  • 带膜食管支架置入治疗食管癌术后胸内食管吻合口瘘

    目的 总结覆硅胶膜镍钛合金食管支架置入治疗食管癌术后胸内食管吻合口瘘的临床经验。方法 回顾性分析 2005年 1月至 2009年 12月上海交通大学医学院附属仁济医院 12例食管癌手术后发生胸内食管吻合口瘘经覆硅胶膜镍钛合金食管支架置入治疗的临床资料,其中男 7例,女 5例;平均年龄 65(45~ 70)岁。胸内食管吻合口瘘发生于术后第 5~ 28 d,均为单个漏口,漏口直径约 0.2~ 1.0 cm。确诊食管吻合口瘘后给予支持治疗 7 d,待病情稳定后置入覆硅胶膜镍钛合金食管支架,支架长 5~ 12 cm ,内径 1.8~ 2.0 cm。结果 术后因进食呛咳死于肺部感染 1例。术后出现胸骨疼痛 7例,支架移位 3例,消化道出血 1例,均经相应的治疗后好转或治愈。生存患者于置入食管支架后第 3~ 15 d经口进食。随访 11例,随访时间 1~ 6个月,食管吻合口瘘愈合良好 9例;支架移位 1例,瘘口未愈合,术后 4个月死于肿瘤复发;在回收食管支架过程中黏膜撕裂,致吻合口狭窄 1例。结论 带膜食管支架置入是治疗食管癌术后胸内食管吻合口瘘的一种有效方法。

  • 食管支架术后延迟性并发症的手术治疗

    目的 探讨食管支架术后延迟性并发症的手术治疗效果和预防措施。 方法  1999年 1月至 2 0 0 2年 8月我院手术治疗 7例支架术后并发食管出血、再狭窄、穿孔或食管气管瘘患者 ,食管大部或食管胃部分切除 (连同支架 ) 6例 ,其中胃代食管 3例 ,结肠代食管 2例 ,胸胃代食管 1例 ;颈部食管胃部分切除、食管胃纵行缝合 1例 ;同期行右肺上中叶切除、气管节段性切除和左主支气管膜部修补术各 1例。 结果  1例术后 13天死亡 ,6例疗效满意 ,随诊5~ 36个月 ,6例患者均生存 ,能进软质食物。 结论 良性食管狭窄应慎用食管内支架治疗 ,食管支架术后并发食管再狭窄或穿孔行手术切除可挽救患者生命 ,提高远期生存率和生活质量。

  • The Clinical Aplication and Efficacy of Domestic Self-expanding Nickel-titanium Memorial Alloying Covered Stent

    目的:探讨国产自膨式镍钛记忆合金食管支架在食管、贲门良恶性狭窄及食管瘘中的临床应用并观察疗效。方法:收集我院2005年11月~2008年8月采用胃镜辅助置入镍钛记忆合金食管支架的患者108例,其中男90例,女18例,年龄37~88岁;包括食管癌性狭窄41例,贲门癌性狭窄5例,放疗后狭窄1例,肺癌压迫致食管狭窄1例,食管、贲门癌术后吻合口顽固性狭窄21例,食管癌术后复发致狭窄6例,食管瘘33例。结果:108例患者共置入食管支架116枚,一次性置入成功率100%,置入后患者吞咽梗阻的症状有不同程度的改善,呛咳的症状基本消失。结论:食管支架置入操作简单、安全,成功率高,是治疗食管良恶性狭窄及封堵瘘口的有效方法。

  • Advance in research of esophageal stent

    The esophageal disease is a major clinical disease. The esophageal stent has extensive clinical applications in the treatment of esophageal diseases. However, the clinical application of esophageal stent is limited, because there are lots of complications after implantation of esophageal stent. Biodegradable esophageal stent has two advantages: biodegradability and good histocompatibility. It is expected to solve a variety of complications of esophageal stent and provide a new choice for the treatment of esophageal diseases. Standardized esophageal stents are not fully applicable to all patients. The application of 3D printing technology in the manufacture of biodegradable esophageal stent can realize the individualized treatment of esophageal stent. And meanwhile, the 3D printing technology can reduce the manufacturing cost of the stent. This review aimed to summarize and discuss the application of esophageal stent, the current research status and prospect of biodegradable esophageal stent and the prospect of 3D printing technology in degradable esophageal stent, hoping to provide evidence and perspectives for the research of biodegradable esophageal stent.
