  • EFFECTIVENESS OF CLAVICULAR HOOK PLATE COMBINED WITH TRAPEZIUS MUSCLE FASCIA FOR RECONSTRUCTION OF ACROMIOCLAVICULAR AND CORACOCLAVICULAR LIGAMENTS TO TREAT COMPLETE ACROMIO-CLAVICULAR JOINT DISLOCATION

    ObjectiveTo explore the effectiveness of the clavicular hook plate combined with trapezius muscle fascia for reconstruction of acromioclavicular and coracoclavicular ligaments to treat acute complete acromioclavicular dislocations. MethodsBetween January 2008 and April 2012, 66 patients with acromioclavicular dislocation were treated with the clavicular hook plate combined with trapezius muscle fascia for reconstruction of acromioclavicular and coracoclavicular ligaments in 32 cases (experimental group) and with the clavicular hook plate in 34 cases (control group). There was no significant difference in gender, age, injured side, the cause of injury, and the time from injury to operation between 2 groups (P>0.05). Visual analogue scale (VAS), Constant shoulder scores, and coracoid clavi-cledistance (CC.Dist) were measured at preoperation and at 2 years after operation. Signal/noise quotiem (SNQ) was measured by MRI at 2 years after operation. The operation complications were observed. ResultsThe patients of 2 groups obtained primary healing of incision. The morbidity of complication in experimental group (12.5%, 4/32) was significantly lower than that in control group (91.2%, 31/34) (χ2=40.96, P=0.00). All the cases were followed up 2.8 years on average (range, 2 to 4 years). VAS scores and CC.Dist significantly decreased at 2 years after operation when compared with preoperative values in the 2 groups (P<0.05). VAS scores and CC.Dist of the experimental group were significantly lower than those in the control group (P<0.05). According to Constant shoulder scores at 2 years after operation, the results were excellent in 19 cases, good in 11 cases, and general in 2 cases with an excellent and good rate of 93.75% in the experimental group; the results were excellent in 7 cases, good in 8 cases, general in 16 cases, and poor in 3 cases with an excellent and good rate of 44.11% in the control group; and significant difference was shown between 2 groups (t=2.30, P=0.03). SNQ was significantly lower in experimental group than in control group at 2 years after operation (t=55.03, P=0.00), indicating that ligament healing was better in experimental group than control group. ConclusionCompared with simple clavicular hook plate fixation, the clavicular hook plate combined with trapezius muscle fascia for reconstruction of acromioclavicular and coracoclavicular ligaments is successful in treating acute complete acromioclavicular dislocations, with the advantages of higher ligament healing, less complication, and early improvement of shoulder functions.

  • Volar Locked Plating for Fractures of the Unstable Distal Radius

    目的:评估使用锁定钢板治疗桡骨远端不稳定骨折的疗效。方法:2005年8月至2009年5月使用锁定钢板治疗桡骨远端不稳定性骨折27例。平均553岁。按AO分类:B2型5例,B3型2例,C1型11例,C2型7例,C3型2例。结果:全部病例得到3~24个月随访,平均162个月。根据Fernandez评分标准进行腕关节功能评分:优14例,良10例,一般1例,差2例。优良率875%。 结论:掌侧锁定钢板治疗桡骨远端不稳定性骨折是安全有效的治疗选择,可提供坚强的固定,早期功能训练。

  • Treating Fracture of Distal End of Femur by AnatomicTCP, LTCP and DCS

    目的:探讨股骨髁解剖钢板、L钢板及DCS在治疗股骨远端骨折中的应用价值。方法:采用股骨髁解剖钢板内固定治疗股骨远端骨折26例,L钢板内固定治疗股骨远端骨折32例, DCS内固定治疗股骨远端骨折21例。结果:79例经3~15月随访,其中采用解剖钢板26例中23例疗效满意,优良率为885%。采用L钢板32例中23例疗效满意,优良率为719%采用DCS 21例中17例疗效满意,优良率为809%。结论:解剖钢板内固定是治疗股骨远端骨折较为有效的方法,是预防膝关节并发症的积极措施。

  • Proximal Femoral Anatomical Plate in Treatment of Intertrochanteric Fracture

    目的:探讨股骨近端解剖型钢板治疗股骨粗隆间骨折的临床疗效。方法:自2005年1月至2009年1月,采用切开复位,股骨近端解剖型钢板治疗股骨粗隆间骨折46 例。结果: 术后随访6个月~3年,平均18个月,所有病例均在术后3~55个月获得骨性愈合,髋关节功能根据Harris评分标准进行评定,优32 例,良4 例。结论: 股骨近端解剖型钢板适用于治疗大多数类型的股骨粗隆间骨折,能达到良好的骨折复位和坚强的固定,促进关节早期功能锻炼,骨折愈合率高,是治疗股骨粗隆间骨折的理想选择。

  • SHORT-TERM EFFECTIVENESS OF RECONSTRUCTIVE LOCKED PLATE FOR TREATING STERNOCLAVICULAR JOINT DISLOCATION

    Objective To investigate the short-term effectiveness of reconstructive locked plate for treating sternoclavicular joint dislocation. Methods Between February 2008 and February 2012, 11 patients with sternoclavicular joint dislocation were treated with reconstructive locked plate, and the clinical data were retrospectively analyzed. There were 7 males and 4 females, aged 30-55 years (mean, 44 years). The causes of injury included traffic accident in 8 cases and crashing in 3 cases. The disease duration ranged from 2 hours to 11 days (median, 6 days). All patients had anterior dislocation of sternoclavicular joint. According to the Grade system, there were 2 cases of type II and 9 cases of type III. Results All patients obtained healing of incisions by first intention after operation. There was no neurovascular injury. The X-ray films showed that satisfactory reduction of joint dislocation and stable internal fixation were obtained at 2 days after operation. All patients were followed up 9-24 months (mean, 16 months). According to the Rockwood criteria, the score was 10-15 (mean, 13.2); the results were excellent in 9 cases and good in 2 cases, with an excellent and good rate of 100% at 9 months after operation. No internal fixation failure or re-dislocation occurred. All internal fixators were removed at 9-15 months after operation. Both the stability and the functions of the shoulder joint were good. Conclusion The reconstructive locked plate in treating sternoclavicular joint dislocation has the advantages of good stability and satisfactory reduction, and the patients can do functional exercises early and obtain good recovery of the shoulder joint function. The short-term effectiveness is satisfactory.

  • EFFECTIVENESS COMPARISON BETWEEN LOCKING COMPRESSION PLATE FIXATION AND LOCKED INTRAMEDULLARY NAIL FIXATION FOR HUMERAL SHAFT FRACTURE OF TYPES B AND C

    Objective To compare the effectiveness between locking compression plate (LCP) and locked intramedullary nail (IMN) for humeral shaft fractures of types B and C. Methods Between January 2010 and January 2012, 46 patients with humeral shaft fractures of types B and C were treated, and the clinical data were retrospectively analyzed. LCP was used for internal fixation in 22 cases (LCP group), and IMN in 24 cases (IMN group). There was no significant difference in gender, age, injury causes, the side of fracture, the site of fracture, the type of fracture, associated injury, and time from injury to operation between 2 groups (P gt; 0.05). The regular clinical examination and evaluation of radiography were done. Shoulder function was evaluated by Neer grading system and elbow function was evaluated by Mayo elbow performance score after operation. Results The operation time and intraoperative blood loss in IMN group were significantly lower than those in LCP group (P lt; 0.05). There was no significant difference in hospitalization time between 2 groups (t=0.344, P=0.733). All patients were followed up 16.8 months on average (range, 12-24 months). At 6 months after operation, bone nonunion occurred in 1 patient of LCP group and in 2 patients of IMN group; the bone healing rate was 95.5% (21/22) in LCP group and 91.7% (22/24) in IMN group, showing no significant difference (χ2=0.000, P=1.000). Except for nonunion patients, the bone healing time was (11.77 ± 0.75) weeks in LCP group and (11.38 ± 0.82) weeks in IMN group, showing no significant difference (t=1.705, P=0.095). Between LCP and IMN groups, significant differences were found in radial nerve injury (4 cases vs. 0 case) and impingement of shoulder (0 case vs. 6 cases) (P lt; 0.05), but no significant difference in superficial infection (1 case vs. 0 case) and iatrogenic fracture (1 case vs. 2 cases) (P gt; 0.05). There was no significant difference in shoulder function and elbow function at 1 year after operation between 2 groups (P gt; 0.05). Conclusion LCP fixation and IMN fixation for humeral shaft fractures of types B and C can achieved satisfactory results. More attention should be paid to avoiding radial nerve injury by fixation of LCP; nail tail should be buried deeply into the cortex of the greater tuberosity and rotator cuff should be protected to decrease the rate of impingement of shoulder by fixation of IMN.

  • LIMITED OPEN REDUCTION AND DOUBLE PLATES INTERNAL FIXATION FOR TREATMENT OF Vancouver TYPE B1 PERIPROSTHETIC FEMORAL FRACTURE AFTER HIP ARTHROPLASTY

    Objective To evaluate the effectiveness of limited open reduction and double plates internal fixation in the treatment of Vancouver type B1 periprosthetic femoral fracture after hip arthroplasty. Methods A retrospective analysis was made on the clinical data of 12 patients with Vancouver type B1 periprosthetic femoral fracture after hip arthroplasty between May 2007 and October 2012, who underwent limited open reduction and double plates internal fixation. Of 12 patients, 4 were male and 8 were female, aged 76-85 years (mean, 81.6 years); the left side was involved in 5 patients and the right side in 7 patients. The cement prosthesis was used in 3 cases and cementless prosthesis in 9 cases; double acting head of hip arthroplasty was performed in 4 cases and total hip arthroplasty in 8 cases. The median time from first hip arthroplasty to re-fracture was 13 months (range, 5 months-5 years). The causes of injury were traffic accident in 2 cases and falling in 10 cases. Combined fractures included contralateral tibial and fibular fractures and ipsilateral distal humeral fracture (1 case), ipsilateral proximal humeral fracture (2 cases), ipsilateral distal radial fracture (1 case), and rib fracture (1 case). The time from injury to operation was 5.6 days on average (range, 3-10 days). Results The incisions all healed by first intention, and no infection or deep venous thrombosis of lower extremity occurred. Twelve cases were followed up 6-24 months (mean, 13.3 months). One female patient died of acute myocardial infarction at 16 months after operation. All the fractures were healed, with X-ray healing time of 12.5 weeks on average (range, 10-16 weeks). The time of full weight bearing was 13 weeks on average (range, 10-18 weeks). Ten cases could walk freely after operation, and 2 cases could walk by the aid of walking aid appliance. At last follow-up, the Harris score of hip function was 87.3 on average (range, 75-93). Conclusion The method of limited open reduction and double plates internal fixation for Vancouver type B1 periprosthetic femoral fracture after hip arthroplasty is effective in maintaining stability, protecting blood supply, promoting fracture healing, and doing functional exercise early. The long-term effectiveness needs further observation.

  • 经尺骨鹰嘴截骨双侧锁定加压钢板内固定治疗C型肱骨远端骨折

    目的总结经尺骨鹰嘴截骨双侧锁定加压钢板治疗C型肱骨远端骨折的疗效。 方法2008年9月-2013年5月,采用经尺骨鹰嘴截骨双侧锁定加压钢板固定治疗21例C型肱骨远端骨折患者。其中男12例,女9例;年龄18~64岁,平均38岁。致伤原因:跌伤7例,交通事故伤14例。均为新鲜闭合骨折。根据国际内固定研究协会(AO/ASIF)分型:C1型9例,C2型6例,C3型6例。受伤至手术时间5~11 d,平均7 d。 结果术后切口均Ⅰ期愈合,无感染及神经损伤发生。21例均获随访,随访时间7~24个月,平均16个月。2例于术后6个月发生异位骨化,术后1 年行异位骨化切除,功能无明显改善。患者骨折均愈合,愈合时间6~10个月,平均7.8个月。随访期间内固定物无松动、断裂及失效发生。术后6个月肘关节活动范围为60~136°,平均110°。术后6个月根据Mayo肘关节功能评分,获65~95分,平均87分;其中优16例,良1例,中4例,优良率为81%。 结论双侧锁定加压钢板治疗C型肱骨远端骨折固定牢固,利于术后早期行肘关节功能锻炼。

  • EFFECT OF PENETRATION OF MINI-PLATE LATERAL MASS SCREWS INTO FACET JOINT ON AXIAL SYMPTOMS IN CERVICAL LAMINOPLASTY

    Objective To investigate the effect of the penetration of mini-plate mass screws into facet joint on axial symptoms in cervical laminoplasty. Methods A retrospective analysis was made on the clinical data of 52 patients who underwent unilateral open-door cervical expansive laminoplasty fixed with Centerpiece mini-plate between September 2009 and December 2011. There were 42 males and 10 females, with a mean age of 61.2 years (range, 34-83 years). Seventeen patients exhibited simple degeneration cervical canal stenosis, 25 patients had multilevel cervical disc protrusion, and 10 patients had ossification of posterior longitudinal ligaments. Disease duration ranged 1-120 months (median, 11 months). The Japanese Orthopedic Association (JOA) score was used to assess neurological function, and JOA recovery rates were calculated. The visual analogue score (VAS) and the neck disability index (NDI) were used to evaluate the axial pain and neck daily activities. The axial symptoms and other complications were recorded. The cervical canal diameter, cervical curvature, cervical canal cross area, and open angle were measured according to the X-ray films, CT scans, and MRI scans. The postoperative CT three dimensional (3-D) reconstruction images were used to identify whether the screws penetrated into the facet joints. All the patients were divided into 3 groups according to involved facet joints: no joint penetrating group (no penetrated facet joint), oligo-joint penetrating group (one or two penetrated facet joints), and multi-joint penetrating group (three or more penetrated facet joints). Results Five patients suffered from C5 nerve palsy, and 2 patients had cerebrospinal fluid leakage. The follow-up time ranged 3-35 months (mean, 15.7 months). At the final follow-up, the JOA scores, NDI, cervical canal diameter, and cervical canal cross area were significantly improved when compared with preoperative ones (P lt; 0.05). At 1 week after operation, CT 3-D reconstruction showed that 16 patients had no penetrated facet joint, 23 patients had one or two penetrated facet joints, and 13 patients had three or more penetrated facet joints. There was no significant difference in age, gender, disease duration, operation time, intraoperative blood loss, and follow-up time among 3 groups (P gt; 0.05). And at the final follow-up, there was no significant difference in JOA score, VAS score, cervical curvature, cervical canal diameter, cervical canal cross area, the JOA recovery rates, and lamiae open angle among 3 groups (P gt; 0.05). The NDI of the multi-joint penetrated group was significantly higher than that of other 2 groups (P lt; 0.05). Axial pain occurred in 1 case of no penetrating group, in 4 cases of oligo-joint penetrating group, and in 5 cases of multi-joint penetrating group, showing no significant difference among 3 groups (χ2=4.881, P=0.087). Conclusion The penetrations of lateral mass screws into articular surface of facet joint may contribute to the axial symptoms after cervical laminoplasty. The risk of axial symptom raises accompany with increased penetrated facet joints.

  • EFFECTIVENESS AND BIOMECHANICAL ANALYSIS OF THREE FIXATION METHODS IN TREATMENT OF POSTERIOR Pilon FRACTURES

    Objective To investigate the effectiveness and biomechanical analysis of 3 fixation methods of screw anterior-posterior fixation (SAPF), screw posterior-anterior fixation (SPAF), and buttress plate fixation (BPF) in treatment of posterior Pilon fractures. Methods Fifteen fresh-frozen skeleto-ligamentous lower leg specimens were harvested to establish the models of posterior Pilon fracture, and then fracture was fixed with BPF (n=5), SAPF (n=5), and SPAF (n=5). Vertical force was loaded to internal fixation failure in a speed of 1 cm/minute with servohydraulic testing machine. The instantaneous loads of 1 mm and 2 mm steps and the failure modes were recorded. Between May 2008 and December 2011, 56 patients with posterior Pilon fracture were treated with SAPF (SAPF group) in 11 cases, or SPAF (SPAF group) in 26 cases, or BPF (BPF group) in 19 cases. There was no significant difference in age, gender, injury cause, side, disease duration, and complications among groups (P gt; 0.05). Clinical and radiographic examinations were used to assess the reduction and healing of fracture; the American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot score and the visual analogue scale (VAS) were used to evaluate the functional outcomes. Results No breaking or bending was observed in all specimens, fixation failure was caused by cancellous bone compression. The instantaneous loads of 1 mm and 2 mm steps were the largest in BPF group, larger in SPAF group, and smallest in SAPF group, showing significant differences among 3 groups (P lt; 0.05). A total of 47 cases were followed up 16-54 months (9 in SAPF group, 22 in SPAF group, and 16 in BPF group), with a mean time of 35.2 months. Fixation failure was found in 2 cases of SAPF group; the other cases obtain bony union within 3 to 4 months (mean, 3.2 months) with no fixation failure. The AOFAS score was significantly lower in SAPF group than in SPAF and BPF groups (P lt; 0.05), but no significant difference was found between SPAF and BPF groups (P gt; 0.05). The VAS score was significantly higher in SAPF group than in SPAF and BPF groups (P lt; 0.05), but no significant difference was found between SPAF and BPF groups (P gt; 0.05). Conclusion SAPF could not reach enough fixation strength for the posterior Pilon fracture; both SPAF and BPF could reach rigid fixation, and have good effectiveness. And from the biomechanical points, BPF could reach better fixation strength than screw fixations.
