  • REPAIRING HALLUX METATARSOPHALANGEAL SKIN AND SOFT TISSUE DEFECTS WITH MEDIAL FLAP WITH DOUBLE BLOOD SUPPLY SYSTEM IN ELDERLY PATIENTS

    Objective To investigate the effectiveness of repairing hallux metatarsophalangeal skin and soft tissue defect with medial flap with double blood supply system in elderly patients. Methods Between June 2011 and March 2012, 9 cases (9 toes) of skin and soft tissue defect at hallux metatarsophalangeal joint were treated with medial flap with double blood supply system. There were 7 males and 2 females, aged 60-70 years (mean, 65.4 years). Injury included crush injury in 5 cases, traffic accident injury in 4 cases. The interval of injury and operation ranged from 3 hours to 5 days (mean, 35 hours). The patients had bone or tendon exposure with pollution of wounds. The size of defects ranged from 2.0 cm × 1.5 cm to 2.5 cm × 2.0 cm. The size of flaps ranged from 2.0 cm × 1.7 cm to 3.0 cm × 2.2 cm. Medial dorsal nerves were anastomosed with toe nerves to recover flap sensation. The donor site was repaired with skin grafting. Results All 9 flaps and skin grafting survived completely, and primary healing of wounds were obtained. Nine patients were followed up 6-8 months (mean, 7 months). The colour, texture, and contour of the flaps were good. The two-point discrimination of the flaps was 12-16 mm (mean, 14 mm) at last follow-up. The patients could walk normally. No scar contracture formed at donor site. No wear occurred. Conclusion The medial flap with double blood supply system can be used to repair hallux metatarsophalangeal skin and soft tissue defect in elderly patients because the flap can be easily obtained, has good blood supply, and has no injury to the main artery and nerve.

  • 吻合血管的足内侧皮瓣修复手掌软组织缺损

    目的 总结以足底内侧动脉浅支或深支为蒂的足内侧皮瓣修复手掌软组织缺损的临床效果。 方法 1999 年5 月- 2007 年5 月,采用吻合血管足内侧皮瓣修复手掌软组织缺损18 例。男10 例,女8 例;年龄15 ~ 56 岁。绞榨伤12 例,热压伤2 例,车祸伤4 例。单纯手掌软组织缺损7 例;手掌软组织缺损伴指伸肌腱损伤3 例,伴掌、指骨开放性骨折6 例,伴手指挤压伤2 例。皮肤缺损范围为4 cm × 2 cm ~ 9 cm × 6 cm。损伤至入院时间1 h ~ 7 d。术中皮瓣切取范围为5 cm × 3 cm ~ 10 cm × 8 cm。供区中厚皮片植皮修复。 结果 18 例皮瓣均成活,切口均Ⅰ期愈合;供区1 例植皮区少量坏死后痂下愈合,余均成活。18 例均获随访,随访时间6 个月~ 8 年。皮瓣质地好,外形无明显臃肿,两点辨别觉为6 ~ 9 mm。手功能按中华医学会手外科学会上肢部分功能评定试用标准进行评定,优7 例,良9 例,可2 例,优良率88.89%。 结论 足底内侧皮瓣修复手掌软组织缺损效果好,是理想术式之一。

  • 远端蒂足内侧皮瓣在儿童足母趾皮肤缺损中的应用

    目的 总结应用远端蒂足内侧皮瓣逆向移位修复儿童足母趾皮肤缺损的临床效果。 方法 2003 年3 月- 2007 年10 月,采用远端蒂足内侧皮瓣修复足母趾皮肤缺损7 例。男5 例,女2 例;年龄2 ~ 13 岁。撕脱伤4 例,重物砸伤3 例。足母趾末节趾腹缺损1 例,近节腹侧缺损4 例, 趾残端皮肤缺损2 例。软组织缺损范围为2.5 cm ×2.0 cm ~ 6.0 cm × 4.0 cm。病程2 h ~ 1 个月。术中切取皮瓣范围3.0 cm × 2.5 cm ~ 6.5 cm × 4.5 cm,供区中厚皮片植皮修复。 结 果 5 例皮瓣及供区植皮均成活;2 例皮瓣远端分别于术后2 d 和3 d 出现0.5 cm × 1.0 cm 水疱,均于2 周后脱痂皮愈合。供受区切口均Ⅰ期愈合。7 例均获随访,随访时间6 个月~ 4 年7 个月。皮瓣外观、血运、弹性好,两点辨别觉为5 ~ 12 mm,无溃疡或磨损,行走无影响。 结论 采用远端蒂足内侧皮瓣修复儿童足母趾皮肤缺损是一种较好的手术方法。

  • 吻合血管足内侧皮瓣修复手部皮肤缺损

    目的 观察应用吻合血管足内侧皮瓣修复手部皮肤缺损的临床效果。方法 2002年10月~2006年3月,应用吻合血管的足内侧皮瓣移植修复6例手部皮肤缺损。其中男4例,女2例;年龄17~49岁。机器辗压伤5例,重物砸伤1例。皮肤缺损范围为4 cm ×6 cm~6 cm×10 cm。损伤至手术时间18~46 d。结果 1例术后第2天腕掌侧瘢痕出现皮瓣下血肿,经拆除部分缝线,充分引流后皮瓣成活;余5例皮瓣成活,创口均Ⅰ期愈合。1例负重后足部供区反复出现渗液,经换药及加强皮肤护理后痊愈,另5例足部供区创面均Ⅰ期愈合。患者均获随访6~13个月,皮瓣质地好,耐磨擦,外形美观不臃肿,关节功能恢复满意。结论 游离足内侧皮瓣是修复手部皮肤软组织缺损的一种有效方法。

  • 携带隐神经终末支的游离足内侧皮瓣在手部高压注射伤中的应用

    目的总结携带隐神经终末支的游离足内侧皮瓣治疗手部高压注射伤的疗效。方法 2014 年 3 月—2018 年 6 月,收治 16 例 16 指高压注射伤患者。其中男 12 例,女 4 例;年龄 21~58 岁,平均 42 岁。拇指 3 例,示指 6 例,中指 5 例,环指 2 例。高压乳胶漆注射伤 10 例,高压油枪注射伤 6 例。受伤至入院时间为 1 h~2 d,平均 6.5 h。急诊行显微外科清创;亚急诊行携带隐神经终末支的游离足内侧皮瓣修复创面,皮瓣范围 4.5 cm×3.0 cm~8.0 cm×5.0 cm。结果术后皮瓣全部成活,未发生指端坏死或部分坏死。患者均获随访,随访时间 8~24 个月,平均 15 个月。末次随访时患指外观满意,皮瓣弹性、血运良好,指端指体饱满,远、近指间关节处横纹初显,无指体冷耐受差发生。皮瓣两点辨别觉为 5~10 mm,平均 7.5 mm。手功能采用手指关节总活动度(TAM)法评定,获优 10 指,良 4 指,可 2 指。供区无并发症发生。结论携带隐神经终末支的游离足内侧皮瓣是治疗手部高压注射伤的一种有效方法,术后手指外形、感觉及功能恢复良好。
