  • TREATMENT OF RECURRENT PATELLAR DISLOCATION WITH ARTHROSCOPIC MEDIAL RETINACULUM PLICATION IN ADOLESCENTS

    Objective To explore the effectiveness of arthroscopic medial retinaculum pl ication (MRP) for recurrent patellar dislocation in adolescents. Methods Between March 2000 and October 2007, 30 adolescent patients with recurrent patellar dislocation underwent arthroscopic MRP, and 28 of them (12 left knees and 16 right knees) completed thefinal follow-up and were enrolled. There were 5 males and 23 females with an average age of 14.7 years (range, 12-19 years). The dislocation duration was 4 to 39 months with an average of 18.8 months. All patients experienced 2-4 episodes of dislocation. Before operation, all the patients showed positive apprehension test; the degree of lateral patellar translation was (2.9 ± 0.7)° and there was no hardness termination in lateral translation. The International Knee Documentation Committee (IKDC), Lysholm, Kujala, and Tegner scores were 47.7 ± 3.7, 52.6 ± 4.9, 66.7 ± 5.9, and 3.1 ± 1.3, respectively. All patients underwent arthroscopic MRP procedure. Results All incisions healed by first intention without compl ication. All the patients were followed up 2-7 years (4.8 years on average). During the follow-up, more and more patients showed positive apprehension test, and at 24 months of follow-up, 12 patients showed positive apprehension test; less and less patients had hardness termination in lateral translation, only 3 patients at 24 months of follow-up; the degree of lateral patellar translation increased, (2.3 ± 1.1)° at 24 months of follow-up. There were significant differences in positive apprehension test and hardness termination between preoperation and 24 months of follow-up (P lt; 0.05). Moreover, 6 patients suffered from redislocation, and 23 patients experienced patella instabil ity. The IKDC, Lysholm, Kujala, and Tegner scores at 24 months of follow-up were 62.5 ± 6.2, 70.7 ± 5.1, 76.6 ± 4.8, and 3.9 ± 0.7, respectively, showing significant differences when compared with preoperative scores (P lt; 0.05). CT examination showed that the congruence angle, lateral patellar angle, and patellar tilt angle were recovered to normal level after operation immediately, however, the final patellar position at 24 months of follow-up was not significantly betterthan that before surgery (P gt; 0.05). There was significant difference in the lateral patellar displacement between preoperation and 24 months of follow-up (P lt; 0.05). Conclusion Arthroscopic MRP is not rel iable for maintaining the corrected position of the patella for recurrent patellar dislocation in adolescents, though functional improvements are significant.

  • QUANTITATIVE STUDY ON MECHANORECEPTORS IN TIBIAL REMNANTS OF RUPTURED ANTERIOR CRUCIATE LIGAMENT IN HUMAN KNEES

    Objective To evaluate the survival condition and quantitative variation of mechanoreceptors in the tibial remnant of ruptured anterior cruciate l igament (ACL) in human knees. Methods Between April 2009 and May 2010, 60 patients who underwent arthroscopic ACL reconstruction were included and divided into 4 groups according to the time of injury, with 15 patients in each group. In group A, there were 14 males and 1 female aged 20-37 years (mean, 27 years); the injury was caused by traffic accident in 5 cases and by sports in 10 cases, and the time between injury and surgery was not more than 3 months. In group B, there were 14 males and 1 female aged 17-43 years (mean, 24 years); the injury was caused by traffic accident in 2 cases and by sports in 13 cases; and the time between injury and surgery was 3 to 6 months. In group C, there were 11 males and 4 females aged 16-53 years (mean, 28 years); the injury was caused by traffic accident in 2 cases and by sports in 13 cases; and the time between injury and surgery was 6 to 12 months. In group D, there were 12 males and 3 females aged 18-41 years (mean, 26 years); the injury was caused by sports in 11 cases, and the other 4 cases had no defined etiology; and the time between injury and surgery was more than 12 months. Specimens were donated by the volunteers from the anteromedial side of the ACL tibial remnant (40 sl ices per specimen) and stained via immunohistochemical method. The type, size, and quantity of mechanoreceptors were observed under l ight microscope. Results Totally 80 Ruffini-l ike corpuscles, 8 Pacini-l ike corpuscles, 5 atypical neural endings and no distinct Golgi organ-l ike corpuscles were observed under l ight microscope in 60 specimens (2 400 sl ices) of 4 groups. There was no obvious difference in size and type of mechanoreceptors among 4 groups. However,some degenerative and atrophic changes could be observed in the specimens of group D. The total number of echanoreceptors was 31 (35.2%) in group A, 22 (25.0%) in group B, 23 (26.1%) in group C, and 12 (13.7%) in group D. There was no significant difference (P gt; 0.05) in Ruffini-l ike corpuscles and Pacini-l ike corpuscles among 4 groups. Mechanoreceptors were found in 9,8, 9, and 6 patients in groups A, B, C, and D, respectively; the detection rates were 60.0%, 53.3%, 60.0%, and 40.0%, respectively; showing no significant difference (P gt; 0.05). Conclusion The mechanoreceptors in tibial residual of ruptured ACL have a long survival without obvious degeneration and atrophy within 12 months. So it may contribute to restoration of proprioceptive function after operation if the remnant is preserved during ACL reconstruction.

  • DEVELOPMENT OF THE MENISCAL REPLACEMENT IN CLINICAL PRACTICE

    Objective To review the details of the current effortsto reconstruct or replace the meniscus.Methods Three kinds of proceduresof reconstructing or replacing the meniscus were analyzed and evaluated by an extensive review of the latest literatures concerned. Results Three kinds of techniques were established to reconstruct the meniscus clinically, i.e., the allograft of the meniscus, meniscal reconstruction with the autotendon, and the meniscus scaffold. There were still a few defects in the meniscal replacement, and so the curative techniques would still be investigated. Conclusion Many efforts have been made to reconstruct the meniscus after its injury or its resection so as to prevent degeneration of the knee joint. The meniscal replacement has been employed for many years, but it has not worked so well. The establishment of an ideal replacement of the meniscus requires further studies. Therefore, reconstruction of the meniscus function is still a challenging problem to the surgeons concerned.

  • TREATMENT OF LARGE BONE DEFECT OF RADIUS WITH VASCULARIZED TUBULATE PERIOSTEUM GRAFT AND BONE FILLING MATERIAL IN RABBITS

    OBJECTIVE: To study the treatment efficacy of vascularized periosteum graft and bone filling material for long bone defect. METHODS: Forty young and forty adult rabbits were divided into four groups respectively according to the bone filling materials. A 3 cm long segment was removed from the middle part of the rabbit radius to make a bone defect model. The periosteum was reserved and restored to set up a vascularized tubulate periosteum graft. On the left side, autogenous bone graft, decalcified allograft, tricalcium phosphate, and hydroxyapatite were used to fill in the bone defect respectively; on the right side, no bone filling material was used as controls. The repairing effect of bone defect was evaluated by roentogenography, biomechanical, and histological methods. RESULTS: In young rabbits, bone defects on both sides healed in the 6th week after operation. The bending strength of radius in the tricalcium phosphate group and in the hydroxyapatite group were lower in the 12th week and there was significant difference when compared with autogenous bone graft group, decalcified allograft group and control group (P lt; 0.05). The repairing mechanism included intramembranous and endochondral ossification, and intramembranous ossification was prevalent. In the adult rabbits, the repairing rates of bone defect were 50% in the autogenous bone graft group, 40% in the decalcified allograft group, 30% in the tricalcium phosphate group and in the hydroxyapatite group and 42.5% in the control group, respectively. CONCLUSION: In young rabbits, large bone defect can be repaired with vascularized tubulate periosteum graft with or without the combining use of bone filling materials. The bone filling material which will be substituted slowly is disadvantageous to the recovery of bone strength. In adult rabbits, vascularized tubulate periosteum graft combined with bone filling materials can not repair the large bone defect effectively.

  • CLINICAL STUDY ON SURGICAL REPAIR OF ACUTE ACROMIOCLAVICULAR DISLOCATION BY MODIFIED WEAVER-DUNN TECHNIQUE

    Objective To investigate the clinicalvalue of modified Weaver-Dunn technique in repair of acute acromioclavicular dislocation. Methods From January 1993 to December 1998, 18 cases of acromioclavicular dislocation were treated bymodified Weaver-Dunn technique, and other 17 cases of the same suffering were treated by tension band fixation of the acromioclavicular joints. All of the patients were followed up for 12-36 months before clinical evaluation of the functionof shoulder joints, according to University of Pennsylvanian Shoulder Score System. Results In short term, the shoulder joints recovered much more rapidly in the cases repaired by modified Weaver-Dunn technique; 12, 24 and 36 months after operation, the scores of the cases repaired by modified Weaver-Dunn technique were (1897±67), (193.7±3.6) and (194.7±3.4) respectively according to the Shoulder Score System, while those of the cases treated by tension band fixation were (167.3±7.8), (170.2±6.3) and (165.6±5.9) respectively. The above data indicated that there was significant difference between two groups (P<0.05). Conclusion The modified Weaver-Dunn technique was a better surgical approach than tension band fixation for repair of acute acromioclavicular dislocation.

  • CLINICAL RESULTS OF ARTHROSCOPIC TREATMENT FOR LOCALIZED PIGMENTED VILLONODULAR SYNOVITIS OF KNEE

    Objective To explore the technique of arthroscopic treatment of local ized pigmented villonodular synovitis of the knee and to evaluate its cl inical results. Methods From February to December 2006, 22 cases of local ized pigmented villonodular synovitis of the knee were treated by arthroscopic excision of the focus and partial synovectomy. There were 8 males and 14 females, with an average age of 24 years old (16 to 35 years old). Eight patients had a trauma history, the others had no obvious inducement. The disease course was from 1 month to 30 months with an average of 10 months. The Lysholm score was 68.5 ± 8.2, and the International Knee Documentation Committee (IKDC) score was 72.7 ± 5.2 before operation. MRI showed that 20 knees had definite focuses and 2 had no ones. In all the cases, routine arthroscopic approach combined with assistant approach adjacent to the focus was used. Results All the patients were diagnosed as having local ized pigmented villonodular synovitis of the knee by pathological examination. The incisions healed at stage I. No compl ications occurred after operation. All patients were followed up 18-28 months (average 22 months). The angle of genuflex was less than 90° in 2 cases after 6 weeks, and the range of motion of the knee was recovery after manipulation release. At last followup, MRI showed no recurrence was found in 19 patients. The IKDC score was 92.8 ± 2.4, and the Lysholm score was 94.5 ± 3.5, respectively, indicating significant differences when compered with before operation (P lt; 0.01). Conclusion Local ized pigmented villonodular synovitis of the knee can be effectively treated by arthroscopic excision of the focus along with a rim of surrounding healthy synovium with most minimal invasive and best knee function.

  • A COMPARATIVE STUDY ON SHORT-TERM EFFECTS OF ARTHROSCOPIC PARTIAL MENISCECTOMY INTREATING MEDIAL VERSUS LATERAL MENISCUS INJURIES

    【Abstract】 Objective To compare the short-term effects of arthroscopic partial meniscectomy in treating medial versuslateral meniscus injuries. Methods From January 2003 to January 2006, 207 patients with meniscus injury(without intraarticularligament injury) underwent arthroscopic partial meniscectomy. The medial meniscus injury group included 115 cases, 50males and 65 females; aged 14 to 78 years(mean 46.9 years); 66 left knees and 49 right knees. Twenty-six cases had injury histories,the delayed time from injury to surgery ranged from 6 d to 6 months (mean 2.1 months). The lateral meniscus injury group included92 cases, 18 males and 74 females; aged 16 to 62 years (mean 41.1 years); 57 left knees and 35 right knees. Twenty-four caseshad injury histories, the delayed time from injury to surgery ranged from 9 d to 6 months (mean 1.9 months). Lysholm score systemwas applied and the scors of pre- and post-operation and were compared between two groups. Results The period of followupranged from 12 to 45 months (mean 31.5 months). In medial meniscus injury group and lateral meniscus injury roup, theLysholm score increased from 61.3±16.9 and 57.4±17.6 preoperation to 95.0±7.9 and 93.3±7.4 postoperation respectively. Therewas statistically significant difference between preoperation and postoperation (P lt; 0.01), and there was no statistically significantdifference between two groups(P gt; 0.05). The excellent and good rates for function of knee joint were 97.39% (excellent in 107cases, good in 5 cases and fair in 3 cases) in medial meniscus injury group and 100%(excellent in 80 cases and good in 12 cases)in lateral meniscus injury group. Conclusion Arthroscopic partial meniscectomy is a safe and effective treatment for meniscusinjury, there is no diference in short-term effects in treating medial versus lateral meniscus injuries.

  • PRIMARY CLINICAL RESULTS OF DOUBLE-BUNDLE ANTERIOR CRUCIATE LIGAMENT RECONSTRUCTION WITH SEMITENDINOSUS ALLOGRAFTS

    Objective To evaluate the primary cl inical results of double-bundle anterior cruciate l igament reconstruction (ACLR) with semitendinosus allografts. Methods From March 2006 to October 2006, 33 patients underwent double-bundle ACLR with semitendinosus allografts. The complete followed-up data of 31 patients was analyzed retrospectively. There were 24 males and 7 females aged 18-35 years old (average 25 years old). The injury was caused by sports accidents in 23cases and traffic accidents in 8 cases, involving the left knee in 18 cases and the right knee in 13 cases. Anterior cruciate l igament rupture were confirmed by MRI and arthroscopy in all the patients, without lateral collateral l igaments injuries and posterior cruciate l igament injuries. The time from injury to operation was 1-43 months (average 11 months). The knee was fixed at 0° position after operation for 2 weeks and got knee joint rehabil itation exercises gradually. Results The incision of 2 patients showed effusion 4 and 7 days after operation, respectively, and healed after symptomatic treatment. The incision of 29 patients healed by first intention. There were no compl ications such as stiffness of knee joint, neurovascular injuries and joint infections. All the patients were followed up for 24-29 months (average 26 months). MRI displayed the anterior cruciate l igament grafts presented with good connection and signal similar to the normal 2 years after operation. There was significant difference between the preoperational value and the final follow-up value in terms of bilateral knee joint difference of prior laxity, Lachman test, and pivot shift test (P lt; 0.05 ). The circumference difference between the injured and the normal was (11.6 ± 7.9) mm before operation and (5.0 ± 3.1) mm at the final follow-up (P lt; 0.05). The Tegner score, Lysholm score, and International Knee Documentation Committee score was 3.83 ± 1.15, 64.38 ± 6.81, and 41.42 ± 6.30, respectively, before operation, and 6.29 ± 0.64, 94.45 ± 3.03, and 95.72 ± 3.10, respectively, at the final follow-up. There was a significant difference between before and after operation (P lt; 0.05). Conclusion The primary cl inical results of double-bundle ACLR with semitendinosus allografts are satisfactory and the allogeneic semitendinosus are good grafts for double-bundle ACLR.

  • 关节镜下自体骨软骨移植修复软骨缺损

    【摘 要】 目的 总结关节镜下自体骨软骨镶嵌移植术(osteochondral autologous transplantation,OAT)修复膝关节软骨缺损的方法及临床效果。 方法 2000 年8 月- 2005 年6 月,采用关节镜下OAT 治疗23 例股骨髁关节软骨缺损。男15 例,女8 例;年龄16 ~ 41 岁。运动冲撞伤13 例,跌伤2 例,8 例无明显诱因或外伤史。病程1 ~ 8 个月。软骨缺损深度按照国际软骨修复协会标准分级:4 级15 例,3 级8 例。缺损范围2.0 ~ 4.0 cm2。Lysholm 评分(69.54 ± 12.36)分。 结 果 术后切口Ⅰ期愈合。15 例获随访,随访时间24 ~ 48 个月,平均33 个月。术后6 个月12 例关节症状完全消失;2 例剧烈活动后关节出现肿胀,1 例上下楼梯时前膝偶有酸痛不适,经对症治疗后痊愈。术后24 个月Lysholm 评分(94.15 ± 9.21)分,与术前比较差异有统计学意义(P lt; 0.05)。 结论 关节镜下OAT 创伤小、能恢复关节面的曲度,是治疗小面积软骨缺损有效方法之一。

  • EFFECTS OF BASIC FIBROBLAST GROWTH FACTOR GENE TRANSFECTION ON BIOCHEMISTRY OF MENISCAL FIBROCHONDROCYTES

    Objective To explore the effects of the basic fibroblast growth factor(bFGF) gene transfection on the meniscal fibrochondrocytes with the reconstructed lentivirus and to observe the response of the meniscal fibrochondrocytes to the bFGF gene transfection. Methods The cultured meniscal fibrochondrocytes were isolated from the same 3-monthold New Zealand rabbit. The cultured first-generation meniscal fibrochondrocytes were divided into 3 groups:Group A (experimental group), Group B (control group), and Group C (blank group). Each group comprised the cells in a 24hole flask in which each hole contained 2×104 cells. At the confluence of 60%, the fibrochondrocytes in Group A were cultured with the reconstructed lentivirus carrying the bFGF gene. The fibrochondrocytes in Group B were cultured with the lentivirus carrying no bFGF gene. The fibrochondrocytes in Group C were cultured without any intervention. After 48 h, the cell cycle, the collagen synthesis ability, the expression of bFGF, and the cell proliferation ability in each group were investigated. Results In Group A, the bFGF expression of 870±60 pg/ml was detected in the cells 48 h afterthe co-culture; however, in Group B and Group C, no expression of bFGF was found. After the co-culture for 6 days, the results of the MTT colorimetry revealed that the cells in Group A had an absorbtance of 0.427±0.037, which had a significant difference when compared with that in Group B and Group C (0.320±0.042,0.308±0.034,Plt;0.01). The cell cycle was significantly shorter in GroupA than in Group B and Group C (Plt;0.05); The durations of G1, S and G2M of the cells in Group A were 16.28, 12.60 and 11.04 h, but those in Group B and Group C were 23.61, 16.90, 21.33 h and 21.56, 19.80, 21.41 h, respectively. The disintegration per minute of the cells was significantly greater in Group A than in Group B and Group C (7281.69±805.50 vs 5916.40±698.11 and 5883.57±922.63,Plt;0.05). Conclusion The lentivirus vector can transfer the bFGF gene into the meniscal fibrochondrocytes, resulting in an increase of the cell proliferation and the collagen synthesis.
