ObjectiveTo systematically review the association of interleukin-33 (IL-33) expression and coronary heart disease (CHD). MethodsWe searched The Cochrane Library, PubMed, EMbase, CBM, VIP, CNKI and WanFang Data up to June 30th, 2014, to collect case-control studies concerning the association of IL-33 expression with CHD. Two reviewers independently screened literature according the inclusion and exclusion criteria, extracted data and assessed the methodological quality of included studies; and then, meta-analysis was performed using the RevMan 5.2 software. ResultsSix case-control studies were included. The results of meta-analysis showed that:there were no significant differences in the levels of IL-33 between stable angina pectoris or ST-elevation myocardial infarction patients and healthy population (MD=-25.15, 95%CI -51.08 to 0.77, P=0.06; MD=-28.97, 95%CI -62.89 to 4.95, P=0.09). However, there were significant differences in the levels of IL-33 between unstable angina pectoris or non-ST-elevation myocardial infarction patients and healthy population (MD=-24.79, 95%CI -50.00 to 0.42, P=0.05; MD=-14.60, 95%CI -20.09 to -9.12, P<0.000 01). ConclusionIL-33 expression may be associated with unstable angina pectoris and non-ST-elevation myocardial infarction patients.
Day surgery is a new mode of diagnosis and treatment, and is greatly honoured and rewarded abroad. In China, day surgery begins rather later, but it has been gradually spread and applied in some large hospitals for its shorter hospital stay and less hospitalization cost. Day surgery management is a process including admission, operation, and discharge that should be finished within 24 hours, so the guarantee of medical quality and patient safety is principal factor of sustainable development influencing day surgery. This paper introduces the specifications for the quality and safety management of day surgery in West China Hospital of Sichuan University, to regulate and promote the application of day surgery, and further improve the medical quality and safety of day surgery.
Medical records play an important role in medical treatment, prevention, teaching, scientific research and hospital management. The epidemic situation of coronavirus disease 2019 is very serious. How to complete the writing of medical records with high quality and high efficiency under the high-intensity, high-pressure environment of the prevention, diagnosis, and treatment of coronavirus disease 2019 is an important issue for medical quality control management departments. West China Hospital of Sichuan University has adopted various measures to continuously improve the quality management of medical records, including optimizing the medical records of fever clinics and isolation wards, and using various methods of medical record quality management under the support of electronic medical record information systems, so as to improve the writing quality and efficiency of medical records during the diagnosis and treatment of coronavirus disease 2019.
Early identification, early diagnosis, early intervention, proper treatment, and the best benefit for patients are the most important manifestations of the ability and quality in large general hospitals. Based on the practical experience of West China Hospital of Sichuan University, we initially sorted out the basic model of multi-disciplinary treatment/multi-disciplinary team (MDT) for the management of patients with coronavirus disease 2019. This article introduces the aspects of team building, division of responsibilities, work flow exploration, MDT discussion basic norms sorting, treatment resources, supervision and coordination of main treatment measures. In the context of the epidemic, all these measures are conducive to promote stronger medical quality assurance and more reasonable resource allocation. The implementation of MDT and shared decision-making model during epidemic provide a reference for medical institutions to carry out hospital quality management, and to improve the treatment for the maximum benefit of patients.
ObjectiveTo investigate the effects of medical simulation (MST) combined with case-based learning (CBL) in training of trainee doctors in emergency department. MethodA total of 120 trainee doctors practicing in the emergency department between March 2008 and December 2014 were randomly divided into two groups:MST combined with CBL group and CBL group, who accepted MST combined with CBL training and merely CBL training, respectively. The training effects were evaluated in terms of theoretical knowledge, practical operation, comprehensive abilities of case analysis and questionnaire survey. The results were compared and analyzed with the t test. The P value less than 0.05 was a significant difference. ResultsTrainee doctors in MST combined with CBL group acquired higher scores in all of the indicators (P<0.05). ConclusionsMST combined with CBL is a feasible method and has a better effect in training of trainee doctors in Emergency Department.
Since its establishment in 2009, adhering to the concept of patient-centered service, the Day Surgery Center of West China Hospital of Sichuan University has been improving the management model constantly, and perfecting the service process gradually, to ensure the medical quality and safety of the hospital, and optimize the utilization of medical resources. In view of the management of day surgery, the hospital has formulated a sound patient access system, surgeon access principle, standardized appointment process and contingency plans, and made changes in patient admission process, responsibilities of surgeons, and nursing care comparing with the traditional hospital operation. The purpose of this paper is to introduce the management model of day surgery in West China Hospital of Sichuan University, discuss the clinical application of centralized and decentralized management in combination with the actual situation, and explore the model of day surgery with high applicability and more consistent with our national conditions.
