  • Effectiveness of Caudal Bupivacaine with Neostigmine for Postoperative Pain Management after Hypospadias Surgery in Children: A Systematic Review

    Objective To assess the analgesic effect of caudal bupivacaine with neostigmine for hypospadias surgery in children. Methods We searched The Cochrane Library (Issue 2, 2008), MEDLINE (1966-2008), EMbase (1980-2008), CBMdisc (1978-2008), CNKI (1989-2008) and Wanfang Database using the keywords “bupivacaine”, “neostigmine” and “hypospadias” to identify randomized control trials (RCTs) of caudal bupivacaine with neostigmine versus bupivacaine alone for hypospadias surgery in children. The methodological quality of included RCTs was evaluated by two reviewers independently, and Meta-analysis was conducted using RevMan 5.0.13 software. Results We identified 3 RCTs including 160 patients which had an uncertain risk of bias. The results of meta-analysis showed no significant difference found between bupivacine alone and bupivacine plus 1 µg/kg neostigmine (WMD= 0.05, 95%CI – 6.76 to 6.86, P= 0.99), but a difference was found between bupivacine alone and bupivacine plus 1 µg/ kg, 2 µg/kg, or 3 µg/ kg neostigmine (WMD= 12.79, 95%CI 10.96 to 14.62, Plt; 0.000 01; WMD= 12.10, 95%CI – 6.76 to 6.86, Plt; 0.000 01; WMD= 11.90, 95%CI 9.17 to 14.63, Plt;0.000 01). The duration of postoperative analgesia of the bupivacaine with neostigmine group was longer than that of bupivacine alone. Conclusions Because of the small sample size and the unclear risk of bias in the included RCTs, the effectiveness of caudal bupivacaine with neostigmine for postoperative pain management after hypospadias surgery in children is not completely confirmed. So more large sample size, double-blind RCTs are needed.

  • 腹腔镜完全腹膜外腹股沟疝修补术中3D补片免固定技术的临床应用体会

    目的探讨3D补片免固定技术在腹腔镜完全腹膜外腹股沟疝修补术中的安全性及疗效。 方法回顾性分析笔者所在医院2011年1月至2013年6月期间施行的96例腹腔镜完全腹膜外腹股沟疝修补术患者的临床资料,所有患者术中均采用3D补片免固定技术。 结果96例患者均成功在全麻下应用3D补片免固定技术行腹腔镜完全腹膜外腹股沟疝修补术,无中转开放手术病例。手术时间31~58 min、(37.6±4.3)min;术中出血5~25 mL、(10.9±2.7)mL;住院时间3~8 d、(4.0±0.3)d。术后均未出现神经血管损伤、阴囊血清肿、腹股沟区疼痛不适及异物感等并发症。术后96例患者全部获访,随访时间均为1年。随访期间所有患者均未复发,均未发生补片感染、慢性疼痛、局部异物感等并发症。 结论在腹腔镜下完全腹膜外腹股沟疝修补术中采用3D补片免固定技术安全可靠,近期疗效满意,具有操作简便、避免了腹股沟区血管神经副损伤等优点。

  • Protective effects of metformin on myocardial injury in patients with COVID-19 combined with coronary heart diseases and diabetes

    ObjectiveTo investigate whether metformin has protective effect on myocardial injury in patients with coronavirus disease 2019 (COVID-19) combined with coronary heart diseases and diabetes.MethodsCOVID-19 patients with coronary heart disease and diabetes who were admitted to Tongji Hospital from January 18 to April 25 in 2020 were enrolled. They were divided into a metformin group and a none-metformin group according to whether the metformin was used. The demographic characteristics, clinical symptoms, laboratory parameters, treatment and clinical outcomes of the two groups were analyzed retrospectively.ResultsThere were 29 patients in the metformin group, 3 patients (12.0%, 3/25) suffered myocardial injury and 1 (3.4%) died of acute respiratory failure complicated by septic shock; 67 patients were in the non-metformin group and 24 (37.5%, 24/64) had myocardial injury but 15 died in hospital among whom 1 died of septic shock complicated by disseminated intravascular coagulation, 1 acute respiratory failure complicated by possible cerebral hemorrhage, 2 acute respiratory failure, 1 fulminant myocarditis, 3 acute myocardial infarction and 7 cardiac arrest. The incidence of myocardial injury (12.0% vs. 37.5%, P=0.019), hospital mortality (3.4% vs. 22.4%, P=0.034) and mortality of cardiovascular events (0.0% vs. 16.4%, P=0.049) in the metformin group were significantly lower than those in the non-metformin group. Multivariate analysis showed that the use of insulins (OR=11.235, P=0.003) was an influencing factor for in-hospital mortality of patients. The use of metformin (OR=0.154, P=0.013) was positively correlated with the myocardial injury.ConclusionWhen patients with coronary heart disease and diabetes are infected with COVID-19, metformin can effectively reduce myocardial damage and has a certain effect on reducing hospital mortality. Combined with clinical considerations, it is worthy of popularization.
