  • Interpretation of the global patterns of breast cancer incidence and mortality: A population-based cancer registry data analysis from 2000 to 2020

    Recently, the National Cancer Center analyzed the disease burden and epidemiological trend of breast cancer based on the global population registration data, providing important reference for the prevention and control of breast cancer and health decision-making. Based on the current situation of prevention and control of breast cancer in China, this paper briefly interpreted the key points of the disease burden and trend of breast cancer.

  • Tumor characteristics and the research progress in diagnosis and treatment of elderly patients with breast cancer

    Objective To summarize the tumor characteristics and the research progress in diagnosis and treatment of elderly patients with breast cancer. Methods To combine the literature review and clinical guidelines, to introduce tumor characteristics, the advance of diagnosis and therapy and expert consensus in elderly breast cancer. Results Compared to the patients in other age group, the elderly patients with breast cancer have special physiological and psychoical characteristics, meanwhile aged patients frequently together with basic disease, the strategy of clinical diagnosis and treatment and prognosis are very different from young and middle-aged patients. Conclusion For the elderly patients with breast cancer, we should formulate the corresponding treatment plan on the premise of the comprehensive evaluation of patients’ life expectancy, with the disease, the degree of benefit, treatment tolerance and other factors.

  • Association analysis of serum lipids and lipoprotein levels with the occurrence of breast cancer and clinicopathological characteristics of breast cancer patients

    Objective This study aimed to explore the relationship between serum lipid and lipoprotein levels and occurrence of breast cancer, and relationship between serum lipid and lipoprotein levels and clinicopathological characteristics of breast cancer patients. Methods Clinical data of 788 patients with breast cancer and 395 patients with benign breast disease were retrospectively collected, who received treatment in The First Affiliated Hospital of Chongqing Medical University from January 2014 to March 2016, and to explore the relationship between levels of total cholesterol (TC)/triglyceride (TG)/high density lipoprotein cholesterol (HDL-c)/low density lipoprotein cholesterol (LDL-c) and occurrence of breast cancer/ clinicopathological characteristics of breast cancer patients. Results ① Influencing factors that affected the occurrence of breast cancer: multifactor logistic analysis showed that, height (OR=0.950, P=0.006), body mass index (OR=1.062, P=0.041), and serum LDL-c level (OR=1.349, P=0.016) were independent influencing factors for occurrence of breast cancer, people had high body mass index and higher level of serum LDL-c had high risk of breast cancer, but people had high height had low risk of breast cancer. ②Association analysis of serum lipid and lipoprotein levels and clinicopathological characteristics of breast cancer patients: the serum TC level was correlated with expression of progesteronereceptors (PR) and lymph node metastasis status (P<0.05), the serum TC levels of patients with negative-expression of PR and lymph node metastasis were slightly higher than that of patients with positive-expression of PR and non-lymph node metastasis; the serum TG level was associated with body mass index (P<0.05), that the serum TG level of patients with body mass index≥25 kg/m2was slightly higher than that of patients with body mass index<25 kg/m2; the serum HDL-c level was correlated with the body mass index and diameter of the tumor (P<0.05), the serum HDL-c level of the patients with body mass index≥25 kg/m2 was slightly lower than that of patients with body mass index<25 kg/m2, the serum HDL-c level of patients with the tumor diameter≤2 cm was slightly higher than that of patients with the tumor diameter >2 cm; the serum LDL-c level was correlated with body mass index, expression of estrogenreceptors (ER) and PR, and molecular typing ( P<0.05), the serum LDL-c level was slightly higher in patients with body mass index≥25 kg/m2, negative expression of ER and PR, and non Luminal type patients, comparing with patients with body mass index<25 kg/m2, positive expression of ER and PR, and Luminal type patients. Conclusions High level of serum LDL-c is strongly associated with occurrence of breast cancer, and levels of serum lipid and lipoprotein are associated with expression of hormone receptor, molecular type of breast cancer, and status of lymph node, but it needs further randomized controlled studies to confirm.

  • Construction and validation of a predicting model for benefit from local surgery for bone-only metastatic breast cancer: a retrospective study based on SEER database

    Objective To predict the patients who can benefit from local surgery for bone-only metastatic breast cancer (bMBC). Methods Patients newly diagnosed with bMBC between 2010 and 2019 in SEER database were randomly divided into a training set and a validation set at a ratio of 7∶3. The Cox proportional hazards model was used to analyze the independent prognostic factors of overall survival in the training set, and the variables were screened and the prognostic prediction model was constructed. The concordance index (C-index), time-dependent clinical receiver operating characteristic curve and area under the curve (AUC), calibration curve and decision curve analysis (DCA) were used to evaluate the discrimination, calibration and clinical applicability of the model in the training set and validation set, respectively. The model was used to calculate the patient risk score and classify the patients into low-, medium- and high-risk groups. Survival analysis was used to compare the survival difference between surgical and non-surgical patients in different risk groups. Results A total of 2057 patients were enrolled with a median age of 45 years (interquartile range 47-62 years) and a median follow-up of 32 months (interquartile range 16-53 months). Totally 865 patients (42.1%) died. Multivariate Cox proportional hazards model analysis showed that the overall survival of patients with surgery was better than that of patients without surgery [hazard ratio=0.51, 95% confidence interval (0.43, 0.60), P<0.001]. Chemotherapy, marital status, molecular subtype, age, pathological type and histological grade were independent prognostic factors for overall survival (P<0.05), and a prognostic prediction model was constructed based on the independent prognostic factors. The C-index was 0.702 in the training set and 0.703 in the validation set. The 1-, 3-, and 5-year AUCs of the training set and validation set were 0.734, 0.727, 0.731 and 0.755, 0.737, 0.708, respectively. The calibration curve showed that the predicted survival rates of 1, 3, and 5 years in the training set and the validation set were highly consistent with the actual survival rates. DCA showed that the prediction model had certain clinical applicability in the training set and the validation set. Patients were divided into low-, medium- and high-risk subgroups according to their risk scores. The results of log-rank test showed that local surgery improved overall survival in the low-risk group (training set: P=0.013; validation set: P=0.024), but local surgery did not improve overall survival in the medium-risk group (training set: P=0.45; validation set: P=0.77) or high-risk group (training set: P=0.56; validation set: P=0.94). Conclusions Local surgery can improve the overall survival of some patients with newly diagnosed bMBC. The prognostic stratification model based on clinicopathological features can evaluate the benefit of local surgery in patients with newly diagnosed bMBC.

  • Validation study in younger breast cancer patients who meeting the criteria of ACOSOG Z0011 trial based on the SEER database

    Objective To explore the axillary lymph node dissection (ALND) could be safely exempted in younger breast cancer patients (≤40 years of age) who receiving breast-conserving surgery combined with radiotherapy in metastasis of 1–2 sentinel lymph node (SLN) and T1–T2 stage. Methods The data of pathological diagnosis of invasive breast cancer from 2004 to 2015 in SEER database were extracted. Patients were divided into SLN biopsy group (SLNB group) and ALND group according to axillary treatment. Propensity matching score (PSM) method was used to match and equalize the clinicopathological features between two groups at 1∶1. Multivariate Cox proportional risk model was used to analyze the relationship between axillary management and breast cancer specific survival (BCSS), and stratified analysis was performed according to clinicopathological features. Results A total of 1 236 patients with a median age of 37 years (quartile: 34, 39 years) were included in the analysis, including 418 patients (33.8%) in the SLNB group and 818 patients (66.2%) in the ALND group. The median follow-up period was 82 months (quartile: 44, 121 months), and 111 cases (9.0%) died of breast cancer, including 33 cases (7.9%) in the SLNB group and 78 cases (9.5%) in the ALND group. The cumulative 5-year BCSS of the SLNB group and the ALND group were 90.8% and 93.4%, respectively, and the log-rank test showed no significant difference (χ2=0.70, P=0.401). After PSM, there were 406 cases in both the SLNB group and the ALND group. The cumulative 5-year BCSS rate in the ALND group was 4.1% higher than that in the SLNB group (94.8% vs. 90.7%). Multivariate Cox proportional hazard analysis showed that ALND could further improve BCSS rate in younger breast cancer patients [HR=0.578, 95%CI (0.335, 0.998), P=0.049]. Stratified analyses showed that ALND improved BCSS in patients diagnosed before 2012 or with a character of lymph node macrometastases, histological grade G3/4, ER negative or PR negative. Conclusions It should be cautious to consider the elimination of ALND in the stage T1–T2 younger patients receiving breast-conserving surgery combined with radiotherapy when 1–2 SLNs positive, especially in patients with high degree of malignant tumor biological behavior or high lymph node tumor burden. Further prospective trials are needed to verify the question.

  • Analysis of metastatic sites and prognosis of newly diagnosed stage Ⅳ breast cancer patients based on SEER database

    Objective To explore the relationship between the metastatic sites and prognosis in newly diagnosed stage Ⅳ breast cancer. Methods The data of newly diagnosed female patients with stage Ⅳ invasive breast cancer with complete follow-up data from SEER database from 2010 to 2015 were grouped according to different metastatic sites, and the differences of breast cancer-specific survival (BCSS) in different metastatic sites were analyzed by univariate and multivariate Cox. Kaplan-Meier method was used to draw the survival curve, and log-rank test was used to analyze the prognostic factors of BCSS in newly diagnosed stage ⅳ breast cancer. Results A total of 8 407 patients were included in the final analysis. Among them, 5 619 (66.84%) patients were confirmed with bone metastasis only, 1 483 (17.64%) patients with lung metastasis only, 1 096 (13.04%) patients with liver metastasis only, and 209 (2.49%) patients with brain metastasis only. The median follow-up time was 22 months, with 4 180 (49.72%) breast cancer-related deaths and a median BCSS of 39 months in those patients. The location of metastasis in newly diagnosed stage Ⅳ invasive breast cancer was significantly correlated with BCSS (χ2=151.07, P<0.001). Multivariate Cox model analysis showed that the BCSS was worse in patients with liver metastasis [HR=1.34, 95%CI (1.21, 1.49), P<0.001], lung metastasis [HR=1.09, 95%CI (1.04, 1.14), P<0.001] and brain metastases [HR=1.28, 95%CI (1.20, 1.36), P<0.001] than in patients with bone metastases. Further subgroup analysis showed that the BCSS of breast cancer patients with different molecular subtypes and different metastatic sites were also significantly different (P<0.05). Patients with brain and liver metastases in the HR+/HER2– subtype had worse BCSS than those with bone metastases (P<0.001). Patients with brain metastases in the HR+/HER2+ subtype had worse BCSS than those with bone metastases (P=0.001). In HR–/HER2+ subtype, the BCSS of patients with liver metastasis, lung metastasis and brain metastasis were worse than that of patients with bone metastasis (P<0.05). In HR–/HER2– subtype, the BCSS of patients with brain metastasis and liver metastasis were worse than that of patients with bone metastasis (P<0.05) . Conclusion The prognosis of newly diagnosed stage ⅳ breast cancer patients with different metastatic sites is different, and the prognosis of different molecular subtypes and different metastatic sites is also different.
