【摘要】 目的 探讨颈椎手术并发症原因及预防措施,以提高手术成功率。 方法 对2000年5月-2009年5月实施颈椎前路手术的脊髓型颈椎病患者201例中20例发生并发症患者的临床资料及随访情况进行回顾性分析。 结果 20例并发症患者获得随访时间3个月~10年,平均5.5年。近期并发症11例,发生率5.47%;远期并发症9例,发生率4.48%。 结论 充分认识脊髓的损伤类型,仔细评估患者的全身情况,熟练的手术操作,良好的术后护理是手术成功的关键。【Abstract】 Objective To investigate the reasons of and prevention for surgical complications of anterior cervical spine. Methods The clinical data of 20 patients with surgical complications in 201 patients who underwent anterior cervical surgery from May 2000 to May 2009 were analyzed retrospectively. Results The patients with complications were followed up for 3 months to 10 years with an average of 5.5 years. The rate of 11 patients with short-term complications was 15%, and the rate of nine patients with long-term complications was 4.48%. Conclusion The key of a successful surgery for anterior cervical spine inludes full realization of type of cervical spinal cord injury, careful evaluation of the patient’s general condition, skilled operation, and good post-operative care.
目的:探讨上颈椎损伤的早期诊断方法和治疗措施。方法:回顾分析2000年1月至2008年7月间收治住院的上颈椎损伤患者35例临床资料,其中寰椎骨折6例,枢椎骨折24例,无骨折的寰枢关节脱位5例。除3例陈旧性齿状突骨折和2例陈旧性寰枢关节脱位外,其余为新鲜损伤。评价其早期诊治方法及其预后。结果:早期漏诊6例,35例患者X线检查后均需结合CT或MRI检查完善诊断及分型。手术治疗18例,其中5例为齿状突骨折早期保守治疗后改手术治疗,2例为漏诊的陈旧性寰枢关节脱位。非手术治愈16例,其中3例齿状突骨折Ⅲ型畸形愈合。1例复合性损伤患者住院3月后诊断出寰枢关节脱位出院。33例得到4~38个月随访。随访的33例患者中,骨折患者均愈合,4例寰枢关节脱位患者脱位整复,上颈椎稳定性均维持良好,神经功能改善。结论:重视上颈椎损伤患者影像检查方法早期合理的分步选择与充分利用,避免漏诊。治疗上,积极地整复骨折与脱位,尽早恢复上颈椎的稳定性。
Objective To investigate the effect of the penetration of mini-plate mass screws into facet joint on axial symptoms in cervical laminoplasty. Methods A retrospective analysis was made on the clinical data of 52 patients who underwent unilateral open-door cervical expansive laminoplasty fixed with Centerpiece mini-plate between September 2009 and December 2011. There were 42 males and 10 females, with a mean age of 61.2 years (range, 34-83 years). Seventeen patients exhibited simple degeneration cervical canal stenosis, 25 patients had multilevel cervical disc protrusion, and 10 patients had ossification of posterior longitudinal ligaments. Disease duration ranged 1-120 months (median, 11 months). The Japanese Orthopedic Association (JOA) score was used to assess neurological function, and JOA recovery rates were calculated. The visual analogue score (VAS) and the neck disability index (NDI) were used to evaluate the axial pain and neck daily activities. The axial symptoms and other complications were recorded. The cervical canal diameter, cervical curvature, cervical canal cross area, and open angle were measured according to the X-ray films, CT scans, and MRI scans. The postoperative CT three dimensional (3-D) reconstruction images were used to identify whether the screws penetrated into the facet joints. All the patients were divided into 3 groups according to involved facet joints: no joint penetrating group (no penetrated facet joint), oligo-joint penetrating group (one or two penetrated facet joints), and multi-joint penetrating group (three or more penetrated facet joints). Results Five patients suffered from C5 nerve palsy, and 2 patients had cerebrospinal fluid leakage. The follow-up time ranged 3-35 months (mean, 15.7 months). At the final follow-up, the JOA scores, NDI, cervical canal diameter, and cervical canal cross area were significantly improved when compared with preoperative ones (P lt; 0.05). At 1 week after operation, CT 3-D reconstruction showed that 16 patients had no penetrated facet joint, 23 patients had one or two penetrated facet joints, and 13 patients had three or more penetrated facet joints. There was no significant difference in age, gender, disease duration, operation time, intraoperative blood loss, and follow-up time among 3 groups (P gt; 0.05). And at the final follow-up, there was no significant difference in JOA score, VAS score, cervical curvature, cervical canal diameter, cervical canal cross area, the JOA recovery rates, and lamiae open angle among 3 groups (P gt; 0.05). The NDI of the multi-joint penetrated group was significantly higher than that of other 2 groups (P lt; 0.05). Axial pain occurred in 1 case of no penetrating group, in 4 cases of oligo-joint penetrating group, and in 5 cases of multi-joint penetrating group, showing no significant difference among 3 groups (χ2=4.881, P=0.087). Conclusion The penetrations of lateral mass screws into articular surface of facet joint may contribute to the axial symptoms after cervical laminoplasty. The risk of axial symptom raises accompany with increased penetrated facet joints.
Objective To evaluate the early outcome of anterior cervical discectomy and fusion (ACDF) using a Zero-profile implant system (Zero-P) for interbody fusion in the treatment of cervical spondylosis. Methods Between March 2010 and June 2011, 25 patients with cervical spondylosis underwent ACDF with Zero-P. There were 13 males and 12 females with an average age of 44.2 years (range, 26-67 years), including 14 cases of nerve root cervical spondylosis, 6 cases of spinal cervical spondylosis, and 5 cases of mixed cervical spondylosis. The disease duration was 3-120 months (median, 25 months). Single segment was involved in 20 cases, 2 segments in 4 cases, and 3 segments in 1 case. A total of 31 Zero-P were implanted (3 at C3, 4, 8 at C4, 5, 12 at C5, 6, and 8 at C6, 7). Primary cervical operation was performed in 23 cases and re-operation in 2 cases. Before and after operation, the height of intervertebral space and the cervical Cobb angle were measured; clinical outcome was evaluated using visual analogue scale (VAS) score for pain in the neck and upper limb, and Japanese Orthopaedic Association (JOA) score for myelopathy; and intervertebral fusion and the incidence of dysphagia were also observed. Results All incisions healed by first intention. All the patients were followed up 12-16 months (mean, 13.9 months). Interbody bone fusion was obtained, and the fusion time was 2.7-6.0 months (mean, 3.8 months). Three patients had dysphagia after operation; symptom disappeared at 1 week and 3 months after operation in 2 cases and 1 case, respectively. No fixation loosening, subsidence, or breakage occurred. The height of intervertebral space was significantly improved (P lt; 0.05) from (4.5 ± 0.5) mm at preoperation to (6.0 ± 0.7) mm at 1 week and (5.7 ± 0.6) mm at 12 months after operation; the cervical Cobb angle was significantly improved (P lt; 0.05) from (11.9 ± 6.1)° at preoperation to (21.2 ± 4.1)° at 1 week and (20.2 ± 3.7)° at 12 months after operation; and there was no significant difference between at 1 week and 12 months after operation (P gt; 0.05). The VAS score was significantly reduced (P lt; 0.05) from 7.1 ± 0.8 at preoperation to 1.9 ± 0.8 at 3 months and 1.0 ± 0.5 at 12 months after operation; the JOA score was significantly increased (P lt; 0.05) from 9.6 ± 1.3 at preoperation to 13.5 ± 1.0 at 3 months and 14.9 ± 1.0 at 12 months after operation; and there was significant difference between at 3 months and at 12 months after operation (P lt; 0.05). Conclusion The early outcome of ACDF using a Zero-P in the treatment of cervical spondylosis is satisfactory and reliable, and it can restore and maintain the cervical alignment and disc height, and disc has low incidence of postoperative dysphagia.
Objective To determine the surgical indications for posterior expansive open-door laminoplasty (EOLP) extended to the C1 level. Methods Seventeen patients undergoing C3-7 or C2-7 open-door laminoplasty were included as the case group between September 2005 and January 2010, whose spinal cord injury symptoms were not alleviated or aggravated again because of the cervical stenosis at C1-4 level, and the causes of the surgery itself were eliminated, all of these patients underwent reoperation with decompress upward to C1 level. Fifteen patients with cervical stenosis who underwent C2-7 laminoplasty and C1 laminectomy were selected as the control group. There was no significant difference in gender, age, and disease duration between 2 groups (P gt; 0.05). The pre- and post-operative cervical curvature and spinal cord compression were evaluated according to the patients’ imaging data; the pre- and post-operative neurological recovery situation was evaluated by Japanese Orthopaedic Association (JOA) 17 score and spinal cord function Frankel grade; the neurological recovery rate (according to Hirabayashi et al. method) was used to assess the postoperative neurological recovery situation. Results In the case group, 8 patients underwent primary C3-7 laminoplasty. In 3 of these patients, there was a cervical stenosis at C1, 2 level, and discontinuous cerebrospinal fluid around the spinal cord was observed; 5 of them with a compression mass which diameter was exceed 7.0 mm in the C2-4 segments. The remaining 9 patients in the case group underwent primary C2-7 laminoplasty, and the diameter of the compression mass was exceed 7.0 mm in the C2-4 segments. In all 17 patients of the case group, reoperation was performed with the decompression range extended to the C1 level, and the follow-up time was 35-61 months with an average of 45.6 months. Cervical curvature: there were 11 cases of cervical lordosis, 4 cases of straight spine, and 2 cases of cervical kyphosis before operation; but after operation, 2 cases of cervical lordosis became straight spine and 1 straight case became kyphosis. The postoperative neurological improvement was excellent in 8 cases, good in 7, and fair in 2. In the control group, all the patients had a compression mass which anteroposterior diameter was exceed 7.0 mm in the C2-4 segments before operation. The follow-up time was 30-58 months with an average of 38.7 months. Cervical curvature: there were 13 cases of cervical lordosis and 2 cases of straight spine before operation; but after operation, 1 case of cervical lordosis became straight spine. The postoperative neurological improvement was excellent in 8 cases, good in 6, and fair in 1. No significant difference was found in the JOA score at pre- and post-operation between 2 groups (P gt; 0.05); however, there were significant differences (P lt; 0.05) in the JOA score between at last follow-up and at preoperation. Conclusion The initially surgical indications which can be used as a reference for EOLP extended to C1 are as follows:① Upper cervical (C1, 2) spinal stenosis: C1 posterior arch above the lower edge part of cerebrospinal fluid around the spinal cord signal is not continuous, and the anteroposterior diameter of the spinal canal actual is less than 8.0 mm as judgment standard. ②There is a huge compression at the lower edge of C2-4 vertebrae, and the most prominent part of the diameter is exceed 7.0 mm, which can not be removed through the anterior cervical surgery, or the operation is high-risk.
Objective To compare the biomechanical properties of the anterior transpedicular screw-artificial vertebral body (AVB) and conventional anterior screw plate system (AP) in lower cervical spine by finite element study. Methods CT images (C1-T1) were obtained from a 38-year-old female volunteer. The models of intact C3-7 (intact group), AP fixation (AP group), and AVB fixation (AVB group) were established and analyzed by Mimics 14.0, Geomagic Studio 2013, and ANSYS 14.0 softwares. The axial force of 74 N and moment couple of 1 N·m were loaded on the upper surface and upper facet joint surfaces of C3. Under conditions of flexion, extension, lateral bending, and rotation, the Von Mises stress distribution regularity and maximum equivalent stree of AP and AVB groups were recorded, and the range of motion (ROM) was also analyzed of 3 groups. Results The intact model of lower cervical spine (C3-7) was established, consisting of 286 382 elements and 414 522 nodes, and it was successfully validated with the previously reported cadaveric experimental data of Panjabi and Kallemeyn. The stress concentrated on the connection between plate and screw in AP group, while it distributed evenly in AVB group. Between AP and AVB groups, there was significant difference in maximum equivalent stress values under conditions of 74 N axial force, flexion, extension, and rotation. AVB group had smaller ROM of fixed segments and larger ROM of adjacent segments than AP group. Compared with intact group, whole ROM of the lower cervical spine decreased about 3°, but ROM of C3, 4 and C6, 7 segments increased nearly 5° in both AP and AVB groups. Conclusion As a new reconstruction method of lower cervical spine, AVB fixation provides better stability and lower risk of failure than AP fixation.
Objective To study the correlation between the cervical posture in the cervical disc replacement (CDR) and the cervical curve restoration in neutral position after surgery. Methods Between January 2008 and August 2010, 51 patients underwent single segmental PRESTIGE LP replacement, and the clinical data were retrospectively analyzed. During the surgery, the patient was supinely placed and the lordosis of the cervical spine was mantained with a pillow placed beneath the neck. Of them, 28 were male and 23 were female, aged 30-64 years (mean, 45 years); 32 were diagnosed as having cervical spondylotic myelopathy, 7 having radiculopathy, and 12 having myelopathy and radiculopathy. The disease duration was 3-48 months (mean, 15 months). CDR was performed at C4, 5 in 5 cases, at C5, 6 in 42 cases, and at C6, 7 in 4 cases. The Cobb angles of the cervical alignment, targeted functional spinal unit (FSU), and targeted disc were measured by sagittal X-ray film of the cervical spine in neutral position before and after surgery, as well as the intraoperative C-arm fluroscopy of the cervical spine. Linear correlation and regression were performed to analyze the relation between cervical Cobb angle difference at intraoperation and improvement of the Cobb angles at 3 months after operation. Results The cervical Cobb angles at intraoperation and 3 months after operation were larger than those at preoperation (P lt; 0.05). The difference of the Cobb angle between intra- and pre-operation was (6.72 ± 9.13)° on cervical alignment, (2.10 ± 5.12)° on targeted FSU, and (3.33 ± 3.75)° on targeted disc. At 3 months after operation, the Cobb angle improvement of the cervical alignment, targeted FSU, and targeted disc was (6.30 ± 7.28), (3.99 ± 5.37), and (4.29 ± 5.36)°, respectively. There was no significant difference in the Cobb angle improvement between the targeted FSU and the targeted disc (t= — 0.391, P=0.698), and between the targeted disc and the cervical alignment (t= — 1.917, P=0.061), but significant difference was found between the targeted FSU and the cervical alignment (t= — 2.623, P=0.012). The linear correlation between the Cobb angle difference and the Cobb angle improvement of the cervical spine was observed (P lt; 0.05). Conclusion A slightly lordotic cervical posture during CDR is an important factor to maintaining normal physiological lordosis of the cervical spine after surgery.
Objective To explore the occurrence condition of the neck axial symptom (AS) after cervical Bryan artificial disc replacement combined with anterior cervical discectomy and fusion (Hybrid surgery) and traditional anterior cervical discectomy and fusion (ACDF surgery) to treat the two-level cervical disease, and to do contrastive analysis. Methods Between August 2006 and March 2010, 18 patients underwent Hybrid surgery (group A) and 30 patients underwent two-level ACDF surgery (group B). There was no significant difference in age, gender, disease duration, type, and operated segment between 2 groups (P gt; 0.05). The Japanese Orthopaedic Association (JOA) score, neck disability index (NDI) score, cervical curvature of the operated segment, total range of motion (ROM) of C2-7, ROM of the adjacent segment, and incidence of neck AS were recorded and compared between before operation and at last follow-up. Results All the patients were followed up 18-34 months (24.1 months on average). In both groups, the JOA and NDI scores at last follow-up had significantly improvement when compared with preoperative scores (P lt; 0.01), but there was no significant difference between 2 groups at preoperation and last follow-up (P gt; 0.05). The kyphosis incidence of the operated segment in group B was significantly higher than that in group A (χ2=5.333, P=0.021). There was no significant difference in the total ROM of C2-7 between at preoperation and last follow-up in group A (t=0.410, P=0.685); the total ROM of C2-7 at last follow-up was significantly lower than that at preoperation in group B (t=3.007, P=0.006); and significant difference was found between 2 groups at last follow-up (t=2.664, P=0.013). At last follow-up, ROM of the superior and inferior adjacent segments in group B increased obviously (P lt; 0.05) and was significantly higher than that in group A (t=2.252, P=0.033; t=2.203, P=0.037). The incidence of neck AS were 16.7% in group A and 46.7% in group B, showing significant difference at last follow-up (χ2=4.427, P=0.035). Conclusion Compared with two-level ACDF surgery, Hybrid surgery has good outcomes. At the same time, it can maintain the curvature of operated segments and total ROM, avoid excessive increased ROM of the adjacent segments, and reduce the incidence of neck AS.
Objective To evaluate the effectiveness of combined posterior decompression with laminoplasty and anterior decompression with fusion for the treatment of cervical spinal canal stenosis with reverse arch. Methods Between May 2009 and February 2012, 13 cases of cervical spinal canal stenosis with reverse arch underwent posterior decompression with laminoplasty surgery in prone position and then anterior decompression with fusion surgery in supine position. There were 7 males and 6 females with an average age of 43.5 years (range, 38-62 years) and an average disease duration of 25 months (range, 18-60 months). All the patients had neck axial symptoms and spinal cord compressed symptoms, and lateral computer radiology (CR) of the neck showed reverse arch of cervical vertebrae. Segments of intervertebral disc protrusion included C3-6 in 4 cases, C4-7 in 4 cases, and C3-7 in 5 cases. After operation, anteroposterior and lateral CR was used to observe the cervical curvature change and fixation loosening, MRI to observe the change of the compression on spinal cord, visual analogue scale (VAS) score to evaluate the improvement of axial symptom, and Japanese Orthopaedic Association (JOA) score to assess the nerve function improvement. Results All incisions healed by first intention. All patients were followed up 9-32 months (mean, 15.4 months). Internal fixator had good position without loosening or breaking and the compression on spinal cord improved significantly after operation. All the patients obtained bony fusion at 6 months after operation. The axial symptoms and the nerve function at last follow-up were improved. VAS score at last follow-up (3.25 ± 1.54) was significantly lower than that at preoperation (6.55 ± 1.52) (P lt; 0.05); JOA score at last follow-up (10.45 ± 4.23) was significantly higher than that at preoperation (7.05 ± 1.32) (P lt; 0.05); and cervical curvature value at last follow-up [(6.53 ± 3.12) mm] was significantly higher than that at preoperation [(3.22 ± 5.15) mm] (P lt; 0.05). Conclusion Combined posterior decompression with laminoplasty and anterior decompression with fusion for the treatment of cervical spinal canal stenosis with reverse arch is a safe and effective surgical method.
