  • Using Shape Memory Alloy Embracing Fixator and Self-ilium Grafting to Surgically Treat Vancouver Type-B1 Periprosthetic Femoral Fractures

    目的 探讨人工髋关节置换术后Vancouver B1型股骨假体周围骨折的治疗方法。 方法 2006年4月-2011年2月采用记忆合金抓握式接骨板固定结合自体髂骨植骨治疗6例Vancouver B1型股骨假体周围骨折。其中男2例,女4例;年龄55~78岁,平均68.5岁。6例均为初次行人工关节置换术后6个月~3年,平均18.4个月发生假体周围骨折;骨折至手术时间为3~6 d,平均4.2 d。 结果 术后切口均Ⅰ期愈合,无深静脉血栓形成、肺部感染、肺栓塞等并发症发生。6例均获随访,随访时间13个月~4年,平均28.6个月。X线片示骨折全部愈合,愈合时间12~20周,平均14.8周。末次随访时Harris评分76~93分,平均83.6分;获优3例,良3例。无1例出现接骨板折断、松动,骨折再移位、骨不连、股骨假体松动等并发症。 结论 记忆合金抓握式接骨板结合自体髂骨植骨治疗Vancouver B1型股骨假体周围骨折具有固定可靠、操作简便的特点,可获满意临床疗效。

  • 空心加压螺钉联合带旋髂深血管骨瓣治疗青壮年股骨颈骨折

    目的总结空心加压螺钉联合带旋髂深血管骨瓣治疗青壮年股骨颈骨折的疗效。 方法2005年8 月-2011年2月,收治13例青壮年股骨颈骨折患者。男8例,女5例;年龄18~46岁,平均34.5岁。致伤原因:交通事故伤9例,高处坠落伤3例,跌伤1例。受伤至手术时间1~10 d,平均3.5 d。根据Garden分型标准分型,Ⅲ型5例,Ⅳ型8 例。采用空心加压螺钉固定联合带旋髂深血管骨瓣移植治疗。 结果除1例患者术中发现血管变异改用缝匠肌肌骨瓣移植外,其余患者均顺利完成手术。术后切口均Ⅰ期愈合,无相关并发症发生。13例均获随访,随访时间18~52个月,平均33.5个月。12例联合带旋髂深血管骨瓣移植者均达骨性愈合,愈合时间3.0~4.5个月,平均3.5个月;无股骨头缺血性坏死发生;术后18个月Harris 评分89~100分,平均96分。1例联合缝匠肌肌骨瓣移植者术后9个月复查示股骨头缺血性坏死并进行性加重。 结论对于青壮年股骨颈骨折,应用空心加压螺钉固定联合带旋髂深血管骨瓣移植重建血运,能促进骨折愈合,减少股骨颈缺血性坏死几率,是一种有效的治疗方法。

  • BIOMECHANICAL EFFECTS OF ILIAC SCREW PLATES ON STABILITY OF LUMBO-ILIAC FIXATION CONSTRUCT

    Objective To evaluate the biomechanical effect of a self-made iliac screw plate on the stability of lumbo-iliac fixation construct before and after fatigue loading. Methods Twelve fresh lumbo-pelvic specimens from donated adult cadavers with formalin embalm were used in the study. According to whether use the iliac screw plate or not, the specimens were randomly assigned into group A (with iliac screw plate, n=6) and group B (without iliac screw plate, n=6). The bone mineral density (BMD) of L1-4 was measured using dual-energy radiograph absorptiometry. The pedicle screw and iliac screw fixation were given at L3-5, and bilateral facetectomy and diskectomy at L5, S1 level were performed to prepare the model of the intervertebral destabilization. The biomechanical testing was conducted on a material testing machine under 0-600 N compression and — 7-7 N · m torsion loading modes for the initial compressive stiffness and torsional stiffness evaluation. And then 20 000 cyclic compressive loading of 40-400 N was given to the specimen, the stiffness evaluation was repeated. Then the maximum pull-out strength of screws at every level was measured and compared. Gross observation and radiological observation were performed during experiment. Results The BMD values of groups A and B were (1.15 ± 0.13) g/cm2 and (1.12 ± 0.11) g/ cm2 respectively, showing no significant difference between 2 groups (t=0.428, P=0.678). All pedicle screws and iliac screws were inserted in good position; no loosening or breaking of screw was observed during loading. After fatigue loading, the incidence of halo ring around the iliac screws of groups A and B was 16.7% (1/6) and 50.0% (3/6), respectively. The compressive stiffness and torsional stiffness after fatigue loading were significantly lower than those in initial state in groups A and B (P lt; 0.05); there was no significant difference in compressive stiffness and torsional stiffness between groups A and B before fatigue loading (P gt; 0.05). However, group A had higher compressive stiffness than group B (t=2.664, P=0.024) after fatigue loading, and there was no significant difference in torsional stiffness between 2 groups (t=0.410, P=0.690). No significant difference was found in screw pull-out strength of pedicle screws at L3, L4, and L5 levels between groups A and B (P gt; 0.05); however, the pull-out strength of the iliac screws in group A was significantly higher than that in group B (t=3.398, P=0.007). In groups A and B, the pull-out strength of L3 screw was significantly lower than that of L4 and L5 screws (P lt; 0.05). In group A, pull-out strength of the iliac screws was significantly higher than that of L3, L4, and L5 screws (P lt; 0.05); in group B, the pull-out strength of iliac screws was significantly lower than that of L4 and L5 screws (P lt; 0.05). Conclusion In the lumbo-iliac reconstruction, the use of iliac screw plate could resist iliac screw loosening, therefore, it has the potential to increase the stability of lumbo-iliac fixation construct.

  • TREATMENT OF ADULT AVASCULAR NECROSIS OF FEMORAL HEAD BY TRANSPLANTING ILIAC BONE FLAP WITH DEEP ILIAC CIRCUMFLEX VESSELS AND CANCELLOUS BONE

    Objective To investigate the effectiveness of transplanting iliac bone flap with deep iliac circumflex vessels and cancellous bone for the treatment of adult avascular necrosis of the femoral head (ANFH). Methods A retrospective analysis was made on the clinical data of 685 patients (803 hips) with ANFH, who underwent iliac bone flap transplantation with deep iliac circumflex vessels and cancellous bone between March 2002 and January 2010. There were 489 males (580 hips) and 196 females (223 hips) with a mean age of 40.4 years (range, 18-63 years), including 567 unilateral cases (303 left hips and 264 right hips) and 118 bilateral cases. The causes of ANFH included alcohol-induced in 223 cases, steroid-induced in 179 cases, alcohol + steroid-induced in 21 cases, traumatic in 136 cases, acetabular dysplasia in 8 cases, bone cyst in 5 cases, septic arthritis in 2 cases, joint tuberculosis in 3 cases, rheumatoid arthritis in 5 cases, and idiopathic in 103 cases. According to Steinberg staging, 211 hips were rated as stage II, 513 hips as stage III, and 79 hips as stage IV. The preoperative Harris hip score was 60.30 ± 7.02. Results Fat necrosis occurred in 2 cases after operation, primary healing of incision was obtained in the other cases; delayed infection, lower extremity deep vein thrombosis, and pulmonary embolism occurred in 2 cases, respectively. All patients were followed up 36-60 months (mean, 49 months). Harris hip score at last follow-up (83.50 ± 7.31) was significantly higher than that at preoperation (t= — 2 266.980, P=0.000), and the scores were significantly higher than those at preoperation in different stages (P lt; 0.05). The results were excellent in 523 hips, good in 185 hips, fair in 65 hips, and poor in 30 hips, and the excellent and good rate was 88.2%. X-ray examination showed bone fusion of transplanted bone flap and bone graft with an average of 4.2 months (range, 3-6 months); according to Steinberg staging, imaging stable rate was 78.3% (629/803) at last follow-up. Conclusion Iliac bone flap transplantion with deep iliac circumflex vessels and cancellous bone has the advantages of complete decompression of the femoral head, exact flap blood supply, improved blood supply of the femoral head, new support for the femoral head, and participation of osteoinductive effect for the treatment of adult ANFH, so it is an effective treatment for the retention of the femoral head.

  • COMPUTER-AIDED PRECISE RESECTION OF PELVIC TUMOR AND FUNCTION RECONSTRUCTION

    Objective To investigate the value of computer-aided design (CAD) in defining the resection boundary, reconstructing the pelvis and hip in patients with pelvis tumors. Methods Between November 2006 and April 2009, 5 cases of pelvis tumors were treated surgically using CAD technology. There were 3 males and 2 females with an average age of 36.4 years (range, 24-62 years). The cause was osteosarcoma, giant cell tumor of bone, and angiosarcoma in 1 case, respectively,and chondrosarcoma in 2 cases. According to the Enneking system for staging benign and mal ignant musculoskeletal tumors, regions I, I + II, III, IV, and I + IV is in 1 case, respectively. According to the principle of reverse engineering, 5 patients with pelvis tumors were checked with lamellar CT/MRI scanning, whose two-dimensional data were obtained in disease area. The three-dimensional reconstruction of pelvic anatomical model, precise resection boundary of tumor, individual surgical template, individual prosthesis, and surgical simulation were precisely made by computer with CAD software. Based on the proposal of CAD, the bone tumor was resected accurately, and allograft il ium with internal fixation instrument or allogeneic il ium with personal ized prosthetic replacement were used to reconstruct the bone defect after tumor was resected. Results The operation was successfully performed in 5 cases. The average operation time was 7.9 hours, and the average blood loss was 3 125 mL. Hemorrhage and cerebrospinal fluid leakage occurred in 1 case, respectively, and were cured after debridement. Five patients were followed up from 24 to 50 months (mean, 34.5 months). All patients began non-weight bearing walk with double crutches at 4-6 weeks after operation, and began walk at 3-6 months after operation. Local recurrence developed in 2 patients at 18 months after operation, and resection and radiotherapy were performed. According to International Society of Limb Salvage criteria for curative effectiveness of bone tumor l imb salvage, the results were excellent in 2 and good in 3. Conclusion The individual surgical template, individual prosthesis, and surgical simulation by CAD ensure the precision and rel iabil ity of pelvis tumors resection. The CAD technology promotes pelvis tumor resection and the reconstruction of pelvis to individual treatment stage, and good curative effectiveness can be obtained.

  • CLINICAL APPLICATION OF NEW BONE HARVESTER

    Objective To investigate the cl inical appl ication effect of the new bone harvester. Methods Between August 2006 and October 2009, 112 patients underwent autogenous il iac bone graft and were followed up. There were 71 males and 41 females with a median age of 42 years (range, 11-71 years), including 45 cases of comminuted fracture of the l imbs, 47cases of bone nonunion, and 20 cases of benign bone tumor. According to different methods of bone harvesting, the patients were divided into 5 groups: group A (n=32, with new bone harvester), group B (n=29, with tricortical bone harvester), group C (n=15, with internal lamina harvester), group D (n=23, with external lamina harvester), and group E (n=13, with bicortical il iac bone by saw). There was no significant difference in general data of 5 groups (P gt; 0.05). The incision length, operation time, and peri-operative bleeding volume were compared. At 4 days, 7 days, 60 days, and 1 year postoperatively, the pain degree was assessed using visual analogue scale (VAS), and the incidence rates of peri pheral nerve injury, fracture, haematoma, ventral hernia, il iac crest sag, and chronic pain were observed. Results The incision length and the operation time in group A was shorter than those in other groups, the peri-operative bleeding volume in group A was less than in other groups, the VAS in group A was the lowest among 5 groups, showing significant differences (P lt; 0.05). The occurrence rates of chronic pain and il iac crest sag in group A were lower than those in group B, showing significant differences at 1 year after operation (P lt; 0.01). There was no significant difference in the incidence rates of peripheral nerve injury, fracture, haematoma, ventral hernia, il iac crest sag, and chronic pain between group A and groups C, D, E (P gt; 0.05), and in the incidence rates of peri pheral nerve injury, fracture, haematoma, and ventral hernia between group A and group B (P gt; 0.05). The total compl ication in group A was the lowest among 5 groups, showing significant difference (P lt; 0.05). Conclusion The new bone harvester is minimally invasive bone harvester,which has the advantages of short operation time and incision length, less bleeding volume, low VAS and complications.

  • TREATMENT OF AVASCULAR NECROSIS OF FEMORAL HEAD AFTER FEMORAL NECK FRACTURE WITH PEDICLED ILIAC BONE GRAFT

    Objective To explore the effectiveness of pedicled il iac bone graft transposition for treatment of avascular necrosis of femoral head (ANFH) after femoral neck fracture. Methods Between June 2002 and December 2006, 22 cases (22 hips, 16 left hips and 6 right hips) of ANFH after femoral neck fracture were treated with il iac bone graft pedicled with ascending branch of the lateral femoral circumflex vessels. There were 18 males and 4 females with an age range from 28 to 48 years (mean, 37.5 years). The time from injury to internal fixation was 2-31 days, and all fractures healed within 12 months after internal fixation. The ANFH was diagnosed at 15-40 months (mean, 22 months) after internal fixation. The ANFH duration was 3-11 months (mean, 8 months). According to Association Research Circulation Osseous (ARCO) staging system, 2 hips were classified as stage IIa, 3 hips as stage IIb, 3 hips as stage IIc, 3 hips as stage IIIa, 7 hips as stage IIIb, and 4 hips as stage IIIc. The preoperative Harris hip score (HHS) was 64.10 ± 5.95. Results All incisions healed by first intention and the patients had no compl ication of lung embol ism, sciatic nerve injury, lower l imb deep venous thrombosis, and numbness and pain of donor site. All patients were followed up 2.5 to 6.3 years (mean, 4.8 years). The fracture heal ing time was 8-12 months, and no femoral neck fracture recurred. The HHS was 90.20 ± 5.35 at last follow-up, showing significant difference when compared with the preoperative value (t= —18.447, P=0.000). The hi p function were excellent in 11 hi ps, good in 10 hips, fair in 1 hip, and the excellent and good rate was 95.5%. Four hips were radiographically progressed in ARCO staging, 18 hips remained stable with a stable rate of 81.8%. Conclusion Pedicled il iac bone graft transposition is an ideal option for treatment of ANFH after internal fixation of femoral neck fracture for the advantages of femoral head revascularization, sufficient cancellous bone supply, and relatively simple procedure.

  • 带旋髂深血管蒂髂骨瓣联合空心加压螺钉内固定治疗青壮年股骨颈骨折的疗效分析

    目的 总结带旋髂深血管蒂髂骨瓣联合松质骨空心加压螺钉内固定治疗青壮年股骨颈骨折的疗效。 方法 2006 年1 月- 2008 年12 月,采用带旋髂深血管蒂髂骨瓣联合松质骨空心加压螺钉内固定治疗23 例外伤致股骨颈骨折患者。其中男17 例,女6 例;年龄19 ~ 47 岁,平均31.4 岁。骨折按Garden 分型:Ⅱ型3 例,Ⅲ型14 例,Ⅳ型6 例。受伤至手术时间1 ~ 9 d,平均5 d。 结果 术后患者切口均Ⅰ期愈合。23 例均获随访,随访时间13 ~ 36个月,平均15.3 个月。骨折愈合时间9 ~ 15 个月。术后15 个月Harris 评分为73 ~ 92 分,其中优5 例,良16 例,中2 例,优良率91.3%。1 例术后33 个月发现股骨头缺血性坏死(avascular necrosis of femoral head,ANFH),其余患者无ANFH及股骨头晚期塌陷等并发症发生。所有患者无神经损害并发症,无术区感染;1 例术后伴供骨区疼痛,12 个月后疼痛自行消失。 结论 带旋髂深血管蒂髂骨瓣联合松质骨空心加压螺钉内固定治疗青壮年股骨颈骨折手术操作简便、安全,可获得较好疗效。

  • BIOMECHANICAL EFFECTS OF SACRECTOMY EXTENTS ON STABILITY OF LUMBO-ILIAC FIXATION USING ILIAC SCREW TECHNIQUES

    Objective To evaluate the effects of sacrectomy extent on the stabil ity of lumbo-il iac fixation using single or dual il iac screw technique, and to determine which conditions require the dual il iac screw technique. Methods Nine fresh L2 to pelvic specimens were harvested from donated adult cadavers. After testing the intact state simulated by L3-5 pedicle screw fixation, sequential partial sacrectomies and L3 to il iac fixation using bilateral single il iac screw (Single) wereconducted on the same specimen as follows: in group A, under S1 partial sacrectomy and Single; in group B, under 1/2 S1 partial sacrectomy and Single; in group C, one-side (left) sacroil iac joint resection and Single; in group D, total sacrectomy and Single; and in group E, the single il iac screw was replaced by dual il iac screws based on group D. Biomechanical testing was performed on a material testing machine under 0-800 N compression and —7-7 N•m torsion loading modes for construct stiffness evaluation. Results The compressive stiffness of intact condition was (392 ± 119) N/mm, groups A, B, C, D, and E obtained 106.4% ± 9.5%, 102.7% ± 8.0%, 92.2% ± 10.1%, 72.7% ± 8.0%, and 107.7% ± 10.7% of intact condition, respectively. No significant differences were found among groups A, B, C, and the intact state (P gt; 0.05), however, the four groups showed significantly higher compressive stiffness than group D (P lt; 0.05). Although group E exhibited a comparable compressive stiffness with groups A, B, and intact state (P gt; 0.05), it displayed markedly higher compressive stiffness than groups C and D (P lt; 0.05). The torsional stiffness of intact state was (3.22 ± 1.23) N•m/deg. Groups A, B, C, D, and E acquired 105.4% ± 10.1%, 89.8% ± 12.3%, 75.9% ± 10.6%, 71.2% ± 10.2%, and 109.1% ± 16.9% of intact state, respectively. No significant differences were detected among groups A, B, E, and the intact state (P gt; 0.05). However, groups C and D showed remarkably lower torsional stiffness than groups A, E, and the intact state (P lt; 0.05). Importantly, group E offered remarkably higher torsional stiffness than group B (P lt; 0.05). Conclusion After under 1/2 S1 partial sacrectomy, single il iac screw technique could effectively restore local stabil ity; whereas it could hardly provide adequate stabil ity for further resection of one-side sacroil iac joint or total sacrectomy; in such situation, the use of dual il iac screw technique could obtain sufficient construct stabil ity. Therefore, in the surgical treatment of sacral tumor, the dual il iac screw technique should be considered for the unstable conditions of totalsacrectomy or under 1/2 S1 sacrectomy with one-side sacroil iac joint resection.

  • TUMOR-SEGMENTAL RESECTION OF HAND-FOOT-GIANT CELL TUMOR OF BONE AND AUTOLOGOUS ILIAC BONE GRAFT RECONSTRUCTION

    To evaluate the effectiveness of tumor-segmental resection and autologous il iac bone graft reconstruction combined with internal fixation in treating hand-foot-giant cell tumor of bone. Methods Between August 1997 and April 2008, 8 cases of hand-foot-giant cell tumor of bone were treated, including 3 males and 5 females with an average age of 28.5 years (range, 16-42 years). The locations were metacarpal bones in 3 cases, metatarsal bones in 4 cases, and phalanges of toes in 1 case. According to Campanacci’s gradation of X-ray films, there were 1 case of grade I and 7 cases of gradeII; according to pathological examination before opration, there were 3 cases of grade I to II, 4 cases of grade II, and 1 case of grade II to III; and according to TNM staging, there were 1 case of TisN0M0, 4 cases of T1N0M0, and 3 cases of T2N0M0. There were 2 cases of recurrence, the time from the first operation to recurrence were 11 and 14 months, respectively. The tumor size was 1.8 cm × 1.0 cm to 6.0 cm × 2.0 cm, the cortical bone became thinner, and the boundary between tumor and periosteum was clear. All patients underwent tumor-segmental resection combined with autologous il iac bone graft reconstruction, and miniplate internal fixation by lumbar anesthesia or trachea cannula anesthesia. Results All incision healed by first intention. Eight patients were followed up 10 to 84 months with an average of 46 months. Radiographs showed that fracture union was achieved at 3 to 9 months (mean, 5 months). No significant rotation, angular, and shortening deformity occurred in il iac bone graft. The function of il iac bone donor site recovered excellently. The pathological examination showed giant cell tumor of bone in all cases, including 2 case of grade I-II, 5 cases of grade II, and 1 case of grade II-III. The hand or foot function recovered excellently. No tumor recurrence or lung metastasis occurred during follow-up. Conclusion Tumor-segmental resection combined with autologous il iac bone graft reconstruction plus internal fixation has excellent effectiveness for hand-foot-gaint cell tumor of bone.
