  • Relationship between the Inspiratory Capacity and Clinical Characters of Stable Chronic Obstructive Pulmonary Disease

    【摘要】 目的 探讨深吸气量(inspiratory capacity,IC)与慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者临床特征的相关性。 方法 2009年12月-2010年6月纳入84例稳定期COPD患者,测定6分钟步行试验(6-minute walk test,6MWT)、圣乔治生活问卷评分(St George′s respiratory questionnaire,SGRQ)及肺功能检查:第1秒用力呼气容积(forced expirotovy volume in one second,FEV1)、IC、IC与肺总量(total lung capacity,TLC)比值(IC/TLC)等相关指标,并进行相关性分析。 结果 6MWT值与FEV1无明显直线相关(r=0.14,Pgt;0.1);6MWT值与FEV1≥60%无明显直线相关(r=0.16,Pgt;0.1);6MWT值与IC值呈正的直线相关(r=0.317,Plt;0.01);6MWT值与IC/TLC值呈正的直线相关(r=0.274,Plt;0.01);SGRQ值与FEV1呈负的直线相关(r=-0.307,Plt;0.01);SGRQ值与IC值无直线相关(r=-0.001,Pgt;0.25);SGRQ值与IC/TLC值无直线相关(r=-0.003,Pgt;0.25)。 结论 对COPD患者,IC比FEV1更准确地反映患者的运动耐量的程度,FEV1比IC更准确地反映患者呼吸困难的严重程度。【Abstract】 Objective To explore the relationship between the inspiratory capacity (IC) and clinical characters of stable chronic obstructive pulmonary disease (COPD). Methods Between December 2009 and June 2010, 84 patients with stable COPD were enrolled. Lung function (FEV1, IC, IC/TLC) ,6 minute walk-test (6MWT) and St George′s respiratory questionnaire (SGRQ) were examined. The relationship among FEV1, IC, IC/TLC, and the results of 6MWT and SGRQ by Pearson correlation analysis. Results There was no linear correlation between 6MWT and FEV1 (r=0.14,Pgt;0.1), and 6MWT and FEV1≥60% (r=0.16,Pgt;0.1). There was positive correlation between 6MWT and IC (r=0.317,Plt;0.01), and 6MWT and IC/TLC (r=0.274,Plt;0.01). There was negative correlation between SGRQ and FEV1 (r=-0.307,Plt;0.01); and no linear correlation between SGRQ and IC (r=-0.001,Pgt;0.25), and SGRQ and IC/TLC (r=-0.003,Pgt;0.25).  Conclusion In stable COPD patients, IC may be more accurate than FEV1 in refection of exercise tolerance while FEV1 may be more sensitive than IC in evaluation of dyspnea.

  • The Feasibility and Effect of Early Pulmonary Rehabilitation in Patients after Acute Exacerbation of Chronic Obstructive Pulmonary Disease in A District Hospital

    ObjectiveTo investigate the feasibility and effect of early pulmonary rehabilitation (PR) in patients after acute exacerbation of chronic obstructive pulmonary disease (COPD) in a district hospital. MethodsA single-centre prospective study was conducted. The COPD patients after an episode of acute exacerbation and referred to the outpatient department were recruited from January 2013 to December 2014. They were randomized to a group with PR (PR group) and a group without PR (wPR group). The following data were recorded and evaluated including age, gender, forced vital capacity (FVC), forced expiratory volume in one second (FEV1), and FEV1 as a percentage of the predicted value (FEV1% pred).The baseline and the post-PR medical research council scale (MRC), St. George's respiratory questionnaire (SGRQ), and six-minute walk distance (6MWD) were also compared. ResultsA total of 91 cases were enrolled with 46 cases in the PR group and 45 cases in the wPR group. The age, gender, the severity of COPD were similar in two groups (P > 0.05). The MRC score and SGRQ score of the PR group were significantly improved 3 months later compare with the baseline (P < 0.05), and did not changed significantly in the wPR group (P > 0.05). There were 26 patients whose SGRQ scores decreased > 4 in the PR group (26/46, 56.5%), which was significantly higher than the wPR group (7/45, 15.6%) (P < 0.05). The 6MWD of the PR group was significantly increased 3 months later compare with the baseline (P < 0.05), and did not changed significantly in the wPR group (P > 0.05). There were 22 patients whose 6MWD increased > 54 meters in the PR group (22/46, 47.8%), which was significantly higher than the wPR group (9/45, 20.0%) (P < 0.05). ConclusionsIt is feasible and safety to perform early PR in patients after acute exacerbation of COPD in the district hospital. The early PR can improve the MRC score, SGQR score, and 6MWD in COPD patients.

  • The application value of 6-minute walking test in clinical evaluation of chronic heart failure

    ObjectivesTo analyze the application value of 6-minute walking test (6MWT) in the clinical evaluation of chronic heart failure (CHF).MethodsPubMed, EMbase, The Cochrane Library, CBM, VIP, WanFang Data and CNKI databases were searched online to collect randomized controlled trials (RCTs) of 6-minute walking distance (6MWD) as the CHF evaluation index. Two reviewers independently screened literature, extracted data, and then analyzed data by using SPSS 17.0 statistical software. The 6MWD with symptom, quality of life, exercise tolerance (ETT), left ventricular ejection fraction (LVEF), peak oxygen consumption (pVO2) were analyzed by Kappa consistency test, and the possible influencing factors of 6MWD were analyzed by logistic regression.ResultsA total of 158 RCTs involving 17 853 patients were included. The results of statistical analysis showed that: 6MWD was consistent with the improvement of symptoms, quality of life, ETT, LVEF and pVO2 (Kappa>0.4). Baseline 6MWD (OR=2.91, 95%CI 1.278 to 6.634,P=0.011) and NYHA Ⅲ-Ⅳ ratio (OR=2.59, 95%CI 1.091 to 6.138, P=0.031) were the independent influencing factors for 6MWD improvement separately.ConclusionsThe 6MWT is an objective and reliable indicator of CHF evaluation.
