  • The Strategic Supporting Role of a Regional State-level Hospital during Medical Rescue after Wenchuan Earthquake

    Shortly after Wenchuan earthquake, the administrative leaders of West China Hospital accurately defined the role of the hospital during the medical rescue work as the treatment center for the seriously wounded, the supporting center for the local hospitals/clinics of the disaster areas, and the logistic supporting center for medical teams from other provinces. Integrated leadership of management and with efficient multi-department coordination and cooperation were emphasized. The mode of the hospital was immediately transformed from the regular state into a double-track emergent state. Scientific allocation and dispatch of resources were ensured to meet the ever-changing demand from all levels of rescue work. Three stages were defined based on the conditions of the wounded delivered to the hospital, with different main focuses for each stage. Owing to the multi-disciplinary cooperation and concerted efforts of a large number of experts from other provinces or even other countries, effective and efficient medical rescue service has been offered to all the wounded. Up to June 2nd, 2?618 cases from the disaster area have been treated, of whom 1 751 admitted into the inpatient department, 1 135 seriously wounded, 127 admitted into the Intensive Care Unit, 1 239 underwent operations and 77 treated with hemodialysis, with an inpatient mortality lower than 0.7%. Moreover, even during such a period of time, the routine medical service has been offered as regular to patients other than the wounded in the disaster.

  • The Experience Learnt from Wenchuan Earthquake Guide the Medical Rescue in Yushu Earthquake△

    A 7.1-magnitude earthquake hit china again, 702 days after Wenchuan Earthquake at Yushu prefecture of Qinghai province, 4000 meters above sea level. Up to April 19th, the death toll climbed to 1706, with 256 missing and 12128 injured. West China Hospital, as a regional state-level hospital of Ministry of Health, organized the first batch of medical team with relief supplies at the first day after Yushu earthquake and rushed to Yushu taking part into the rescue effort in golden seventy-two hours. The second day after Yushu earthquake, medical apparatus and drugs valued 5,000,000 RMB were delivered to the rescue site and the second batch of medical team were positioned. Within thirty three hours after the earthquake, 102 people, including 93 earthquake related injuries and 9 armed police with severe altitude illness, were sent to our hospital by air in four batches successively. Based on the first-hand experience of medical rescue in Wenchuan Earthquake, a series of diagnosis and treatment criteria which were built and developed with evidence-based method, and “four concentration principles”, namely concentrating the wounded, concentrating the experts, concentrating resources, and concentrating treatments, our medical rescue in Yushu earthquake were carried out appropriately. Up to 12 am., April 19th, 93 cases from the disaster area have been treated, of whom 54 seriously wounded, 58 underwent operations and none death. The experience learnt from Wenchuan Earthquake have been used, improved and sublimated more rapidly, more appropriately, more effectively in the Yushu earthquake medical rescue.

    Yushu earthquake bettered a record all round in ability and speed about emergency command, provisioning, medical rescue, epidemic prevention, rebuilding and policy ensuring.   • Machine learning-based method for interpreting the guidelines of the diagnosis and treatment of COVID-19

    The outbreak of pneumonia caused by novel coronavirus (COVID-19) at the end of 2019 was a major public health emergency in human history. In a short period of time, Chinese medical workers have experienced the gradual understanding, evidence accumulation and clinical practice of the unknown virus. So far, National Health Commission of the People’s Republic of China has issued seven trial versions of the “Guidelines for the Diagnosis and Treatment of COVID-19”. However, it is difficult for clinicians and laymen to quickly and accurately distinguish the similarities and differences among the different versions and locate the key points of the new version. This paper reports a computer-aided intelligent analysis method based on machine learning, which can automatically analyze the similarities and differences of different treatment plans, present the focus of the new version to doctors, reduce the difficulty in interpreting the “diagnosis and treatment plan” for the professional, and help the general public better understand the professional knowledge of medicine. Experimental results show that this method can achieve the topic prediction and matching of the new version of the program text through unsupervised learning of the previous versions of the program topic with an accuracy of 100%. It enables the computer interpretation of “diagnosis and treatment plan” automatically and intelligently.

  • Science and Ethics in Conducting Cardiothoracic Surgery with Da Vinci Surgical System in China: A Systematic Review

    ObjectiveTo systematically evaluate the effectiveness, safety and ethics of Da Vinci Surigcal System (dVSS)in the field of cardiothoracic surgery, and provide evidence for its clinical application. MethodsWe searched VIP, CNKI, WanFang Database, CBM, official website of National Health and Family Planning Commission of People's Republic of China, PubMed, Cochrane Library, Google Scholar, WHO ICTRP search portal, ClinicalTrials.gov, Chinese Clinical Trial Registration Center, Australian-New Zealand Clinical Trials Registry and ISRCTN to collect clinical trials of dVSS for thoracic and cardiovascular diseases to evaluate its effectiveness, safety and ethics. The search time was from the establishment of each database to January 2014. The quality of included literatures was evaluated by Grades of Recommendations Assessment, Development and Evaluation (GRADE). ResultsA total of 31 studies were identified including 4 case control studies and 27 retrospective studies involving 1 037 patients. The quality of included literatures was all very low. dVSS had been widely used in cardiothoracic surgery including coronary artery bypass grafting, valvuloplasty or valve replacement, thoracic tumor resection, etc. Compared with conventional surgery and thoracoscopy, the safety and effectiveness of dVSS was good, while its economic burden was heavier for the patients. dVSS could benefit patients and improve clinical level of cardiothoracic surgery. The disciplines and qualification of dVSS application was legal, but the maturity of its clinical use needed improvement, and relevant measures were required. The ethics in the research, introduction and application of dVSS had some problems which deserved more attention. ConclusionsClinical use of dVSS in cardiothoracic surgery is relatively safe and effective, but research quality needs to be improved. More studies on the economics and ethics of dVSS are needed in the field of cardiothoracic surgery. Relevant measures of precaution, accountability and patient protection are required to further improve clinical application of dVSS in cardiothoracic surgery, and whole-process ethical supervision should be conducted with the help of existing clinical ethical review platform.

  • To Promote Good Research Practice, Good Publication Practice and Good Transplation by Implementing Research Ethic and Publication Ethic

    Medical ethic and publication ethic was more and more and important with the development of trial registration and systematic review registration promoted by the World Health Organization and the Cochrane Collaboration. Chinese Trial Registration and Publication Collaboration (ChiCTRPC) published the first statement to implement trial registration in China. We suggested that trial registration should be combined with ethic review and described the new definition of publication ethic in the second meeting by ChiCTRPC in 2010. In the the third meeting of ChiCTRPC in 2011, Experts from many fields communicated adequately and reached consensus. We summarized the experiences and lessons in the 5 years after the first statement of ChiCTRPC was published. Based on the latest progress of research ethic, publication ethic, trial registration, and systematic review registration, we drafted this explanation document for the second statement of ChiCTRPC to realize good research practice (RGP), good publication practice (GPP), and good translation practice (GTP) by implementing research ethic and publication ethic.
