and bleeding will helpfully reduce the complication of CT-guided lung biopsy.
Abstract: Objective To evaluate the diagnostic accuracy for the the assessment of coronary artery stenosesusing 64-multislice spiral computed tomography (64-MSCT) scanner compared with selective coronary angiography(SCA). Methods 64-MSCT and SCA were both performed in 93 patients with 74 males and 19 females at mean age of (58.2±8.5) years in West China Hospital between April 2004 and December 2010. The cardiacrhythm of all the patients was stably sinus. Patients with initial heart rates≥90 time/min were received treatments of β-blockers. All available coronary segments(internal diameter ≥ 2.0 mm)were included in the evaluation. Lesions with ≥ 50% luminalnarrowing were considered as significant stenosis. According to the image detail of segments and existance of artfacts,the image quality was randed Ⅰ to Ⅳ ,with rank Ⅰ to Ⅲ meeting demands of image evaluation. Evaluations had been done concerning the 64-MSCT scanner for detecting the stenosis of the variant branches or segments of the coronary artery. Results 64-MSCTprovided a full image correspondence(100%,1 238/1 238)of all the segments clearly displayed in the SCA. All of the coronary segments involved (n=1 238) met the quality demands of being evaluated as rankⅠ to Ⅲ. Considering SCA as the golden standard, overall sensitivity for classifying stenosis using 64-MSCT scannerwas 88.8%(427/481), specificity was 91.7%(694/757), positive predictive value was 87.1%(427/490),and negative predictive value was 92.8%( 694/748).While the stenosis diagnostic accuracy of proximal left circumflexbranch and the first obtuse marginal branch of left coronay artery is lower than other branches: the sensitivity of the proximal left circumflex branch was 68.3%(41/60),specificity was 60.6%(20/33),positive predictive value was 75.9%(41/54),negative predictive value was 51.3%(20/39);the sensitivity of the first obtuse marginalbranch was 58.8%(10/17),specificity was 93.5%(58/62),positive predictive value was 71.4%(10/14),negative predictive value was 89.2%(58/65). Arterial wall calcification and false image of cardiac movements were the maininfluence factors for accuracy of the the assessment of coronary artery stenosis with 64-MSCT scanner. Conclusion 64-MSCT scanner provids a high diagnostic accuracy in assessing stenosis of the coronary artery. The anatomical location and luminal area of coronary artery were the main influence factors of diagnostic accuracy. Thus the diagnostic accuracy in proximal left circumflex branch and the first obtuse marginal branch of left coronary artery was lower than other coronarysegments.
Abstract: Objective To explore the diagnosis and treatment of descending necrotizing mediastinitis (DNM). Methods We retrospectively analyzed the records of eight DNM patients treated at Tangdu Hospital between 2006 and 2009 year. There were 7 males and 1 female aged from 21-98 years with a median age of 49.5 years. The diagnostic criteria included clinical manifestations, neck and chest CT scans, and bacteriological culture. Six of the patients had odontogenic infections and six had diabetes. Antibiotic treatment, incision drainage, and other symptomatic treatments were applied. Two patients received cervical incision drainage, five received thoracotomy, and one received video-assisted thoracoscopic surgery (VATS). Results After treatment, six patients recovered and two died of heart failure and neck vessel rupture. According to the bacterial culture, six patients presented mixed infections, and four of these presented mixed aerobic and anaerobic infections. The mean operation time was 75.6 minutes, the average volume of pus removed during the operation was 318.7 ml, and the average inpatient stay was 18 days. At six months follow-up, all six surviving patients showed improvements in quality of life. Conclusion The valid diagnosed criteria of DNM include history, sign, symptom, neck and chest CT scanning, and secretion culture.DNM mortality can be reduced by employing broad spectrum antibiotics early in treatment, individual surgical managements, and effective treatments for complicating illnesses.
